RESUMO

To analyze the distribution and factors associated with lack of knowledge about the human papillomavirus (HPV) vaccine among Brazilian adolescents.Cross-sectional study using the 2019 National School Health Survey among 17,805 Brazilian students of public and private schools, aged 13-17 years. The outcome variable was "lack of knowledge about the HPV vaccine" and the explanatory variables were sociodemographic, behaviors, knowledge, and health conditions. Logistic regression model was used to calculate Odds Ratio (OR) and 95% confidence intervals (95%CI). Spatial analysis techniques were used to determine the formation of clusters in the federated units with similar proportions of adolescents who were unaware of the vaccine. The lack of knowledge about the HPV vaccine was reported by 45.54% of Brazilian students. There was a higher chance of lack of knowledge having had sexual intercourse (OR 1.43; 95% CI 1.20-1.70); attending public school (OR 1.72; 95%CI 1.47-2.02) and located in the Northeast Region (OR 1.35; 95%CI 1.08-1.69). The lower chance of lack of knowledge were female gender (OR 0.41; 95% CI 0.35-0.48), higher maternal education (OR 0.62; 95% CI 0.50-0.77) self-rated health as Poor/very poor (OR 0.64; 95% CI 0.49-0.86) and receiving contraceptive counseling (OR 0.77; 95% CI 0.65-0.91). The proportion of lack of knowledge about the HPV vaccine was higher with the formation of High-High spatial clusters in the states of Maranhão, Piauí, and Pernambuco. Sociodemographic, health, and behavioral conditions and knowledge of students, as well as school characteristics, were associated with lack of knowledge about the HPV vaccine. A higher frequency of lack of knowledge about the HPV vaccine among adolescents was found in the states of the Northeastern Region.

RESUMO

BACKGROUND: Low polio vaccine coverage can result in the spread of Poliovirus to areas free from viral circulation. This study analyzed the temporal trends and spatial distribution of polio vaccine coverage in one year-old children in Brazil, between 2011 and 2021. METHODS: This was an ecological, time-series study (2011 to 2021) with annual vaccine coverages against poliomyelitis, extracted from the Information System of the National Immunization Program from the 26 States and the Distrito Federal (DF). The percentage reductions in vaccination coverage in Brazil and in the Regions were calculated. Prais-Winsten regression models were used to analyze time series for the Regions and States, and spatial analysis identified the distribution of clusters (high-high; low-low; high-low and low-high) of vaccination coverages across Brazilian municipalities, using a 5% significance level. RESULTS: From 2011 to 2021, the coverage of polio vaccines decreased by 29,9%. There was a progressive increase observed in clusters resulting in low vaccination coverages (140 low-low Brazilian municipalities in 2011 vs. 403 in 2021), mostly reported in the North and Northeast regions of the country. There was a downward trend in vaccination coverages in 24 of the 26 States and DF (p ≤ 0.05). CONCLUSIONS: The reduction in polio vaccine coverage, as observed in the North and Northeast regions of Brazil, may favor the spread of Poliovirus. Therefore, vaccination strategies should be prioritized for children residing in areas with sharp and recurrent declines in vaccination coverages, including travelers, migrants, and refugees.

RESUMO

The current study combined chemical data on trace elements and polycyclic aromatic hydrocarbons (PAH) from sediment and used a multibiomarker approach in the catfish Sciades herzbergii to evaluate two different sites in São Marcos Bay, Brazil. Higher diffuse contaminations by trace elements and PAH were detected in the sediment of Porto Grande (PG) harbor than in the Ilha dos Caranguejos (IC) reference area. A multibiomarker was used in catfish to evaluate the bioavailability of PAH in bile and the effects of pollutants in target tissues. The parameters considered were oxidative stress biomarkers (SOD, CAT, GSH, GST and LPO) and histopathological alterations and were compared between two seasons. The biological responses revealed adverse effects on the population, as indicated by the presence of trace elements and PAH as stressors. Principal component analysis (PCA) of the biomarkers corroborated these results and indicated that fish from the PG site during the rainy season in 2019 exhibited many biological effects compared to 2018. Overall, the present study showed that environmental contamination increased over the years and provides information on the contamination of sediments in the São Marcos Bay, Brazil. The results showed that the presence of contaminants was correlated with the health status of the catfish S. herzbergii.

RESUMO

This study aimed to compare biochemical and histological biomarkers in oysters to identify impacted areas in a Brazilian port region. Oysters belonging to the Crassostrea genus were collected in two points in São José Bay (Brazil): (A1) Curupu Island (control area) and (A2) Braga Port (impacted area). Digestive glands from oysters were used to analyze the enzymatic activity of glutathione S-transferase and Catalase. The gills were used for standard histology analyses. Water samples were collected for metal analyses. Our results indicated that there was a change in the activity of oyster GST and CAT enzymes, especially in A2. Histological gill analysis indicated more frequent changes in A2. The analyzed metals presented higher values in A2. The results of this study suggest that enzymatic alterations, histological changes and higher metal values are indicative of initial stress caused by contaminants in São José Bay, especially in the port region.

RESUMO

The aim of this study was to evaluate enzymatic (glutathione-S-transferase and catalase) and histological (branchial lesions) biomarkers in Ucides cordatus (Crustacea, Decapoda) from an industrial port region on the north coast of Brazil. The crabs were collected in two distinct locations of the Brazilian coast: A1 = region under influence of port activities; and A2 = low-impacted area. We performed histological examination in the gills and glutathione-S-transferase (GST) and catalase activity in the hepatopancreas. The most frequent and severe histological lesions were found in A1, especially rupture of pilaster cells and lamellar collapse. Catalase activity did not show a pattern capable of differentiating the two analyzed areas. On the other hand, GST activity presented a more pronounced response in the crabs of the port area (p < 0.05), coinciding with the most frequent branchial lesions in these same organisms. These results suggest that the species is susceptible to environmental stress, once alterations at different organizational levels were verified.

RESUMO

OBJECTIVE: To assess the impact of asthma in a population of inner-city Brazilian children. METHODS: In a cross-sectional study, we selected children with asthma and healthy controls from public schools (8-16 years) from a capital city of Southern Brazil. Divided into three phases, questionnaires were administered, assessing lung function, body mass index and allergic sensitization. RESULTS: From 2500 children initially included in the study (48.4% males; mean age of 11.42 ± 2.32 years), asthma prevalence was detected in 28.6% (715/2500). The disease was not controlled in 42.7% (305/715) of the children, with 7.6% of hospitalization rate. School absenteeism (at least one day of missing school because of asthma) and sedentary behavior were high (57.1 and 67.2%, respectively), with 47.9% of subjects requiring oral steroids in the previous year, and physical well-being significantly lower than controls, directly interfering with quality of life, and therefore in the daily activities of these students. Moreover, 38% of the parents admitted to being non-adherent to treatment with their children and 31.1 and 53.6%, respectively, believed that rescue medication and exercise might be harmful. CONCLUSIONS: The burden of asthma in Brazilian children seems to be substantial. New international guidelines with a special focus in developing countries settings, with more pragmatic approaches, should be a priority for discussion and implementation actions.

RESUMO

BACKGROUND: Primary health care (PHC) renewal processes are currently ongoing in South America (SA), but their characteristics have not been systematically described. OBJECTIVE: The study aimed to describe and contrast the PHC approaches being implemented in SA to provide knowledge of current conceptions, models and challenges. METHODS: This multiple case study used a qualitative approach with technical visits to health ministries in order to apply key-informant interviews of 129 PHC national policy makers and 53 local managers, as well as field observation of 57 selected PHC providers and document analysis, using a common matrix for data collection and analysis. PHC approaches were analysed by triangulating sources using the following categories: PHC philosophy and conception, service provision organization, intersectoral collaboration and social participation. RESULTS: Primary health care models were identified in association with existing health system types and the dynamics of PHC renewal in each country. A neo-selective model was found in three countries where coverage is segmented by private and public regimes; here, individual and collective care are separated. A comprehensive approach similar to the Alma-Ata model was found in seven countries where the public sector predominates and individual, family and community care are coordinated under the responsibility of the same health care team. CONCLUSIONS: The process of implementing a renewed PHC approach is affected by how health systems are funded and organized. Both models face many obstacles. In addition, care system organization, intersectoral coordination and social participation are weak in most of the countries.

RESUMO

Ethanol alters motricity, learning, cognition, and cellular metabolism in the cerebellum. We evaluated the effect of ethanol on apoptosis in Golgi, Purkinje, and granule cells of the cerebellum in adult rats. There were two groups of 20 rats: a control group that did not consume ethanol and an experimental group of UChA rats that consumed ethanol at 10% (<2 g ethanol/kg body weight/day). At 120 days old, rats were anesthetized and decapitated, and their cerebella were collected and fixed. Cerebellar sections were subjected to immunohistochemistry for terminal deoxynucleotide transferase dUTP nick end labeling (TUNEL), caspase-3, X-linked inhibitor of apoptosis protein (XIAP), and insulin-like growth factor 1-receptor (IGF-1R); real-time PCR (RT-PCR) to determine caspase-3, XIAP, and IGF-1R gene expression; and transmission electron microscopy (TEM). We identified fragmentation of DNA and an increase in caspase-3 protein and XIAP in Purkinje cells, whereas granule cells exhibited increased caspase-3 and XIAP. IGF-1R expression was unchanged. There was no significant difference in gene expression of caspase-3, XIAP, and IGF-1R. There were an increase in lipid droplets, a reduction in the cellular cytoplasm in electron-dense nuclei, and changes in the myelin sheath in the cerebellar cortex. In conclusion, our data demonstrated that ethanol induced apoptosis in the Purkinje and granule cells of the cerebellum of adult UChA rats.

RESUMO

OBJECTIVES: to map the available evidence on resources used to promote health literacy among caregivers of prematurely born children during outpatient follow-up. METHODS: the Joanna Briggs Institute's scope review protocol was utilized. The search encompassed six databases, incorporating studies from 2012 to 2022. RESULTS: the three included publications revealed that the resources employed are: mobile applications, phone calls, individual counseling, videos, educational pamphlets, and group discussions. Implementing an education protocol during the transition home enhances scientifically grounded health promotion rates. CONCLUSIONS: there is limited literature addressing the health literacy of these caregivers. The nursing team plays a crucial role in health education and in developing resources applicable to these families.

RESUMO

The article analyzes the fight against COVID-19 in three Latin American countries: Argentina, Brazil, and Mexico. A multiple case study was carried out in a comparative perspective, based on a bibliographic review, documentary analysis, and secondary data, considering characteristics of the countries and the health system, evolution of COVID-19, national governance, containment and mitigation measures, health systems response, constraints, positive aspects and limits of responses. The three countries had distinct health systems but were marked by insufficient funding and inequalities when hit by the pandemic and recorded high-COVID-19 mortality. Structural, institutional, and political factors influenced national responses. In Argentina, national leadership and intergovernmental political agreements favored the initial adoption of centralized control measures, which were not sustained. In Brazil, there were limits in national coordination and leadership related to the President's denialism and federative, political, and expert conflicts, despite a universal health system with intergovernmental commissions and participatory councils, which were little used during the pandemic. In Mexico, structural difficulties were associated with the Federal Government's initial reluctance to adopt restrictive measures, limits on testing, and relative slowness in immunization. In conclusion, facing health emergencies requires strengthening public health systems associated with federative, intersectoral, and civil society coordination mechanisms and effective global solidarity mechanisms.

RESUMO

The insulin therapy used to treat Diabetes Mellitus (DM) lacks educational tools to assist self-care. Thus, we aimed to develop and validate an educational tool for the relationship between glycemic variation and insulin therapy for adults with type 1 and 2 DM. The study was developed in three stages: i) elaboration of the educational tool; ii) validation of content and display by a panel of judges; iii) pre-test with a target audience. Ten judges participated in the second stage, and 12 insulin-dependent adults with type 1 or 2 DM participated in the third stage. The Content Validity Index (CVI) was used to evaluate the adequacy of the material by the judges. The percentages of the agreement by item were calculated for validation by the target audience. The My Treatment Diary (MTD) educational tool was then developed. It obtained a mean CVI of 99.6% and a percentage of agreement of 99%. The results demonstrated that the content and display of the MTD tool were validated and culturally appropriate for the population of adults with type 1 and 2 DM.

OBJECTIVE: To analyze the cognitive development of preterm infants at six and 12 months of corrected age and the associations with perinatal and socioeconomic factors. METHODS: Cognitive development of 40 infants (20 preterm and 20 full-term) at six and 12 months of age was evaluated using the Bayley-III scale. Correlations between cognitive outcome and associated factors were assessed using Spearman correlation. Stepwise multiple linear regression analysis with covariance was applied to identify changes on cognitive score between six and 12 months. RESULTS: Bayley-III cognitive score in preterm group was significantly lower than in full-term group at both six and 12 months of age. Birth weight correlated with cognitive performance at six months and head circumference at birth at 12 months, in full-terms infants. The occurrence of necrotizing enterocolitis was inversely associated with cognitive score in preterms at 12 months. An increase in cognitive score was observed between six and 12 months in both groups, but the gain was more pronounced in preterms. CONCLUSIONS: These findings suggest some cognitive recovery capacity in the first year despite the restrictions imposed by premature birth and emphasize the importance of early interventions in this population.

RESUMO

This study aimed to compare preterm (PT) and full-term (FT) infants' adaptive behavior and functional cortical response to tactile stimulus, as measured by Test of Sensory Functions in Infants and functional Near-Infrared Spectroscopy (fNIRS). Outcome measures were taken at 6 (PT = 26/FT = 21 infants) and 12 months (PT = 15/FT = 14 infants). At 6 months, poorer tactile reactivity was observed in PT, but not confirmed at 12 months. At 6 months, cortical response to tactile stimulus was found in the primary sensorimotor cortex and differences between groups did not reach significance. At 12 months, cortical response was found in the primary sensorimotor cortex and premotor area and in the somatosensory associative area, with significant less frequent response in premotor area in PT. The findings reinforce fNIRS as a tool to complement the knowledge of tactile adaptive behaviors in PT in early life.

RESUMO

Cardiomyopathy is a well-known complication of sepsis that may deteriorate when accompanied by obesity. To test this hypothesis we fed C57black/6 male mice for 6 week with a high fat diet (60% energy) and submitted them to endotoxemic shock using E. coli LPS (10 mg/kg). Inflammatory markers (cytokines and adhesion molecules) were determined in plasma and heart tissue, as well as heart mitochondrial biogenesis and function. Obesity markedly shortened the survival rate of mouse after LPS injection and induced a persistent systemic inflammation since TNFα, IL-1ß, IL-6 and resistin plasma levels were higher 24 h after LPS injection. Heart tissue inflammation was significantly higher in obese mice, as detected by elevated mRNA expression of pro-inflammatory cytokines (IL-1ß, IL-6 and TNFα). Obese animals presented reduced maximum respiratory rate after LPS injection, however fatty acid oxidation increased in both groups. LPS decreased mitochondrial DNA content and mitochondria biogenesis factors, such as PGC1α and PGC1ß, in both groups, while NRF1 expression was significantly stimulated in obese mice hearts. Mitochondrial fusion/fission balance was only altered by obesity, with no influence of endotoxemia. Obesity accelerated endotoxemia death rate due to higher systemic inflammation and decreased heart mitochondrial respiratory capacity.

RESUMO

This article analyzes the configuration of public-private relations in Chile's health system between 2000 and 2018, focusing on organization and regulation, funding and service delivery. The following data collection methods were employed: literature review, content analysis of official documents and secondary data, and semi-structured interviews. With regard to organization and regulation, the findings show a lack of institutional mechanisms to mitigate risk selection and that access to private services is intimately linked to ability to pay. The funding model is incapable of sustaining the public health system. With respect to service delivery, despite the implementation of strategies that suggest advances, the segmentation of the system is sustained by the fragmentation of care and purchase of private services. Our findings show that the nature of public-private relations in Chile's health system reinforces the segmentation of population groups produced by the market-oriented approach. Although the reforms implemented during the study period mitigate the effects of segmentation, they were unable to produce structural changes in the configuration of the health system.

In this study, we analyzed biomarkers and heavy metals in Ucides cordatus specimens caught in mangrove areas [A1 = reference (low impact area); A2, A3 and A4 = port complex (potentially impacted area)] of the Amazonian northern coast of Brazil. Sediments and crab muscles sampled from the potentially impacted mangroves showed high levels of metals. Additionally, catalase and glutathione S-transferase enzymes in the gills and hepatopancreas of the collected crabs were elevated in crabs from A2, A3 and A4 relative to those from A1. The histological characteristics of the gills and hepatopancreas differed in crabs collected from A2, A3 and A4 compared with those collected from A1. The crabs were larger and heavier in the rainy season in A1 and A3 and heavier in the dry season in A4. Heavy metal analyses of sediments, crab muscles and biological responses indicated that the animals are subjected to stress levels differentiated in the mangroves due to a gradient (A4>A3>A2>A1) of anthropogenic pressure in São Marcos Bay. In conclusion, the analyses of metals in the sediment and in the muscles of crabs, as well as the biochemical biomarker and histological analyses, suggest that crabs from mangroves in the port complex region are subject to pollutants that compromise their health.

RESUMO

In the 1980s, during the military dictatorship, Chile was a forerunner in Latin America in radical health system reform, expanding the private sector's participation in health insurance and services provision and influencing reforms in other countries of the region. The article analyzes health policies in Chile from 2000 to 2018, in the context of four democratic government administrations, considering continuities and changes in the policies' development and their conditioning factors. The analytical reference drew on contributions from historical institutionalism. Literature and document searches were performed, besides semi-structured interviews with national policymakers from the period under study. Analysis of the trajectory of health policies in Chile during the democratic period revealed continuities and changes in the agendas and strategies adopted by governments with different political positions. Incremental reforms throughout this period produced progress and improvements in health services access and provision. However, reform proposals to alter the health system's public-private arrangement encountered resistance, and the dual and segmented structure shaped in the 1980s was maintained, with strong private participation. Historical-structural, institutional, and political conditioning factors in State-market relations and the health system's configuration under the dictatorship hindered comprehensive changes in public-private relations in health, producing an example of path dependence and corporate interests' power in the health sector.

The goal of this study was to evaluate biomarkers (glutathione S-transferase and catalase) and microorganisms in soft tissues of Crassostrea rhizophorae to assess possible contamination of seafood in Brazil. The oysters were sampled from a reference area (Ports 1 and 2) and an impacted area (Ports 3 and 4) in Brazil (São Luís Island, Maranhão). Six attributes were examined in sampled oysters: glutathione S-transferase activity, catalase activity, concentrations of total coliforms and thermotolerant coliforms, and levels of Escherichia coli and Aeromonas hydrophila. Water samples were analysed for aluminium, cadmium, iron, manganese, lead, mercury, phenolics, and polychlorinated biphenyls. We found that Ports 3 and 4 are impacted by several contaminants (mercury, phenolics, and polychlorinated biphenyls), while Ports 1 and 2 are still relatively free of these contaminants. Changes in enzymes activity as well as the highest tissue bacterial concentrations were recorded in oysters from Ports 3 and 4 during the rainy season.

RESUMO

This article shows the results of a study which analyzed healthcare actions carried out by Family Health Teams. It is a qualitative research carried out in Belo Horizonte and Contagem, state of Minas Gerais. The subjects were 28 members of the Family Health Team. Data were collected through interviews. The results show a tendency to work a concept of health promotion, associated to disease prevention activities. Permanent education is very elementary is at a very early stage in the health services. We conclude that the activities carried out with a focus on inter-sectoral work are limited, compromising the consolidation of the merely technical healthcare model, which is based on health promotion. It can be seen, based on analysis of the practice of the teams, that there is little incentive to the empowerment of the subjects.

RESUMO

OBJECTIVE: To identify aspects that hinder or facilitate the work of nurses in primary health care when disclosing difficult news. METHOD: Qualitative, descriptive and exploratory study addressing 15 nurses from ten primary health care units located in the south of Brazil. Data were collected from February to April 2017, using a semi-structured interview. Interviews were audio-recorded and analyzed based on discourse analysis. RESULTS: The aspects that hinder communication include the demand for services; work organization; the characteristics of patients; and personal aspects. Facilitating aspects include privacy and being in the community. Also hindering or facilitating communication: the way the network functions and the staff; academic training; professional experience; personal aspects. CONCLUSION: This topic is seldom addressed during academic training and this lack of preparedness is one of the main barriers hindering the disclosure of difficult news. Hence, it is essential that teaching institutions establish a discussion regarding this topic.
