OBJECTIVE: The objective of the study was to determine total hospital costs and net hospital income for different types of minimally invasive hysterectomy and financial impact if a subset of patients underwent total vaginal hysterectomy (TVH) instead of their selected procedure. STUDY DESIGN: A retrospective chart review was performed of patients who underwent hysterectomy for benign disease by TVH, laparoscopic assisted vaginal hysterectomy (LAVH), total laparoscopic hysterectomy (TLH), and robotic hysterectomy (RH) between Jan. 1, 2007, and April 30, 2010, at Thomas Jefferson University Hospital. The hospital decision support database was used to calculate net hospital income. A subset of patients with at least 1 prior vaginal delivery, no more than 1 laparotomy, and a uterine size less than 14 weeks who had undergone RH, TLH, or LAVH was identified as potential TVH candidates. The financial impact of performing TVH over the selected hysterectomy was calculated. RESULTS: Three hundred thirty-four cases of minimally invasive hysterectomy were identified. Fifty-five percent were TVH, 33% LAVH, 3% TLH, and 9% RH. Mean total hospital costs for TVH were $7903, $10,069 for LAVH, $11,558 for TLH, and $13,429 for RH (P < .0001). Net hospital income was $1260 for TVH. The hospital incurred losses of $-1306 for LAVH, $-4049 for TLH, and $-4564 for RH (P = .03). Our criteria to determine the mode of hysterectomy increased TVH from 57% to 76% of all minimally invasive hysterectomy. CONCLUSION: Hospital costs were greater with LAVH, TLH, and RH than for TVH. The hospital incurred financial losses with LAVH, TLH, and RH. TVH was the only minimally invasive modality of hysterectomy that generated net hospital income. Our conservative criteria to determine the route of hysterectomy would increase the number of TVHs by more than 30%.

PURPOSE/OBJECTIVES: Clinical teaching in dentistry has cycled through specialty-based "departmental" teaching to group practice scenarios. The aims of this study were to ascertain third-year dental students' perceptions of a specialty-based rotation supplemented by online educational tools and to compare their Objective Structured Clinical Exam (OSCE) scores to those of the students in the previous year. METHODS: This retrospective research design included the analysis of OSCE scores, and student responses to survey questions regarding their perceptions of the clinical oral pathology (COP) rotation. This study was completed in 2022. It included data from 2020 to 2021 and 2021 to 2022 corresponding to input from the Classes of 2022 and 2023, respectively. The response rate was 100%. RESULTS: The students evaluated the focused COP rotation and the online teaching modules as a positive experience. The results of the OSCE were similar to those of the previous class and showed a high average score. CONCLUSIONS: This study shows that students had a positive perception to specialty-based learning with online educational tools and that it enhanced the education they received in the comprehensive care clinic. The OSCE scores were similar to those of the previous class. These findings suggest a method of maintaining high-quality dental education through challenges as it continues evolving.

BACKGROUND: Creating a curriculum that can effectively develop critical thinking skills is an elusive outcome for undergraduate nursing programs. Despite the best efforts of faculty, employers continue to note an inability of new graduates to apply critical thinking concepts that improve patient outcomes. The lack of an operational definition of critical thinking appears to be a key contributor to this issue. METHOD: A concept analysis was developed to identify a definition of critical thinking that allows for clear, measurable learning outcomes. RESULTS: Despite extensive discussion within the literature, a consistent definition of critical thinking is lacking; instead, a rather extensive list exists of the attributes and antecedents consistently found in individuals who think critically. CONCLUSION: Curricular student learning outcomes framed around operational attributes can promote the development of critical thinking skills and improve patient outcomes. OBJECTIVE: The objective of this pilot study is to determine the prevalence of pseudoxanthoma elasticum (PXE)-like connective changes in an oral biopsy service and compare it with the estimated prevalence of PXE as well as to the prevalence of the mutated PXE gene ABCC6. STUDY DESIGN: This prevalence study utilized 500 oral mucosal biopsy specimens received from the biopsy service of the Oral Pathology Consultants at the Ohio State University. Each specimen was microscopically evaluated using hematoxylin and eosin, Verhoeff-van Gieson and von Kossa stains. RESULTS: A prevalence of 9.8% was identified for PXE-like changes in the connective tissue of oral biopsy specimens submitted to this service. CONCLUSIONS: The overall prevalence of PXE-like connective tissue changes found in routine oral mucosal biopsy specimens (9.8%) was much higher than either the suspected prevalence of PXE (0.001%-0.004%) or the estimated prevalence of the mutated gene ABCC6 (0.625%-1.25%).

Aggressive osteoblastoma is a rare primary bone neoplasm with the potential for local invasion and recurrence. While the vertebrae or long bones are most commonly affected, few well-documented cases have been reported in the jaws. A 25-year-old man presented with a palatal mass of several months' duration. He reported the lesion had undergone gradual enlargement and, while generally asymptomatic, had recently become increasingly painful. An incisional biopsy was interpreted as "osteoblastic neoplasm" most suggestive of osteoblastoma. However, final diagnosis was deferred until the resection specimen could be evaluated. Following partial maxillectomy, histopathologic examination revealed a proliferation of large epithelioid cells with eccentric nuclei and prominent nucleoli associated with broad, irregular deposits of osteoid and trabeculae of bone. The lesional cells exhibited minimal pleomorphism with infrequent, normal-appearing mitotic figures and numerous osteoclast-like giant cells were observed within an associated loose fibrovascular stroma. Transformation of "blue bone" to more organized eosinophilic trabeculae of woven bone was noted at the periphery of the lesion and there was no evidence of invasion. A diagnosis of aggressive osteoblastoma was made. Previous reports of gnathic aggressive osteoblastoma are reviewed and the features that distinguish this process from conventional osteoblastoma or osteoblastoma-like osteosarcoma are presented.
