RESUMO

In this paper the authors describe the clinical efficacy in treatment of vertigo of various etiology. A group of 31 patients were treated with Vertigoheel medication: 14 patients suffered from vertebrobasilar arterial insufficiency, 8 patients were diagnosed as Meniere's disease, 5 patients complained of vertigo of traumatic origin and 4 patients suffered from neuronitis vestibularis. The authors found regression of clinical symptoms in the majority of cases in the investigated group who were treated with Vertigoheel.

RESUMO

Indications for hearing implants in profoundly deaf patients have been presented. Auditory pathway status was evaluated by application of electric stimulation of cochlear nerve. The electric stimulus originated from Cochlear Nerve Tester. Obtained sound sensations might be revealed on ABR pattern.

The authors present a graphical method for the assessment of the sensitivity of the vestibular system. With the aid of this method it is possible, with a high degree of probability, to exclude the existence of disturbances (80%) or confirm the presence of disease.

101 past workers of artificial fibres plant (average age 50.5 years +/- 6.1, average length of employment 21.8 years +/- 6.0, disabled due to recognized chronic carbon disulphide poisoning of various clinical forms, have been examined. Apart from routine ENG examination (recording at rest and after kinetic, caloric and optokinetic stimulation), the pendular swivel chair test has been carried out. Tönnies electronystagmograph coupled with electronically controlled swivel chair D-420 has been used, programmed for pendular motion with angular acceleration values 1 degree/sek2. Analysis of electronystagmographic records after pendular stimulation has revealed the characteristic intermittent nystagmus reaction in 50.5% of subjects. Occurrence of such nystagmus pattern increased with patients age and CS2 exposure duration, amounting in particular clinical forms of poisoning to the following: 91% in encephalopolyneuropathy, 70% in encephalopathy and 35% in polyneuropathy. The authors regard the electronystagmographic evaluation of vestibular system done with the pendular swivel chair as significant aid in diagnosing toxic labyrinthopathies due to CS2 poisonings. On the other hand, intermittent nystagmus reaction may be indicative of the central nature of the lesions.

RESUMO

35 males aged 48--65 were spirometrically examined prior to treatment and following an operation. Excision of neoplastic larynx and tracheostoma breathing were found to result in defected lung ventilation. The defected lung ventilation depends on the size of neoplastic infiltration and its location, but primarily on the degree of narrowing of larynx lumen.

RESUMO

Sonometric and dosimetric measurements of noise at a dozen workplaces of three divisions in a refinery-petrochemical plant have been made. The equivalent level allowable with a view to hearing protection was found to be exceeded by 1-3 dBA at some workplaces. The tonal audiometric test was used to determine threshold levels for 250-8000 Hz frequencies in 122 workers. The present acoustic conditions at the test workplaces do not pose a risk of a significant hearing loss. Only a slight high-frequency hearing loss which might result from noise exposure was diagnosed. Not a single case of occupational hearing impairment conformable with the mandatory diagnostic and certificatory rules was reported.

The authors have presented results of examinations of the smell and taste senses in 547 petrochemistry, workers, and 100 cotton industry workers composing a control group. In both groups, those aged 26 -- 35, with long duration of employment, predominated. In the primary group i.e. petroleum chemistry workers, dysgeusia has been found in 319 (58.3%) subjects with no reaction at all in 102 (18.6%) persons. Hyposmia has been found in 238 (43.5%) and anosmia in 50 (9.2%) subjects. In the control group, dysgeusia has been found in 69%, whereas anosmia in 24% of subjects.

RESUMO

The prevalence of occupational hearing impairment constantly increases, thus there are more and more people who become deaf and disabled from carrying on their jobs and normal social contacts. The technical efforts aimed at limiting exposure to noise, as well as medical prophylactic activities are not efficient enough. Therefore the authors analyse the possibilities of occupational hearing impairment treatment. Pharmacologic preparations likely to affect the temporary and permanent threshold shift, are considered. Within operative treatment, microsurgical technique of implanting microelectrodes to the cochlea is presented, basing on the authors' experiments with 4 patients op]erated and rehabilitated with this method.

The author presents 12 theories which try to evaluate the significance of paranasal sinuses. A vast amount of references on this subject and additionally this paper do not explain the role of paranasal sinuses in human. In the author's opinion the most probable is the amortizative and constructive theory based on antropometric studies and experimental head traumas.

The author presents his own experience in surgical treatment of otosclerosis in 373 patients. He particularly exposes results concerning hearing level after surgical procedures and discusses progress of symptoms which accompany otosclerosis like vertigo or dizziness and tinnitus in the context of bibliography data. He also looks at the complications occurring during procedures and following them. Worth noticing is presentation of long term results after 10-year follow-up.

The most often used skin-flap techniques in one-step and two-steps closed tympanoplasties are discussed. The correct plastic surgery of the meatal skin gave good results. In the ENT Clinic of the Medical Academy in Lódz techniques most often used are two-flaps tympanoplasties with anterior and posterior pedunculated flaps or tympanoplasties with a single pedunculated flap.

The authors described the results of distant field registration of evoked responses from the vestibular part of acoustic nerve. The investigations were carried out as a preliminary pilot study and were successful in obtaining the repeatable responses in guinea pigs. The detailed informations about the stimulation, reception and average technique equipment were enumerated. The variant proposition of the extraction of side motor disturbances was done.

The clinical analysis of the patients after tympanic membrane reconstruction with the temporal muscle fascia has been conducted. The total number of 145 patients, operated at the Otosurgery Department Medical University of Lodz, was divided into 2 Groups. Group 1 (n = 75) patients with chronic otitis media (central perforation and mobile ossicular chain). The preoperative mean conductive hearing deficit of 20 dB was observed in all evaluated patients. After surgery a satisfactory hearing improvement was observed in 90% of the patients. Group 2 (n = 70) patients with the reconstructed postero-superior part of the tympanic membrance and if necessary external auditory canal wall reconstruction. The mean conductive hearing deficit of up to 30 dB was observed before the surgery. Postoperatively in 75% of patients the hearing improvement was observed as well as the satisfactory healing of fascia implant. The postoperative evaluation indicates a better hearing improvement after the myringoplasties with the prefascial lamina than in the myringoplasties performed in the traditional ways.
