[The effect of omeprazole on healing of duodenal ulcers, Helicobacter pylori and gastritis]. / Vliv omeprazolu na hojení duodenálního vredu, Helicobacter pylori a gastritidy.

Losec (omeprazole) Astra Co. is a blocker of the proton pump of the parietal cell. It inhibits basal and stimulated HCl secretion. It is used for treatment of gastroduodenal ulcers, reflux oesophagitis and Zollinger Ellison's syndrome. In a group of 17 patients with duodenal ulcers the authors investigated the effect of omeprazole on (1) healing of duodenal ulcers and bulbitis after 2-4 weeks of therapy, (2) elimination of Helicobacter pylori in the antrum, (3) chronic antral gastritis. Ad 1. After two weeks of treatment the authors found that 5 of 17 chronic duodenal ulcers were healed in the remainder substantial regression was found. Four-week treatment led to healing of 16 from a total of 17 ulcers (P < 0.001), i. e. 94%. In subjects with ulcers and bulbitis (12 patients) the ulcer healed in 11 instances, in 7 patients residual bulbitis persisted. Ad 2. H. pylori was detected before treatment in 16 of 17 patients, after treatment only in 5 (P < 0.001). Ad 3. Chronic gastritis was recorded before treatment in all patients. Treatment reduced its activity and the presence of H. pylori.

A dysontogenetic cyst in the edge of the tongue in 1-year old boy was found lined with gastric, duodenal, and intestinal mucosa with its own lamina muscularis mucosae, with a thin layer of submucous connective tissue encircled by bundles of smooths muscle imprecisely separated from the striated lingual muscle.

[Bilateral pheochromocytoma of the adrenal glands and a solitary neurofibroma of the dorsal mucosa of the tongue associated with a solid medullary carcinoma of the thyroid gland in Hashimoto's lymphocytic thyroiditis]. / Oboustranný feochromocytom nadledvin a solitární neurofibrom sliznice hrbetu jazyka, sdruzený se solidním medulárním karcinomem stítné zlázy v hashimotove lymfocytární tyreoiditide.

A female aged 22 died 13 hours after having given birth to a live mature fetus. On autopsy, bilateral phoechromyocytomas, a mucosal neurofibroma of the tongue and a solid medulary carcinoma of the thyroid with amyloid associated with Hashimoto's lymphocytic thyroiditis were detected. After parturition, there occurred bleeding into the left-side pheochromocytoma followed by a release of phenyl alkylamines into the circulation. This resulted in fatal peripheral circulatory failure.

[Campylobacter pylori and treatment of duodenal ulcer]. / Campylobacter pylori a lécba duodenálního vredu.

The authors subjected to endoscopic examination 90 patients with duodenal ulcers. From each patient they collected a bioptic specimen of the gastric antrum and duodenal bulbus for microbiological and histological examination. They found that for detection of C. pylori the microscopic examination was most useful. The presence of C. pylori in the gastric antrum was detected in 87% of the subjects and in the duodenal bulb only in 48%. The difference is significant at the 0.01 level. To 43 patients of the group Ranitidine was administered. The ulcer was cured within 4 weeks in 35 subjects. Ranitidine did not affect the presence of C. pylori and antral gastritis. After cure of the ulcer the authors administered Tarivid to 10 patients with persisting C. pylori positivity. The presence of C. pylori and antral gastritis were not affected by this treatment.

[Cystic lymphangioma of the retroperitoneum]. / Cystický lymfangiom retroperitonea.

The authors present the case-history of a patient with a cystic retroperitoneal lymphangioma. Its diagnosis was established only by histological examination of the resected tissue. The authors draw attention to the symptomatology, diagnosis, therapy and discuss contemporary views regarding the cause of this disease.

[Unusual cause of death of a parturient from circulatory failure (author's transl)]. / Ungewöhnliche Todesursache durch Kreislaufversagen bei einer Gebärenden.

Bilateral phaeochromocytoma was histologically recorded from a primigravida and primipara, 22 years of ager, who had died 13 hours after precipitate spontaneous birth of a living mature child. Mucosal neurofibroma on the tongue and solid medullary carcinoma of the thyroid gland with amyloid, associated with Hashimoto's lymphocytic thyreoiditis, were additional findings. Coincidence of such carcinoma with Hashimoto's thyreoiditis so far has not been described in international literature. When the abdominal cavity was relieved of pressure after parturition, bleeding started into the phaeochromocytoma on the left side. Death then was caused by resulting peripheral circulatory failure.

[Acute reversible liver necrosis after valproate therapy (immunologic hypersensitivity)]. / Akute Reversible Lebernekrose nach Valproat-Behandlung (immunologisch bedingte Idiosynkrasie).

We describe a case of a 9-year-old boy with the severe acute liver injury caused by idiosyncrasy after the administration of valproate. The liver biopsy performed during the first days after the onset of the disease revealed the necrosis of more than 50% of hepatocytes but the injury was almost fully reversible which was well demonstrated by the second biopsy five months later. After five years an evidence for the immunologic idiosyncrasy caused by valproate was still present, which was demonstrated in vitro by blastic transformation of lymphocytes from the patient. The boy is quite healthy and epilepsy is under control of suximide. The family was informed that the boy could be treated with valproate never more.
