BACKGROUND: There was a need for a solid asthma guideline in Mexico to update and unify asthma management. Because high-quality asthma guidelines exist worldwide, in which the latest evidence on asthma management is summarized, the ADAPTE approach allows for the development of a national asthma guideline based on evidence from already existing guidelines, adapted to national needs. OBJECTIVE: To fuse evidence from the best asthma guidelines and adapt it to local needs with the ADAPTE approach. METHODS: The Appraisal of Guidelines for Research and Evaluation (AGREE) II asthma guidelines were evaluated by a core group to select 3 primary guidelines. For each step of asthma management, clinical questions were formulated and replied according to (1) evidence in the primary guidelines, (2) safety, (3) Cost, and (4) patient preference. The Guidelines Development Group, composed of a broad range of experts from medical specialties, primary care physicians, and methodologists, adjusted the draft questions and replies in several rounds of a Delphi process and 3 face-to-face meetings, taking into account the reality of the situation in Mexico. We present the results of the pediatric asthma treatment part. RESULTS: Selected primary guidelines are from the British Thoracic Society and Scottish Intercollegiate Guidelines Network (BTS/SIGN), Global Initiative for Asthma (GINA), and Spanish Guidelines on the Management of Asthma (GEMA) 2015, with 2016 updates. Recommendations or suggestions were made for asthma treatment in Mexico. In this article, the detailed analysis of the evidence present in the BTS/SIGN, GINA, and GEMA sections on the (non) pharmacologic treatment of pediatric asthma, education, and devices are presented for 2 age groups: children 5 years or younger and children 6 to 11 years old with asthma. CONCLUSION: For the pediatric treatment and patient education sections, applying the AGREE II and Delphi methods is useful to develop a scientifically sustained document, adjusted to the Mexican situation, as is the Mexican Guideline on Asthma.

OBJECTIVE: To assess the presence of CLDN4 in bronchoalveolar lavage fluid (BALF) and pulmonary tissue as an early indicator of LIRI and its relationship with changes in pulmonary physiology, edema formation and histology in an experimental porcine model of LTx with CIT of 50 min or 6 h. METHODS: In 12 pigs, LIRI was produced by: group I (n = 6) LTx with 50 min of CIT (LTx-50 min-CIT); and group II (n = 6) LTx with 6 h of CIT (LTx-6h-CIT). The lung function, edema formation, macroscopic and microscopic changes were assessed. CLDN4 expression in BALF and pulmonary tissue were determined. RESULTS: Both groups presented similar clinical, edema, and histological damage, as well as similar expression of CLDN4 in BALF and tissue (p > 0.05, RM-ANOVA). CONCLUSION: CLDN4 expressed in BALF and the pulmonary tissue during the first 5 h within 72 h of the PGD window are not associated by the deterioration of lung function, edema and lung histological injury, in LTx with CIT 50 min or 6 h, CLDN4 does not seem to be a valuable indicator of LIRI.

Currently, research in physiology focuses on molecular mechanisms underlying the functioning of living organisms. Reductionist strategies are used to decompose systems into their components and to measure changes of physiological variables between experimental conditions. However, how these isolated physiological variables translate into the emergence -and collapse- of biological functions of the organism as a whole is often a less tractable question. To generate a useful representation of physiology as a system, known and unknown interactions between heterogeneous physiological components must be taken into account. In this work we use a Complex Inference Networks approach to build physiological networks from biomarkers. We employ two unrelated databases to generate Spearman correlation matrices of 81 and 54 physiological variables, respectively, including endocrine, mechanic, biochemical, anthropometric, physiological, and cellular variables. From these correlation matrices we generated physiological networks by selecting a p-value threshold indicating statistically significant links. We compared the networks from both samples to show which features are robust and representative for physiology in health. We found that although network topology is sensitive to the p-value threshold, an optimal value may be defined by combining criteria of stability of topological features and network connectedness. Unsupervised community detection algorithms allowed to obtain functional clusters that correlate well with current medical knowledge. Finally, we describe the topology of the physiological networks, which lie between random and ordered structural features, and may reflect system robustness and adaptability. Modularity of physiological networks allows to explore functional clusters that are consistent even when considering different physiological variables. Altogether Complex Inference Networks from biomarkers provide an efficient implementation of a systems biology approach that is visually understandable and robust. We hypothesize that physiological networks allow to translate concepts such as homeostasis into quantifiable properties of biological systems useful for determination and quantification of health and disease.

BACKGROUND: The need for a national guideline, with a broad basis among specialists and primary care physicians was felt in Mexico, to try unifying asthma management. As several high-quality asthma guidelines exist worldwide, it was decided to select the best three for transculturation. METHODS: Following the internationally recommended methodology for guideline transculturation, ADAPTE, a literature search for asthma guidelines, published 1-1-2007 through 31-12-2015 was conducted. AGREE-II evaluations yielded 3/40 most suitable for transculturation. Their compound evidence was fused with local reality, patient preference, cost and safety considerations to draft the guideline document. Subsequently, this was adjusted by physicians from 12 national medical societies in several rounds of a Delphi process and 3 face-to-face meetings to reach the final version. RESULTS: Evidence was fused from British Thoracic Society Asthma Guideline 2014, Global Initiative on Asthma 2015, and Guía Española del Manejo del Asma 2015 (2016 updates included). After 3 Delphi-rounds we developed an evidence-based document taking into account patient characteristics, including age, treatment costs and safety and best locally available medication. CONCLUSIONS: In cooperation pulmonologists, allergists, ENT physicians, paediatricians and GPs were able to develop an evidence-based document for the prevention, diagnosis and treatment of asthma and its exacerbations in Mexico.
Antecedentes: Con el objetivo de unificar el manejo del asma en México se estructuró una guía clínica que conjunta el conocimiento de diversas especialidades y la atención en el primer nivel de contacto. Se seleccionaron 3 guías publicadas en el ámbito internacional para su transculturación. Métodos: Conforme a la metodología ADAPTE se usó AGREE II después de la búsqueda bibliográfica de guías sobre asma publicadas entre 2007 y 2015. Se fusionó la realidad local con la evidencia de 3/40 mejores guías. El documento inicial fue sometido a la revisión de representantes de 12 sociedades médicas en varias rondas Delphi hasta llegar a la versión final. Resultados: Las guías base fueron la British Thoracic Society Asthma Guideline 2014, la Global Initiative on Asthma 2015 y la Guía Española del Manejo del Asma 2015. Después de 3 rondas Delphi se desarrolló un documento en el que se consideraron las características de los pacientes según edad, costos de los tratamientos y perfiles de seguridad de los fármacos disponibles en México. Conclusión: Con la cooperación de neumólogos, alergólogos, otorrinolaringólogos, pediatras y médicos generales se llegó a un consenso basado en evidencia, en el que se incluyeron recomendaciones sobre prevención, diagnóstico y tratamiento del asma y sus crisis.

BACKGROUND: Some technologists worry that patients with very severe lung disease are unable to complete several spirometry maneuvers, which require considerable effort. METHODS: We retrospectively selected all spirometry tests with an FEV1 < 35% predicted done by adult subjects sent to our pulmonary function laboratory during a 3-y period. We determined the rates and correlates of poor quality test sessions. RESULTS: Approximately 90% of the tests done by the 558 subjects with very severe lung-function impairment (of > 30,000 subjects tested during the 3-y period) had adequate quality spirometry. Subjects with airway obstruction were less likely to meet FVC repeatability goals. A poor spirometry quality grade was associated with a very low FVC and a low body mass index, but not older age. CONCLUSIONS: Severe lung disease should not be used as an excuse for not meeting spirometry quality goals.

The use of dry gases during mechanical ventilation has been associated with the risk of serious airway complications. The goal of the present study was to quantify the plasma levels of TNF-alpha and IL-6 and to determine the radiological, hemodynamic, gasometric, and microscopic changes in lung mechanics in dogs subjected to short-term mechanical ventilation with and without humidification of the inhaled gas. The experiment was conducted for 24 hours in 10 dogs divided into two groups: Group I (nâ=â5), mechanical ventilation with dry oxygen dispensation, and Group II (nâ=â5), mechanical ventilation with oxygen dispensation using a moisture chamber. Variance analysis was used. No changes in physiological, hemodynamic, or gasometric, and radiographic constants were observed. Plasma TNF-alpha levels increased in group I, reaching a maximum 24 hours after mechanical ventilation was initiated (ANOVA pâ=â0.77). This increase was correlated to changes in mechanical ventilation. Plasma IL-6 levels decreased at 12 hours and increased again towards the end of the study (ANOVA p>0.05). Both groups exhibited a decrease in lung compliance and functional residual capacity values, but this was more pronounced in group I. Pplat increased in group I (ANOVA pâ=â0.02). Inhalation of dry gas caused histological lesions in the entire respiratory tract, including pulmonary parenchyma, to a greater extent than humidified gas. Humidification of inspired gases can attenuate damage associated with mechanical ventilation.

Resumen La exploración adecuada del tórax tiene una secuencia que ayuda al clínico a seguir varios pasos e integrar sus hallazgos en síndromes, mismos que lo llevarán con más seguridad hacia el diagnóstico más adecuado. Al cumplirse los 200 años de la invención del estetoscopio por René Laënec, tomamos esta oportunidad para recordarlo y detallar los pasos de la exploración del tórax. El hecho de contar con más herramientas tecnológicas para el diagnóstico, no le resta importancia a esta etapa tan importante de la relación entre el médico y su paciente, que es la exploración.
Abstract Thoracic exploration has a sequence of steps that help the clinician to integrate its findings in syndromes, which will lead the physician to better diagnosis. This year the stethoscope, invented by René Laënec, turns 200 years old and we use this opportunity to remember him and provide a detailed description of the thoracic exploration. To have access to more sophisticated diagnostic tools should not diminish the relevance of a direct exploration of the patient, which is a very important step in the patient-physician relationship.

Inhaled therapy has been used for over 200 years in asthma treatment. It is, to this day, considered to be the most effective and preferred therapy for asthma patients. Aerosols have therapeutic effect on the airway when the particle size (measured in mass median aerodynamic diameter MMAD) is between 1 and 5 microns. Jet and ultrasonic nebulizers are still the favorite means of administration for they have proven themselves very effective. The metered dose (MDI) and dry powder (DPI) inhalers are also frequently used in the treatment of asthma. They have been proven effective with no disadvantages when compared with nebulization. The most commonly used inhaled drugs include B2 agonists and steroids. There is no significant difference in asthma treatment when the different methods of administration are compared, however, the inhalation technique, especially with MDI with and without spacer and DPI, is directly related to the effectiveness of the drug.

Objetivo: Informar los resultados iniciales en la extracción de cuerpos extraños de la vía aérea de pacientes pediátricos mediante broncoscopía flexible. Material y métodos: Todos los pacientes menores de 18 años de edad referidos al Servicio de Broncoscopía del Instituto Nacional de Enfermedades Respiratorias con diagnóstico de aspiración de cuerpo extraño durante el periodo de marzo a diciembre de 2004. Resultados: Ocho pacientes entre 18 meses y 12 años (promedio 3 años, 5 meses), 5 del género masculino y 3 del femenino. En 4, el cuerpo extraño estaba en tráquea, 1 en bronquio intermediario, 1 en bronquio para el lóbulo inferior izquierdo y 2 en bronquio principal derecho. En 6 se logró extraer el cuerpo extraño en el primer intento con un broncoscopio flexible de 6.3 mm de diámetro, no se presentaron complicaciones mayores. En 2 pacientes, la extracción fue imposible con instrumental flexible y se realizó con broncoscopio rígido; en ambos, el cuerpo extraño estaba firmemente impactado en el bronquio después de tres meses y un año, respectivamente, de haber sido aspirado. No se presentaron complicaciones mayores. Conclusiones: Este estudio aporta evidencia útil para apoyar la broncoscopía flexible en la extracción de cuerpos extraños de las vías aéreas de pacientes pediátricos.
Objective: Report our experience in the removal of tracheobronchial foreign bodies in infants and children by flexible bronchoscopy. Patients and methods: All infants and children referred to the Department of Bronchoscopy from March to December 2004. Results: Eight patients from 18 months to 12 years, average 3 years and 5 months; 5 male, 3 female. Localization: 4 in the trachea, 1 in the intermediary bronchus, 1 in the left lower lobe bronchus and 2 in the main right bronchus. The foreign body was removed at the first attempt in 6 cases with a 6.3 mm dm flexible bronchoscope. Rigid bronchoscopy was successfully used in the other 2 after failure with the flexible instrument because the foreign bodies were heavily impacted; they had been aspirated 3 and 12 months before. There were no major complications. Conclusions: This study provides support for the removal of tracheobronchial foreign bodies in infants and children by flexible bronchoscopy.
