RESUMO

An anterior open bite (AOB) is an occlusal disorder that causes the patient both an aesthetic and functional handicap. The lower third of the face is disproportionately larger. Patients are unable to properly occlude with the anterior part of dental arch and occlusion only happens in the premolar and/or molar regions. An anterior open bite may be the result of anatomical anomalies. Long term stability as well as an immediate outcome of the surgery depends on the choice of a suitable treatment strategy. In this article, we review options of AOB treatment, from classical orthodontic treatment to current combined orthodontic and surgical approach with a benefit of an anchor system.

The aim of this study was a detailed clinicopathological investigation of sinonasal NUT midline carcinoma (NMC), including analysis of DNA methylation and microRNA (miRNA) expression. Three (5%) cases of NMC were detected among 56 sinonasal carcinomas using immunohistochemical screening and confirmed by fluorescence in situ hybridization. The series comprised 2 males and 1 female, aged 46, 60, and 65 years. Two tumors arose in the nasal cavity and one in the maxillary sinus. The neoplasms were staged pT1, pT3, and pT4a (all cN0M0). All patients were treated by radical resection with adjuvant radiotherapy. Two patients died 3 and 8 months after operation, but one patient (pT1 stage; R0 resection) experienced no evidence of disease at 108 months. Microscopically, all tumors consisted of infiltrating nests of polygonal cells with vesicular nuclei, prominent nucleoli and basophilic cytoplasm. Abrupt keratinization was present in only one case. Immunohistochemically, there was a diffuse expression of cytokeratin (CK) cocktail, CK7, p40, p63, and SMARCB1/INI1. All NMCs tested negative for EBV and HPV infection. Two NMCs showed methylation of RASSF1 gene. All other genes (APC, ATM, BRCA1, BRCA2, CADM1, CASP8, CD44, CDH13, CDKN1B, CDKN2A, CDKN2B, CHFR, DAPK1, ESR1, FHIT, GSTP1, HIC1, KLLN, MLH1a, MLH1b, RARB, TIMP3, and VHL) were unmethylated. All NMCs showed upregulation of miR-9 and downregulation of miR-99a and miR-145 and two cases featured also upregulation of miR-21, miR-143, and miR-484. In summary, we described three cases of sinonasal NMCs with novel findings on DNA methylation and miRNA expression, which might be important for new therapeutic strategies in the future.

RESUMO

We study symmetry breaking at the Dicke quantum phase transition by coupling a motional degree of freedom of a Bose-Einstein condensate to the field of an optical cavity. Using an optical heterodyne detection scheme, we observe symmetry breaking in real time and distinguish the two superradiant phases. We explore the process of symmetry breaking in the presence of a small symmetry-breaking field and study its dependence on the rate at which the critical point is crossed. Coherent switching between the two ordered phases is demonstrated.

BACKGROUND: Wound dehiscence complicated by gastrointestinal (GI) fistula to belong ,,abdominal catastrophe". Therapy is prolonged and connected with high morbidity and mortality rate. METHODS: In the period from October 2006 to July 2009 we performed 12 reconstructive surgical procedures on gastrointestinal tract in patients with abdominal catastrophe. Treatment of 12 consecutive patients (9 men, 3 women) was managed according to a standardize protocol. The protocol consists of treatment of septic complications, optimisation of nutritional state, special wound procedures, diagnosis of gastrointestinal fistulas and GI tract, timing of surgical procedures, reconstruction of GI tract and postoperative care. RESULTS: Reconstructive surgery of GI tract was successful on 11 patients. One patient developed recurrence of early GI fistula. In four patients we let open abdomen to heal per secundam. We observed no deaths after operation. CONCLUSION: With regard to complex character of therapy of abdominal catastrophe there is a need of multidisciplinary approach. Considering long-lasting and expensive therapy there is logical step to concentrate these patients into special centres which are experienced, equipped and their staff is trained in treatment of such a seriously impaired patients.

RESUMO

A case of a 24-year-old woman with a 6 weeks lasting nodule of the right margin of the tongue is described. The nodule was 20 mm in diameter and showed surface ulceration. The diagnosis of low grade myofibroblastic sarcoma was supported by histological, immunohistochemical and electronmicroscopic examination. Although the tumor resection was not complete, the patient is free of disease 1 year after operation. The differential diagnostics of low grade myofibroblastic sarcoma is discussed.

UNLABELLED: The aim of our study was to investigate if there is any significant relationship between adrenal steroid hormone dehydroepiandrosterone-sulphate (DHEAS) and insulin resistance, as there are some previous signs in the literature suggesting such relationship but the results are not conclusive. METHODS: In our study participated 50 obese women and 20 obese men both with BMI over 27 kg/m2. Insulin resistance was calculated using HOMA insulin resistance index (HOMA-IR) that is based on fasting glycaemia and basal insulin levels. RESULTS: The negative correlation between DHEAS and HOMA-IR was found in the whole group of obese women but close behind statistical significance (r = -0.27, p = 0.055). But the group of obese women does not seem to be homogenous in correlation between DHEAS and insulin resistance. The significant negative correlation between DHEAS and insulin resistance was found in the subgroup (n = 21) of obese type 2 diabetic women (r = -0.55, p = 0.01) but no correlation was found in the subgroup (n = 29) of obese non-diabetic women (r = -0.06, p = 0.77). The relationship between DHEAS and glycaemia homeostasis was also confirmed by significant negative correlation between DHEAS and glycated haemoglobin (HbA1c) both in the subgroup of type 2 diabetic women (r = -0.51, p = 0.03) and in the subgroup of non-diabetic women (r = -0.70, p = 0.001). There was not found any significant correlation between DHEAS and insulin resistance neither in the whole group (n = 20) of obese men (r = 0.04, p = 0.87) nor in the subgroup (n = 11) of obese type 2 diabetic men (r = 0.55, p = 0.08) or in the subgroup (n = 9) of obese non-diabetic men (r = -0.42, p = 0.26). CONCLUSIONS: Significant negative correlation between DHEAS and HOMA-IR was found in the group of obese type 2 diabetic women but not in obese non-diabetic women suggesting that low DHEAS level might be connected to the development of insulin resistance and type 2 diabetes mellitus in obese women.

Long-range interactions in quantum gases are predicted to give rise to an excitation spectrum of roton character, similar to that observed in superfluid helium. We investigated the excitation spectrum of a Bose-Einstein condensate with cavity-mediated long-range interactions, which couple all particles to each other. Increasing the strength of the interaction leads to a softening of an excitation mode at a finite momentum, preceding a superfluid-to-supersolid phase transition. We used a variant of Bragg spectroscopy to study the mode softening across the phase transition. The measured spectrum was in very good agreement with ab initio calculations and, at the phase transition, a diverging susceptibility was observed. The work paves the way toward quantum simulation of long-range interacting many-body systems.

RESUMO

Inelastic neutron-scattering experiments on the high-temperature superconductor La1.855Sr0.145CuO4 reveal a magnetic excitation gap Delta that decreases continuously upon application of a magnetic field perpendicular to the CuO2 planes. The gap vanishes at the critical field required to induce long-range incommensurate antiferromagnetic order, providing compelling evidence for a field-induced soft-mode driven quantum phase transition.
