BACKGROUND: Furuncular myiasis is a parasitic disease caused by the development of human botfly larva in the skin. It affects people living in tropical countries and travelers returning from these countries and concerns a number of medical specialties. One form of treatment involves surgical extraction of the parasites. PATIENTS AND METHODS: We report the case of a 47-year-old man returning from Guyana presenting two furuncle-like nodules of the skin on the right buttock and on the right shoulder blade. Extemporaneous intraoperative macroscopic examination of the buttock nodule resulted in diagnosis of myiasis caused by the human botfly, Dermatobia hominis. DISCUSSION: The diagnosis of furuncular myiasis is made primarily on clinical grounds and should be suspected on observation of an abscess in subjects returning from a tropical region. It is consequently rare to find D. hominis in biopsy specimens. In the present case, macroscopic examination showed an extremely rare image of the edge of the intact larva in a longitudinal cut, which to our knowledge has never been published to date.

Out of 344 patients with newly diagnosed non-Hodgkin's lymphoma (NHL), this study identified 16 patients presenting Burkitt-like cells (BLCs) after cytological and/or histological review. Conventional cytogenetic analysis showed at diagnosis complex chromosomal abnormalities in 13 cases and a normal karyotype in three cases. However, neither t(8;14)(q24;q32) nor the variants t(2;8)(p12;q24) or t(8;22)(q24;q11) was detected. FISH studies showed c-MYC amplification in all cases with four to more than seven copies in 10 - 77% metaphase or inter-phase cells. This study did not observe any gene fusion signal for c-MYC/IgH excluding a t(8;14) translocation and partial tri or polysomy of chromosome 8. It also excluded in that cases a break apart for the c-MYC locus. This study also never detected IgL/c-MYC, IgK/c-MYC or X-c-MYC. The BLCs were present whatever the lymphoma sub-type: follicular lymphoma (FL) was diagnosed in six out of 16 patients, mantle cell lymphoma (MCL) in four out of 16 patients, marginal zone lymphoma (MZL) in two out of 16 patients and diffuse large B-cell lymphomas (DLBCL) in three out of 16 patients. One additional patient presented a T-cell lymphoma. The clinical course was aggressive with a poor prognosis, as death occurred in nine patients, within 6 months after diagnosis for eight of them. These data could suggest a sub-group of NHL patients (15 B-NHL, 1 T-NHL) have been identified with a poor prognosis characterized by the association of Burkitt-like cells and c-MYC amplification without t(8;14)(q24;q32) or its variants. The possibility that this profile may represent a distinct morphologic NHL sub-set remains to be determined on a large cohort of patients.

INTRODUCTION: Intestinal metastasis from primary lung cancers is rare and usually asymptomatic. CASE REPORT: We report two cases of symptomatic small bowel metastasis. The first one presented with a persistent anaemia occurring in a 54 year old man who had undergone a left lower lobectomy for a large cell carcinoma. The second case was a 75 year old man who presented with peritonitis which led to discovery of a lung primary. CONCLUSION: Intestinal metastasis can be the presenting feature of, or complicate lung cancer. Symptomatology depends on metastasis localisation. Their treatment is difficult and they are associated with a poor prognosis.

RESUMO

Intestinal angiostrongyliasis caused by Angiostrongylus costaricensis is a rare nematodiasis which is present mainly in Costa-Rica, and in some other countries in the northern part of South America, Central America, and in the southern part of Mexico. This is a report of the first case from Martinique involving a sixteen month old child, and proved by an intestinal pathologic study. The epidemiology of the disease, its clinical, parasitological, histopathologic and therapeutic aspects are emphasized.

RESUMO

Primary signet-ring cell carcinoma of urinary bladder is an uncommon primitive bladder tumor. We report the first case occurring on a diverted neurogenic bladder. Except of adenocarcinoma of urachal origin, about 60 cases have been reported to date. The histogenesis of these tumors remains controversial.
