RESUMO

For analytical approach-based word recognition techniques, the task of segmenting the word into individual characters is a big challenge, specifically for cursive handwriting. For this, a holistic approach can be a better option, wherein the entire word is passed to an appropriate recognizer. Gurumukhi script is a complex script for which a holistic approach can be proposed for offline handwritten word recognition. In this paper, the authors propose a Convolutional Neural Network-based architecture for recognition of the Gurumukhi month names. The architecture is designed with five convolutional layers and three pooling layers. The authors also prepared a dataset of 24,000 images, each with a size of 50 × 50. The dataset was collected from 500 distinct writers of different age groups and professions. The proposed method achieved training and validation accuracies of about 97.03% and 99.50%, respectively for the proposed dataset.

RESUMO

Glomeruli are interconnected capillaries in the renal cortex that are responsible for blood filtration. Damage to these glomeruli often signifies the presence of kidney disorders like glomerulonephritis and glomerulosclerosis, which can ultimately lead to chronic kidney disease and kidney failure. The timely detection of such conditions is essential for effective treatment. This paper proposes a modified UNet model to accurately detect glomeruli in whole-slide images of kidney tissue. The UNet model was modified by changing the number of filters and feature map dimensions from the first to the last layer to enhance the model's capacity for feature extraction. Moreover, the depth of the UNet model was also improved by adding one more convolution block to both the encoder and decoder sections. The dataset used in the study comprised 20 large whole-side images. Due to their large size, the images were cropped into 512 × 512-pixel patches, resulting in a dataset comprising 50,486 images. The proposed model performed well, with 95.7% accuracy, 97.2% precision, 96.4% recall, and 96.7% F1-score. These results demonstrate the proposed model's superior performance compared to the original UNet model, the UNet model with EfficientNetb3, and the current state-of-the-art. Based on these experimental findings, it has been determined that the proposed model accurately identifies glomeruli in extracted kidney patches.

As multimedia technology is developing and growing these days, the use of an enormous number of images and its datasets is likewise expanding at a quick rate. Such datasets can be utilized for the purpose of image retrieval. This research focuses on extraction of similar images established on its different features for the image retrieval purpose from huge dataset of images. In this paper initially, the query image is searched within the available dataset and, then, the color difference histogram (CDH) descriptor is employed to retrieve the images from database. The basic characteristic of CDH is that it counts the color difference stuck among two distinct labels in the L ∗ a ∗ b ∗ color space. This method is experimented on random images used for various medical purposes. Various unlike features of an image are extracted via different distance methods. The precision rate, recall rate, and F-measure are all used to evaluate the system's performance. Comparative analysis in terms of F-measure is also made to check for the best distance method used for retrieval of images.

Small changes in retinal blood vessels may produce different pathological disorders which may further cause blindness. Therefore, accurate extraction of vasculature map of retinal fundus image has become a challenging task for analysis of different pathologies. The present study offers an unsupervised method for extraction of vasculature map from retinal fundus images. This paper presents the methodology for evolution of vessels using Modified Pixel Level Snake (MPLS) algorithm based on Black Top-Hat (BTH) transformation. In the proposed method, initially bimodal masking is used for extraction of the mask of the retinal fundus image. Then adaptive segmentation and global thresholding is applied on masked image to find the initial contour image. Finally, MPLS is used for evolution of contour in all four cardinal directions using external, internal and balloon potential. This proposed work is implemented using MATLAB software. DRIVE and STARE databases are used for checking the performance of the system. In the proposed work, various performance metrics such as sensitivity, specificity and accuracy are evaluated. The average sensitivity of 76.96%, average specificity of 98.34% and average accuracy of 96.30% is achieved for DRIVE database. This technique can also segment vessels of pathological images accurately; reaching the average sensitivity of 70.80%, average specificity of 96.40% and average accuracy of 94.41%. The present study provides a simple and accurate method for the detection of vasculature map for normal fundus images as well as pathological images. It can be helpful for the assessment of various retinal vascular attributes like length, diameter, width, tortuosity and branching angle.

RESUMO

Granulicatella adiacens is a nutritionally variant streptococcus species. These bacteria are rarely isolated in the laboratory due to their fastidious growth requirements. These have been mostly reported from bloodstream infections, infective endocarditis, infections of orbit, nasolacrimal duct and breast implants. Here, we are reporting two cases of subcutaneous abscesses caused by G. adiacens. In first case, it was isolated from abscess around elbow joint and second case was a suprapatellar abscess. We have also reviewed the published data concerning diagnosis and antimicrobial susceptibility pattern of Granulicatella infections and included some Indian cases.

The choice of implant length is an essential factor in deciding the survival rates of these implants and the overall success of the prosthesis. Placing an implant in the posterior part of the maxilla and mandible has always been very critical due to poor bone quality and quantity. Long implants can be placed in association with complex surgical procedures such as sinus lift and bone augmentation. These techniques are associated with higher cost, increased treatment time and greater morbidity. Hence, there is need for a less invasive treatment option in areas of poor bone quantity and quality. Data related to survival rates of short implants, their design and prosthetic considerations has been compiled and structured in this manuscript with emphasis on the indications, advantages of short implants and critical biomechanical factors to be taken into consideration when choosing to place them. Studies have shown that comparable success rates can be achieved with short implants as those with long implants by decreasing the lateral forces to the prosthesis, eliminating cantilevers, increasing implant surface area and improving implant to abutment connection. Short implants can be considered as an effective treatment alternative in resorbed ridges. Short implants can be considered as a viable treatment option in atrophic ridge cases in order to avoid complex surgical procedures required to place long implants. With improvement in the implant surface geometry and surface texture, there is an increase in the bone implant contact area which provides a good primary stability during osseo-integration.

The routine dental practice involves various dental procedures which needs the application of local anesthetics. Generally, there are very few complications associated with these procedures. Complications such as tissue necrosis can occur following the rapid injection of local anesthetic solutions. Palate is a favorable site for soft tissue lesions, various factors such as direct effects of the drug, blanching of the tissues during injection, a relatively poor blood supply, and reactivation of the latent forms of herpes can all promote to tissue ischemia and a lesion in the palate.

Aims. This study was aimed at knowing the prevalence of vancomycin and high level aminoglycoside resistance in enterococcal strains among clinical samples. Study Design. It was an investigational study. Place and Duration of Study. It was conducted on 100 Enterococcus isolates, in the Department of Microbiology, Pt. BDS PGIMS, Rohtak, over a period of six months from July to December 2014. Methodology. Clinical specimens including urine, pus, blood, semen, vaginal swab, and throat swab were processed and Enterococcus isolates were identified by standard protocols. Antibiotic sensitivity testing of enterococci was performed using Kirby-Bauer disc diffusion method. Results. High level gentamicin resistance (HLGR) was more common in urine samples (41.5%) followed by blood (36%) samples. High level streptomycin resistance (HLSR) was more common in pus samples (52.6%) followed by blood samples (36%). Resistance to vancomycin was maximum in blood isolates. Conclusion. Enterococci resistant to multiple antimicrobial agents have been recognized. Thus, it is crucial for laboratories to provide accurate antimicrobial resistance patterns for enterococci so that effective therapy and infection control measures can be initiated.

Tuberculosis is a chronic granulomatous systemic infectious disease caused by Mycobacterium tuberculosis. The oral lesions found in tuberculosis are relatively rare and may present as ulcers, erythematous patches, indurated lesions, nodules or as bony jaw lesions. Oral tubercular lesions sometimes present a confusing clinical presentation and can be overlooked. Hence, we document a case of tuberculous osteomyelitis of the maxilla in a 19-year-old female patient, who was initially treated for multiple periodontal dental abscesses, which later proved to be tubercular osteomyelitis of the maxilla. Although it is a rare occurrence, the differential diagnosis of tuberculous osteomyelitis must always be considered when it fails to respond to routine therapy.
