Сhronic endometritis is a medical and social problem that leads to impaired reproductive health of women. Even after elimination of the causative agent of inflammation detect changes in the endometrium by determining immunohistochemical markers (СD 138). The purpose of this study is to analyze the incidence of chronic endometritis in women of reproductive age with reproductive health disorders.; We performed hysteroscopy and determination СD 138 30 patients with recurrent pregnancy loss, 47 women with primary infertility and 36 women who applied for a pregnancy planning examination and endometrial polyps detected during routine ultrasound. In patients with habitual miscarriage, primary infertility, and women in the planning of pregnancy and endometrial polyps, a high frequency of bacterial vaginosis and recurrent inflammatory diseases of the lower parts of the reproductive system was noted (p>0.01). In the study of infectious agents of the lower parts of the reproductive system of patients of all groups found in the pathological growth of ureoplasmic infection and a group of non-specific infections (Enterococcus faecalis, Esherichia coli, Proteus sp, Klebsiella sp, S. aureus, agactactia) (p>0.05). Hysteroscopic changes in endometrial pathology were present in 57% of women with habitual miscarriage and primary infertility (p>0.05). In immunohistochemical examination of the endometrium, chronic endometritis was diagnosed in 80% of patients with habitual miscarriage, in 55% of women with primary infertility and in 61% of women when planning pregnancy and polyps of the endometrium (p>0.01).

RESUMO

Videothoracoscopy constitute a secure miniinvasive method of diagnosis of intrathoracic lymphadenopathy syndrome. Pulmonary hemorrhage and injury constitute intraoperative videothoracoscopic complications, and pulmonary collapse, hemorrhage, purulent complications ­ postoperative complications. Satisfactory intraoperative visualization, guaranteeing optimal position of the patient's body on operative table and sufficient pulmonary collapse on the intervention side, application of medical аlpha­cyanacrylate adhesive with hemostatic sponge for hemostasis in a biopsy zone, systemic application of antibiotics constitute the main prophylactic methods for videothoracoscopic complications and optimization of conditions for videothoracoscopic biopsy of intrathoracic lymphatic nodes. Application of the methods proposed have permitted to reduce the intraoperative complications rate from 19.2 tо 2.8%, and a postoperative one ­ from 23 tо 2.8%.

RESUMO

In Department of Thoracic Surgery and Invasive Methods of Diagnosis in 2008-2014 yrs 132 biopsy specimen of intrathoracic lymph nodes with further cytological and histological investigation, taken in patients, who were treated for the intrathoracic lymphadenopathy syndrome of unknown etiology, were analyzed. Sensitivity, specificity and general accuracy of cytological and histological diagnosis were determined, while applying various methods of the biopsy material selection.

Enhancement of rate of the intrathoracic lymphadenopathy syndrome (ILS) revealing have caused a necessity to find out the informative, secure and miniinvasive method of its diagnosis. There was established, that sensitivity, specificity and accuracy of videothoracoscopic (VTS) biopsy of intrathoracic lymph nodes (ILN) have constituted, accordingly, 96.0, 98.1 and 97.1%, and of videoassisted thoracoscopy (VATS) - 93.8, 93.6 and 93.5%. Low frequency of intraoperative and postoperative complications (accordingly 4.4 and 7.7%), small intraoperative blood loss (29 ml), short duration of operative intervention (45 min), rapid rehabilitation of patients postoperatively (10.6 days) witnesses about expediency of application of a VTS biopsy and VATS as a safe and miniinvasive procedures in ILS.

Conducted a retrospective analysis of the results transpleural (videothoracoscopic and videoassisted) biopsy of intrathoracic lymph nodes (ITLN) performed in 91 patients with hilar lymphadenopathy syndrome of various etiologiy for the period from 2003 to 2014. Endoscopic ITLN biopsy performed in the mediastinum in the high risk zone, so patients need to create adequate and safe intraoperative comfort, in particular, the position of lying on side, imposing artificial pneumothorax before surgery, one-lung ventilation, lifting the head end of the operating table at 300. These measures provide optimal con- ditions for handling and minimizing the frequency of intraoperative complications or prevention after surgery.

Highly oriented pyrolytic graphite (HOPG) is an inert substrate with a structural honeycomb lattice, well suited for the growth of a two-dimensional (2D) silicene layer. It was reported that when Si atoms are deposited on the HOPG surface at room temperature, they arrange into two configurations: silicene nanosheets and three-dimensional clusters. In this work we demonstrate, by using scanning tunneling microscopy (STM) and Raman spectroscopy, that a third configuration stabilizes in the form of Si 2D nanosheets intercalated below the first top layer of carbon atoms. The Raman spectra reveal a structure located at 538 cm-1 which we ascribe to the presence of sp2 Si hybridization. Moreover, the silicon deposition induces several modifications in the graphite D and G Raman modes, which we interpret as experimental evidence of the intercalation of the silicene nanosheets. The Si atom intercalation at room temperature takes place at the HOPG step edges and it detaches only the outermost graphite layer inducing a strong tensile strain mainly concentrated on the edges of the silicene nanosheets. Theoretical calculations of the structure and energetic viability of the silicene nanosheets and of the strain distribution on the outermost graphite layer and its influence on the Raman resonances support the STM and Raman observations.
