OBJECTIVE: The incidence of otitis media with effusion in children with cleft palate is high. There are numerous reports looking at early insertion of ventilation tubes (VT) with associated complications. We believe that this is the first paper that discusses the use of hearing aids (HA) as the first line of management. METHODS: Children with cleft palate are managed in a special multidisciplinary clinic in our hospital. Detailed records of these children are maintained. We studied the otological management of 70 children with repaired cleft palate. RESULTS: 12 of the 70 (17.1%) had VT inserted, 17 (24.3%) were provided with HA, 14 (20%) had both grommets and HA and 27 (38.6%) had neither. Of the 31 (44.3%) children who had HA, 16 (51.6%) had good compliance with the HA. The hearing, speech and language developments in these children have been good. Twelve of the 70 patients (17.1%) have had one or more otological complications. These were significantly higher in children treated with VT. CONCLUSION: We have successfully treated 62.9% of our patients with non-surgical intervention, and show a low incidence of long term complications.

The clinical notes of all new patients with T(1) squamous cell carcinoma of the glottis seen in one head and neck cancer unit between 1989 and 1996 were reviewed. Fifty-three patients were treated with radical radiotherapy and of these 42 (79.2 per cent) had no loco-regional recurrence, after a median follow-up of seven years. Eleven (20.8 per cent) developed local recurrence and were treated with salvage surgery. Fourteen of the 53 (26.4 per cent) tumours involved the anterior commissure and eight of these 14 (57.1 per cent) developed recurrence, whereas only three of the 19 (15.8 per cent) tumours arising from the anterior half of the fold but not involving the anterior commissure had recurrence. None of the remaining tumours recurred. This difference is statistically significant (p<0.001). Anterior commissure involvement is a predictor of poor response to radiotherapy. This may be the result of understaging as none of the cases had computed tomography (CT) scans performed. Technical radiotherapy factors may also be important, although in all cases of anterior commissure involvement steps were taken to ensure adequate radiation dose to this region.

RESUMO

The presence of heterotopic gastric mucosa in the upper oesophagus has been reported to occur in up to 10 per cent of individuals but it is usually asymptomatic. We present two patients with symptomatic oesophageal heterotopic gastric mucosa and discuss the aetiology, pathogenesis, and management of the condition.

RESUMO

Pott's puffy tumour is a rare clinical entity in this era of antibiotics. It is usually seen as a complication of frontal sinusitis. This is the first report of Pott's puffy tumour presenting as a complication of maxillary sinusitis. This is also the first reported case of Pott's puffy tumour treated with debridement and gentamicin beads. We discuss the clinical presentation and successful treatment of this rare disease.

RESUMO

Angiosarcomas are uncommon malignant tumours of vascular endothelium. In the head and neck, the scalp is the commonest site of origin. A rare case of spindle-cell angiosarcoma of the oropharynx, treated by surgery and post-operative radiotherapy, is reported. We discuss the clinical presentation and histopathological diagnosis of this lesion.

RESUMO

Basal cell carcinoma (BCC) is the most common malignancy of the body and most frequently occurs in the head and neck. 'Problematic aggressive' BCCs are either frequently recurrent, often after histologically confirmed clearance, or are 'giant/horrifying' and invasive of critical anatomy. Three patients that illustrate different clinical features and outcomes of 'problematic aggressive' craniofacial BCCs are reported that required craniofacial resection and reconstruction with microsurgical free flap cover, and a re-evaluation of our care pathway for these uncommon presentations.

Tonsillectomy, traditionally performed using reusable instruments (RIs), has recently been performed using disposable instruments (DIs), to prevent the theoretical risk of spreading new variant Creutzfeldt-Jakob disease. Following this, there have been concerns of increased risk of complications, especially haemorrhage. We audited our hospital's complication rates of 100 tonsillectomies performed using RI and 100 using DIs. We analysed the overall complication rates and the postoperative haemorrhage rates. and compared them with the broader groups of RIs versus DIs. We also compared patients who did not have diathermy with those who did have diathermy. The overall complication rate was 6% with RIs and 33% with DIs and the haemorrhage rate was 1% and 12%, respectively. With DIs, these rates were significantly higher in the diathermy subgroup. We believe this is the first study to look at the complications due to DIs.

RESUMO

Recurrent acute tonsillitis, peritonsillar infections and quinsy (peritonsillar abscess) are common clinical conditions that present to a general practitioner. Management in general practice and in hospital is confounded by apparently conflicting treatment options in the medical literature. We have developed clear guidelines as part of an ongoing audit cycle which include performing quinsy or 'hot' tonsillectomy in selected cases, alongside conservative treatments to ensure the speedy recovery and minimal hospitalisation of all patients, with resultant cost benefits.

There are few reports on the long-term results of laser-assisted uvulopalatoplasty (LAUP), with some casting doubt about its long-term efficacy. We report satisfactory results in patients with more than four years' follow-up. We conducted a retrospective study of patients who underwent LAUP at the Princess of Wales Hospital over a period of five years. Thirty-three patients underwent LAUP as a day case procedure. They had had a mean follow-up period of four years and five months. Twenty-eight of these were asked details of their pre- and postoperative levels of snoring regarding frequency, volume and disturbance to their partners. There was a 50% reduction in frequency of snoring, a 51.85% reduction in volume and a 53.7% reduction in disturbance to their partners. All these figures are statistically significant (p<0.001). Overall, 60.8% of the patients felt the surgery was successful and had reduced their snoring to acceptable levels. We conclude that LAUP is an effective treatment for snoring, even in the long term.

The Carhart notch is a dip in the bone conduction at 2000 Hz without a corresponding dip in the air conduction. The main objective of this study was to establish how reliable is the presence of the Carhart notch in a preoperative audiogram in predicting the presence of glue at myringotomy. A prospective study has been carried out in 50 children presenting with glue ear to find out the association between the Carhart notch and the presence of glue at myringotomy. Children were seen before the operation and an audiogram and tympanogram were carried out. Myringotomy was carried out in 95 ears and the presence or absence of glue was recorded. The significance of the air-bone gap and the type of tympanogram in predicting a middle ear effusion were also examined. The audiograms of 37 ears showed a Carhart notch; of these, 36 ears were noted to have glue ear. The correlation between the presence of a Carhart notch in the preoperative audiogram and the presence of glue at myringotomy was found to be significant (P < 0.001) (chi-square test). Using the decision rule that the presence of a Carhart notch predicts the presence of glue, the following operating characteristics were determined: correct, 83 ears (87.4%); false positive, one ear (1.1%); false negative, 11 ears (11.6%). The presence of a Carhart notch was found to be a strong predictor of the presence of glue at myringotomy.

RESUMO

A rapidly enlarging lump in the neck of an adolescent, is a cause for concern. Amongst the various clinical differential diagnoses, including lymphoma and other malignant lesions, focal myositis is an unusual cause. To date, only two such cases affecting the sternomastoid have been reported worldwide. We report a further similar case.
