RESUMO

PURPOSE: To describe chronic intestinal pseudo-obstruction (IPO) syndromes that occur after radiotherapy or chemotherapy (or both) for gynecologic cancer. METHODS: All 48 patients in the study population had a history of gynecologic cancer, treatment with radiotherapy or chemotherapy (or both), and suspected chronic IPO. The final diagnosis was based on clinical symptoms, radiographic imaging, motility studies, and surgical findings. Treatment was expectant for 27 patients and surgical for 21. RESULTS: In six of the 21 surgical patients, the final diagnosis was mechanical obstruction. In the other 15, it was IPO syndrome: six had an idiopathic dysfunction (ID) and nine had a thick fibrinous coating (FC) on the serosal surface. Intestines of these 15 patients had patent lumens but decreased motility. The ID and FC groups differed in mean age, chemotherapy administration, and mean time from radiotherapy to surgery. Symptoms improved in 67% of FC patients compared with 17% of ID patients. Among patients treated expectantly, symptoms improved in 50% of the ID patients and in 38% of the FC patients. Motility studies were useful for distinguishing ID from FC or mechanical obstruction. CONCLUSION: Clinical history and motility studies may assist in diagnosing IPO syndrome in gynecologic cancer patients treated with radiotherapy or chemotherapy (or both) and in identifying patients who might benefit from surgical intervention.

OBJECTIVE: To present the largest single institutional review of demographics, associated primary diseases, and survival of patients receiving home parenteral nutrition (HPN). MATERIAL AND METHODS: We conducted a retrospective review of medical records of all Mayo Clinic patients treated with HPN between 1975 and 1995. The probability of survival was calculated by using Kaplan-Meier analysis. RESULTS: In the 225 study patients requiring HPN (median age, 51 years), the main underlying primary diseases were as follows: inflammatory bowel disease (IBD) (N = 50), nonterminal active cancer (N = 39), ischemic bowel (N = 35), radiation enteritis (N = 32), motility disorder (chronic pseudo-obstruction) (N = 26), and adhesive intestinal obstruction (N = 18). Other conditions included intestinal and pancreatic fistula, refractory sprue, dumping syndrome, and protein-losing enteropathy. The overall probability of 5-year survival during HPN was 60%. The probability of survival at 5 years based on the primary disease was 92% for IBD, 60% for ischemic bowel, 54% for radiation enteritis, 48% for motility disorder, and 38% for cancer. The probability of 5-year survival stratified by age at initiation of HPN was as follows: younger than 40 years, 80%; 40 through 60 years, 62%; and older than 60 years, 30%. Most deaths during therapy with HPN were attributable to the primary disease. Among the 20 patients who died of an HPN-related cause, 11 deaths were from catheter sepsis, 4 from liver failure, 2 from venous thrombosis, and 2 from metabolic abnormalities. CONCLUSION: Survival of HPN-treated patients is best predicted on the basis of the primary disease and the age at initiation of HPN. Patients with IBD and age younger than 40 years have a better 5-year survival in comparison with other groups. Most deaths during treatment with HPN are a result of the primary disease; HPN-related deaths are uncommon.

In cortex, neighboring 0.05 mm minicolumns have distinctly different receptive fields (RFs). Within minicolumns, neighboring cells have very similar RFs, but differ prominently in their stimulus-evoked temporal behaviors. This is reproduced in a cortical model that has strong inhibition and semi-random lateral connections among cells with similar RFs. Within modeled minicolumns, non-linear dynamics amplify small differences in cells' lateral inputs into large differences in temporal behaviors. Cells' stimulus-evoked behaviors, though complex, are surprisingly orderly, in that (1) time courses of cells' responses are very stimulus-specific and (2) in the presence of a stimulus, activities of some cells fluctuate coherently, and the patterns of coherence are also stimulus-specific. Thus the model, like real cortex, represents stimulus information in the overall strengths and temporal patterns of cells' responses and in patterns of temporal coherence among cells.

In the period 1961-84, the number of deaths in Ireland due to the testis cancer rose by 64%. This was due both to significant male population expansion (25.3%) and to an increased mortality rate. In the 25-34 years age group, one of the groups at highest risk, the mortality rate rose by 123%. In contrast, in England and Wales, although the male population has risen by 8.5% since 1961, the number of deaths has decreased by 17%. This is due to falling mortality rates, for example an 18% decrease in those aged 25-34 years. The highest rate of all occurred in those Irish over 75 years old. The changing Irish trends appear to lag behind those in England and Wales by some decades, and this raises important aetiological considerations.

The capacity of 8 neurologically healthy adults to distinguish direction of motion on the skin overlying the mental foramen was determined. The velocity, orientation, and the length and width of skin traversed by the moving tactile stimuli were precisely controlled. Directional sensitivity, d', was found to depend on both stimulus velocity and the length of skin traversed. Since the relationship between d' and velocity at each traverse length was well described by a generalized gamma function, it was possible to quantitatively characterize the effects of changes in traverse length on the relationship between d' and velocity. Specifically, peak (i.e., maximal) directional sensitivity increased as the length of skin traversed was increased, yet the velocity which resulted in peak directional sensitivity (i.e., the optimal or model velocity) remained invariant over the range of traverse lengths investigated (0.35-1.0 cm). The effect of stimulus velocity on directional sensitivity was least at the longest traverse lengths used. The generalized gamma function model fit the relationship between directional sensitivity and velocity equally well at all traverse lengths studied. The results lead us to anticipate that stimuli of the type used in this study should prove valuable for the detection and quantification of disturbances in orofacial tactile spatiotemporal integration in patients with peripheral nerve injury.

RESUMO

Repairs to the external silicone central venous catheter segment are done to correct mechanical defects caused by natural wear and tear or traumatic catheter injury. The ability to repair a damaged catheter obviates the need for surgical replacement. Complications specific to repaired Hickman catheters have not been reported. We report a case in which the metal piece separated from the replacement segment of a Hickman catheter. This metal piece lodged 1 cm from the catheter entrance to the anterior chest and caused incomplete catheter occlusion.

We consider models of the form mu chi = -x + p + WF(x) where x = x(t) is a vector whose entries represent the electrical activities in the units of a neural network. W is a matrix of synaptic weights, F is a nonlinear function, and p is a vector (constant or slowly varying over time) of inputs to the units. If the map WF(x) is a contraction, then the system has a unique equilibrium which is globally asymptotically stable; consequently the network acts as a stable encoder in that its steady-state response to an input is independent of the initial state of the network. We consider some relatively mild restrictions on W and F(x), involving the eigenvalues of W and the derivative of F, that are sufficient to ensure that WF(x) is a contraction. We show that in the linear case with spatially-homogeneous synaptic weight, the eigenvalues of W are simply related to the Fourier transform of the connection pattern. This relation makes it possible, given cortical activity patterns as measured by autoradiographic labeling, to construct a pattern of synaptic weights which produces steady state patterns showing similar frequency characteristics. Finally, we consider the relationships, in the spatial and frequency domains, between the equilibrium of the model and that of the linear approximation mu chi = -x + p + Wx; this latter equilibrium can be computed easily from p in the homogeneous case using discrete Fourier transforms.

The capacity of 4 patients who had previously experienced trauma to their mandibular nerves to distinguish opposing directions of tactile motion over the distribution of the mental nerve was compared to that of 8 neurologically normal adults. Brushing stimuli were delivered to the perioral region and were precisely controlled for their velocity, the length of skin traversed, the width of skin contacted, and the orientation and direction of motion. A temporal, 2-alternative, forced choice method was used to obtain estimates of directional sensitivity, d'. It was discovered that impairment in cutaneous directional sensitivity could be readily detected within areas of hypaesthesia. Although directional sensitivity was found to increase linearly with the length of skin traversed for both the patients and the neurologically normal adults, the slope and the x-intercept of the linear relationship differed between the two groups. The difference in the slope suggests that direction discrimination within the hypaesthetic areas is relatively insensitive to changes in the length of skin traversed. The difference in the x-intercept suggests that a greater length of skin must be traversed before any information about direction is made available at the hypaesthetic sites. The dependency of the capacity of neurologically normal and impaired individuals to process information about direction of tactile motion on the length of skin traversed and the velocity of stimulation suggests that a high degree of stimulus control is required for the detection and quantification of subtle neurosensory deficits.

Twenty-eight right-handed, young adults participated in a sensory testing experiment to evaluate spatial resolution at 10 positionally matched sites on the right- and left-hand sides of the face. An adaptive psychophysical (i.e. tracking) procedure was used to estimate the threshold spatial separation for perceiving two points of contact at each site. Estimates of the threshold at one site on both sides of the face were also obtained with a method-of-limits procedure similar to that employed for clinical evaluation of patients. In addition, each individual was asked to rate (i) his(her) overall facial sensitivity to touch and (ii) the degree to which he(she) could discern subtle changes in lip, cheek and chin position during speech, chewing and facial expression. Analysis of the estimates of the threshold separation obtained with the tracking procedure revealed a significant effect of gender (p < 0.04) and of site (p < 0.001). Females were more spatially sensitive than males: average threshold separations were 1.55 mm less. Most notably, the threshold increased ninefold with distance posterolaterally from the oral opening. The vermilion of the upper lip was the most spatially sensitive site (population geometric mean = 2.4 mm) and the preauricular skin the least spatially sensitive site (20.9 mm). Significant effects of side and of interactions among gender, side and site were not observed. The estimates obtained with the method-of-limits procedure were very similar to those obtained with the tracking procedure: the latter were 0.67 mm less on the average. Individuals' ratings of overall facial sensitivity to touch were similar for males and females (p > 0.70). Females, however, reported greater ability to discern subtle changes in lip, cheek and chin position than males (p < 0.03). The ratings of this sensory function correlated negatively with the estimates of the threshold separation on the vermilion of the upper lip (p < 0.03).

BACKGROUND AND PURPOSE: The purpose of this study was to compile information about and define variables that are influential in the career satisfaction of tenure-track, full-time, physical therapy faculty who have been employed in academia for 5 years or less but who do not yet have tenure. SUBJECTS AND METHODS: An investigator-developed instrument was used to collect the data. The self-report instrument contained 80 items in four categories: demographics, social supports, teaching, and scholarly activity. Questionnaires were sent to junior physical therapy faculty at the 127 physical therapist schools in the United States and Puerto Rico listed by the American Physical Therapy Association in 1993. There were 163 responses to the survey, representing an estimated 85% of the population of junior faculty as defined by this survey. RESULTS: Eighty-three percent of junior faculty surveyed were satisfied with having taken an academic position, despite feelings of loneliness, tenure anxiety, heavy work loads, and the desire for more guidance from colleagues. CONCLUSION AND DISCUSSION: Social and collegial supports such as relationship with senior faculty and experienced colleagues are key elements influencing faculty satisfaction. Information is given that could be utilized by directors and faculty who are planning to guide the professional development of new faculty.

The purpose of this study was to determine whether the attitudes of students in a physical therapy education program toward the elderly could be influenced positively by (1) having classroom discussion led by an individual with expertise in geriatrics and (2) having only individuals aged 65 years or older participate in a specially designed geriatric mock clinic (GMC). Subjects were 47 first-year students divided into a control group (n = 24) and an experimental group (n = 23). Students in the control group participated in two 4-week clinical rotations. Students in the experimental group participated in one 5-week clinical rotation and in one 3-week GMC. Students in both groups were given a pretest and a posttest administration of the Kogan Old People Scale to assess their attitudes toward the elderly. No significant difference was found between the two groups' overall attitude scores on the pretest or the posttest results. Overall, positive attitude scores increased significantly for both groups, although the increase was greater for the experimental group than for the control group. Negative attitude scores decreased significantly for the experimental group, but did not change for the control group. We concluded that both traditional clinical rotations and a specially designed GMC influenced the students positively. Suggestions have been made for the Clinical Instructor who wishes to influence physical therapy students' attitudes to meet the increasing needs of the elderly patient population.

BACKGROUND: Extracorporeal shock-wave lithotripsy (ESWL) is a relatively new technological adjunct in the treatment of renal calculi, but availability is limited. AIM: The aim of this study was to assess the outcome of ureteroscopic procedures in a unit without on-site ESWL facility. METHODS: A retrospective study of all cases of ureteric calculi presenting to this hospital during the period 1991 to 1997 was performed. RESULTS: One hundred and thirty five patients with a mean age of 46.8 years were evaluated. Ninety-four (69.5%) had their stone successfully manipulated and/or extracted at ureteroscopy. Using strict criteria, 41 patients (30.5%) had failed ureteroscopies. Fourteen (10.4%) suffered complications related to their ureteroscopy, thirteen had their complications treated conservatively and three (2.3%) needed open procedures. CONCLUSION: Ureteroscopy has a high success rate and low morbidity. We continue to advocate ureteroscopy in the initial management of ureteric calculi.

BACKGROUND: Patients with prostate cancer with a pre-operative prostate-specific antigen (PSA) >15 ng/ml who undergo radical retropubic prostatectomy (RRP) generally do not have a good outcome, yet may have organ-confined cancer and should be offered the option of surgery. AIM: To assess the outcome of patients who underwent RRP with a pre-operative PSA >15 ng/ml. METHODS: Thirty-four patients, mean pre-operative PSA: 25.46 ng/ml (15.03-76.6) and mean Gleason score: 6.4 (5-9) were assessed. RESULTS: Two groups were identified. Group I: 41% (14/34) have no biochemical recurrence to mean follow up of 58 months (30-106). Mean PSA: 18.8 ng/ml (15.03-25.84). Mean Gleason score: 6.1 (5-7). Clinical stage: T1c in 80%. No patient had seminal vesicle or lymph node involvement. Group II: 59% (20/34) have biochemical recurrence or died (3) from their disease to mean follow up of 66 months (36-98). Mean PSA: 28.9 ng/ml (15.28-76.6). Mean Gleason score: 6.7 (5-9). Clinical stage: T1c in 25%. Eleven patients had seminal vesicle (8) involvement or positive lymph nodes (3) or both (2). CONCLUSION: RRP seems feasible in patients whose pre-operative PSA is between 15 and 25 ng/ml with stage T1c, Gleason score < or = 7 and negative lymph node frozen section.

Because four successive weekly exposures of the gastric mucosa of intact dogs to bile did not alter the appearance or the barrier function of the mucosa during subsequent challenges with bile, the effects of chronic continuous exposure to bile were tested. This was accomplished by diversion of the flow of bile from the duodenum into the stomach by cholecystogastrostomy and diversion of the common bile duct. After four weeks, on endoscopic examination the mucosa was dark red but covered in some areas by a creamy coloured, strongly adherent pseudomembrane. Histologically the mucosa was normal. Ion fluxes, when an acid test solution was used, were close to normal. Differences between control dogs and those with chronic bile diversion became very evident, however, when the mucosa was exposed to increasing concentrations of bile. The control dogs displayed increases in net fluxes of H+, Na+, Cl- and K+ as the concentration of the bile was increased but the dogs with chronic bile diversion did not. Also the changes in PD and fluxes in K+ were less in the dogs with bile diversion. In the intact control dogs bile placed in the stomach always produced bleeding and hemorrhagic erosion; in the dogs with chronic bile diversion added bile in the stomach never caused bleeding and the mucosa appeared normal on endoscopic and histological examination. We conclude that the resistance of the gastric mucosa to the barrier breaking action of bile was increased in the dogs with chronic gastric bile diversion.

Persistent organic pollutants remain a serious threat to many food-chain systems. New pollutants continue to emerge. The present study has created novel extraction vessels which are compatible with readily available commercial instrumentation to validate the analysis of one class of persistent organic pollutants, polychlorinated biphenyls (PCBs), in avian blood. The volumes used can be reasonably sampled without sacrificing individuals, or comprising breeding or migratorial success. The procedure consists of the pressurized solvent extraction (PSE) of analytes in a novel PSE extraction vessel. The new extraction cell contains a 38-cm long, coiled, re-packable, in situ clean-up column. Lipid elimination, using Florisil, occurs within the coiled region of the extraction vessel, eliminating the requirement for post extraction clean-up. For development, 0.2 g samples of chicken whole blood have been used. Extract volumes are reduced from (30 to 10) cm(3), compared to unmodified systems. The new PSE vessel with its integrated clean-up method showed satisfactory performance for the analysis of ten environmentally relevant PCB congeners in chicken whole blood samples with recoveries in the range of (70-130)%. Detection limits using gas chromatography coupled with large volume injection ion-trap mass spectrometry (GC-LVI-ITMS-MS) were in the range of (0.05-0.5) ng g(-1). The relative standard deviations for all congeners investigated were better than 5%. This is the first PSE validation to have been conducted on unaltered whole blood samples.
