RESUMO

Transitioning a patient with partial edentulism through hard and soft tissue grafting to an implant restoration with an interim removable dental prosthesis (IRDP) presents a challenge to the restorative dentist. The management of grafted sites requires care, and without the appropriate design, an IRDP may impede surgical outcomes and place the graft at risk for displacement or necrosis. A site development IRDP (SDIRDP) for a grafted site must fulfill restorative goals and promote the surgical objectives for site development. A technique is described for fabricating an SDIRDP that facilitates surgical procedures and maintains prosthetic goals.

RESUMO

The procedure for creating a computer-aided design/computer-aided manufactured surgical template with the NobelClinician software was originally designed for the completely edentulous arch. With the development of new techniques and expanded functions in the software, guided surgery for the partially edentulous became a viable option. As of yet, few protocols have been reported for the guided surgery of immediate implants with the NobelClinician software. A new technique is described whereby the radiographic template is modified to allow the use of guided surgery for the immediate placement of dental implants after extractions.

RESUMO

As dental implant therapy has evolved since its early days more than five decades ago, its effects have been mostly extremely positive. However, over the course of time, several undesirable, unanticipated consequences have also materialized, most notably peri-implant disease. In this article, the authors propose that many of the challenges clinicians see today regarding increased peri-implant disease represent negative effects of interventions that, though well-intended, had unforeseen shortcomings. Sufficient time has now passed for these adverse effects to manifest, and clinicians today are better able to understand how their attempts to solve problems in some ways produced new challenges. The article also shows how new innovations in engineering and digital technology allow clinicians to address the problems of the past and avoid increasing the risk of peri-implant disease.
