PURPOSE: To evaluate the hardness profile of three resin-based restorative composites (RBC) (Filtek Z250XT, Filtek One Bulk Fill, Filtek Bulk Fill Flow) polymerized by a multi-wave curing light. METHODS: Specimens (n= 12) were prepared by inserting 2 mm RBC increments into a split-mold and polymerized from the top using either 20- or 40-second exposure times. Specimen curing was performed directly at a 1 mm distance (control-group) or through an ivorine-tooth slot preparation at a 5 mm distance (experimental-group). Specimens were stored (37 ± 1°C/24 hours), then subjected to Knoop indenter (25g/5 seconds). Specimens' KHN values were obtained from the upper and lower surfaces. Relative hardness (RH) (lower-to-upper ratio) was calculated for each specimen. Data were analyzed with three-way ANOVA and Tukey's HSD (α= 0.05). RESULTS: There was no significant RH difference among RBCs in the control group, regardless of the exposure time (P> 0.05). Average RH ratios for all RBCs tested in this group were greater than 0.80. However, the average RH values of the experimental RBC group were significantly lower. The RH for Z250 was 0.39 in the 20-second group, while RH was 0.63 in the 40-second group. BF had an RH ratio of 0.70 in the 20-second and 0.72 in the 40-second group, while One Bulk had a ratio of 0.65 in the 20-second and 0.71 in the 40-second groups. Doubled exposure time substantially increased RH of all tested materials at a 1 mm tip-to-material distance. Clinically relevant 5 mm light-tip to material-surface distance significantly reduced polymerization efficacy of RBC specimens, regardless of the exposure time. CLINICAL SIGNIFICANCE: Adequate light-polymerization of resin-based direct restoratives is necessary for long-term clinical success. Polymerizing Class 2 restorations is challenging due to a hard-to-reach location and an increased distance between the light source and the restorative material. Insufficient polymerization is often seen at the bottom of the proximal box of the Class 2 cavity, with a detrimental effect on restoration longevity.

STATEMENT OF PROBLEM: Postpolymerization methods influence the properties of 3-dimensionally (3D) printed materials. Nevertheless, it is a time-consuming step that could lead dentists to avoid the chairside use of 3D printing. Information assessing the impact of different postpolymerization workflows on flexural strength and color stability is sparse. PURPOSE: The purpose of this in vitro study was to analyze the impact of different postpolymerization methods on the flexural strength and color stability of 3D-printed resins used for interim restorations. MATERIAL AND METHODS: Three resin materials: CT (Cosmos Temp; Yller Biomaterials), DT (Denture Teeth; Formlabs), and CB (C&B MFH; NextDent) were submitted to 4 methods of postpolymerization: a Form Cure polymerization chamber (FC) (Form Cure; Formlabs), a broadband LED device (Valo Grand; Ultradent Products, Inc) operating at 1000 mW/cm2 for 2 different polymerization times (40 seconds/V40 or 120 seconds/V120), and a custom-made polymerization chamber with a 24 W UV LED Strip (CC). The specimens were assessed for flexural strength using an universal testing machine immediately after polymerization or after 10 000 thermal cycles, and the color stability was evaluated using a spectrophotometer at baseline, after 7 days in dark, dry storage at 37 °C and after 24 hours of artificial aging in water at 60 °C. The data were evaluated using 3-way ANOVA (flexural strength) and 3-way repeated measurements ANOVA (color stability), followed by the Tukey HSD test (α=.05). RESULTS: Flexural strength showed significant differences for resin, postpolymerization method, time, and for the interaction between all effects. CT showed the highest overall flexural strength, while DT showed the lowest. In general, V120 produced specimens with higher flexural strength. Thermocycling did not reduce flexural strength except for CB polymerized with FC and V120, and DT polymerized with V40. For color stability, significant differences were found for resin, postpolymerized method, time, and for the interaction between all effects. The highest ΔE values were observed for DT polymerized using V40 after artificial aging. The lowest values were observed for the same resin but polymerized using V120 after 7 days in dark, dry storage at 37 °C. CONCLUSIONS: Postpolymerization with the broadband LED device for 120 seconds can produce specimens with higher flexural strength values and higher color stability when compared with the other postpolymerization workflows tested in this study.

STATEMENT OF PROBLEM: A reliable and validated protocol for nondestructive analysis of the entire restorative interface that could be applicable in studies assessing the adaptation of direct and indirect restorations is lacking. PURPOSE: The purpose of this in vitro study was to validate a 3D protocol for microleakage assessment in class II mesio-occluso-distal (MOD) cavities restored with composite resin. MATERIAL AND METHODS: Sixteen human third molars received standardized class II MOD preparations with cervical margins located 1 mm apical to the cemento-enamel junction. The specimens were restored with Filtek Bulk Fill Posterior (FBP) or Filtek Bulk Fill Flowable (FBF) and submitted to 10 000 thermocycles (5 °C and 55 °C with a 30-second dwell time at each temperature and 5-second transfer time). The specimens were scanned with microcomputed tomography (µCT) before and after immersion in 50% ammoniacal silver nitrate solution for 12 hours. Scanned data were processed, and the 3D amount of infiltrated silver nitrate was analyzed through the entire adhesive interface. In addition, the linear distance of silver nitrate infiltration was assessed using 3 axial images obtained from each specimen proximal box (2D analysis). Data were tested for homogeneity by using the Shapiro-Wilk test and evaluated by using 1-way ANOVA, followed by the post hoc Tukey HSD test (α=.05). RESULTS: FBP showed significantly lower leakage than FBF for both 2D and 3D analyses. A low correlation was observed between the assessment methods. CONCLUSIONS: Three-dimensional analysis of microleakage using µCT is a predictable and more reliable method for nondestructive and comprehensive analysis of the entire specimen.

PURPOSE: To evaluate the color change caused by post-irradiation conversion (ΔE24h and ΔE8 days ) and artificial aging (ΔEAGING ) of resin cements light activated through ceramics of different opacities immediately and 6 minutes after manipulating the material. MATERIALS AND METHODS: Resin cement disks (Allcem and LuxaCore) were light activated through ceramics (without ceramic, e.max HT, LT, and MO) immediately and 6 minutes after the manipulation (n = 10). Spectrophotometry was used to evaluate ΔE24h and ΔE8 days after 37˚C-dry-dark-storage for 24 hours and 8 days, and ΔEAGING after 60˚C-water-storage for 24 hours. Data were analyzed with three-way ANOVA and Tukey's HSD (α = 0.05). RESULTS: Regarding the post-irradiation color change, significant differences were found between the cements (p < 0.0001), light activation protocols (p = 0.037), and ceramics (p < 0.001). The immediate activation (16.2 ± 0.1) showed lower ΔE values than the delayed activation (16.5 ± 0.1). Ceramics influenced the ΔE values as follows: MO (14.0 ± 1.2) < LT (14.9 ± 1.2) ≈ HT (15.4 ± 1.2) < control (21.2 ± 1.2). Regarding ΔEAGING , significant differences were found between cements (p < 0.001), light activation (p = 0.006), and ceramics (p < 0.001). The delayed activation (8.4 ± 0.1) showed lower values than the immediate activation (8.9 ± 0.1). Ceramic spacers influenced ΔEAGING as follows: control (5.6 ± 0.2) < MO (6.6 ± 0.2) < HT (11.0 ± 0.2) ≈ LT (11.5 ± 0.2). CONCLUSION: The delayed light activation resulted in increased color stability, while the ceramic interposition resulted in lower color stability.

RESUMO

Root canal perforation and root resorption are challenging clinical conditions to correctly diagnose and treat, especially when they occur in anterior teeth. This clinical report describes the computed tomography findings, endodontic treatment, prosthetic rehabilitation, and clinical outcome of an iatrogenic root perforation and internal resorption in a maxillary central incisor. The case management consisted of endodontic retreatment, periodontal surgery, and prosthetic rehabilitation. Gray mineral trioxide aggregate (MTA) was used to fill the resorption space and seal the perforation. The prosthetic treatment was performed with glass fiber-reinforced dowels and all-ceramic crowns. No signs or symptoms, including discomfort, pain, or esthetic defects were observed in 30 months of follow-up.

OBJECTIVES: In this study hybrid nanofibers embedded with niobium pentoxide (Nb2O5) were synthesized, incorporated in self-adhesive resin cement, and their influence on physical-properties was evaluated. METHODS: Poly(D,L-lactide), PDLLA cotton-wool-like nanofibers with and without silica-based sol-gel precursors were formulated and spun into submicron fibers via solution blow spinning, a rapid fiber forming technology. The morphology, chemical composition and thermal properties of the spun fibers were characterized by field emission scanning electron microscopy (FE-SEM), energy dispersive X-ray spectroscopy (EDS) and Fourier-transform infrared spectroscopy (FTIR), and differential scanning calorimetry (DSC), respectively. Produced fibers were combined with a self-adhesive resin cement (RelyX U200, 3M ESPE) in four formulations: (1) U200 resin cement (control); (2) U200+1wt.% PDLLA fibers; (3) U200+1wt.% Nb2O5-filled PDLLA composite fibers and (4) U200+1wt.% Nb2O5/SiO2-filled PDLLA inorganic-organic hybrid fibers. Physical properties were assessed in flexure by 3-point bending (n=10), Knoop microhardness (n=5) and degree of conversion (n=3). Data were analyzed with One-way ANOVA and Tukey's HSD (α=5%). RESULTS: Composite fibers formed of PDLLA-Nb2O5 exhibited an average diameter of ∼250nm, and hybrid PDLLA+Nb2O5/SiO2 fibers were slightly larger, ∼300nm in diameter. There were significant differences among formulations for hardness and flexural strength (p<0.05). Degree of conversion of resin cement was not affected for all groups, except for Group 4 (p<0.05). SIGNIFICANCE: Hybrid reinforcement nanofibers are promising as fillers for dental materials. The self-adhesive resin cement with PDLLA+Nb2O5 and PDLLA+Nb2O5/SiO2 presented superior mechanical performance than the control group.

OBJECTIVES: To evaluate the colour-stability and gloss-retention of silorane versus dimethacrylate composites exposed to accelerated aging from daylight radiation. METHODS: Five disc-shaped specimens of photo-cured resin-composites were prepared and manually polished for each material (Filtek Silorane, Herculite XRV, Tetric Evoceram and QuiXfil). Colour and gloss were evaluated before and after periods (baseline, 24, 72, 120 and 192 h) of accelerated photo-aging in xenon light following ISO 7491:2000. Colour measurements were performed with a colourimeter according to the CIE-Lab colour-space. The colour change (DeltaE) for each time was calculated. The surface gloss was measured using a glossmeter. Results were evaluated using one-way ANOVA and Tukey tests (alpha=0.05). Correlations between logtime, DeltaE and gloss were evaluated using Pearson's correlation (alpha=0.05). RESULTS: Materials generally decreased in L and a and increased in b. The strong exception was Filtek Silorane which maintained a and b. DeltaE was found to be a positive linear function of logtime for all materials. Materials varied in the magnitude and rate of increase of DeltaE with logtime: QuiXfil>Tetric EvoCeram>(Filtek Silorane>or=Herculite XRV). DeltaE remained<3.3 for Filtek Silorane and Herculite XRV. Gloss was found to be a negative linear function of logtime. Gloss was maximal in the sequence: Filtek Silorane approximately Tetric EvoCeram>Herculite XRV>QuiXfil. CONCLUSIONS: Silorane gave the best overall performance in stability over time, compared to a set of representative dimethacrylate composites.

OBJECTIVES: This study has investigated the influence of Titanium dioxide nanotubes (TiO2-nt) addition to self-adhesive resin cement on the degree of conversion, water sorption, and water solubility, mechanical and biological properties. METHODS: A commercially available auto-adhesive resin cement (RelyX U200™, 3M ESPE) was reinforced with varying amounts of nanotubes (0.3, 0.6, 0.9wt%) and evaluated at different curing modes (self- and dual cure). The DC in different times (3, 6, 9, 12 and 15min), water sorption (Ws) and solubility (Sl), 3-point flexural strength (σf), elastic modulus (E), Knoop microhardness (H) and viability of NIH/3T3 fibroblasts were performed to characterize the resin cement. RESULTS: Reinforced self-adhesive resin cement, regardless of concentration, increased the DC for the self- and dual-curing modes at all times studied. The concentration of the TiO2-nt and the curing mode did not influence the Ws and Sl. Regarding σf, concentrations of both 0.3 and 0.9wt% for self-curing mode resulted in data similar to that of dual-curing unreinforced cement. The E increased with the addition of 0.9wt% for self-cure mode and H increased with 0.6 and 0.9wt% for both curing modes. Cytotoxicity assays revealed that reinforced cements were biocompatible. SIGNIFICANCE: TiO2-nt reinforced self-adhesive resin cement are promising materials for use in indirect dental restorations. Taken together, self-adhesive resin cement reinforced with TiO2-nt exhibited physicochemical and mechanical properties superior to those of unreinforced cements, without compromising their cellular viability.

BACKGROUND: Dentin hypersensitivity treatments are based on the physical obliteration of the dentinal tubules to reduce hydraulic conductance. The aim of the present study is to evaluate the hydraulic conductance of bovine root dentin after irradiation with a 980-nm diode laser, with or without associated fluoride varnish. METHODS: Sixty bovine root dentin specimens were divided into six groups (n = 10 in each group): G1, G3, and G5 (0.5 W, 0.7 W, and 1 W diode laser, respectively); G2, G4, and G6 (fluoride varnish application + 0.5 W, 0.7 W, and 1 W diode laser, respectively). The dentin hydraulic conductance was evaluated at four time periods with a fluxmeter: 1) with smear layer, 2) after 37% phosphoric acid etching, 3) after the treatments, and 4) after 6% citric acid challenge. After the dentinal fluid flow measurements, specimens were also evaluated for mineral composition using energy dispersive X-ray spectroscopy (EDS). RESULTS: Analysis demonstrated a better result with increased irradiation power (P < 0.001), especially if the diode laser irradiation was associated with the application of fluoride varnish (P < 0.001), ensuring a greater reduction in permeability. Considering the groups treated only with laser irradiation, the 1 W group was superior when compared with the 0.5 W and 0.7 W irradiated groups immediately after treatment (P < 0.001). After citric acid testing, all groups showed similar results, except when comparing the 1 W groups with the 0.5 W groups (P = 0.04). EDS results of the irradiated groups showed an increase in the proportion of calcium and phosphorus ions, which demonstrates a superficial composition modification after laser treatments. CONCLUSION: Laser irradiation of exposed dentin promoted significant reduction in the dentin hydraulic conductance, mainly with higher energy densities and association with fluoride varnish.

Objective: multidisciplinary treatment has become increasingly common in dental offices and this treatment strategy is based on the achievement of a healthy, harmonious, and pleasant smile. In addition, the development of new ceramic systems and the possibility of making indirect restorations with a low thickness, such as laminates veneers, allowed combining highly satisfactory aesthetic properties, preserving the dental structure through minimally Invasive technique. Case report: this paper describes a multidisciplinary approach involving periodontics and restorative dentistry for the successful, functional, and aesthetic treatment in an adult patient. The digital smile design was performed as a guide to plan the clinical case considering factors such as tooth inclination and position, golden proportion, tooth size and shape. Discussion: based on these concepts, the present work reports, through a clinical case, the restoration of aesthetics and function with ceramic laminates veneers in lithium disilicate, in the case of anatomical correction and diastema closure. Conclusion: overall, the multidisciplinary treatment plan demonstrated to have greater advantages both for the aesthetics and function of the patient, as well as for the work team. During the 24-month follow-up, this case presented great functional and aesthetics results. (AU)


Introdução: o tratamento multidisciplinar tornou-se cada vez mais comum nos consultórios odontológicos, sendo uma estratégia baseada no tratamento da obtenção de um sorriso saudável, harmonioso e agradável. Além disso, o desenvolvimento de novos sistemas cerâmicos e a possibilidade de fazer restaurações indiretas com baixa espessura, como facetas laminadas, permitiram combinar propriedades estéticas altamente satisfatórias, preservando a estrutura dentária com o mínimo desgaste. Caso clínico: este artigo descreve uma abordagem multidisciplinar envolvendo Periodontia e Odontologia restauradora para um tratamento funcional e estético em um paciente adulto. O desenho digital do sorriso foi realizado como um guia para planejar o caso clínico, considerando fatores como inclinação e posição do dente, proporção áurea, tamanho e formato do dente. Discussão: com base nesses conceitos, o presente trabalho relata, através de um caso clínico, a restauração estética e funcional com laminados cerâmicos em dissilicato de lítio para correção anatômica e fechamento de diastemas. Conclusão: em geral, o plano de tratamento multidisciplinar demonstrou ter maiores vantagens tanto para a estética e função do paciente, quanto para a equipe de trabalho. Durante os 24 meses de acompanhamento, esse caso apresentou ótimos resultados funcionais e estéticos. (AU)

OBJECTIVES: To evaluate the influence of different tertiary amines on degree of conversion (DC), shrinkage-strain, shrinkage-strain rate, Knoop microhardness, and color and transmittance stabilities of experimental resins containing BisGMA/TEGDMA (3:1wt), 0.25wt% camphorquinone, 1wt% amine (DMAEMA, CEMA, DMPT, DEPT or DABE). Different light-curing protocols were also evaluated. METHODS: DC was evaluated with FTIR-ATR and shrinkage-strain with the bonded-disk method. Shrinkage-strain-rate data were obtained from numerical differentiation of shrinkage-strain data with respect to time. Color stability and transmittance were evaluated after different periods of artificial aging, according to ISO 7491:2000. Results were evaluated with ANOVA, Tukey, and Dunnett's T3 tests (α=0.05).
