OBJECTIVES: To detect a potential hidden dissemination of the blaOXA-48 gene among Proteus mirabilis isolates obtained from a single centre. METHODS: P. mirabilis from diverse clinical samples presenting an ESBL phenotype or obtained from blood cultured from 2017 to 2019 were evaluated. Bacterial identification was performed using MALDI-TOF MS. MICs were determined using International Organization for Standardization (ISO) standard microdilution and interpreted following EUCAST guidelines. WGS was performed using both short- and long-read technologies and assemblies were done using Unicycler. Resistomes were assessed using the ResFinder database. SNPs were detected using the PATRIC bioinformatics platform. Cloning experiments were performed using the pCRII-TOPO cloning kit. RESULTS: Thirty-one out of 108 (28.7%) isolates were positive for blaOXA-48 and blaCTX-M-15. Twenty-nine out of 31 of the isolates were susceptible to temocillin, piperacillin/tazobactam, ertapenem and meropenem, whereas only 2/31 showed a resistance phenotype against these antibiotics. Both blaOXA-48 and blaCTX-M-15 genes were detected within the same chromosomally integrated new transposon in all isolates. The resistant isolates displayed a single mutation located in the putative promoter upstream of blaOXA-48. Cloning experiments confirmed that the mutation was responsible for the resistance phenotype. CONCLUSIONS: The presence of a chromosomal copy of blaOXA-48 did not confer resistance to carbapenems, but a single mutation in the promoter could lead to an increase in resistance. This study shows a hidden circulation of OXA-48-positive, but carbapenem- and piperacillin/tazobactam-susceptible, P. mirabilis isolates that can become resistant to ß-lactams after a single mutation.

AIM: Describe gender-based violence by intimate partners against female nurses in a sample of nurses in Andalucia, Spain. DESIGN: Descriptive transversal study. SETTING: Hospitals and primary health care districts in Andalucia. PARTICIPANTS: Six hundred and twenty-two female nurses that work as nurses in the eight provinces in Andalucia (Spain). MEASURES: Social-demographic characteristics and presence of abuse (psychological, physical and sexual). RESULTS: 78.5% of the nurses were married or with a regular partner and had the economic income based on both salaries; 71.1% had a child or an elderly dependent person. It was proved that there can be a statistical association between abuse and: marital status; life together; familiar economic support and children and/or dependent elderly person. The average age was 42.5±8.1 years old (22-62 years) and presented statistical age differences comparing both groups: abused (average 44 years) and non-abused (average 41.8 years). Between the married couples studied, 21.7% of them belong to the social class I and 16.9% to the social class II. Between all studied nurses, 33.0% suffered abuse, among which 75.1% were psychologically abused. Of all the abuse cases 60% were less severe and 40% more serious. CONCLUSIONS: It was confirmed the presence of intimate partner violence (IPV) against nurses, which was predominantly psychological abuse, but others classes of abuse were present too.

OBJECTIVE: To describe the distribution of frequent attenders (FA) through the different primary care practices in Cordoba-Guadalquivir Health District (Córdoba, Spain). METHODS: An ecological study was performed, including data from 2011 to 2015. Defining FA as those subjects who made12 or more appointments per year; independently analysed for nursing, general practice and paediatrics. Prevalence of frequent attendance and FA/professional ratio were used as dependent variables. Demographic characteristics from district population, number of health professionals and use of general facilities were also examinated. Aiming to understand FA distribution, primary health settings were classified according to facility size and environmental location (urban, suburban and rural). RESULTS: The mean prevalence for FA was 10.86% (0.5 SE) for nursing; general practice 21.70% (0.7 SE) and for paediatrics 16.96% (0.7 SE). FA/professional ratios for the different professional categories were: 101.07 (5.0 SE) for nursing, 239.74 (9.0 SE) for general practice and 159.54 (9.8 SE) for paediatrics. CONCLUSIONS: A major part of primary health care users make a high number of consultations. From this group, women overuse nursing and general practitioner services more compared to men. A higher prevalence of FAs was observed in smaller settings, in rural areas. Although taking the FAs:professional ratio as the bar, medium-size practices are more highly overused.
