OBJECTIVES: The aim of this study was to compare the effect of three different access cavities on the tissue removal, deflection, fracture resistance, and stress distribution of extracted maxillary central incisors. MATERIALS AND METHODS: Forty human maxillary central incisors were randomly assigned in four experimental group (n = 10) including conservative access cavity "CAC," traditional access cavity "TAC," invasive access cavity "IAC," and without access cavity (control group). Cone-beam computed tomography "CBCT" scans were used to evaluate the tissue removal during the different access cavities. All specimens were restored with composite resin (Admira Fusion, Voco, Cuxhaven, Germany) and embedded in acrylic resin blocks after simulating the periodontal ligament using red wax, then the specimens were submitted to the deflection test applying a load of 250 N and to the load-to-fracture test after artificial aging in a mechanical cycling machine (150 N, 5 × 106 cycles, 10 Hz). Lastly, stress distribution was assessed by three-dimensional finite element analysis (3D-FEA), simulating the specimens restoration by two types of composite resins of low and high elastic modulus (8 and 18 GPa respectively) after the access cavities. The data were submitted to Shapiro-Wilk and KS normality tests. Then, they were analyzed by one-way ANOVA and Tukey tests with a significance level (α ≤ 0.05). RESULTS: CBCT scans showed a significant difference of worn tissues in CAC and TAC when compared to the IAC (P < 0.0001). In deflection test, CAC showed lower deformation values than the TAC and IAC. Load-to-fracture test presented no significant difference among the three experimental groups (P = 0.6901). 3D-FEA showed that the more conservative the access cavity, the higher the stress magnitude. CONCLUSIONS: CAC promote less worn tissue; however, this does not improve the stress distribution or fracture resistance of endodontically treated maxillary incisors. CLINICAL RELEVANCE: Clinicians should reconsider the pros and cons of the conservative access cavity.

(1) Background: In hospitals, medical and dental clinics, antiseptics or disinfectants play an essential role in the control of nosocomial infections. This study aimed to evaluate R. officinalis and P. paniculata glycolic extracts regarding: (I) their antimicrobial action on planktonic and biofilm (monotypic and cutaneous biofilm model-S. aureus, S. epidermidis and C. acnes); and (II) their cytotoxicity on human keratinocytes (HaCaT). (2) Methods: Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were performed (CLSI protocol M7-A6 and M11-A8). MTT analysis was used to evaluate the antibiofilm activity of the extracts on biofilms and their cytotoxicity on human keratinocytes. (3) Results: The combined glycolic extracts MIX A (75% P. paniculata + 25% R. officinalis); MIX B (50% P. paniculata + 50% R. officinalis); and MIX C (25% P. paniculata + 75% R. officinalis) promoted MBC values by 50 mg/mL on S. aureus, absent on S. epidermidis, and ranged 6.25-50 mg/mL for C. acnes. The cutaneous biofilm model was reduced more than 90%. In addition, it showed biocompatibility with human keratinocytes, resulting in percentages of viability greater than 50%. (4) Conclusions: The combination of extracts promoted antimicrobial action on planktonic cultures, and monotypic and heterotypic biofilms of skin pathogens. Additionally, these extracts are biocompatible against human keratinocytes.

The aim of this study was to evaluate in vitro the effect of calcium hydroxide [Ca(OH)2], 2% chlorhexidine gel (CHX) on macrophages (RAW 264.7) to produce pro-inflammatory cytokines and nitric oxide after pretreatment with lipoteichoic acid (LTA) of Enterococcus faecalis. Forty-eight human single-rooted teeth were instrumented with R25.08 (RECIPROC) and sterilized by gamma irradiation. LTA was inoculated in the root canal of each specimen for 96 hours. Specimens were instrumented with 40.06 and 50.05 (RECIPROC) and medicated with: I) Pyrogen-free saline solution (SS); II) 2% CHX gel; III) Ca(OH)2 + SS; or IV) Ca(OH)2 + CHX for 14 days. Three samples (S) were performed of the root canal of each specimen at: S1) immediately after instrumentation; S2) after Ethylenediaminetetraacetic acid (EDTA); S3) after intracanal medication removal. Subsequent quantification of cytokines (IL-1ß, TNF-α, MIP-1α, IP-10, G-CSF and IL-6) by immunosorbent assay (ELISA) and nitric oxide by the Griess method was carried-out. Data were submitted to a normality test and then analyzed with one-way ANOVA and Tukey test with a significance level of 5% using GraphPad Prism 6. Ca(OH)2 + SS and Ca(OH)2 + CHX presented lower levels of TNF-α, TNF-α, IL-6, G-CSF and nitric oxide. Ca(OH)2 + SS was the most effective in reducing MIP-1α. CHX was effective in reducing IL-6 and G-CSF. Therefore, the combined intracanal medication of calcium hydroxide and chlorhexidine is effective in reducing the cytokines TNF-α, IL-1ß, IL-6, G-CSF and nitric oxide.

AIM: The aim was to compare cyclic fatigue, torsional failure, and flexural resistance of NiTi endodontic files: Hyflex CM (HYF), Genius files (GEN), WaveOne Gold (WOG), and ProTaper Universal (PTU). MATERIALS AND METHODS: Fifteen files of each brand were used in cyclic fatigue test and other fifteen files for flexural test and torsional failure test. To the cyclic fatigue test, used torque limit and revolutions per minute were set according to the respective manufacturer guidelines. The test was performed under deionized water at 36°C, and all files were tested in a 3 mm radius of curvature with an angle of curvature of 60°, time of the fracture was recorded. Torsional fatigue test was performed in the torsional machine (Instron MT, USA), recording the fractured time and torque data by the machine software. Flexural fatigue test was performed in 60° of curvature. All data were statistically analyzed by one-way analysis of variance, and Tukey test for multiple comparisons. RESULTS: Cyclic fatigue (seconds)= HYF: 744.1 ± 231.9/GEN: 477.3 ± 220.5/WO: 278.4 ± 57.0/PTU: 152.4 ± 65.2; torsional failure (N × cm)= HYF: 6.85 ± 1.484/GEN: 6.55 ± 0.828/WOG: 5.73 ± 0.360/PTU: 4.43 ± 0.900; flexural resistance (N × mm)= HYF: 0.33 ± 0.294/GEN: 0.19 ± 0.136/WOG: 0.98 ± 0.216/PTU: 1.85 ± 0.276. CONCLUSION: HYF and GEN showed the best results for cyclic fatigue, torsional failure, and flexural resistance, followed by WOG and PTU.

This study aimed to evaluate sodium hypochlorite (NaOCl), limewater (LW), and Polymyxin B (PMB) as irrigants over MMP-3, MMP-8 and MMP-9. Thirty-three patients with apical periodontitis of single-rooted teeth were treated according to three-experimental groups (n=11): group-1: 2.5% NaOCl was used as irrigant; group-2: 2.5% NaOCl for the first two files and LW: [0.14% Ca(OH)2] for the last two files; group-3: 2.5% NaOCl for the first two files and PMB for the last two files. The association of Ca(OH)2 and CHX was used as an intracanal medication in all groups. Four root canal samplings (S) were collected: S1) immediately after access cavity; S2) after biomechanical preparation; S3) after EDTA application; and S4) after removal of the intracanal medication. After quantification of MMP-3, MMP-8, and MMP-9, the data were analyzed by Friedman and Kruskal-Wallis tests and completed by Dunn test (5%). Regardless the used irrigant, there was no difference in reducing MMP-3 or MMP-8 (P=0,5273, P=0,7048 respectively). However, in reducing MMP-9 (P=0,0246) the NaOCl group was the most effective followed by NaOCl+LW group and NaOCl+PMB group respectively. The intracanal medication [Ca(OH)2 + CHX] with the NaOCl and NaOCl+LW was effective in reducing MMP-8 (P<0,0001, P=0,0025) and MMP-9 (P=0,0007, P=0,0047) respectively, but not for the group of NaOCl+PMB which was not effective in reducing MMP-8 or MMP-9 (P=0,1718, P=0,1953) respectively. NaOCl and NaOCl+LW were effective in reducing MMP-9 levels, and this effectivity could be improved by the use of the intracanal medication [Ca(OH)2 + CHX] in reducing MMP-8 and MMP-9 levels.

Endodontic treatment consists of decontamination of the root canal using irrigants like Sodium Hypochlorite (NaOCl). However, some microorganisms resist conventional therapies, causing relapse and secondary infections. Substances with microbicidal power and potential to become new irrigants need to be studied. Thus, the objective of the present study was to evaluate the antimicrobial potential of Psidium guajava L. (guava tree) extract against clinical and standard (ATCC) strains of Enterococcus faecalis and Candida albicans. For this purpose, the extract was produced from the raw material (leaves and shoots) diluted in the hydroethanolic vehicle (EtOH: H2O / 50:50) for subsequent microbiological analysis. The Minimum Inhibitory Concentration (MIC) and Minimum Microbicidal Concentration (MMC) of the plant extract were tested, according to the Clinical and laboratory standards institute (CLSI) guidelines, on four strains: E. faecalis (ATCC and clinic) and C. albicans (ATCC and clinic). The extract of P. guajava did not produce antifungal action on C. albicans, however, it showed microbicidal potential against strains of E. faecalis, showing MIC of 0.20%. This concentration was lower than the MIC of NaOCl which was 0.31%, a solution that is commonly used in dental clinics. In conclusion, the hydroethanolic extract of P. guajava presents bactericidal action against E. faecalis, being a natural product with potential for future studies regarding the development of new endodontic irrigants.


O tratamento endodôntico consiste na descontaminação do canal radicular com emprego de soluções irrigadoras, como por exemplo, o Hipoclorito de Sódio (NaOCl). Contudo, alguns microrganismos resistem as terapias convencionais, causando recidiva e infecções secundárias. Substâncias com poder microbicida e potencial para se tornarem novas soluções irrigadoras precisam ser estudadas. Assim, o objetivo do presente estudo foi avaliar o potencial antimicrobiano do extrato de Psidium guajava L. (goiabeira) contra cepas clínicas e padrão (ATCC) de Enterococcus faecalis e Candida albicans. Para tanto, o extrato foi produzido a partir da matéria bruta (folhas e brotos) diluído no veículo hidroetanólico (EtOH: H2O / 50:50) para posterior análise microbiológica. Foram testadas a Concentração Inibitória Mínima (CIM) e Concentração Microbicida Miníma (CMM) do extrato vegetal, de acordo com as normas da Clinical and laboratory standards institute (CLSI), sobre quatro cepas: E. faecalis (ATCC e clínica) e C. albicans (ATCC e clínica). O extrato de P. guajava não produziu ação antifúngica sobre C. albicans, contudo, apresentou potencial microbicida contra cepas de E. faecalis, exibindo CIM de 0,20%. Esta concentração foi menor que a CIM do NaOCl que foi de 0,31%, uma solução que é comumente utilizada nas clínicas odontológicas. Em conclusão, o extrato hidroetanólico de P. guajava apresenta ação bactericida contra E. faecalis, sendo um produto natural com potencial para futuros estudos quanto ao desenvolvimento de novos irrigantes endodônticos.


El tratamiento endotónico consiste en la descontaminación del canal raíz con el uso de soluciones de irrigación, como el Hipoclorito de Sodio (NaOCl). Sin embargo, algunos microorganismos se resisten a las terapias convencionales, causando recaídas e infecciones secundarias. Es necesario estudiar las sustancias con poder de microbicida y el potencial para convertirse en nuevas soluciones de riego. Por lo tanto, el objetivo de este estudio fue evaluar el potencial antimicrobiano del extracto de Psidium guajava L. (guava) frente a cepas clínicas y estándar (ATCC) de Enterococcus faecalis y Candida albicans. Con este fin, el extracto se produjo a partir de la materia prima (hojas y brotes) diluida en el vehículo hidroetanólico (EtOH: H2O / 50:50) para análisis microbiológicos posteriores. La Concentración Mínima Inhibidora (CMI) y la Concentración de Microbicidas Minima (CCM) del extracto vegetal se probaron de acuerdo con el Clinical and laboratory standards institute (CLSI) en cuatro cepas: E. faecalis (ATCC y clínico) y C. albicans (ATCC y clínico). El extracto de P. guajava no produjo acción antifúngica sobre C. albicans, sin embargo mostró potencial microbicida frente a cepas de E. faecalis, presentando CIM 0,20%. Esta concentración fue menor que la CMI de NaOCl, que fue del 0,31%, una solución comúnmente utilizada en clínicas dentales. En conclusión, el extracto hidroetanólico de P. guajava muestra una acción bactericida frente a E. faecalis, siendo un producto natural con potencial para futuros estudios sobre el desarrollo de nuevos irrigantes endotónicos.

The aim of this study was to evaluate the effect of nicotine lozenge on teeth staining with/without bleaching in animal model study. A total of 15 Wistar rats were exposed in an acrylic container to 10 cigarettes smoke three times a day for 8 minutes per time, and sacrificed after 60 days. A total of 30 incisor teeth were treated (n=10) as the following: Group-1: in-office bleaching, at-home bleaching and immersion in artificial saliva; group-2: in-office bleaching, at - home bleaching and immersion in nicotine lozenge solution and in artificial saliva; group-3: immersion in nicotine lozenge solution and in artificial. The specimens of all groups were photographed using a stereomicroscope at T1) immediately after the extraction and before any treatment; T2) after one month of the treatment; and T3) after two months of the treatment. Four equidistant points of each specimen were analyzed using CMYK shade guide. The data were analyzed one-way ANOVA test followed by Tukey test for multiple comparisons with (a ≤ 0.05). In group-1, there was a significant difference of the color saturation of specimens between T1 and T2, and between T1 and T3 readings (P<0.0001). In group-2, there was a significant difference of the color saturation of specimens between T1 and T2, between T2 and T3 readings (P<0.0001). In group-3, there was a significant difference of the color saturation of specimens between T1 and T2, and between T1 and T3 readings (P<0.0001). The usage of nicotine lozenge promotes teeth lighting with/without bleaching.


El objetivo de este estudio fue evaluar el efecto de comprimidos de nicotina sobre la tinción de los dientes con/sin blanqueamiento en un estudio de modelo animal. Un total de 15 ratas Wistar fueron expuestas en un recipiente acrílico al humo de 10 cigarrillos tres veces al día durante 8 minutos por vez, y sacrificadas después de 60 días. Se trataron un total de 30 dientes incisivos (n=10) de la siguiente manera: Grupo-1: blanqueamiento en consultorio, blanqueamiento en casa e inmersión en saliva artificial; grupo-2: blanqueamiento en consultorio, blanqueamiento en casa e inmersión en solución de comprimidos de nicotina y en saliva artificial; grupo-3: inmersión en solución de nicotina en comprimidos y en artificial. Los especímenes de todos los grupos fueron fotografiados utilizando un microscopio estereoscópico en T1) inmediatamente después de la extracción y antes de cualquier tratamiento; T2) después de un mes del tratamiento; y T3) a los dos meses del tratamiento. Se analizaron cuatro puntos equidistantes de cada espécimen utilizando la guía de colores CMYK. Los datos se analizaron con la prueba ANOVA unidireccional seguida de la prueba de Tukey para comparaciones múltiples con (a ≤ 0,05). En el grupo 1, hubo una diferencia significativa de la saturación de color de las muestras entre T1 y T2, y entre las lecturas T1 y T3 (P<0,0001). En el grupo 2, hubo una diferencia significativa de la saturación de color de las muestras entre T1 y T2, entre las lecturas de T2 y T3 (P<0.0001). En el grupo 3, hubo unadiferencia significativa de la saturación de color de las muestras entre T1 y T2, y entre las lecturas T1 y T3 (P<0,0001). El uso de comprimidos de nicotina promueve la iluminación de los dientes con/sin blanqueamiento.

Aim: To compare the accuracy of the panoramic radiography with cone-beam computed tomography (CBCT) scans in measuring the distances between root apexes and the adjacent anatomical structures including the maxillary sinus and the mandibular canal. Material and Methods: A total of 200 CBCT scans (100 maxillary and 100 mandibular) from patients who also had corresponding panoramic radiography were selected. Linear measurements (in mm) presenting centralized image were made between the apexes of the maxillary teeth and the inferior wall of the maxillary sinus, and between the apexes of the mandibular teeth and the superior border of the mandibular canal by using specific software for panoramic radiography and the measurements on the coronal sections in CBCT scans. Data were submitted to inferential statistical analysis and Student's t-test for comparison between measurements. Results: CBCT scans were significantly more accurate than panoramic radiography to measure the distances between the apexes of the maxillary teeth and the inferior wall of the maxillary sinus (p<0.05) and between the apexes of the mandibular teeth and the superior border of the mandibular canal or mental foramen (p<0.05). Conclusion: CBCT scans present more accurate measurements than panoramic radiography.


Objetivo: Comparar la precisión de la radiografía panorámica con las exploraciones de la tomografía computarizada dental de haz en cónico (CBCT) para medir las distancias entre los vértices radiculares y las estructuras anatómicas adyacentes, incluidos el seno maxilar y el canal mandibular. Material y Métodos: Se seleccionaron un total de 200 tomografías CBCT (100 maxilares y 100 mandibulares) de pacientes que además tenían la correspondiente radiografía panorámica. Se realizaron mediciones lineales (en mm) que presentaban imagen centralizada entre los ápices de los dientes maxilares y la pared inferior del seno maxilar, y entre los ápices de los dientes mandibulares y el borde superior del canal mandibular mediante software específico para radiografía panorámica. y las mediciones en las secciones coronales en escaneos CBCT. Los datos se sometieron a análisis estadístico inferencial y prueba t de Student para comparación entre mediciones. Resultados: Las exploraciones CBCT fueron significativamente más precisas que la radiografía panorámica para medir las distancias entre los ápices de los dientes maxilares y la pared inferior del seno maxilar (p<0,05) y entre los ápices de los dientes mandibulares y el borde superior de los dientes mandibulares. canal o agujero mentoniano (p<0.05). Conclusión: Las exploraciones CBCT presentan mediciones más precisas que la radiografía panorámica.

Resumo O objetivo deste estudo foi avaliar in vitro o efeito do hidróxido de cálcio [Ca(OH)2], clorexidina gel a 2% (CHX) em macrófagos (RAW 264.7) na produção de citocinas pró-inflamatórias e óxido nítrico após pré-tratamento com ácido lipoteicóico (LTA) de Enterococcus faecalis. Quarenta e oito dentes humanos uniradiculares foram instrumentados com R25.08 (RECIPROC) e esterilizados por irradiação gama. LTA foi inoculado no canal radicular de cada espécime por 96 horas. Os espécimes foram instrumentados com 40.06 e 50.05 (RECIPROC) e medicados com: I) solução salina apirogênica (SS); II) CHX 2%; III) Ca(OH)2 + SS; ou IV) Ca(OH)2 + CHX durante 14 dias. Três amostras (S) foram realizadas do canal radicular de cada espécime em: S1) imediatamente após a instrumentação; S2) após ácido etilenodiaminotetracético (EDTA); S3) após a retirada da medicação intracanal. Foi realizada a quantificação subsequente de citocinas (IL-1β, TNF-α, MIP-1α, IP-10, G-CSF e IL-6) por imunoensaio (ELISA) e de óxido nítrico pelo método de Griess. Os dados foram submetidos a um teste de normalidade e então analisados com ANOVA one-way e teste de Tukey com nível de significância de 5% usando GraphPad Prism 6. Ca(OH)2 + SS e Ca(OH)2 + CHX apresentaram níveis mais baixos de TNF -α, TNF-α, IL-6, G-CSF e óxido nítrico. Ca(OH)2 + SS foi o mais eficaz na redução de MIP-1α. CHX foi eficaz na redução de IL-6 e G-CSF. Sendo assim a associação de hidróxido de cálcio e clorexidina é eficaz na redução das citocinas TNF-α, IL-1β, IL-6, G-CSF e óxido nítrico.

Acinetobacter baumannii is a multi-drug resistant microorganism. The objective of this study was to evaluate the antimicrobial and antibiofilm action of the pomegranate natural extract against eight strains of multidrug resistant Acinetobacter baumanii and to assess the extract cytotoxicity in a culture of Human Keratinocytes (HaCat). Broth microdilution method was used to determine the minimum inhibitory and minimum microbicidal concentrations of the extracts. The extract antibiofilm action was analysed by the MTT colorimetric test. The cytotoxicity evaluation was performed by the MTT colorimetric test, which analysed the mitochondrial cellular action, after contact of the extract for 5 min. The results were statistically analysed by ANOVA and Tukey test with a significance level α≤ 0.05. Punica granatum L. (pomegranate) extract had antimicrobial action on all the 8 clinical strains of Acinetobacter baumannii evaluated. The extract showed a significant reduction in metabolic action in biofilm for all the strains, with results statistically different from growth control (p≤0.05%). P. granatum extract applied for 5 min on human keratinocytes (HaCat) promoted cell viability in all the tested concentrations. The pomegranate extract is effective in reducing the multidrug-resistant clinical strains of Acinetobacter baumanni and is biocompatible. (AU)


Acinetobacter baumannii é um microrganismo multirresistente. O objetivo deste estudo foi avaliar a ação antimicrobiana e antibiofilme do extrato natural de romã contra oito cepas de A. baumanii multirresistente e avaliar a citotoxicidade do extrato em uma cultura de queratinócitos humanos (HaCat). O método de microdiluição em caldo foi utilizado para determinar as concentrações inibitórias mínimas e microbicidas mínimas dos extratos. A ação antibiofilme do extrato foi analisada pelo teste colorimétrico MTT. A avaliação da citotoxicidade foi realizada pelo teste colorimétrico MTT, que analisou a ação celular mitocondrial, após contato do extrato por 5 min. Os resultados foram analisados ​​estatisticamente por ANOVA e teste de Tukey com nível de significância α≤ 0,05. O extrato de Punica granatum L. (romã) apresentou ação antimicrobiana em todas as 8 cepas clínicas avaliadas de A. baumannii. O extrato apresentou redução significativa na ação metabólica no biofilme para todas as linhagens, com resultados estatisticamente diferentes do controle de crescimento (p≤0,05%). Objective: this questionnaire aimed to evaluate the approval of a new pictograph system for endodontic diagnosis by undergraduate and graduate students/endodontist instead of a conventional method. Methods: after a presentation to a total of 224 participants explaining the pictograph system, the participants (166 undergraduate students and 58 graduate students/endodontist) received a standardized questionnaire with three questions. The data were analyzed using the mult prop macro basing on Tukey's test for multiple comparison with (P ≤ 0.05). The first question was to define the education level of the participants; the second question about which diagnosis method is preferred; and the third question about the intention of using one of the methods in the future. The questionnaire was realized in May 2019. The response rate was 97.7%. Results: the first question showed that 74.10% and 25.90% were under graduate and graduate students/endodontist respectively. After statistical analysis there was no significant difference between the groups in the second and the third questions. In the second question (71.08% and 62.06%) of undergraduate and graduate student/endodontists respectively preferred the pictograph method over the conventional method. In the third question (60.24% and 51.72%) of undergraduate and graduate student/endodontists respectively showed interest in using the pictograph method in their professional career. Conclusion: the pictograph method in endodontic diagnosis is accepted by the majority of undergraduate and graduate students/endodontists. Training and experience affect the diagnosis making. (AU)


Objetivo: este questionário teve como objetivo avaliara aprovação de um novo sistema de imagem para diagnóstico endodôntico por estudantes de graduação e pós-graduação/endodontista, em vez de um método convencional. Métodos: após uma apresentação a um total de 224 participantes explicando o sistema de pictograma, os participantes (166 estudantes de graduação e 58 de pós graduação/endodontista) receberam um questionário padronizado com três perguntas. Os dados foram analisados usando a macro multi propósito baseada no teste de Tukey para comparação múltipla com (P ≤ 0,05). A primeira pergunta foi definir o nível de escolaridade dos participantes; a segunda pergunta sobre qual método de diagnóstico é preferido; e a terceira pergunta sobre a intenção de usar um dos métodos no futuro. O questionário foi realizado em maio de 2019. A taxa de resposta foi de 97,7%. Resultados: a primeira pergunta mostrou que 74,10% e 25,90% eram estudantes de graduação e pós-graduação / endodontista, respectivamente. Após análise estatística, não houve diferença significativa entre os grupos na segunda e na terceira questões. Na segunda questão (71,08% e 62,06%) dos estudantes de graduação e pós-graduação / endodontistas, respectivamente, preferiram o método de pictograma ao método convencional. Na terceira questão (60,24% e 51,72%) dos estudantes de graduação e pós-graduação /endodontistas, respectivamente, demonstraram interesse em utilizar o método de pictograma em sua carreira profissional. Conclusão: o método do pictograma no diagnóstico endodôntico é aceito pela maioria dos estudantes de graduação e pós-graduação / endodontistas.
