RESUMO

The article is devoted to scientific biography of the prominent neuroanatomist, Doctor of Medical Sciences, Professor L. A. Shangina, representative of the scientific school of Tonkov-Bushmakin. Her input into becoming of the chairs of normal anatomy of medical institutes in Irkutsk, Moscow, Kursk, Dushanbe (Tajikistan) and Smolensk and in organization of scientific community of neuromorphologists in the USSR in the middle and second half of the twentieth century is demonstrated. The special attention is paid to the results of scientific studies of L. A. Shangina and her post-graduate students of topography of nerves innervating human heart, of topography of accessory nerve (XI pair of cranial nerves), of structure of receptors of autonomic nervous system in walls of alimentary tract and urinary system.

A semiclassical model describing the charge transfer collisions of C60 fullerene with different slow ions has been developed to analyze available observations. These data reveal multiple Breit-Wigner-like peaks in the cross sections, with subsequent peaks of reactive cross sections decreasing in magnitude. Calculations of charge transfer probabilities, quasi-resonant cross sections, and cross sections for reactive collisions have been performed using semiempirical interaction potentials between fullerenes and ion projectiles. All computations have been carried out with realistic wave functions for C60's valence electrons derived from the simplified jellium model. The quality of these electron wave functions has been successfully verified by comparing theoretical calculations and experimental data on the small angle cross sections of resonant C60+C60 + collisions. Using the semiempirical potentials to describe resonant scattering phenomena in C60 collisions with ions and Landau-Zener charge transfer theory, we calculated theoretical cross sections for various C60 charge transfer and fragmentation reactions which agree with experiments.

Posterior reversible encephalopathy (PRES) is a clinical syndrome characterized by headaches, seizures, cortical blindness, and altered mental status up to coma and by a radiological syndrome defined by edematous alteration on Magnetic Resonance Imaging (MRI)-Scans of the white matter. In the following, we present the case of a 73 years old post-operative patient with PRES in the context of hypertension and endocarditis with E. coli. The initial presentation included a series of seizures, sopor, and cortical blindness. In MRI-Scans a marked cerebral edema could be identified. After successful treatment of underlying conditions, the patient has made a good recovery. However, homonymous hemianopsia towards right due to laminar necrosis in the left occipital lobe remained as a residuum.

RESUMO

The objective of the article is a comparative and legal study of the models for the organization of public procurements in the healthcare field in the EU and Ukraine. Theoretical and empirical methods of cognition were used to achieve this goal, namely, methods of analysis, synthesis, generalization, as well as comparative and legal method in analyzing the experience of the EU countries in organizing public procurements. The conducted study suggests that the public procurement mechanism in the healthcare sector exists in most states. However, the legal regulation in this area differs significantly in different states. It has been established that a significant part of the states use centralized procurements. The shortcomings of the legal regulation of public procurements in Ukraine have been revealed and the ways to solve them have been offered. The authors have indicated the necessity of using international experience in organizing public procurements in the healthcare sector, in particular, with respect to: centralized procurements with the introduction of procurement groups or consortia (Great Britain, France, Germany, Italy); activities of specially created centralized organizations (Austria, Belgium, Spain, Hungary, Portugal) decentralized procurements (Sweden). In order to improve the legal regulation of public procurements in the healthcare field, the authors have proved the necessity of making amendments to the Law of Ukraine "On Public Procurements".

The glaucoma is a leading cause of irreversible blindness in all countries. In the nearest years an increasing of patients with the given pathology is prognosticated. Nowadays, development of disease is related to alteration of interleukin profile in lacrimal fluid. however, diagnostic significance of interleukins is studied insufficiently. The purpose of study. To determine diagnostic significance of interleukins on a local level for amelioration of diagnostic and prognostication of development of glaucoma. The content of interleukins in lacrimal fluid was analyzed in 80 patients with glaucoma stage II and 45 healthy individuals using technique of sandwich-linked enzyme-linked immunosorbent assay with calculation of informativeness of indices of immunity. The results. The analysis established high diagnostic significance of IL-2, IL-17, IL-8 for verification of glaucoma. They are proposed to be applied in laboratory diagnostic and prognostication of disease. The developed regression model ensures with high accuracy prognostication of development of glaucoma in case of increased level of IL-2, IL-17, IL-8.

Ovarian cancer is a common pathology in the structure of tumors among elderly women. The aim of the work is to identify the leading indicators of interleukin profile to improve the diagnosis of ovarian cancer in the elderly. Enzyme immunoassay studied serum levels of interleukins in 78 patients with ovarian cancer stage IIB-IIIB according to the classification of FIGO at the age of 65-70 years and 42 women of similar age without ovarian cancer at the time of the study and in history. The differentiation of the compared groups is carried out by means of discriminant method. The leading diagnostic interleukins for ovarian cancer in the elderly are IL-1α, IL-1ß, IL-8 and TNF-α. The constructed discriminant models allow to differentiate patients with ovarian cancer and control women with an accuracy of 93.2-93.8%. The use of enzyme immunoassay and discriminant analysis improve the diagnosis of ovarian cancer in the elderly.

Analysis of pathomorphological changes which taking place in muscle tissue after reperfusion of previously ischemic rats limbs allowed to identify three phases of the experimental reperfusion syndrome (RS): the first or ischemic, the second or initial reperfusional, the third or late reperfusional. Morphological changes of the skeletal muscles in the first stage are characterized by presence of dystrophic-necrotic processes and reflect the compensatory-adaptive reaction of the organism to hypoxia. In the third stage one can see the progress of morphological damages, which develop during the ischemic period against a background of exhaustion of proteinase inhibitors. This indicates the intensity of endogenous intoxication of the organism with the products of disturbed metabolism and determines the irreversibility of destructive processes and probability of multiple organ failure. Proceeding from the character of the pathomorphological changes and the state of proteinase-inhibitor system one can suppose, that the optimal time for medical measures is the first stage and the first hours of the second stage (to increase the ischemic tolerance of the skeletal muscles). Taking into account the direct relation between the intensity of pathomorphological injuries and the imbalance of proteinase-inhibitor system, the usage of proteinase-inhibiting medicines for correction of RS-development and reduction of the destructive changes during first and second stages is substantiated. When reperfusion syndrome lasts for a long time, medical measures become ineffective due to the high degree of pathological changes.

RESUMO

The results of the long term work of the Russian Scientific Centre of Roentgenology and Radiology on me- dical investigation of the participants in the liquidation of the consequences of Chernobyl power plant acci- dent have been summarized. It has been stated that circulatory system and tumor diseases occupy the leading position in the disease rate among the affected liquidators. The important role of cytogenetic investigation was pointed out. It allows us not only to determine the efficient impact on the human body but also to evaluate the effective dose of radiation, the information about which allows us to predict the development of distant post-irradiation pathology. The results of cytogenetic investigations testify to the interrelation between the level of chromosomal abnormalities and cardiovascular diseases and confirm the clinical data on the non- neoplastic.pathology among the liquidators of the accident.

RESUMO

Conducting postoperative radiotherapy in a mode of hypofranctionation with SFD-3Gy to TFD-36-39Gy (EQD2 = 43,246,8Gy) in combined treatment of patients with non-small cell lung cancer allows significantly increasing a 5-year disease-free survival at IIB-IIIB stages of the disease (pN1-2) as well as the central cancer, squamous cell morphological type of tumor after surgery in a volume of lob-bilobectomy. The clearest effect of postoperative radiotherapy is assessed by survival without locoregional recurrence where radiation therapy in the adjuvant setting allows achieving a statistically significant increase in local control of the disease to the level of 85-95% regardless of stage of the disease, tumor size, regional lymph nodes lesion and the surgical treatment.

It is presented 40-years experience of tracheal adenocystic carcinoma diagnosis and management in 144 patients. Peculiarities of clinical course and choice of diagnostic and therapeutic measures are discussed in relation to stenosis and hypoxia degree and severity of accompanying inflammatory complications of trachea, bronchi and lungs. Technical tools and original methods of resection and reconstruction of trachea and it's bifurcation after extended lung resection are described. Surgical management is carried out in 96 patients: all patients underwent circular resection of trachea (77) or it's bifurcation (19). Morbidity rate was 22.9% and mortality - 3.1%. Additional radiotherapy was carried out in 50 patients. 41 patients underwent radiotherapy alone. Radiotherapy improves remote results of management. 5-and 10-year survival rate was 78.3±6.1 and 45.9±7.9% after surgery alone, 92.0±3.9 and 77.1±6.5% after combined treatment and 76.4±6.5 and 55.0±10.7% after radiotherapy alone respectively.

Sphingolipids play an important role in the development of insulin resistance. Ceramides are the most potent inhibitors of insulin signal transduction. Ceramides are generated in response to stress stimuli and in old age. In this work, we studied the possible contribution of different pathways of sphingolipid metabolism in age-dependent insulin resistance development in liver cells. Inhibition of key enzymes of sphingolipid synthesis (serine palmitoyl transferase, ceramide synthase) and degradation (neutral and acidic SMases) by means of specific inhibitors (myriocin, fumonisin B1, imipramine, and GW4869) was followed with the reduction of ceramide level and partly improved insulin regulation of glucose metabolism in "old" hepatocytes. Imipramine and GW4869 decreased significantly the acidic and neutral SMase activities, respectively. Treatment of "old" cells with myriocin or fumonisin B1 reduced the elevated in old age ceramide and SM synthesis. Ceramide and SM levels and glucose metabolism regulation by insulin could be improved with concerted action of all tested inhibitors of sphingolipid turnover on hepatocytes. The data demonstrate that not only newly synthesized ceramide and SM but also neutral and acidic SMase-dependent ceramide accumulation plays an important role in development of age-dependent insulin resistance.

To confirm the hypothesis about the involvement of EGF-stimulated MAP-kinase ERK1/2 in the regulation of microtubule (MT) system, the influence of two widely used ERK1/2 inhibitors, U0126 and PD98059, on the organization of tubulin cytoskeleton in interphase HeLa cells during EGF receptor endocytosis has been investigated. We have found that addition of U0126 or PD98059 to not-stimulated with EGF ells for 30 min has no effect on radially organized MT system. However, in the case of U0126 addition before EGF endocytosis stimulation, the number of MT per cell decreased within 15 min after such stimulation and was followed by complete MT depolymerization by 60-90 min. Stimulation of EGF endocytosis in the presence of PD98059 resulted only in insignificant depolymerization of MT and it could be detected mainly from their minus-ends. At the same time, MT regions close to plasma membrane became stabilized, which was proved by increase in tubulin acetylation level. This situation was characteristic for all period of the experiment. It has been also found that the inhibitors affect endocytosis dynamics of EGF-receptor complexes. Quantitative analysis demonstrated that the stimulation of endocytosis in the presence of U0126 generated a greater number of endosomes compared to control cells, and their number did not change significantly during the experiment. All these endosomes were localized peripherally. Effect of PD98059 resulted in the formation of lower number of endosomes that in control, but they demonstrated very slow clusterization despite the presence of some intact MT. Both inhibitors decreased EGFR colocolization with early endosomal marker EEA1, which indicated a delay in endosome fusions and maturation. The inhibitors were also shown to affect differently phospho-ERK 1 and 2 forms: U0126 completely inhibited phospho-ERK1 and 2, white, in the presence of PD98059, the two ERK forms demonstrated sharp transient activation in 15 min after stimulation, but only phospho-ERK2 could be detected after 60 min of endocytosis. In both cases, MAP-kinase activation dynamics was significantly different from the control. Our results suggest involvement of EGF-stimulated MAP-kinase pathway in cytoskeleton regulation. At the same time, they demonstrate that the two studied and widely used inhibitors are not equivalent with respect to not only the effect on MAP-kinase activity but also to such interdependent processes such as changes in cytoskeleton organization and signaling receptor' endocytosis.

The aim of the article was to study the occurrence of periodontal diseases in children with type I diabetes mellitus. The examination of 78 children revealed periodontal diseases in 40 children with type I diabetes. OHI-S, CPITN, PMA indices were determined. Pathological changes in periodontal tissues were revealed in 100% of cases. The following were identified: gingival hemorrhage (100%), over - and under-gingival dental tartar (100%), inflammation of gingival papilla (87,5%) marginal (80%) and alveolar gingiva (55%). Spread of periodontal disease among children with I type diabetes is characterized as high and is equal to 100%. Degree of periodontal sickness is evaluated as average and is M=2,28; SD=0,47 according to CPITN index. Treatment and preventive measures should be carried out taking into account major somatic disease.

Ceramides are a novel class of biologically active molecules involved in the regulation of different signaling pathways. Ceramide is involved in regulation of the phospholipase D (PLD) activity and development of cell resistance to insulin. In this work, we have studied age-related features of insulin regulation of PLD activity and glucose metabolism in intact cells and modeled their resistance to insulin by exogenous ceramide and palmitic acid. Contents of ceramides and of free fatty acids (FFA) are found to increase with age, as well as on incubation of liver cells of young rats in the presence of the ceramide precursor palmitic acid. Under these conditions, the ability of insulin to activate PLD, the cell uptake of glucose, and glycogen synthesis sharply decreased. On incubation of hepatocytes of young animals in the presence of exogenous C2-ceramide, the contents of endogenous ceramides increased but not the contents of FFAs and of neutral lipids. These events were accompanied by suppression of the insulin-induced production of phosphatidylethanol (a result of ethanol transphosphatidylation by PLD), glucose uptake, and glycogen synthesis. Incubation of insulin-resistant liver cells of young rats and also of hepatocytes of old rats in the presence of myriocin (an inhibitor of the de novo synthesis of ceramide) was associated with a decrease in ceramide content in the cells and an increase in the cell sensitivity to insulin. The findings indicate an important role of ceramide in disturbance of insulin signaling due to inhibition of the PLD-dependent link in the liver cells of old animals.

The aims of the study were to determine fecundity of several strongylid species parasitizing domestic horses and analyze possible relations between numbers of eggs in female uteri and size of both the eggs and the nematodes as well as the influence of fecundity on proportion of species in the strongylid community. Twenty-five specimens from each of 15 strongylid species (Strongylus vulgaris, Strongylus edentatus, Triodontophorus serratus, Triodontophorus brevicauda, Triodontophorus tenuicollis, Cyathostomum catinatum, Coronocyclus coronatus, Cylicocyclus nassatus, Cylicocyclus insigne, Cylicocyclus leptostomus, Cylicostephanus calicatus, Cylicostephanus goldi, Cylicostephanus longibursatus, Cylicostephanus minutus, and Poteriostomum imparidentatum) collected after necropsy were studied. The reproductive system was extracted from the female body; all eggs were removed, counted, and measured under a light microscope. Significant differences in number of eggs in female uteri of various strongylid species were observed (Kruskal-Wallis test, p < 0.001); the least numbers of eggs were registered in C. longibursatus (average = 49) and C. leptostomus (63) and the largest number in S. edentatus (5,918). Significant correlation between nematode body size and number of eggs was observed (p < 0.001). Correlation between size of eggs and body size was insignificant (Spearman R = 0.11, p = 0.70). Negative correlation was observed between number of eggs in female uteri and proportion of these species in strongylid community (Spearman R = -0.78, p < 0.001). Multiple linear regression of species proportion in the community on three predictors (number of eggs, body size, and egg size) was not significant (p > 0.05). However, the question on influence of fecundity on proportion of species in strongylid community needs further studies.

In this paper, we report our investigation of the translational energy relaxation of fast S((1)D) atoms in a Xe thermal bath. The interaction potential of Xe-S was constructed using ab initio methods. Total and differential cross sections were then calculated. The latter have been incorporated into the construction of the kernel of the Boltzmann equation describing the energy relaxation process. The solution of the Boltzmann equation was obtained and results were compared with those reported in experiments [G. Nan, and P. L. Houston, J. Chem. Phys. 97, 7865 (1992)]. Good agreement with the measured time-dependent relative velocity of fast S((1)D) atoms was obtained except at long relaxation times. The discrepancy may be due to the error accumulation caused by the use of hard sphere approximation and the Monte Carlo analysis of the experimental data. Our accurate description of the energy relaxation process led to an increase in the number of collisions required to achieve equilibrium by an order of magnitude compared to the number given by the hard-sphere approximation.

RESUMO

Our study was conducted in patients with primary squamous cell carcinoma of the trachea and adenoid cystic carcinoma (191) (radical surgery--90; combined treatment--101). The former pathology was relatively more aggressive. Combined treatment proved more effective as compared with surgery alone (5- and 10-year survival after surgery was 79 and 64%; combined treatment--92 and 79%, respectively).

RESUMO

The paper deals with the importance of treatment of primary cancer of the trachea and, in particular, the experience of the Center in using radical therapy for inoperable squamous cell tumors and adenoid cystic carcinoma. An original system of tumor staging developed at the Center draws on international classifications (TNIVI) and methods of radiotherapy depending on tumor localization. It is shown that tracheal malignancies, especially adenoid cystic carcinomas, are characterized by relatively high radiosensitivity.

RESUMO

The authors summarize the results of investigations of specific anatomical features of paranasal sinuses in more than 12,000 subjects. The patients with suspected cerebral pathology and lesions in the paranasal sinuses were examined using computed tomography. Structural variants of ethmoid bone and ethmoidal labyrinth, nasal septum, maxillary, frontal, and sphenoidal sinuses are described. The morphometric analysis revealed the zonal relationship between different structures of nasal mucosa and the new variants of regulatory structures in the cavernous sinus vessels.

RESUMO

The paper deals with immediate and end results of surgical treatment of 435 elderly patients (70-years-old or older) with gastric cancer and the specifics of postoperative complications influencing the postoperative course of the disease. Metastasis to the regional lymph nodes appeared to be a prognostic factor of crucial importance in the latter period: 5-year recurrence-free (N0) survival was 60.3% +/- 5.2 vs. 17.6% +/- 10.3 (in 1-6 lymph nodes) and 5.2% +/- 5.0 (in 7-15 lymph nodes) (N1).
