RESUMO

This paper explores the opportunities and challenges for classifying human posture in indoor scenarios by analyzing the Frequency-Modulated (FM) radio broadcasting signal received at multiple locations. More specifically, we present a passive RF testbed operating in FM radio bands, which allows experimentation with innovative human posture classification techniques. After introducing the details of the proposed testbed, we describe a simple methodology to detect and classify human posture. The methodology includes a detailed study of feature engineering and the assumption of three traditional classification techniques. The implementation of the proposed methodology in software-defined radio devices allows an evaluation of the testbed's capability to classify human posture in real time. The evaluation results presented in this paper confirm that the accuracy of the classification can be approximately 90%, showing the effectiveness of the proposed testbed and its potential to support the development of future innovative classification techniques by only sensing FM bands in a passive mode.

The Research Octane Number (RON) is a key quality parameter for gasoline, obtained offline through complex, time-consuming, and expensive standard methods. Measurements are usually only available a few times per week and after long delays, making process control very challenging. Therefore, alternative methods have been proposed to predict RON from readily available data. In this work, we report the development of inferential models for predicting RON from process data collected in a real catalytic reforming process. Data resolution and synchronization were explicitly considered during the modelling stage, where 20 predictive linear and non-linear machine learning models were assessed and compared using a robust Monte Carlo double cross-validation approach. The workflow also handles outliers, missing data, multirate and multiresolution observations, and processes dynamics, among other features. Low RMSE were obtained under testing conditions (close to 0.5), with the best methods belonging to the class of penalized regression methods and partial least squares. The developed models allow for improved management of the operational conditions necessary to achieve the target RON, including a more effective use of the heating utilities, which improves process efficiency while reducing costs and emissions.

BACKGROUND: This non-inferiority randomised clinical trial aimed to evaluate the survival of direct bulk fill composite resin restorations in primary molars using different methods of moisture control: rubber dam isolation (RDI-local anaesthesia and rubber dam) and cotton roll isolation (CRI-cotton roll and saliva ejector). Secondary outcomes included baseline and 2-year incremental cost, self-reported child's pain scores and patient behaviour during the restorative procedure. METHODS: A total of 174 molars (93 children) with dentine caries lesions were randomly allocated to study groups (RDI or CRI) and restored with bulk fill composite resin by trained operators. Two blinded examiners assessed the restorations for up to 24 months. Wong-baker faces and Frankl's behaviour rating scales were used for accessing the child's pain and behaviour, respectively. The primary outcome (restoration survival) was analysed using the two-sample non-inferiority test for survival data using Cox Regression (non-inferiority/alternative hypothesis HR > 0.85; CI = 90%). Bootstrap Linear regression was used for cost analysis and logistic regression for pain and behaviour analysis (α = 5%). RESULTS: After 2-years, 157 restorations were evaluated (drop-out = 9.7%). The survival rate was RDI = 60.4% and CRI = 54.3%. The non-inferiority hypothesis was accepted by the Cox Regression analysis (HR = 1.33; 90% CI 0.88-1.99; p = 0.036). RDI was 53% more expensive when compared to the CRI group. No differences were found between the groups regarding pain (p = 0.073) and behaviour (p = 0.788). CONCLUSION: Cotton roll isolation proved to be non-inferior when compared to rubber dam for composite restorations longevity in primary molars. Furthermore, the latest presented the disadvantage of higher cost and longer procedure time. Clinical Significance The moisture control method does not influence the longevity of composite restorations in primary molars. Cotton roll isolation proved to be non-inferior to rubber dam isolation and is a viable option for restoring primary molars. Clinical trial registration registered NCT03733522 on 07/11/2018. The present trial was nested within another clinical trial, the CARies DEtection in Children (CARDEC-03-NCT03520309).

BACKGROUND: Glass ionomer cements (GIC) have been considered the top option to restore primary teeth by dentists. The most common supply forms are hand mixed and encapsulated GIC. There is a lack of information about the impact of different GIC supply forms on restoration survival. METHODS: This randomized clinical trial compared the survival rate of occlusal and occlusoproximal restorations in primary molars using two glass ionomer cements versions: hand-mixed (H/M) and encapsulated (ENC) after 24 months. Children aged 3-10 years who presented dentin caries lesions in primary molars were selected at School of Dentistry, University of São Paulo, Brazil. They were randomly assigned to groups: H /M (Fuji IX®, GC Europe) or ENC (Equia Fill®, GC Europe). The occurrence of restoration failure was evaluated by two blinded and calibrated examiners. The analyses were performed in Stata 13 (StataCorp, USA). To evaluate the primary outcome (restoration survival), we  performed a survival analysis. Additionally an intention to treat (ITT) analysis were done at 24 months of follow-up. Cox Regression with shared frailty was performed to assess association between restoration failure and independent variables (α = 5%). RESULTS: A total of 324 restorations were performed in 145 children. The survival for H/M group was 58.2% and 60.1% for ENC, with no difference (p = 0.738). Occlusoproximal restorations had lower survival rate when compared to occlusal ones (HR = 3.83; p < 0.001). CONCLUSIONS: The survival rate in primary molars is not influenced by the different supply forms of GIC. Also, occlusoproximal restorations present reduced performances when compared to occlusal cavities. In addition to seven known alkaloids (2, 6-11) and 1,2,4-trimethoxybenzene (1), three isoquinoline-derived alkaloids (3-5), namely, duguetinine (3), a compound based on an unprecedented oxahomoaporphine scaffold, and two new 8-oxohomoaporphine alkaloids, duguesuramine (4) and 11-methoxyduguesuramine (5), and a new asarone-derived phenylpropanoid (10) were isolated from the bark of Duguetia surinamensis. The isolation workflow was guided by HPLC-HRESIMS/MS and molecular networking-based analyses. Twenty-four known alkaloids were dereplicated from the D. surinamensis alkaloid-rich fraction network and were assigned by manual MS/MS interpretation. Their cytotoxic potential was evaluated.

BACKGROUND: The physiological stress of critically ill patients can trigger several complications, including digestive bleeding due to stress ulcers (DBSU). The use of acid secretion suppressants to reduce their incidence has become widely used, but with the current understanding of the risks of these drugs, their use, as prophylaxis in critically ill patients, is limited to the patients with established risk factors. AIM: To determine the appropriateness of the use of prophylaxis for stress ulcer bleeding in acutely ill patients admitted to intensive care units and to analyze the association of risk factors with adherence to the prophylaxis guideline. METHODS: Retrospective, analytical study carried out in three general adult intensive care units. Electronic medical records were analyzed for epidemiological data, risk factors for DBSU, use of stress ulcer prophylaxis, occurrence of any digestive bleeding and confirmed DBSU. The daily analysis of risk factors and prophylaxis use were in accordance with criteria based on the Guidelines of the Portuguese Society of Intensive Care for stress ulcer prophylaxis. RESULTS: One hundred and five patients were included. Of the patient days with the opportunity to prescribe prophylaxis, compliance was observed in 95.1%. Of the prescription days, 82.35% were considered to be of appropriate use. Overt digestive bleeding occurred in 3.81% of those included. The occurrence of confirmed DBSU was identified at 0.95%. Multivariate analysis by logistic regression did not identify risk factors independently associated with adherence to the guideline, but identified risk factors with a negative association, which were spinal cord injury (OR 0.02 p <0.01) and shock (OR 0.36 p=0.024). CONCLUSION: The present study showed a high rate of adherence to stress ulcer prophylaxis, but with inappropriate use still significant. In the indication of prophylaxis, attention should be paid to patients with spinal cord injury and in shock.

The Bromeliaceae family has been used as a model to study adaptive radiation due to its terrestrial, epilithic, and epiphytic habits with wide morpho-physiological variation. Functional groups described by Pittendrigh in 1948 have been an integral part of ecophysiological studies. In the current study, we revisited the functional groups of epiphytic bromeliads using a 204 species trait database sampled throughout the Americas. Our objective was to define epiphytic functional groups within bromeliads based on unsupervised classification, including species from the dry to the wet end of the Neotropics. We performed a hierarchical cluster analysis with 16 functional traits and a discriminant analysis, to test for the separation between these groups. Herbarium records were used to map species distributions and to analyze the climate and ecosystems inhabited. The clustering supported five groups, C3 tank and CAM tank bromeliads with deep tanks, while the atmospheric group (according to Pittendrigh) was divided into nebulophytes, bromeliads with shallow tanks, and bromeliads with pseudobulbs. The two former groups showed distinct traits related to resource (water) acquisition, such as fog (nebulophytes) and dew (shallow tanks). We discuss how the functional traits relate to the ecosystems inhabited and the relevance of acknowledging the new functional groups.

BACKGROUND: Different approaches have been used by dentists to base their decision. Among them, there are the aesthetical issues that may lead to more interventionist approaches. Indeed, using a more interventionist strategy (the World Dental Federation - FDI), more replacements tend to be indicated than using a minimally invasive one (based on the Caries Around Restorations and Sealants-CARS). Since the resources related to the long-term health effects of these strategies have not been explored, the economic impact of using the less-invasive strategy is still uncertain. Thus, this health economic analysis plan aims to describe methodologic approaches for conducting a trial-based economic evaluation that aims to assess whether a minimally invasive strategy is more efficient in allocating resources than the conventional strategy for managing restorations in primary teeth and extrapolating these findings to a longer time horizon. METHODS: A trial-based economic evaluation will be conducted, including three cost-effectiveness analyses (CEA) and one cost-utility analysis (CUA). These analyses will be based on the main trial (CARDEC-03/ NCT03520309 ), in which children aged 3 to 10 were included and randomized to one of the diagnostic strategies (based on FDI or CARS). An examiner will assess children's restorations using the randomized strategy, and treatment will be recommended according to the same criteria. The time horizon for this study is 2 years, and we will adopt the societal perspective. The average costs per child for 24 months will be calculated. Three different cost-effectiveness analyses (CEA) will be performed. For CEAs, the effects will be the number of operative interventions (primary CEA analysis), the time to these new interventions, the percentage of patients who did not need new interventions in the follow-up, and changes in children's oral health-related quality of life (secondary analyses). For CUA, the effect will be tooth-related quality-adjusted life years (QALYs). Intention-to-treat analyses will be conducted. Finally, we will assess the difference when using the minimally invasive strategy for each health effect (∆effect) compared to the conventional strategy (based on FDI) as the reference strategy. The same will be calculated for related costs (∆cost). The discount rate of 5% will be applied for costs and effects. We will perform deterministic and probabilistic sensitivity analyses to handle uncertainties. The net benefit will be calculated, and acceptability curves plotted using different willingness-to-pay thresholds. Using Markov models, a longer-term economic evaluation will be carried out with trial results extrapolated over a primary tooth lifetime horizon. DISCUSSION: The main trial is ongoing, and data collection is still not finished. Therefore, economic evaluation has not commenced. We hypothesize that conventional strategy will be associated with more need for replacements of restorations in primary molars. These replacements may lead to more reinterventions, leading to higher costs after 2 years. The health effects will be a crucial aspect to take into account when deciding whether the minimally invasive strategy will be more efficient in allocating resources than the conventional strategy when considering the management of restorations in primary teeth. Finally, patients/parents preferences and consequent utility values may also influence this final conclusion about the economic aspects of implementing the minimally invasive approach for managing restorations in clinical practice. Therefore, these trial-based economic evaluations may bring actual evidence of the economic impact of such interventions. TRIAL REGISTRATION: NCT03520309 . Registered May 9, 2018. A alta prevalência de cárie ao redor do mundo faz com que os tratamentos restauradores figurem entre os procedimentos mais executados pelos cirurgiõesdentistas. Também é bastante comum os profissionais relatarem novas lesões de cárie ao redor das restaurações ou até mesmo fraturas parciais ou totais das mesmas, fatores que podem ser considerados como "falhas" do procedimento restaurador. Diante dessas situações, a maioria dos profissionais entende que é necessário realizar a substituição completa da restauração. No entanto, existem outras técnicas mais conservadoras e que podem ser tão efetivas quanto a substituição/troca, como por exemplo a realização de reparo das restaurações apresentando defeitos. O objetivo deste artigo é apresentar de forma clara e objetiva aos clínicos que se deparam diariamente com este cenário, qual seria o melhor momento para intervir, e quais as alternativas de tratamento, baseadas na melhor evidência científica disponível, a se realizar frente às falhas dos procedimentos restauradores, sempre alinhadas com a filosofia de Mínima Intervenção.


The high prevalence of caries worldwide makes restorative treatments some of the most commonly performed dental treatments. It is pretty common to find new caries lesions around the restorations or even partial or total fractures, factors that can be considered a "failure" for the restorative procedure. In these situations, most professionals understand that it is necessary to replace the restoration, but other more conservative techniques are as effective as a replacement, such as repairing the restorations. This article aims to present a clear and evidence-based when is the best time to intervene and what is the best treatment to be carried out in case of failure of the restorative procedures, in line with Minimal Intervention principles.

RESUMO Racional: O estresse fisiológico dos pacientes críticos pode desencadear várias complicações, entre elas o sangramento digestivo por úlcera de estresse (SDUE). O uso de supressores da secreção ácida para reduzir sua incidência passou a ser amplamente utilizado, mas com o atual entendimento dos riscos destes medicamentos sua utilização, como profilaxia em doentes críticos, está limitada aos pacientes com fatores de risco estabelecidos. Objetivos: Determinar a adequação do uso de profilaxia para sangramento por úlcera de estresse em pacientes agudamente enfermos internados em unidades de terapia intensiva e analisar a associação dos fatores de risco com a adesão à diretriz de profilaxia. Métodos: Estudo retrospectivo, analítico, realizado em três unidades de terapia intensiva gerais de adultos. Os prontuários eletrônicos foram analisados para dados epidemiológicos, fatores de risco para SDUE, uso de profilaxia para SDUE, ocorrência de qualquer sangramento digestivo e de SDUE confirmado. A análise diária dos fatores de risco e uso de profilaxia foram de acordo com critérios baseados nas Diretrizes da Sociedade Portuguesa de Cuidados Intensivos para profilaxia da úlcera de estresse. Resultados: foram incluídos 105 pacientes. Dos pacientes-dia com oportunidade de prescrição de profilaxia, foi observada adesão em 95,1%. Dos dias de prescrição foram considerados de uso apropriado 82,35%. Sangramento digestivo visível ocorreu em 3,81% dos incluídos. A ocorrência de SDUE confirmado foi identificada em 0,95%. A análise multivariada por regressão logística não identificou fatores de risco independentemente associados com a adesão à diretriz, mas identificou fatores de risco com associação negativa, que foram lesão da medula espinhal (OR 0.02 p<0,01) e choque (OR 0.36 p=0.024). Conclusão: O presente estudo evidenciou alta taxa de adesão à profilaxia para SDUE, mas com uso inapropriado ainda significativo. Na indicação de profilaxia deve-se ter atenção aos pacientes com lesão de medula espinhal e choque.

At present, there is a concern about the quality of milk and diseases related to its consumption, as it can generate discomfort and allergic reactions in some individuals due to its protein components. Thus, the present study was developed to identify the allele and genotype frequencies of genes for ß-casein, A1 and A2, in dairy herds in the region of Araguaína-TO, Brazil. Genetic material from 421 animals (crossbred dairy cattle in lactation) was used. All animals were numbered for identification, and DNA samples were extracted from hair bulbs. Samples for two markers from the polymorphic regions were characterized and confirmed by real-time PCR using the ABI Prism® 7500 Sequence Detection System (Applied Biosystems). Allele and genotype frequencies were determined using the TaqMan™ detection system, where the primer and probe release different fluorescence signals for each allele of the polymorphism. The sampled herd showed frequencies of 28.27% for the A1 allele and 71.73% for the A2 allele. Genotype frequencies were 52.96% (223/421) for A2A2; 37.53% (158/421) for the A1A2 genotype; and 9.50% (40/421) for the A1A1 genotype. The frequency of the A1 allele for ß-casein in dairy herds from the northern region of Tocantins was low and is per the results of previous studies. Although the A2A2 genotype of ß-casein had a high relative frequency, the A1A2 genotype is still rather frequent, warranting greater selection pressure.(AU)


Atualmente existe uma preocupação em relação à qualidade e doenças que estão relacionadas ao consumo de leite, pois o mesmo pode gerar desconfortos e reações alergicas em alguns indivíduos devido aos seus constituintes protéicos. Assim, o presente estudo teve como objetivo identificar a frequência alélica e genotípica de genes para beta caseína, A1 e A2, em rebanhos leiteiros da região de Araguaína-TO. Foram utilizados material genético de 421 animais (bovinos leiteiros mestiços em lactação), e todos os animais foram numerados para identificação e amostras de DNA foram extraídas de bulbo de folículos pilosos. As amostras para dois marcadores das regiões polimórficas foram caracterizadas e confirmadas por PCR em tempo real, usando um sistema de detecção de sequências ABI Prism® 7500 (Applied Biosystems). As frequências alélicas e genotípicas foram determinadas utilizando o sistema de detecção TaqMan ™, no qual o primer e a sonda emitem diferentes sinais de fluorescência para cada alelo do polimorfismo. Observou-se frequência do alelo A1 de 28,27%, e do alelo A2 de 71,73% no rebanho amostral. A frequência genotípica de A2A2 foi de 52,96% (223/421), com genótipo A1A2 de 37,53% (158/421), e de 9,50% (40/421) animais com genótipo A1A1. A frequência do alelo A1 para beta-caseína em rebanhos leiteiros da região norte do Tocantins foi baixa e seguiu a mesma tendência já observada em estudos anteriores. Os genótipos A2A2 da beta-caseína apresentaram frequência relativa alta, entretanto o genótipo A1A2 ainda é bastante frequente, necessitando de maior pressão de seleção.(AU)

Introdução: O desperdício é um grande problema enfrentado pelo Brasil, em função dos hábitos alimentares, sendo o aproveitamento de cascas de frutas, folhas e talos de hortaliças ainda baixo. Sabe-se, no entanto, que a utilização dessas partes tradicionalmente não aproveitadas pode contribuir para a produção de alimentos mais saudáveis, nutritivos e com menor impacto negativo para o meio ambiente. Objetivo: Aproveitar os vegetais na sua forma integral ou de suas partes alimentícias não convencionais, e caracterizar as farinhas obtidas, com vistas à alimentação humana. Métodos: As farinhas foram obtidas de banana verdes (casca e polpa), casca de tangerina, casca e polpa de berinjela e bagaço de uva oriundo da produção de suco de uva, que foram secas em estufa ventilada a 40°C por 48 horas. Foram realizadas análises físico-químicas e microbiológicas. Resultados: As farinhas apresentaram alto teor de fibras (17,7 a 48,94 g/100g) e compostos fenólicos (778,81 a 2708,11 mg EAG/100g), e baixo teor de lipídeos (0,70 a 3,40 g/100g). Para a quantidade de proteínas, a farinha de berinjela foi a que mais se destacou (13,81 g/100g), podendo ser uma alternativa aos indivíduos que não consomem produtos de origem animal. Conclusões: Todas as farinhas são seguras do ponto de vista microbiológico, de acordo com a legislação vigente, e portanto, podem ser utilizadas na alimentação humana para aumentar o aporte de nutrientes e diminuir os impactos da poluição, contribuindo para a manutenção do meio ambiente e gerando renda. Introduction: Waste is a great problem faced by Brazil, due to dietary habits, with a low use of fruit peels, leaves and stems of vegetables. However, it is known that the use of these parts traditionally not used can contribute to the production of healthier and more nutritious food with less negative impact on the environment. Objective: Take advantage of vegetables in their entirety or from their non-conventional food parts, and to characterize the flours obtained, for human consumption. Methods: Flours were obtained from green bananas (peel and pulp), tangerine peel, eggplant peel and pulp and grape marc from grape juice production, which were dried in a ventilated oven at 40 ° C for 48 hours. Physical-chemical and microbiological analyzes were realized. Results: Flours showed a high fiber (17.7 to 48.94 g/100g) and phenolic compounds (778.81 to 2708.11 mg EAG/100g) content, and low lipid content (0.70 to 3.40 g/100g). For the amount of proteins, eggplant flour was the one that stood out the most (13.81g/100g), it can be an alternative to individuals who do not consume animal products. Conclusions: All flours are safe in a microbiological point of view, according to current legislation, and therefore they can be used in food to increase the supply of nutrients and reduce the impacts of pollution, contributing to the environment maintenance and income generating. RESUMO

This work relates the development of an analytical methodology to simultaneously determine three methylxanthines (caffeine, theobromine, and theophylline) in beverages and urine samples based on reversed-phase high-performance liquid chromatography. Separation is made with a Bondesil C18 column using methanol-water-acetic acid or ethanol-water-acetic acid (20:75:5, v/v/v) as the mobile phase at 0.7 mL/min. Identification is made by absorbance detection at 273 nm. Under optimized conditions, the detection limit of the HPLC method is 0.1 pg/mL for all three methylxanthines. This method is applied to urine and to 25 different beverage samples, which included coffee, tea, chocolate, and coconut water. The concentration ranges determined in the beverages and urine are: < 0.1 pg/mL to 350 microg/mL and 3.21 microg/mL to 71.2 microg/mL for caffeine; < 0.1 pg/mL to 32 microg mL and < 0.1 pg/mL to 13.2 microg/mL for theobromine; < 0.1 pg/mL to 47 microg/mL and < 0.1 pg/mL to 66.3 microg/mL for theophylline. The method proposed in this study is rapid and suitable for the simultaneous quantitation of methylxanthines in beverages and human urine samples and requires no extraction step or derivatization.

RESUMO

O objetivo deste ensaio clínico randomizado foi comparar a taxa de sobrevida das restaurações oclusais e oclusoproximais em molares decíduos usando duas versões de cimentos de ionômero de vidro: pó-líquido, manipulado manualmente (MAN) e encapsulado (ENC) após 24 meses. Crianças entre 3 e 10 anos de idade que apresentaram lesões de cárie em dentina nas superfícies oclusais e/ou oclusoproximais de molares decíduos foram selecionadas na Faculdade de Odontologia da Universidade de São Paulo, Brasil. Elas foram alocadas aleatoriamente em dois grupos: M/M (Fuji IX®, GC Europa) ou ENC (Equia Fill®, GC Europa). A ocorrência de falha das restaurações foi avaliada por duas examinadoras calibradas e cegas e em relação aos grupos. As análises estatísticas foram realizadas no software Stata 13 (StataCorp, EUA). A análise de Kaplan-Meier foi utilizada para ilustrar a sobrevida e o comportamento das restaurações ao longo dos 24 meses de acompanhamento enquanto a Regressão de Cox com fragilidade compartilhada foi realizada para avaliar a associação entre falha de restauração e variáveis independentes (=5%). Realizamos ainda a análise por intenção de tratar (ITT) considerando os 24 meses de acompanhamento. Um total de 323 restaurações foram realizadas em 145 crianças. A sobrevida para o grupo MAN foi de 58,2% e 60,1% para o grupo ENC, sem diferença estatisticamente significativa (p=0,738). As restaurações oclusoproximais tiveram menor taxa de sobrevida quando comparadas às oclusais (HR=3,83; p<0,001). A taxa de sobrevida nos molares decíduos não é influenciada pelas diferentes formas de apresentação do CIV Registro do ECR: Este ensaio clínico randomizado foi registrado no ClinicalTrials.Gov em 15/10/2014 sob protocolo (NCT 02274142).

Aim: To present the report of a female patient, 10 years old, referred for treatment in the Dentistry Clinic of a Brazilian public university. Case report: The patient's oral health condition was unfavorable with biofilm accumulation, pain report and chewing difficulty. After anamnesis, clinical and radiographic examination, the treatment plan included removal of infectious foci (54, 55, 64, 65, 74, 75, 85, 16, 36 and 46) followed by prosthetic rehabilitation. At this stage, the functional restoration of the lower arch was restricted by the imminent eruption of the premolars. In the upper arch, the maintenance of the mesio-distal diameters aims to guarantee the chronological sequence of successors irruption and establishment of normal occlusion. During the monthly follow-up consultations the patient presents a more spontaneous smile, although she still needs constant reinforcement in oral hygiene habits. Conclusions: The rehabilitation of patients with loss of permanent teeth during the mixed dentition phase should consider the peculiarities of this period for the restoration of health without altering the normal pattern of occlusal development.
