BACKGROUND: Neuronal ceroid lipofuscinoses, (NCLs or Batten disease) are a group of inherited, early onset, fatal neurodegenerative diseases associated with mutations in 13 genes. All forms of the disease are characterized by lysosomal accumulation of fluorescent storage material, as well as profound neurodegeneration, but the relationship of the various genes' function to a single biological process is not obvious. In this study, we used a well-characterized mouse model of classical late infantile NCL (cLINCL) in which the tripeptidyl peptidase 1 (Tpp1) gene is disrupted by gene targeting, resulting in loss of detectable TPP1 activity and leading to progressive neurological phenotypes including ataxia, increased motor deficiency, and early death. METHODS: In order to identify genes and pathways that may contribute to progression of the neurodegenerative process, we analyzed forebrain/midbrain and cerebellar transcriptional differences at 1, 2, 3 and 4 months of age in control and TPP1-deficient mice by global RNA-sequencing. RESULTS: Progressive neurodegenerative inflammatory responses involving microglia, astrocytes and endothelial cells were observed, accompanied by activation of leukocyte extravasation signals and upregulation of nitric oxide production and reactive oxygen species. Several astrocytic (i.e., Gfap, C4b, Osmr, Serpina3n) and microglial (i.e., Ctss, Itgb2, Itgax, Lyz2) genes were identified as strong markers for assessing disease progression as they showed increased levels of expression in vivo over time. Furthermore, transient increased expression of choroid plexus genes was observed at 2 months in the lateral and fourth ventricle, highlighting an early role for the choroid plexus and cerebrospinal fluid in the disease pathology. Based on these gene expression changes, we concluded that neuroinflammation starts, for the most part, after 2 months in the Tpp1-/- brain and that activation of microglia and astrocytes occur more rapidly in cerebellum than in the rest of the brain; confirming increased severity of inflammation in this region. CONCLUSIONS: These findings have led to a better understanding of cLINCL pathological onset and progression, which may aid in development of future therapeutic treatments for this disease.

BACKGROUND: ProSeal (PLMA) and Supreme (SLMA) laryngeal mask airways are effective ventilator devices with distinctive designs that may require different anaesthetics for insertion. Sevoflurane induction provides acceptable conditions for laryngeal mask insertion, and remifentanil significantly decreases the minimum alveolar concentration of sevoflurane required for that insertion. The study aimed to evaluate the optimal end-tidal (ET) sevoflurane concentration for successful insertion of PLMA versus SLMA in patients receiving a remifentanil infusion without a neuromuscular blocking agent. METHODS: Altogether, 45 patients ASA (American Society Anaesthesiologists) physical status I-II, aged 18-60 years were scheduled for elective ambulatory surgery. Exclusion criteria were a difficult airway, recent respiratory infection, reactive airway, obstructive sleep apnoea syndrome, gastric aspiration's risk factors, pregnancy, and lactation. Patients were randomly allocated to receive the SLMA or the PLMA. Sevoflurane induction with co-administration of remifentanil was performed at an effect-site concentration of 4 ng mL- 1. ET50 was calculated with a modified Dixon's up-and-down method (starting at 2.5% in steps of 0.5%). Predetermined sevoflurane concentration was kept constant during the 10 min before LMA insertion. Patient's response to LMA insertion was classified as "movement" or "no movement". Sevoflurane ET50 was determined as the midpoint concentration of all the independent pairs that manifested crossover from "movement" to "no movement". RESULTS: The ET50 sevoflurane concentration co-administered with remifentanil required for PLMA insertion was 1.20 ± 0.41% (95% confidence interval 0.76 to 1.63%). For SLMA insertion, it was 0.55 ± 0.38% (95% confidence interval 0.14 to 0.95%) (p = 0.019). CONCLUSIONS: The end-tidal sevoflurane concentration with co-administered remifentanil required to allow insertion of the SLMA was 54% lower than that needed for inserting the PLMA. BACKGROUND: Postzygotic de novo mutations lead to the phenomenon of gene mosaicism. The 3 main types are called somatic, gonadal, and gonosomal mosaicism, which differ in terms of the body distribution of postzygotic mutations. Mosaicism has been reported occasionally in patients with primary immunodeficiency diseases (PIDs) since the early 1990s, but its real involvement has not been systematically addressed. OBJECTIVE: We sought to investigate the incidence of gene mosaicism in patients with PIDs. METHODS: The amplicon-based deep sequencing method was used in the 3 parts of the study that establish (1) the allele frequency of germline variants (n = 100), (2) the incidence of parental gonosomal mosaicism in families with PIDs with de novo mutations (n = 92), and (3) the incidence of mosaicism in families with PIDs with moderate-to-high suspicion of gene mosaicism (n = 36). Additional investigations evaluated body distribution of postzygotic mutations, their stability over time, and their characteristics. RESULTS: The range of allele frequency (44.1% to 55.6%) was established for germline variants. Those with minor allele frequencies of less than 44.1% were assumed to be postzygotic. Mosaicism was detected in 30 (23.4%) of 128 families with PIDs, with a variable minor allele frequency (0.8% to 40.5%). Parental gonosomal mosaicism was detected in 6 (6.5%) of 92 families with de novo mutations, and a high incidence of mosaicism (63.9%) was detected among families with moderate-to-high suspicion of gene mosaicism. In most analyzed cases mosaicism was found to be both uniformly distributed and stable over time. CONCLUSION: This study represents the largest performed to date to investigate mosaicism in patients with PIDs, revealing that it affects approximately 25% of enrolled families. Our results might have serious consequences regarding treatment and genetic counseling and reinforce the use of next-generation sequencing-based methods in the routine analyses of PIDs.

OBJECTIVE: The purpose of this qualitative study was to examine the experiences of certified peer specialists (CPSs) in delivering telehealth services during the COVID-19 pandemic. METHODS: Fourteen semistructured interviews with CPSs were conducted from August to November 2020. Data were analyzed by using thematic analysis. RESULTS: Helpful support from employers during the transition to telehealth included clear communication and access to training on telehealth technologies. Main barriers and facilitators were related to interpersonal relationships and logistical factors. CPSs reported that telehealth allowed them to continue to support the peers they serve but that the quality of interactions was not as good as with in-person services. Logistical challenges included inadequate equipment and Internet access. CONCLUSIONS: To support CPSs in delivering telehealth, employers can engage them in open and transparent communication about guidelines for service delivery, provide training and equipment, and allow them the flexibility to tailor service delivery modalities to the peers they serve.

Aim: Faces pain scales are widely used to measure pain. So far, no faces pain scale has ever been constructed by Rasch modeling. Hence the authors aimed to construct a new scale by this method. Methods: Rasch modeling was used to provide an initial calibration and development of the 'Balparda-Herrera Pain Scale' (BHPS) and this scale was compared with the existing Faces Pain Scale - Revised. The scale was later refined. Results: Both the existing scale and the initial version of the BHPS required category collapsing. Statistical tests demonstrated an excellent concordance between both scales. The final version of the BHPS was found to behave excellently and to be capable of adequately measuring pain. Conclusion: The BHPS provides an excellent instrument for measuring pain in the adult population.


Lay abstract Pain is an unpleasant experience for patients, and it is useful for physicians to try to measure how much pain the patient is experiencing. Some scales used by physicians include faces with different expressions to represent various amounts of pain. Rasch modeling is a mathematical approach to develop better scales. In this study, the researchers performed two surveys in order to develop and improve a new scale for measuring pain: the Balparda­Herrera Pain Scale. They used mathematical and statistical approaches to optimize the scale and to compare with another well-known scale. The authors found that the new scale is very useful for measuring pain.

Since 1983, several recombinant antibodies have been expressed in important agronomic plant species. However, to date no evaluation has been published about prolonged antibody stability within plant tissues under cryo-preservation conditions. This current report presents an approach to the KDEL-plantibody HB-01 (PHB-01) stability in frozen tobacco leaves by presenting scientific evidence about the stability of a plantibody to a prolonged low temperature exposure in this biological source. Results clearly show that the PHB-01 amount is maintained during the storage of tobacco leaves at -20 degrees C for 90 days. The PHB-01 recovery was not affected by any irreversible physical and/or chemical change produced in tobacco leaves after this cryo-preservation time. The amount of total soluble proteins in the clarified extract decreased in proportion with the storage time and the PHB-01 molecules isolated from frozen leaf extracts were highly pure, >95%, according to an SDS-PAGE assessment under reducing conditions. Low temperature exposure of tobacco leaves did not reveal visible changes in frozen leaves, which is essential for the further extractability of proteins. The PHB-01 is stable in tobacco leaves at -20 degrees C during 90 days, which offers the possibility to overcome problems associated with detrimental climate conditions and optimize purification capabilities.

Full house nephropathy is defined as the simultaneous detection of IgA, IgG, IgM, C3, and C1q deposits by immunofluorescence, usually indicating lupus nephritis. There are patients with this immunofluorescence pattern, but with negative autoantibody serology, which means they cannot be diagnosed with systemic lupus erythematosus. Patients presenting with full house nephropathy but no other criteria for lupus are diagnosed as having nonlupus full house nephropathy. Here, we describe two cases: A male patient who debuted with rapidly progressive glomerulonephritis and a female patient with nephrotic syndrome. Both had negative autoantibody serology, findings in the renal biopsy of class IV lupus nephritis and afull house immunofluorescence pattern. Histological findings in non-lupus full house nephropathy are similar to those in lupus nephritis and, probably, similar physiopathological bases. However, prospective studies are needed to determine risk factors and the renal prognosis and to make suggestions for specific treatments.


La nefropatía full house se refiere a la detección simultánea de depósitos de IgA, IgG, IgM, C3 y C1q en la inmunofluorescencia, lo que generalmente indica la presencia de nefritis lúpica. Hay pacientes con este patrón de inmunofluorescencia, pero con serología negativa para autoanticuerpos, por lo que no se les puede diagnosticar un lupus eritematoso sistémico. Este tipo de nefropatía, en la que no se presentan otros criterios para lupus, se denomina nefropatía full house no lúpica. En esta presentación, se describen dos casos: un paciente que ingresó con una glomerulonefritis rápidamente progresiva y una paciente con síndrome nefrótico, ambos con serología negativa para autoanticuerpos, hallazgos en la biopsia renal indicativos de nefritis lúpica de clase IV y un patrón full house en la inmunofluorescencia. La nefropatía full house no lúpica tiene rasgos histológicos similares a los de la nefritis lúpica y, probablemente, sus bases fisiopatológicas son parecidas. Sin embargo, se necesitan estudios prospectivos para conocer los factores de riesgo y el pronóstico renal, y poder hacer sugerencias sobre tratamientos específicos.

La enfermedad inflamatoria intestinal es un problema de salud que actualmente genera alta morbilidad en el paciente pediátrico. Existen múltiples métodos diagnósticos que permiten su evaluación, entre los cuales y de creciente uso, está la ecografía digestiva. El objetivo de la presente investigación fue mostrar una panorámica actualizada de la utilidad e idoneidad de esta técnica en el seguimiento de la enfermedad inflamatoria intestinal en pacientes pediátricos. Se hizo una recopilación y revisión de la literatura nacional e internacional de mayor novedad y profundidad en el tratamiento del tema, en un período de 4 meses (febrero de 2022 a mayo de 2022). La estrategia de búsqueda adoptada fue el uso de las palabras clave o descriptores en español e inglés como: enfermedad inflamatoria intestinal, pediatría, enfermedad de Crohn, colitis ulcerosa, ultrasonido, ecografía digestiva. En total se utilizaron 50 artículos, de diferentes bases de datos como:Scielo, PubMed/Medline, Ebesco, Clinical Key y Springer. Se escogieron 30 citas para realizar la revisión, donde se analizó la importancia de la técnica en la descripción de las características ecográficas intestinales en pacientes pediátricos, confirmándose el papel que tiene en la identificación e intervención temprana de la enfermedad inflamatoria intestinal.


Inflammatory bowel disease is a health problem that currently causes high morbidity in pediatric patients. There are several diagnostic methods that allow its evaluation, among which gastrointestinal ultrasound is increasingly used. The aim of this research was to provide an updated review of the usefulness and suitability of this technique in the follow-up of inflammatory bowel disease in pediatric patients. A compilation and review of the national and international literature of greater novelty and depth in the treatment of the subject was made over a period of 4 months (February 2022 to May 2022). The search strategy adopted was the use of keywords or descriptors in Spanish and English such as: inflammatory bowel disease; pediatrics; Crohn's disease; ulcerative colitis; ultrasound; digestive ultrasound. A total of 50 articles were used from different databases such as: Scielo, PubMed/Medline, Ebesco, Clinical Key and Springer. Thirty citations were selected for review, analyzing the importance of the technique in the description of intestinal ultrasound characteristics in pediatric patients, confirming its role in the identification and early intervention of inflammatory bowel disease.


A doença inflamatória intestinal é um problema de saúde que atualmente causa elevada morbilidade no doente pediátrico. Existem vários métodos de diagnóstico que permitem a sua avaliação, entre os quais a ecografia gastrointestinal é cada vez mais utilizada. O objetivo deste trabalho foi fazer uma revisão actualizada da utilidade e adequação desta técnica no seguimento da doença inflamatória intestinal em idade pediátrica. Foi feita uma compilação e revisão da literatura nacional e internacional de maior novidade e profundidade no tratamento do tema, durante um período de 4 meses (fevereiro de 2022 a maio de 2022). A estratégia de busca adotada foi o uso de palavras-chave ou descritores em espanhol e inglês, tais como: doença inflamatória intestinal; pediatria; doença de Crohn; colite ulcerativa; ultrassom; ultrassom digestivo. Foram utilizados 50 artigos de diferentes bases de dados, como: Scielo, PubMed/Medline, Ebesco, Clinical Key e Springer. Foram selecionadas 30 citações para revisão, analisando a importância da técnica na descrição das características ultra-sonográficas intestinais em pacientes pediátricos, confirmando seu papel na identificação e intervenção precoce da doença inflamatória intestinal.

Overt polyautoimmunity (PolyA) corresponds to the presence of more than one well-defined autoimmune disease (AD) manifested clinically in a single patient. The current study aimed to describe the main characteristics of juvenile PolyA in a pediatric rheumatology setting and analyze the chronological aspects, index cases, familial autoimmunity, and clustering pattern. This was a cross-sectional and multicenter study in which 313 children with overt PolyA were included. Patients were systematically interviewed and their medical records reviewed using a questionnaire that sought information about demographic, clinical, immunological, and familial characteristics. A hierarchical cluster analysis was done to determine similarities between autoimmune diseases based on PolyA. PolyA occurred simultaneously in 138 (44%) patients. Multiple autoimmune syndrome was observed in 62 (19.8%) patients. There were 25 index diseases of which, systemic lupus erythematosus (SLE, n = 134, 42.8%), juvenile idiopathic arthritis (JIA, n = 40, 12.7%), Hashimoto's thyroiditis (HT, n = 24, 7.66%), immune thrombocytopenic purpura (ITP n = 20, 6.39%), antiphospholipid syndrome (APS, n = 15, 4.79%), and vitiligo (VIT, n = 15, 4.79%) were the most frequent and represented 79.23% of the total number of patients. Familial autoimmunity influenced PolyA. A high aggregation of autoimmunity was observed (λr = 3.5). Three main clusters were identified, of which SLE and APS were the most similar pair of diseases (based on the Jaccard index) followed by HT and JIA, which were related to ITP and Sjögren's syndrome. The third cluster was composed of localized scleroderma and VIT. Our findings may assist physicians to make an early diagnosis of this frequent condition. Pediatric patients with ADs should be systematically assessed for PolyA.

BACKGROUND: Juvenile Idiopathic Arthritis (JIA) is the most common inflammatory joint disease in children. JIA and autoimmune inflammatory Gastrointestinal (GI) diseases share common etiologic mechanisms, including genetic predisposition and environmental influences. OBJECTIVE: To Investigate association between gastrointestinal, rheumatologic clinical variables and the presence of autoantibodies in patients with JIA in treatment. METHODOLOGY: In a cross-sectional study of patients with JIA according to diagnostic criteria and the ILAR classification. GI symptoms and autoantibody expression were evaluated with respect to their association with JIA clinical variables. Anti-Saccharomyces Cerevisiae IgG/IgA (ASCA), 6 antigen associated with anti polymorphonuclear neutrophil (ANCA), anti Transglutaminase (tTG) IgG/IgA, anti deaminated gliadin peptide (DGP) IgG/IgA autoantibodies, ANAS and IgA were measured in all patients. The association between clinical variables and auto-antibodies were evaluated using the Fisher test with significant value of p <0.05. The study was approved by the ethics committee of the all institutions. RESULTS: Samples were collected from ninety-seven patients, 63% of whom were female. The average age was 14 years. The JIA subtype associated with the most common GI symptoms was enthesitis- related arthritis. Of these patients, 44.3% and 14% reported abdominal pain and diarrhea, respectively. Anti-DPG and anti-tTG antibodies were found in 9.28% and 7.22%, respectively and 11.34% were positive for p-ANCA, and 2% were positive for ASCA. CONCLUSION: GI symptoms and autoantibodies associated with inflammation of the GI mucosa were detected in JIA patients but were not associated with autoantibodies or clinical variables. However, it is the monitoring of these patients diagnosis is important.

Fundamento: el nivel académico superior en la atención primaria de salud tiene importante repercusión en la formación de los futuros profesionales, por lo que es necesario atender las necesidades de superación pedagógica con vistas a perfeccionar la competencia y desempeño en esta tarea. Objetivo: diagnosticar las necesidades de superación pedagógica en médicos de la atención primaria de salud pertenecientes al Policlínico Universitario "Juana Naranjo León" de Sancti Spíritus, Cuba. Métodos: se realizó un estudio observacional, descriptivo, de corte transversal. Su contexto fueron los escenarios docentes asistenciales del Policlínico Universitario "Juana Naranjo León" de Sancti Spíritus, en el período comprendido de 2020-2021. Para fundamentar la investigación se utilizaron métodos teóricos (análisis-síntesis e inducción-deducción) y empíricos (revisión documental y cuestionario). Se emplearon procedimientos de estadística descriptiva (frecuencia absoluta y relativa), para el análisis e interpretación de los resultados. Resultados: se evidenció limitada preparación docente e insuficiente desempeño en la formación educativa para garantizar la calidad del proceso enseñanza aprendizaje, a partir de la baja categorización y el escaso número de actividades e investigaciones de corte académico. Conclusiones: el estudio realizado permitió conocer el estado actual de los conocimientos y preparación docente-metodológica del claustro de la institución en el primer nivel de atención médica. Estos profesionales requieren atención prioritaria para el desempeño formativo, y alcanzar con ello la excelencia en el proceso docente-educativo.


Background: the higher academic level in primary health care has an important impact on the training of future professionals, so it is necessary to meet the needs of pedagogical improvement with a view to improving competence and performance in this task. Objective: to diagnose the needs for pedagogical improvement in PHC doctors belonging to the "Juana Naranjo León" University Polyclinic in Sancti Spíritus, Cuba. Methods: an observational, descriptive, cross-sectional study was carried out. Its context was the teaching assistance scenarios of the "Juana Naranjo León" University Polyclinic in Sancti Spíritus, from 2020 to 2021. Theoretical methods were used to support the research: (analysis-synthesis and induction-deduction) and empirical ones (documentary review and questionnaire). Descriptive statistical procedures (absolute and relative frequency) were used for the analysis and interpretation of the results. Results: limited teaching preparation and insufficient performance in educational training were evidenced to guarantee the quality of the teaching-learning process, taking into account the low categorization and the small number of activities and academic research. Conclusions: the study allowed us to know the current state of knowledge and teaching-methodological preparation of the teaching staff of the institution at the first level of medical care, which require priority attention for training performance, and thereby achieve excellence in the teaching-learning process.

Both healthy ageing and rheumatoid arthritis (RA) are frequently associated with acquired steroid resistance. Here, we investigated the potential involvement of steroid resistance with multidrug resistance (MDR) and explored the impact of pathological ageing on lymphocyte sensitivity to glucocorticoids. Seventy-four RA patients and 26 healthy controls took part in this study. Peripheral blood mononuclear cells were isolated and T-cell sensitivity to glucocorticoids was measured in vitro. The functional activity of P-glycoprotein was analyzed by flow cytometry and ABCB1/MDR-1 gene polymorphisms were assessed in peripheral lymphocytes. Patients and controls had similar sensitivities to glucocorticoids. Only controls presented age-related immunological changes, including reduced T-cell proliferation and relative resistance to corticosterone. Patients had a higher percentage (72%) of lymphocytes actively extruding rhodamine 123 (Rh123(dim)) than controls (60%) in spite of similar P-glycoprotein activity. A higher percentage of Rh123(dim)+ lymphocytes was observed in patients who were more resistant to dexamethasone in vitro. The distribution of ABCB1 genotypes in RA patients did not differ significantly from that in controls and were not associated to steroid sensitiveness or disease activity. These data suggest that peripheral lymphocytes of arthritic patients are fully responsive to GCs in vitro in spite of displaying higher MDR activity.

RESUMEN La especialidad de Imagenología comprende áreas del conocimiento médico que utilizan diferentes tecnologías en creciente desarrollo y métodos que exteriorizan partes del cuerpo humano, por tal motivo, educar a través del diagnóstico por imágenes informatizadas constituye un desafío en la actualidad. El presente artículo plantea describir las características principales de los medios de enseñanza y el interés que brinda especialmente la imagen radiológica digital como recurso didáctico. Se recopiló y revisó bibliografía de mayor novedad y profundidad en el tratamiento del tema. La estrategia de búsqueda adoptada fue la utilización de las palabras clave o descriptores en español e inglés. En la Universidad de Ciencias Médicas las tecnologías modernas traen diversidad de herramientas didácticas en el marco de escenarios docentes que impulsan y motivan el desarrollo rápido de habilidades perceptivas, la utilización cada vez más frecuente de imágenes digitales como medio de enseñanza la convierten en un recurso muy utilizado actualmente que exige habilidades y manejo eficaz. Las imágenes digitales con fines docentes son una alternativa importante, pues reflejan la modernidad e incorporan el conocimiento, debido a que se transforman en un nuevo recurso destinado a comunicar.


ABSTRACT The specialty of Imaging includes areas of medical knowledge that use different technologies in growing development and methods that externalize parts of the human body, for this reason, educating through computerized imaging diagnosis is a challenge today. This article proposes to describe the main characteristics of the teaching aids and the interest that the digital radiological image offers especially as a didactic resource. Bibliography of greater novelty and depth in the treatment of the subject was collected and reviewed. The search strategy adopted was the use of keywords or descriptors in Spanish and English. In the University of Medical Sciences, modern technologies bring diversity of didactic tools within the framework of teaching scenarios that promote and motivate the rapid development of perceptual skills, the increasingly frequent use of digital images as a means of teaching make it a very useful resource. Currently used that requires skills and effective handling. Digital images for teaching purposes are an important alternative, as they reflect modernity and incorporate knowledge, because they become a new resource for communicating.

Resumen: Introducción: la diabetes es un importante factor de riesgo cardiovascular; sin embargo, los análisis de la dinámica cardíaca no son claros al evaluar cuantitativamente el impacto de esta enfermedad. Objetivo: determinar la aplicabilidad clínica de una metodología basada en una ley caótica exponencial para analizar la dinámica cardíaca de pacientes diabéticos. Método: se analizaron registros Holter de 30 pacientes: 10 con antecedentes de diabetes, 10 con antecedente de enfermedades cardiovasculares y 10 normales. Posteriormente, se construyeron atractores con los valores de frecuencia cardíaca y se calcularon los espacios de ocupación para cada caso. Se realizaron cálculos de rendimiento y concordancia diagnóstica. Resultados: los registros normales tuvieron espacios de ocupación mayores a 200 en la rejilla Kp; los registros de personas diabéticas oscilaron entre normalidad y enfermedad aguda cardíaca. Se obtuvo sensibilidad y especificidad del 100% Conclusión: esta metodología detecta alteraciones crónicas y agudas en pacientes con diabetes mellitus tipo 2.


Abstract: Introduction: an important cardiovascular risk factor is diabetes; however, the analysis of cardiac dynamics is not clear when evaluating quantitatively the impact of this disease. Objective: to determine the clinical applicability of a methodology based on an exponential chaotic law to analyze the cardiac dynamics of diabetic patients. Method: 10 Holter records were taken from patients with type 2 diabetes mellitus, 10 from patients with cardiovascular diseases and 10 normal. Afterwards, attractors were constructed with the heart rate values and the occupation spaces were calculated for each case. Sensitivity and specificity calculations and diagnostic agreement were performed. Results: the normal records had occupation spaces in the Kp grid greater than 200; records of people with diabetes ranged from normal to acute heart disease. Sensitivity and specificity of 100% were obtained. Conclusion: this methodology detects chronic and acute alterations in patients with type 2 diabetes mellitus.

Introducción: El carcinoma basocelular es una neoplasia maligna derivada de las células epidérmicas de los folículos pilosos o células no queratinizadas puede ser desfigurante y originar deformidades o pérdida de la función del órgano afectado, es más frecuente en zonas sobresalientes como la nariz. Objetivo: Describir los resultados de la aplicación de la combinación sinérgica de interferones alpha-2b y gamma en una serie de casos con carcinoma basocelular nasal. Material y métodos: Se realizó un estudio de serie de casos clínicos, se incluyeron 28 pacientes con diagnóstico clínico, dermatoscópico e histopatológico de carcinoma basocelular de la nariz que acudieron a consulta de dermatología del Policlínico Centro de Sancti Spíritus en el período de 17 de enero de 2016 a 28 de diciembre de 2021. Se realizó una evaluación inicial y otra final a las 16 semanas después del comienzo del tratamiento. Se administró 10,5 UI de HeberFERON, 3 veces por semana perilesional e intradérmica hasta completar 9 dosis. Las variables principales fueron la respuesta al tratamiento y presencia o no de evento adversos. Resultados: Predominó el sexo masculino, fototipocutáneo II, localización en el dorso y ala nasal, subtipo clínico nódulo ulcerativo y el histológico sólido. En la mayoría de los pacientes desapreció el tumor al culminar el esquema terapéutico. Como eventos adversos más comunes se presentaron dolor en el sitio de inyección, fiebre, edema y eritema perilesional. Conclusiones: La respuesta al tratamiento fue completa en la mayoría de los pacientes y los eventos adversos los descritos en la literatura sin cambios en la actitud farmacológica(AU)


Introduction: Basal cell carcinoma is a malignant neoplasm derived from epidermal cells of hair follicles or non-keratinized cells with a high potential for local destruction, which can be disfiguring and may invade the surrounding tissue, resulting in deformities or loss of function of the affected organ, being more frequent in protruding areas such as the nose. Objective: To describe the results of the application of the synergistic combination of interferon alpha-2b and gamma in a series of cases with nasal basal cell carcinoma. Material and Methods: An observational, descriptive and longitudinal study was carried out in a series of 28 clinical cases with basal cell carcinoma of the nose who attended the dermatology clinic of Policlínico Centro in Sancti Spiritus in the period from january 2016 to december 2021. Cases with a clinical, dermoscopic and histopathological diagnoses were included. An initial and a final evaluation at the end of the 16 weeks of treatment were carried out; also, 10,5 IU of HeberFERON was administered 3 times per week perilesional and intradermally until completing 9 doses. The main variables were response to treatment and presence or absence of adverse events. Results: Male sex, skin phototype II, location on the dorsum and nasal ala, clinical subtype ulcerative nodule, and solid histological subtype predominated. In most of the patients, the tumor disappeared at the end of the therapeutic regime. The most common adverse events were pain at the injection site, fever, edema and perilesional erythema. Conclusions: There was a complete response to treatment in most patients and the adverse events were those described in the literature with no change in the pharmacological attitude(AU)

Introducción: La creciente necesidad de una piel de aspecto juvenil impulsa innovaciones continuas con procedimientos mínimamente invasivos. El plasma rico en plaquetas autólogo representa una terapéutica regenerativa incluida en el novedoso arsenal de intervenciones que buscan este efecto. Objetivo: Evaluar los resultados de la terapéutica con plasma rico en plaquetas autólogo en pacientes con envejecimiento facial. Métodos: La muestra estuvo constituida por 68 pacientes valorados con la escala para valoración clínica de fotoenvejecimiento cutáneo al inicio del tratamiento. Se sometieron a cuatro sesiones de plasma rico en plaquetas cada 15 días y una sesión adicional a los 3 meses de concluir el tratamiento inicial. Los pacientes fueron seguidos durante 6 meses al cabo de los cuales se volvieron a evaluar con el mismo instrumento. Resultados: Las edades estuvieron comprendidas entre 21 y 73 años con una media de 46,80 años, predominó el sexo femenino (89,9 %). Los resultados significativos en el tratamiento de las arrugas, los surcos, la textura de la piel y las lesiones cutáneas estuvieron relacionados con la mesoterapia con plasma rico en plaquetas. El procedimiento produjo una mejoría valorada a través de la escala para valoración clínica de fotoenvejecimiento cutáneo. Conclusiones: La terapia con plasma rico en plaquetas tiene buenos resultados en el tratamiento de pacientes con envejecimiento facial.


Introduction: The growing need for youthful looking skin drives continued innovations with minimally invasive procedures. Autologous platelet-rich plasma represents a regenerative therapeutic included in the new arsenal of interventions that seek this effect. Objective: Assess the results of platelet-rich plasma therapy in patients with facial aging. Methods: The sample consisted of 68 patients assessed with the scale for the clinical assessment of cutaneous photoaging (SCACPH). They underwent four PRP sessions every 15 days and an additional session 3 months after completing the initial treatment. The patients were followed up for 6 months, after which they were reevaluated which the same instrument. Results: The ages of the patients were between 21 and 73 years with a mean of 46.80 years, the female sex predominated (89.9%). Significant results in the treatment of wrinkles, furrows, and skin texture and skin lesions were related to PRP mesotherapy. The procedure produced an improvement assessed with the SCACPH. Conclusions: Autologous PRP therapy has good results in the treatment of patients with facial aging.

Introducción: En Colombia la hemorragia postparto es la segunda causa de mortalidad en mujeres gestantes de 24 a 34 años con 6,9 casos por cada 1000 nacidos vivos. Después del parto se prevé que el 8.2% de las mujeres latinoamericanas presentarán hemorragia postparto. Objetivo: Describir el cuidado de enfermería a mujeres que presentan hemorragia postparto para disminuir el riesgo de shock hipovolémico, a través de una revisión integrativa de la literatura. Metodología: Revisión integrativa de la literatura siguiendo la propuesta por Sasso, de Campos y Galvão, se realizó una búsqueda en ClinicalKey, LILACS, CINAHL, Epistemonikos, Cochrane Library, PubMed, Scielo y Google Scholar; se incluyeron artículos publicados en los últimos cinco años, en español, inglés y portugués, se clasificaron por nivel de evidencia y grado de recomendación. Esta investigación es de bajo riesgo por ser de tipo documental. Resultados: Se recopilaron 41 artículos definitivos. La información se organizó en: cuadro clínico, cuidados de enfermería y dificultades en la atención gineco-obstétrica. Discusión: El profesional de enfermería debe identificar barreras en la atención evaluando la capacidad resolutiva de las instituciones y analizando los casos de muerte materna. Se recomienda el uso de misoprostol con oxitocina o únicamente de carbetocina y la combinación de ergometrina con oxitocina según el volumen de sangrado. Conclusión: Es pertinente realizar un examen físico para reconocer signos de inestabilidad hemodinámica, y de shock hipovolémico. Además, los diagnósticos e intervenciones de enfermería se enfocan en brindar cuidados de calidad, para evitar complicaciones como la muerte.


Introduction: In Colombia, postpartum hemorrhage is the second leading cause of death in pregnant women at a rate of 6.9 deaths per 1,000 live births. After childbirth, 8.2% of Latin American women are expected to have postpartum hemorrhage. Objective: To describe nursing care delivered to women with postpartum hemorrhage to reduce the risk of hypovolemic shock by means of an integrative literature review. Materials and Methods: Following the approach suggested by Sasso, de Campos and Galvão, an integrative literature review was conducted on ClinicalKey, LILACS, CINAHL, Epistemonikos, Cochrane Library, PubMed and ScieELO Google Scholar databases. Articles published in the last five years in Spanish, English and Portuguese were selected and classified by their level of evidence and degree of recommendation. This is a low-risk research due to its documentary nature. Results: 41 articles were finally selected and classified into clinical picture, nursing care and challenges in OBGYN care. Discussion: Nursing professionals should identify barriers to care by evaluating institutional problem-solving capacity and analyzing maternal death cases. Using misoprostol and oxytocin or only carbetocin and ergometrine-oxytocin is suggested based on the quantity of blood loss. Conclusions: Performing a physical examination is important to recognize signs of hemodynamic instability and hypovolemic shock. Nursing diagnoses and interventions focus on providing quality patient care to prevent certain complications such as death.

Esta é uma pesquisa de baixo risco, pois é do tipo documental. Resultados: Foram recolhidos 41 artigos definitivos. A informação foi organizada em: quadro clínico, cuidados de enfermagem e dificuldades nos cuidados gineco-obstetrítricos. Discussão: O profissional de enfermagem deve identificar as barreiras ao cuidado, avaliando a capacidade das instituições e analisando os casos de morte materna. O uso de misoprostol com ocitocina, ou carbetocina isoladamente, e a combinação de ergometrina com ocitocina são recomendados dependendo do volume de sangramento. Conclusão: O exame físico para sinais de instabilidade hemodinâmica e choque hipovolêmico é relevante. Além disso, os diagnósticos e intervenções de enfermagem concentram-se na prestação de cuidados de qualidade para evitar complicações como a morte.

Introducción: La alopecia androgenética es el tipo más prevalente de pérdida de pelo. Se manifiesta a edades tempranas y es más frecuente en el sexo masculino. Objetivo: Evaluar los resultados de la infiltración del plasma rico en plaquetas en una serie de casos con alopecia androgenética. Métodos: Se realizó un estudio observacional, descriptivo y longitudinal en una serie de casos, que incluyó 56 pacientes con diagnóstico clínico y dermatoscópico de alopecia androgenética que recibieron tratamiento con plasma rico en plaquetas en el área centro de la ciudad Sancti Spíritus, durante el período de julio 2015 a marzo de 2021. Se inyectaron 6 mL por vía intradérmica de plasma rico en plaquetas en el área afectada con múltiples inyecciones en patrón lineal a un cm de separación bajo el cuidado aséptico. El tratamiento se repitió cada 3 semanas por 8 sesiones. Las variables principales fueron la prueba de tracción pelo y eventos adversos. Resultados: Predominó el sexo masculino, las edades entre 19 y 51 años. El tiempo de evolución osciló entre 1 y 20 meses, la mayoría tenía antecedentes patológicos familiares de alopecia. Predominó el patrón III de Halminton Norwood. Como eventos adversos se presentaron dolor, ardor y eritema en el sitio de inyección en todos los casos. Conclusiones: La prueba de tracción del pelo en la mayoría de los pacientes tratados con plasma rico en plaquetas fue negativa al culminar el tratamiento(AU)


Introduction: Androgenetic alopecia is the most prevalent type of hair loss, it can be manifested at an early age, being more frequent in males. Objective: To evaluate the results of the infiltration of platelet-rich plasma in a series of cases with androgenetic alopecia. Methods: An observational, descriptive and longitudinal study was carried out in a series of cases, which included 56 patients with a clinical and dermatoscopic diagnosis of androgenetic alopecia who received treatment with platelet-rich plasma in the downtown area of the city of Sancti Spíritus, during the period from July 2015 to March 2021. 6 mL of platelet-rich plasma were injected intradermally into the affected area with multiple injections in a lineal pattern one cm apart under aseptic care. The treatment was repeated every 3 weeks for 8 sessions. The main variables were the hair pull test and adverse events. Results: Male sex predominated, ages between 19 and 51 years, evolution time ranged between 1 and 20 months, most had a family pathological history of alopecia, Halminton Norwood pattern III prevailed. Adverse events included pain, burning, and erythema at the injection site in all cases. Conclusions: The hair-pull test in most patients treated with platelet-rich plasma was negative at the end of treatment(AU)

La vulnerabilidad de los adolescentes se acentúa durante la maternidad debido a las posibles complicaciones biopsicosociales para el binomio madre-hijo. Así, se desarrolló un estudio observacional, descriptivo y de corte transversal, con un enfoque cuantitativo, cuya población estuvo integrada por 62 madres adolescentes, en puerperio inmediato, ingresadas en el Servicio de Ginecología y Obstetricia en el contexto de un hospital del segundo nivel de atención de la ciudad de Riobamba, durante el primer semestre del año 2019. Predominaron las edades perte-necientes a la etapa de la adolescencia tardía (77,4%), las autoidentificadas como mestizas, las que profesaban religión católica, el estado civil de unión libre o solteras y las de profesión de ama de casa. También, aquellas que no planificaron su embarazo (71%) y las que consideraron como deseado a su hijo. La mayoría de los adolescentes declararon no usar métodos anticoncep-tivos, no tener antecedentes patológicos familiares (67,74%) ni personales de interés (45,16%); aunque, entre estos últimos destacaron las infecciones de vías urinarias, que también constitu-yeron la principal enfermedad durante el embarazo. Preponderaron las embarazadas que se hicieron más de 5controles prenatales, las que cumplieron con las indicaciones y acudieron a las consultas de seguimiento, así como, la ausencia de complicaciones en los recién nacidos. El autoconcepto de las adolescentes puérperas participantes no se afectó gravemente en ninguna de estas, existiendo un predominio de afección entre moderada y leve, además de la categoría ninguna. En relación con el desempeño, predominaron aquellas en las que no se afectó.


The vulnerability of adolescents is accentuated during motherhood due to possible biopsychoso-cial complications for the mother-child binomial. An observational, descriptive, and cross-sec-tional study was developed with a quantitative approach. The population was made up of 62 adolescent mothers, in the immediate puerperium, admitted to the Gynecology and Obstetrics Service in the context of a second-level hospital of attention in the city of Riobamba, during the first semester of the year 2019. The predominated results were: the ages belonging to the late adolescence stage (77.4%), those self-identified as mestizo, those who professed a Catholic religion, the civil status of free union or single and those of the profession of housewife. Besides, those ones who did not plan their pregnancy (71%) and those ones who considered their child as desired. Most of the adolescents declared that they did not use contraceptive methods, did not have a family pathological history (67.74%) or personal history of interest (45.16%); although, among the latter, urinary tract infections stood out, which were also the main disease during pregnancy. The preponderance of pregnant women who underwent more than 5 prenatal check-ups, those who complied with the indications and attended follow-up consultations, as well as the absence of complications in the newborns. The self-concept of the participating puer-peral adolescents was not seriously affected in any of them, with a predominance of affection between moderate and mild, including the category none. In relation to performance, those ones that were not affected predominated.

RESUMEN Fundamento: la cirugía plástica ayuda a la mejora de la calidad de vida de las pacientes con cáncer de mama. La reconstrucción mamaria ha evolucionado no solo en las cuestiones estéticas, sino también, a favor de lograr menor morbilidad en las pacientes afectadas. Objetivo: describir los resultados obtenidos con la aplicación de técnicas oncoplásticas en pacientes con cáncer de mama. Método: se realizó un estudio descriptivo de una serie de 41 pacientes en el Hospital Provincial General Camilo Cienfuegos de Sancti Spíritus desde marzo de 2018 hasta octubre de 2020. La muestra incluyó a todas las pacientes intervenidas por cáncer de mama a las que se realizaron técnicas oncológicas conservadoras y radicales, así como, cirugía plástica. Fueron analizadas variables demográficas, clínicas y terapéuticas. Se utilizaron los procedimientos aritméticos y tabulares de la estadística descriptiva. Se mostraron las variables cuantitativas como promedios y desviación estándar, los datos cualitativos como frecuencias y porcentajes. Resultados: la edad media fue de 45 años, con una mínima de 26 años y una máxima de 67. En las pacientes sin deformidad mamaria previa se aplicaron tratamientos radicales, conservadores, reconstrucción con cirugía ahorradora de piel y mastoplastias con mínimas incisiones. En las pacientes con deformidades mayores el tratamiento que predominó fue conservador y las reconstrucciones con mastoplastias. Los estadios tumorales que prevalecieron fueron los T2N1M0 y T1N1M0. El mayor número de complicaciones se observó en las pacientes sometidas a cirugía ahorradora de piel y en aquellas con tratamientos radicales. Conclusiones: la cirugía oncoplástica es una alternativa de tratamiento quirúrgico que actualmente se emplea con más frecuencia en las pacientes con cáncer de mama. Se obtienen resultados similares a los de la cirugía convencional en cuanto a incidencia de complicaciones, recidivas y muerte.


ABSTRACT Background: plastic surgery helps to improve the quality of life of patients with breast cancer. Breast reconstruction has evolved not only in aesthetic matters, but also in favor of achieving less morbidity in each patient affected. Objective: to describe the results obtained with the application of oncoplastic techniques in patients with breast cancer. Method: a descriptive study of a series of 41 patients was carried out at the Camilo Cienfuegos Provincial General Hospital in Sancti Spíritus from March 2018 to October 2020. The sample included all the patients operated on for breast cancer in which techniques were performed. Conservative and radical oncology, as well as, plastic surgery. Demographic, clinical and therapeutic variables were analyzed. The arithmetic and tabular procedures of descriptive statistics were used. Quantitative variables were shown as means and standard deviation. Qualitative data such as frequencies and percentages. Results: the mean age was 45 years, with a minimum of 26 years and a maximum of 67. In patients without previous breast deformity, radical and conservative treatments, reconstruction with skin-sparing surgery and mastoplasties with minimal incisions were applied. In patients with major deformities, the predominant treatment was conservative and reconstructions with mastoplasties. The tumor stages that prevailed were T2N1M0 and T1N1M0. The highest number of complications was observed in patients undergoing skin-sparing surgery and in those with radical treatments. Conclusions: oncoplastic surgery is an alternative surgical treatment that is currently used more frequently in patients with breast cancer. Similar results are obtained to conventional surgery in terms of incidence of complications, recurrences and death.
