RESUMO

BACKGROUND: Older and very old persons have an increased risk of traumatic injuries as well as comorbidities and multimorbidities. The standardized workflow in hospitals can increase the occurrence of typical geriatric complications, such as challenging behavior and delirium, which can result in highly complex care situations. The application of advanced practice nurses (APN) is an international response to such challenges. In Germany, the scientifically based development of APN has so far lagged behind the international standard. METHOD: For this scoping review, a systematic search in the databases PubMed and CINAHL and a supplementary hand search was conducted for the period 2010-2022. RESULTS: Tasks and role profiles for the scope of practice of APN in geriatrics and traumatology already exist in the literature. There is a lack of scientifically proven tasks and role profiles for APN in geriatric traumatology in the literature. DISCUSSION: Based on the current state of research it is not yet possible to derive specific tasks and role profiles for APN in geriatric traumatology. The transferability of tasks and profiles from geriatrics and traumatology seem to be possible. The development of tasks and role profiles for geriatric traumatology APN requires further research, especially to identify the specific needs of geriatric traumatology patients.

RESUMO

Despite of new therapy methods there currently is no perspective for healing or stopping the course of Parkinson's disease. Because of this and the predicted increase of the number of people suffering from parkinsonian syndromes, not only medical care but also nursing care of these syndromes will be of high importance in the future. The main objective of the conducted review was to study professional literature describing the situation of affected people and their relatives allowing to derive the appropriate requirements for a suitable care environment. This systematic review was conducted in the databases PubMed, CINAHL, Embase and Cochrane Library of Systematic Reviews. The results were considered with the focus of two main issues of a social situation (Dreitzel, 1972), the themes and the partners of interaction. The themes were assigned according to the coping needs of Corbin and Strauss (2004). Those needs can be observed during the progression of a chronic disease. Given the basic coping needs of affected people in different life situations, the results of 41 surveys and articles demonstrate the need for a continuous support of people suffering from parkinsonian syndromes and their relatives. Described are central issues such as financial coverage, maintenance of the individual role, getting support in dealing with the diagnosis and experiencing acceptance of one's perception of physical symptoms by health professionals. The professional life, family life, personal relationships, physician-patient and nurse-patient relationships, parent-child relationships as well as being part of society are described to be particularly challenging. The importance of support by a specialised nurse within the meaning of a Parkinson's Disease Nurse Specialist is being discussed.

RESUMO

OBJECTIVE: Care for people with dementia is considered a multi-professional challenge that requires a collaborative approach between health professionals and non-health professionals. Didactic strategies to ensure the same qualifications across these occupational groups are lacking. This article presents the joint learning of selected properties and promotive and obstructive conditions, using the example of a multi-professional Master's programme. It subsequently draws conclusions for didactic concepts. METHODOLOGY: The perceptions of 12 teachers on this Master's programme, all representing different professions, were determined by using a qualitative exploratory survey on the three stated dimensions. With the aid of a summarising content analysis, their statements were condensed and abstracted so as to deduce appropriate requirements for methodical and didactic learning scenarios. RESULTS: In view of the fact that the students have very varied previous knowledge, the main challenge is finding a balance between expertise and tediousness. Establishing essential and common expertise, as well as sensitivity for different perspectives, is made particularly difficult by the fact that health and non-health professions differ greatly in terms of methods and approaches. For a successful outcome, the content focal points and didactic and methodical concepts for a learning group need to take into account the composition of that specific group. Recourse to didactic standard concepts is only possible to a limited extent. CONCLUSIONS: The aim of joint teaching and learning of health and non-health professionals is to enhance the understanding of a profession: This is done by making individuals aware of their role in the chain of care, so they can recognise and organise the mutual conditionality of their own and external professional contributions.

RESUMO

In 2003 the "discourse of science ", which developed while establishing nursing-research in Germany, was investigated by the Hessian Institute of Nursing Research. Ten narrative interviews with nurse scientists who particurlarly participated in this discourse were conducted. The interviews were analysed by means of a qualitative content analysis. Then the interviewees were, according to their respective opinions and attitudes, divided into different groups, representing four ideal type categories. The analytical evaluation of these interviews demonstrated that there are common aspects in the careers of the interviewed, which are above all caused by the structural and institutional conditions of nursing. However, differences regarding the patterns of attitude and principles towards the discourse of science were identified also.

RESUMO

Cortical glutamate/glutamine (Glx) metabolism seems to be affected by a major depressive disorder. Recently, a Glx deficit was detected by proton magnetic resonance spectroscopy (1H-MRS) in the bilateral anterior cingulum of depressives. The aim of this study was to assess the effect of successful electroconvulsive therapy (ECT) on Glx levels in the anterior cingulum. The left anterior cingulum of 17 severely depressed unipolar patients was measured by 1H STEAM spectroscopy before and after ECT, and the results were compared with those for 17 age- and gender-matched controls. We observed significantly reduced Glx levels in the patients' left cingulum compared to healthy controls. In ECT responders, in contrast to non-responders, Glx levels normalized (P=0.04) and then did not differ statistically from controls. Severe depression seems to be associated with a Glx deficit and increasing Glx may be an important mechanism of ECT action.
