A recent meta-analysis of functional neuroimaging contrasts between emotional and neutral face processing has shown that the processing of facial emotions can be better classified according to threat detection than emotional valence, with the authors suggesting that their data are inconsistent with both the right-hemisphere and valence models of emotional laterality. I report empirical and theoretical data indicating that facial expressions are better classified according to threat detection than to the distinction between positive and negative emotions. I challenge, however, the claim that laterality effects provide little support to the right-hemisphere model of emotional laterality. This claim contrasts with neuropsychological and psychophysiological investigations that have shown that the right hemisphere has a graded prevalence for emotions provoked by threatening events. A reanalysis of data obtained in the target study suggests that the reported data are not necessarily inconsistent with a model assuming a graded, right-hemisphere dominance for emotions. I present a model of hemispheric asymmetries that could be consistent with the assumption that the right hemisphere's dominance for emotions may mainly be concerned with threatening events.

PURPOSE OF REVIEW: Pronagnosia is a rare acquired or developmental pathological condition that consists of a selective difficulty to recognize familiar people by their voices. It can be distinguished into two different categories: apperceptive phonagnosia, which denotes a purely perceptual form of voice recognition disorder; and associative phonagnosia, in which patients have no perceptual defects, but cannot evaluate if the voice of a known person is or not familiar. The neural substrate of these two forms of voice recognition is still controversial, but it could concern different components of the core temporal voice areas and of extratemporal voice processing areas. This article reviews recent research on the neuropsychological and anatomo-clinical aspects of this condition. RECENT FINDINGS: Data obtained in group studies or single case reports of phonagnosic patients suggest that apperceptive phonagnosia might be due to disruption of the core temporal voice areas, bilaterally located in the posterior parts of the superior temporal gyrus, whereas associative phonagnosia might result from impaired access to structures where voice representations are stored, due to a disconnection of these areas from structures of the voice extended system. Although these results must be confirmed by further investigations, they represent an important step toward understanding the nature and neural substrate of apperceptive and associative forms of phonagnosia.

In his target article, Morin claims that ideographic codes are exceedingly difficult to use. In my commentary I will show that the use of Bliss symbols does not improve the communicative abilities of aphasic patients with severe language disorders. This failure to remediate communication disorders may result from disruption of inner language allowing to translate ideographic codes into spoken language.

Since the description of anosognosia for hemiplegia by Babinski (who also stressed the links between anosognosia and right hemisphere damage) both motivational and cognitive mechanisms have been advanced to explain this awareness disorder. In this review I will discuss first the neurophysiological mechanisms that can impede the discovery of the motor deficits contralateral to the brain lesion and then suggest that some instances of anosognosia for left-sided hemiplegia may also be due to motivational mechanisms of denial. Among the cognitive mechanisms, sensory feedback and intentional feed-forward disorders can lead to a poor awareness of the motor defects, whereas denial mechanisms could result from an interaction between the right hemisphere dominance for emotions and the anxiety raised by the catastrophic consequences of the brain damage. In particular, a maladaptive reaction to the personal implications of the brain lesion could be revealed by the presence of an implicit acknowledgement of the motor defect.

Acquired prosopagnosia is usually a consequence of bilateral or right hemisphere lesions and is often associated with topographical disorientation and dyschromatopsia. Left temporo-occipital lesions sometimes result in a face recognition disorder but in a context of visual object agnosia with spared familiarity feelings for faces, usually in left-handers. We describe a patient with a left temporo-occipital hemorrhagic lesion unexpectedly resulting in a deficit of face familiarity, which could represent a mild form of associative prosopagnosia. Our patient failed to feel familiarity feelings even with very well-known famous faces but had neither visual object agnosia nor defects with semantics or naming of celebrities. This was confirmed even when the patient was re-tested a year later. We speculate that a graded lateralization of face processing could be at the basis of occasional cases of prosopagnosia.

According to the original "hub-and-spoke" model of conceptual representations, the neural network for semantic memory requires a single convergence zone located in the anterior temporal lobes (ATLs). However, a more recent version of this model acknowledges that a graded specialization of the left and right ATLs might emerge as a consequence of their differential connectivity with language and sensory-motor regions. A recent influential paper maintained that both the format of semantic representations (representational account) and their differential connectivity (connectivity account) could contribute to the cognitive consequences of atrophy to the left versus the right ATL atrophy. That paper, however, also raised questions as to whether the distinction between representational and connectivity accounts is a meaningful question. I argue that an important theoretical difference exists between the representational and the connectivity-based models and that investigations, based on this difference, should allow to choose between these alternative accounts.

OBJECTIVES: Within the large topic of naming disorders, an important and separated chapter belongs to proper names. Defects of proper naming could be a selective linguistic problem. Sometimes, it includes names belonging to various kinds of semantically unique entities, but other times, it has been observed for famous people proper names only. According to Bruce and Young's model, different stages allow to recognize, identify, and name famous people from their faces and voices, subsuming different anatomical pathways, both in right temporal lobe, and their different efficiency in this task. The present study aimed to report the normative data concerning the naming of the same famous people from voice and face. SUBJECTS AND METHODS: One hundred fifty-three normal subjects underwent a test in which they were requested to name famous people from their face and from their voice. The stimuli belonged to the previously published Famous People Recognition Battery. RESULTS: The mean percentage score on naming from face was 84.42 ± 12.03% (range 55.26-100%) and the mean percentage score on naming from voice was 66.04 ± 16.81% (range 28.13-100%). The difference observed in performance by face and by voice resulted significant (t|153 = 15.973; p < 0.001). Regression analyses showed that the percentage score obtained on naming from faces was predicted by education, whereas naming from voice was predicted by education and gender. DISCUSSION: Naming from voice is more difficult than from face, confirming a different difficulty of the two tasks. Education showed high predicting value for faces and less for voices, whereas gender contributed to predict results only for voices.

Disagreement exists regarding representational and connectionist interpretations of semantic knowledge subserved by the right versus left anterior temporal lobes (ATLs). These interpretations predict a different pattern of impairment in patients with a right unilateral ATL lesion. We conducted a neuropsychological study of a selective semantic pictorial defect exhibited by a 57-year-old man who had undergone a right temporal lobectomy due to the presence of a glioblastoma. The patient was given the Thematic and Taxonomic Semantic task, in which individuals must select, within triplets of words or pictures, the best associates of living or nonliving stimuli, related by thematic or taxonomic links, and presented in the verbal or pictorial modality. The selectivity of the defect was documented by a comparison between the results obtained by our patient and those obtained by healthy controls on living items and on pictures with a thematic relation. The selectivity of the defect was confirmed by a within-subject analysis of the results obtained on all of the task's triplets and those obtained on the stimuli representing living entities with a taxonomic relation. The selectivity of this semantic pictorial defect mainly concerning living entities is consistent with the representational account of semantic defects observed in our patient. In the present case report, a right temporal lobectomy resulted in a selective semantic pictorial defect with the qualitative features predicted by the representational account of semantic defects observed after a unilateral ATL lesion.

The target article carefully describes the memory system, centered on the temporal lobe that builds specific memory traces. It does not, however, mention the laterality effects that exist within this system. This commentary briefly surveys evidence showing that clear asymmetries exist within the temporal lobe structures subserving the core system and that the right temporal structures mainly underpin face familiarity feelings.

Changes in health care and disease epidemiology have shifted the attention of neuropsychologists and cognitive neuroscientists from vascular lesions to degenerative diseases or other bilateral brain lesions. This displacement of attention from vascular patients to patients with degenerative brain diseases allowed the discovery of hitherto unexplored and unheralded aspects of the neural substrates of human cognition. Three aspects of research on the anterior parts of the temporal lobes (ATLs) are the focus of the present review. The first aspect is category-specific semantic disorders, including current accounts of categorical brain organization, the anatomical substrate of different categories (stressing the role of the ATLs with respect to the biological categories), and the "sources of knowledge" that contribute to construction of those categories. The second aspect is the role of the ATLs in conceptual knowledge, including the "hub-and-spokes" model of semantic representation and semantic control. The third aspect is the role of the right ATL in recognition of familiar people, including the distinction made between associative prosopagnosia and multimodal disorders of person recognition. Consistencies and inconsistencies of results obtained across these different domains are discussed, and the clinical implications of these findings are considered.

Earlier studies, which suggested that anosognosia of hemiplegia might be related to right hemisphere (RH) lesions did not control for the influence of confounding variables, such as aphasia, in patients with left-hemisphere lesions and unilateral neglect in those with RH lesions. These confounding variables are absent in patients with degenerative brain disease, where a prevalence of right-sided lesions is often associated with emotional and behavioural disturbances. This review, which can be considered a 'qualitative synthesis', aimed, therefore, to determine whether the unawareness phenomena observed in degenerative brain diseases are linked to the RH dominance for emotions. Results of the review confirmed that the neural correlates of anosognosia are often right lateralised in patients with degenerative brain diseases and that emotional disturbances are associated with right frontal lesions and anosognosia in the behavioural variant, i.e., frontotemporal dementia (bvFTD). However, they also showed that anosognosia is a heterogeneous phenomenon and that the role of right frontal lesions is much greater when the loss of insight concerns emotion-linked aspects of personality or behaviour than when it concerns particular aspects of cognition or memory.

Poor disease awareness ('anosognosia') is often observed in patients with various disabilities caused by brain damage. The lack of disease awareness can be due to the disruption of specific cognitive mechanisms and the development of psychodynamic mechanisms of denial. The aim of this paper is to review how these phenomena were discovered and evolved over time and to consider the relationships between them and the right hemisphere dominance for emotions. It is not clear whether the term 'anosognosia' refers to a basic mechanism that can explain similar awareness defects in different behavioural domains or whether it must be viewed as a multifaceted phenomenon in which both the disruption of cognitive or sensorimotor mechanisms and the emergence of motivational factors can play different roles in various forms of disease unawareness and in different kinds of 'anosognosic' patients.

In this normative study, we investigated famous people recognition through personal name, using as stimuli the names of the same 40 Italian famous persons whose faces and voices had been utilized for the normative study of the Famous People Recognition Battery. For each famous people, we assessed name familiarity, person identification (when the name had been considered as familiar), and false alarms. The investigation was carried out on 143 normal subjects who varied in age and education. Name familiarity and semantic scores were affected by educational level, whereas age influenced false alarms. A comparison between results obtained with names in this research and with faces and voices of the same famous people in our previous study showed that familiarity scores were higher for personal names than those for faces and voices, which obtained the worst scores. Person identification scores were not significantly different from names and from faces, but both these scores were significantly higher than the semantic scores obtained by voices. Taken together, these results are inconsistent with the influential interactive activation and competition model of person recognition.

This paper reviews some controversies concerning the original and revised versions of the 'hub-and-spoke' model of conceptual representations and their implication for abstraction capacity levels. The 'hub-and-spoke' model, which is based on data gathered in patients with semantic dementia (SD), is the most authoritative model of conceptual knowledge. Patterson et al.'s (Nature Reviews Neuroscience, 8(12), 976-987, 2007) classical version of this model maintained that conceptual representations are stored in a unitary 'amodal' format in the right and left anterior temporal lobes (ATLs), because in SD the semantic disorder cuts across modalities and categories. Several authors questioned the unitary nature of these representations. They showed that the semantic impairment is 'multi-modal'only in the advanced stages of SD, when atrophy affects the ATLs bilaterally, but that impariments can be modality-specific in lateralised (early) stages of the disease. In these cases, SD mainly affects lexical-semantic knowledge when atrophy predominates on the left side and pictorial representations when atrophy prevails on the right side. Some aspects of the model (i.e. the importance of spokes, the multimodal format of representations and the graded convergence of modalities within the ATLs), which had already been outlined by Rogers et al. (Psychological Review, 111(1), 205-235, 2004) in a computational model of SD, were strengthened by these results. The relevance of these theoretical problems and of empirical data concerning the neural substrate of concrete and abstract words is discussed critically. The conclusion of the review is that the highest levels of abstraction are due more to the structuring influence of language than to the format of representations.

In semantic tasks, sex-related categorical differences, in the form of better processing of fruits and vegetables by women and of artifacts (human-made objects) and animals by men, have been reported both in healthy participants and in brain-damaged patients. Researchers' interpretation of these sex-related categorical asymmetries has, however, been controversial, being connected with the more general (innatist versus experience-dependent) interpretations that had been given of the mechanisms subsuming the categorical organization of the brain. I begin this review with a brief reminder of the debate between supporters of the innatist and the experience-related accounts of categorical brain organization. Then I summarize results that have documented a preference by women for fruits and vegetables and a preference by men for artifacts and animals, and I discuss the innatist and social role-related interpretations that have been given of these results. I conclude that sex-related categorical effects disappear in generations in which the traditional social roles have almost completely disappeared, and these differences are not seen in young individuals raised in societies that emphasize sex equality.

Several anatomo-clinical investigations have shown that familiar face recognition disorders not due to high level perceptual defects are often observed in patients with lesions of the right anterior temporal lobe (ATL). The meaning of these findings is, however, controversial, because some authors claim that these patients show pure instances of modality-specific 'associative prosopagnosia', whereas other authors maintain that in these patients voice recognition is also impaired and that these patients have a 'multimodal person recognition disorder'. To solve the problem of the nature of famous faces recognition disorders in patients affected by right ATL lesions, it is therefore very important to verify with formal tests if these patients are or are not able to recognize others by voice, but a direct comparison between the two modalities is hindered by the fact that voice recognition is more difficult than face recognition. To circumvent this difficulty, we constructed a test battery in which subjects were requested to recognize the same persons (well-known at the national level) through their faces and voices, evaluating familiarity and identification processes. The present paper describes the 'Famous People Recognition Battery' and reports the normative data necessary to clarify the nature of person recognition disorders observed in patients affected by right ATL lesions.

The domain of emotions corresponds to the consciousness model proposed by Morsella et al. The action schemata unconsciously activated by spontaneous emotions are indeed automatically selected from a small number of innate operative patterns, whereas behavioral responses selected at the cognitive level of emotional processing consist of strategic plans, leading to a conscious integration of goal-directed actions.

AIMS: To investigate the relationship between psychotic symptoms and cognitive impairment in Alzheimer's disease (AD). METHODS: A total of 108 subjects affected by AD were subdivided into subjects without delusions (ND), subjects with paranoid delusions (PD), subjects with delusional misidentifications (DM), subjects with both DM and PD (DM+PD), subjects with visual hallucinations (v-HALL), and subjects without visual hallucinations (N-HALL). RESULTS: PD and ND subjects performed similarly on neuropsychological tests, while DM patients performed significantly worse than PD and ND patients. v-HALL patients performed worse than N-HALL patients on memory, visuospatial, and executive functions. As for behavioral features, DM and v-HALL subjects reported higher scores on the abnormal motor behavior subscale of the neuropsychiatric inventory (NPI); PD subjects reported higher scores on the disinhibition subscale of the NPI. The severity of PD was predicted by the severity of disinhibition (B = 0.514; p = 0.016) but not by neuropsychological performances. The severity of DM was predicted by age (B = 0.099; p = 0.048) and MMSE (B = -0.233; p = 0.001). The severity of v-HALL was predicted by age (B = 0.052; p = 0.037) and scores on an immediate visual memory task (B = -0.135; p = 0.007). CONCLUSIONS: The occurrence of PD may require the relative sparing of cognitive functions and be favored by frontal lobe dysfunction, while DM is associated with the overall level of cognitive impairment. Finally, v-HALL are associated with the impairment of visuospatial abilities.

An important debate exists in contemporary cognitive neuroscience about the innate or experience-dependent origin of the brain representation of conceptual categories. The 'domains of knowledge' hypothesis maintains that innate factors subsume the categorical organization at the brain level of animals, plant life and artefacts. On the other hand, the 'sensory-motor model of conceptual knowledge' and the embodied cognition theory attribute this categorical organization to experience-dependent factors. I tried to clarify this issue by surveying the influence that handedness could have on the lateralization of tools representation in the inferior fronto-parietal and posterior middle temporal cortices of the left hemisphere. The underlying assumption was that, if this lateralization results from innate mechanisms, then handedness should not influence this hemispheric asymmetry. If, on the other hand, this lateralization is due to the motor and somatosensory experiences made with the right dominant hand during the manipulation of tools and other artefacts, then this asymmetry should be inverted or strongly attenuated in left-handers. Results of the review strongly suggest that manual experience acquired during tool manipulation can influence the hemispheric representation of tools and other artefacts. They also suggest, however, that handedness-related embodiment is not fixed, but influenced by personal motor experiences (such as those made by left-handers who have been forced to use their right hand) and by social visual experiences (such as the fact that, living in a right-handed world, left-handers see more people in their environment who use the right rather than the left hand) during tool manipulation.

Since the early 1960s, human neuropsychology, the study of brain-behavior interrelations, mainly based on the analysis of their pathological variations, brought about by brain damage, has had a remarkable systematical development in Italy. All this started in Milan, with the neurologist Ennio de Renzi, and his collaborators (Luigi Vignolo, then Anna Basso, Pietro Faglioni, Hans Spinnler, François Boller, and, more autonomously, Edoardo Bisiach), in the Clinic of Nervous and Mental Diseases. Scientists of the "Milan group" investigated several neuropsychological deficits caused by focal hemispheric lesions in large series of left- and right-brain-damaged patients, and control participants, comparable for cultural and demographic variables. Standardized tests and advanced statistical methods were used, which also applied to the diagnosis and rehabilitation of aphasia. Subsequently, neuropsychology developed in Italy extensively, reaching high international reputation. Leading neuropsychologists have been the neurologists Guido Gainotti (Rome), and Franco Denes (Padua), the physicians and psychologists Luigi Pizzamiglio (Rome), and Carlo Umiltà (Parma, with fruitful interactions with the neurophysiologists Giovanni Berlucchi, Giacomo Rizzolatti, and Carlo Marzi, from the school of Giuseppe Moruzzi in Pisa) A second scientific generation of neuropsychologists has then developed in the 1970s, trained by the abovementioned scientists, further boosting and spreading high-level basic and applied research (diagnosis and rehabilitation of neuropsychological deficits of patients with brain damage or dysfunction throughout the life span, from childhood to the elderly). Available techniques include structural and functional imaging (CT, PET, SPET, MRI and fMRI Scans, DTI), electrophysiological recording (EEG, ERPs), non-invasive brain stimulation (TMS, tES), and their combined use.
