OBJECTIVE: Improving the understanding of the adaptation to stress of urinary continence. METHOD: A transversal analysis between physics of materials and the female anatomy. Laws of physics of the materials and of their viscoelastic behavior are applied to the anatomy of the anterior vaginal wall. RESULT: The anterior vaginal wall may be divided into two segments of different viscoelastic behavior, the vertical segment below the urethra and the horizontal segment below the bladder. If the urethra gets crushed on the first segment according to the hammock theory, the crushing of the bladder on the second segment is, on the other hand, damped by its important elasticity. The importance of this elasticity evokes an unknown function: damping under the bladder that moderates and delays the increase of intravesical pressure. This damping function below the bladder is increased in the cystocele, which is therefore a continence factor; on the other hand, it is impaired in obesity, which is therefore a factor of SUI. CONCLUSION: It is necessary to include in the theory of stress continence, the notion of a damping function below the bladder.

We developed a theoretical model to understand the extrinsic factors of the bladder-urethral system involved in stress urinary incontinence and their relationships with cystocele or the protective effect of dorsal decubitus and with overfilled-bladder-related dysuria. The model is based on known physical rules (elasticity, pressure-volume relationships in a closed cavity - vagina and its supportive tissue, Retzius space) applied to anatomy. The anterior vaginal wall, divided in two parts, plays a major role in stress urinary incontinence by moderating the rise in the abdominal pressure on the bladder and also by increasing the pressure in the Retzius space that increases the resistance to flow in the urethra. The tent canvas hypothesis applied to the anterior portion of the vaginal wall under the urethra explains the relationship between defective vaginal wall resistance and mobility of the urethra-bladder junction.

The authors report about one case of a tumoral syndrome discovered immediately after delivery, the etiology of which proved to be an infarction of omentum tumoral-like. On the basis, they study the pregnancy influence on etiology of this very rare pathology.

Epidural anaesthesia for elective induction of labour allows a better planning with many advantages for the medical team. A retrospective study was carried out to know the opinion parturients who benefited from this technique. This induction of labour, though appearing to a slight extent unnatural, is well accepted by the parturients. The reasons of satisfaction are mainly the security of childbirth, but also a better family organization.

It is extremely rare for pregnancy to occur spontaneously in a woman with a Turner' syndrome, pure 45 XO or mosaic. Furthermore, these pregnancies have been plagued by chromosome anomalies and fetal malformations, but also by spontaneous abortions, and pre-eclampsia. The author, having one personal case, try and point out the interest about antiplatelet therapy, these patients having uterine hypoplasia and hypovascularization.

The author uses a case to review the link between adenomyosis and pathological delivery. Such a link has been suspected in the past, and was confirmed in the present case by hysteroscopy after spontaneous abortion. Coagulation-diathermy of adenomyosis via hysteroscopy enabled a subsequent pregnancy culminating in normal delivery.
