BACKGROUND: As the gatekeepers of rural residents' health, teams of village doctors play a vital role in improving rural residents' health. However, the high turnover of village doctors, both individually and collectively, threaten the stability of village medical teams. This research evaluated the influence of job satisfaction, resilience, and work engagement on the village doctors' turnover intention, and explored the mediating role of work engagement and resilience between job satisfaction and the turnover intention of village doctors in China. METHODS: A quantitative study using a self-administered questionnaire containing mostly structured items was conducted among village doctors with a sample size of 2693 from 1345 rural clinics in Shandong province, China, during May and June 2019. All variables including demographic characteristics, job satisfaction, resilience, work engagement and turnover intention were based on available literature, and measured on a 5- or 6-point Likert scale. Such statistical methods as one-way ANOVA, bivariate correlation, exploratory factor analysis (EFA), and Structural Equation Modelling (SEM) were used. RESULTS: Up to 46.9% of the subjects had a higher turnover intention and more than 26.3% of them had a medium turnover intention. The job satisfaction of village doctors could not only have a direct negative effect on turnover intention (ß = - 0.37, p < 0.001), but also have an indirect effect through work engagement (ß = - 0.04,=< 0.001). Meanwhile, work engagement also had a direct negative impact on turnover intention (ß = - 0.13, p < 0.001), and resilience had an indirect negative impact on turnover intention through work engagement (ß = - 0.09, p < 0.001). The above results of this study strongly confirmed that job satisfaction, resilience, and work engagement were early, powerful predicators of village doctors' turnover intention. CONCLUSION: According to the results, the following should be taken seriously to improve job satisfaction: reasonable and fair income, effective promotion mechanism, fair social old-age security, reasonable workload, and strong psychological coping mechanisms for work stress. The turnover intention of village doctors could be reduced through improving job satisfaction, resilience and work engagement.

BACKGROUND: Osteoarthritis (OA) was a chronic degenerative joint disease. The dysregulation of circular RNAs (circRNAs) has been identified in OA progression. However, the function and regulation mechanism of circ_0114876 in OA remains largely unknown. METHOD: Firstly, we used LPS-treated C28/I2 cells as a cellular model of OA. Quantificational real-time polymerase chain reaction (qRT-PCR) was used to determine the expression levels of circ_0114876, miRNA-1227-3p, and ADAM10 in OA chondrocytes. Cell Counting Kit-8 (CCK8), 5-ethynyl-20-deoxyuridine (EdU) incorporation assays, flow cytometry, Enzyme-linked immunosorbent assay (ELISA) kit, and western blot were applied to confirm cell proliferation, apoptosis, inflammation, and extracellular matrix. of circ_0114876 in vitro. The interaction between circ_0114876 and its downstream target (miR-1227-3p) and mRNA target ADAM metallopeptidase domain 10 (ADAM10), was evaluated by luciferase assay and RNA immunoprecipitation (RIP) assay. RESULT: Circ_0114876 and ADAM10 were upregulated and miR-1227-3p was decreased in OA tissues and LPS-treated chondrocytes. Low expression of circ_0114876 promoted proliferation and inhibited apoptosis, inflammation, and extracellular matrix of the LPS-treated chondrocytes. Mechanistically, circ_0114876 functioned in human chondrocytes through targeting miR-1227-3p and ADAM10. Furthermore, miRNA-1227-3p inhibitor reversed the effect of circ_0114876 knockdown on the OA chondrocytes, and ADAM10 overexpression reversed the effect of miR-1227-3p mimic on the OA chondrocytes. CONCLUSION: Circ_0114876 was increased in OA tissues and cells. Circ_0114876 facilitated the progression in the LPS-induced OA cell model via regulating the miR-1227-3p/ADAM10 axis. This study would provide a potentially effective therapeutic strategy for OA progression.

The overlying strata of the lower coal seam is easy to be collapsed causing the roof caving accident at the end face of the mining working face under repeated mining in close-distance coal seams. In order to predict the roof instability of the end face, the mechanical model of the granular arch structure is established in this study to further analyze its main influencing factors. The results show that the mining height of the working face, the advancing speed, the distance of coal seams, the tip-to-face distance, the strength of the surrounding rock and the support setting the load of the support are the main influencing factors on the roof caving of the end face. Subsequently, the prediction model of roof instability in the end face under repeated mining is constructed through the radial basis function neural network (RBFNN) and the above main influencing factors are regarded as input layer indexes. Meanwhile, the roof subsidence, coal wall deformation and support load are determined as the output layer indexes. The predicted results are closer to the results of sample tests. Finally, the early warning system, including monitoring and early warning, data query, emergency management, user management, and system settings, is designed to monitor roof conditions of the end face and timely warn the roof accidents. The field application proves that the system has good practical value, which is of great significance to intelligent prediction of coal mine stope disaster and prevent the end face roof disaster under repeated mining and. This will promote the safe and efficient construction of coal mine production.

Aiming at the problem of coal face failure of lower coal seam under the influence of repeated mining in close coal seams, with the working face 17,101 as a background, the coal samples mechanics test clarified the strength characteristics of the coal face under repeated mining, through similar simulation experiments, the development of stable roof structure and surrounding rock cracks under repeated mining of close coal seams are further explored. And based on this, establish a coal face failure mechanics model to comprehensively analyze the influence of multiple roof structural instabilities on the stability of the coal face. Finally, numerical simulation is used to further supplement and verify the completeness and rationality of similar simulation experiment and theoretical analysis results. The results show that: affected by repeated mining disturbances, the cracks in the coal face are relatively developed, the strength of the coal body is reduced, and the coal face is more prone to failure under the same roof pressure; During the mining of coal seam 17#, the roofs of different layers above the stope form two kinds of "arch" structures and one kind of "voussoir beam" structure, and there are three different degrees of frequent roof pressure phenomenon, which is easy to cause coal face failure; Under repeated mining of close coal seams, the roof pressure acting on the coal face is not large. The main controlling factor of coal face failure is the strength of the coal body, and the form of coal face failure is mostly the shear failure of soft coal. The research results can provide a theoretical basis for coal face failure under similar conditions.

Aiming at the low mining rate in mines, Xingelao, Dabianyao, and Dongliang Coal Mines in Shenmu Mining Area, Shaanxi Province, China were taken as research objects. Based on this, this study constructed an evaluation index system for the mining capacity of the mines from the perspectives of geological factors, mechanical equipment, humans, and mining design. Moreover, the factors influencing the mining capacity of the mines were evaluated using a combination weighting approach based on an improved analytic hierarchy process and an entropy weight method. A standard cloud was generated based on the mapping standards of each index and a comprehensive cloud was obtained according to comprehensive weight and a backward cloud algorithm. Finally, by combining the comprehensive cloud with local and overall scores of the mines, the mining capacities of the mines were evaluated. The research results demonstrate that the key factor restricting the mining capacity of the mines is the geological environment and five major third-grade indexes affecting mining capacity are igneous rock intrusion, collapse column, scouring zone of the ancient river bed, mechanization level and coal pillar width. In addition, the corresponding suggestions and measures were put forward according to the main factors influencing the mining rate of the mines. In accordance with the weights and scores of each index, the overall scores of the mines were calculated. Dongliang, Dabianyao, and Xingelao Coal Mines were ranked in order based on scores. The research results provide a theoretical basis for improving the mining capacity of the mines under similar geological conditions.

BACKGROUND: Knee osteoarthritis (KOA) is the most common cause of musculoskeletal pain and disability worldwide. Chinese herbal fumigation, an external therapy, is commonly used for the treatment of KOA, while there is no systematic review or meta-analysis designed to evaluate the effects of Chinese herbal fumigation on KOA. METHODS: Seven databases including Cochrane Central Register of Controlled Trials, EMBASE, PubMed, China National Knowledge Infrastructure, Wanfang data, VIP, and Chinese Biomedical Literature Database will be searched up to October 31, 2020. Data that meet the inclusion criteria will be extracted and analyzed by using RevMan V.5.3 software. Two reviewers will assess quality of the included studies based on the Cochrane Collaboration risk of bias tool. The funnel plot and Begg test will be used to evaluate publication bias. And Grading of Recommendations Assessment, Development and Evaluation will be employed to assess the quality of evidence. RESULTS: This study will provide high-quality evidence for Chinese herbal fumigation for the treatment of KOA in terms of effectiveness and safety. CONCLUSION: This systematic review will provide evidence to help us confirm the clinical efficacy of Chinese herbal fumigation in the treatment of KOA.

Background: Postoperative cognitive dysfunction (POCD) is a common surgical complication in elderly patients undergoing hip and knee replacement. Electroacupuncture (EA) may have a protective effect on postoperative cognitive function, but relevant evidence remains uncertain. Objective: To systematically evaluate the evidence of EA for the prevention of POCD after total joint arthroplasty. Methods: PubMed, Embase, Cochrane Central Register of Controlled Trials (CENTRAL), China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP, and Chinese Biomedical Literature Database (CBM) databases were searched until May 1, 2021. Randomized controlled trials (RCTs) in which patients undergoing hip and knee replacement pretreated with EA for preventing POCD were included. The risk of bias was assessed by the Cochrane Collaboration tool. Meta-analysis was performed using Review Manager version 5.4. Results: A total of 11 RCTs with 949 patients were identified. Meta-analysis showed that compared with controls, EA pretreatment significantly reduced the incidence of POCD at 1, 3, and 7 days and 3 and 6 months after the operation. EA was also superior in improving the Mini-Mental State Examination (MMSE) scores on the third postoperative day, but not on the first postoperative day. Neuron-specific enolase (NSE) and interleukin-1ß (IL-1ß) in the EA group were significantly lower than that in the control group. There was no difference in S100ß between the EA group and the control group. Compared to the control group, tumor necrosis factor-α (TNF-α) levels were not significantly lower in the EA group at postoperative hour 0, while significantly decreased at postoperative hours 24 and 48. Conclusion: Our results suggest that EA pretreatment is an effective adjunctive therapy for reducing the incidence of POCD for patients receiving total joint replacement surgery. Its effect was embodied in improving the MMSE scores and NSE, IL-1ß, and TNF-α levels, whereas it had no significant effect on S100ß levels. Meanwhile, the benefits of EA for improving POCD need further strengthening and support from more large-scale, high-quality, and good-homogeneity RCTs.
