RESUMO

Magnetars are strongly magnetized, isolated neutron stars1-3 with magnetic fields up to around 1015 gauss, luminosities of approximately 1031-1036 ergs per second and rotation periods of about 0.3-12.0 s. Very energetic giant flares from galactic magnetars (peak luminosities of 1044-1047 ergs per second, lasting approximately 0.1 s) have been detected in hard X-rays and soft γ-rays4, and only one has been detected from outside our galaxy5. During such giant flares, quasi-periodic oscillations (QPOs) with low (less than 150 hertz) and high (greater than 500 hertz) frequencies have been observed6-9, but their statistical significance has been questioned10. High-frequency QPOs have been seen only during the tail phase of the flare9. Here we report the observation of two broad QPOs at approximately 2,132 hertz and 4,250 hertz in the main peak of a giant γ-ray flare11 in the direction of the NGC 253 galaxy12-17, disappearing after 3.5 milliseconds. The flare was detected on 15 April 2020 by the Atmosphere-Space Interactions Monitor instrument18,19 aboard the International Space Station, which was the only instrument that recorded the main burst phase (0.8-3.2 milliseconds) in the full energy range (50 × 103 to 40 × 106 electronvolts) without suffering from saturation effects such as deadtime and pile-up. Along with sudden spectral variations, these extremely high-frequency oscillations in the burst peak are a crucial component that will aid our understanding of magnetar giant flares.

The Tavis-Cummings model is intensively investigated in quantum optics and has important applications in generation of multi-atom entanglement. Here, we employ a superconducting circuit quantum electrodynamic system to study a modified Tavis-Cummings model with directly-coupled atoms. In our device, three superconducting artificial atoms are arranged in a chain with direct coupling through fixed capacitors and strongly coupled to a transmission line resonator. By performing transmission spectrum measurements, we observe different anticrossing structures when one or two qubits are resonantly coupled to the resonator. In the case of the two-qubit Tavis-Cummings model without qubit-qubit interaction, we observe two dips at the resonance point of the anticrossing. The splitting of these dips is determined by Δ λ=2g12+g32, where g1 and g3 are the coupling strengths between Qubit 1 and the resonator, and Qubit 3 and the resonator, respectively. The direct coupling J12 between the two qubits results in three dressed states in the two-qubit Tavis-Cummings model at the frequency resonance point, leading to three dips in the transmission spectrum. In this case, the distance between the two farthest and asymmetrical dips, arising from the energy level splitting, is larger than in the previous case. The frequency interval between these two dips is determined by the difference in eigenvalues (Δ λ=ε 1+-ε 1-), obtained through numerical calculations. What we believe as novel and intriguing experimental results may potentially advance quantum optics experiments, providing valuable insights for future research.

OBJECTIVE: Children with hearing loss experience greater difficulty understanding speech in the presence of noise and reverberation relative to their normal hearing peers despite provision of appropriate amplification. The fidelity of fundamental frequency of voice (f0) encoding-a salient temporal cue for understanding speech in noise-could play a significant role in explaining the variance in abilities among children. However, the nature of deficits in f0 encoding and its relationship with speech understanding are poorly understood. To this end, we evaluated the influence of frequency-specific f0 encoding on speech perception abilities of children with and without hearing loss in the presence of noise and/or reverberation. METHODS: In 14 school-aged children with sensorineural hearing loss fitted with hearing aids and 29 normal hearing peers, envelope following responses (EFRs) were elicited by the vowel /i/, modified to estimate f0 encoding in low (<1.1 kHz) and higher frequencies simultaneously. EFRs to /i/ were elicited in quiet, in the presence of speech-shaped noise at +5 dB signal to noise ratio, with simulated reverberation time of 0.62 sec, as well as both noise and reverberation. EFRs were recorded using single-channel electroencephalogram between the vertex and the nape while children watched a silent movie with captions. Speech discrimination accuracy was measured using the University of Western Ontario Distinctive Features Differences test in each of the four acoustic conditions. Stimuli for EFR recordings and speech discrimination were presented monaurally. RESULTS: Both groups of children demonstrated a frequency-dependent dichotomy in the disruption of f0 encoding, as reflected in EFR amplitude and phase coherence. Greater disruption (i.e., lower EFR amplitudes and phase coherence) was evident in EFRs elicited by low frequencies due to noise and greater disruption was evident in EFRs elicited by higher frequencies due to reverberation. Relative to normal hearing peers, children with hearing loss demonstrated: (a) greater disruption of f0 encoding at low frequencies, particularly in the presence of reverberation, and (b) a positive relationship between f0 encoding at low frequencies and speech discrimination in the hardest listening condition (i.e., when both noise and reverberation were present). CONCLUSIONS: Together, these results provide new evidence for the persistence of suprathreshold temporal processing deficits related to f0 encoding in children despite the provision of appropriate amplification to compensate for hearing loss. These objectively measurable deficits may underlie the greater difficulty experienced by children with hearing loss.

AIM: To evaluate the diagnostic value and clinical applicability of single-photon-emission computed tomography (SPECT) for lumbar spondylolysis using meta-analysis. MATERIALS AND METHODS: Stata 12.0, was used to test the heterogeneity, and the pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, diagnostic odds ratio, and other effect sizes were collected to generate the summary receiver operating characteristic (SROC) curve for comprehensive evaluation. Meta-regression analysis was used to explore the source of heterogeneity, and subgroup analysis was performed. Funnel plots, Fagan's line diagrams, and likelihood ratio dot plots were drawn to evaluate publication bias and clinical applicability. RESULTS: Eight studies involving 785 patients were included. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, odds ratio, and area under the SROC curve of SPECT for the diagnosis of lumbar spondylolysis were 0.85 (95% confidence interval [CI]: 0.70 0.93), 0.92 (95% CI: 0.60 0.99), 11.01 (95% CI: 1.61 75.18), 0.17 (95% CI: 0.08 0.35), 0.92 (95% CI: 0.90 0.94). Meta-regression analysis showed that the sources of heterogeneity were region and age. Subgroup analysis showed that the specificity of the child and adolescent subgroup was significantly higher than that of the middle-aged and elderly subgroups. Deek's funnel plots showed no significant publication bias. The pooled effect of the likelihood ratio dot plot for diagnosis is in the upper-right quadrant. CONCLUSION: As a diagnostic tool for spondylolysis, SPECT has a high degree of specificity, moderate sensitivity, and relatively high diagnostic effectiveness. It can be used as an auxiliary sign in the diagnosis and treatment of lumbar spondylolysis.

Children's speech understanding is vulnerable to indoor noise and reverberation: e.g., from classrooms. It is unknown how they develop the ability to use temporal acoustic cues, specifically amplitude modulation (AM) and voice onset time (VOT), which are important for perceiving distorted speech. Through three experiments, we investigated the typical development of AM depth detection in vowels (experiment I), categorical perception of VOT (experiment II), and consonant identification (experiment III) in quiet and in speech-shaped noise (SSN) and mild reverberation in 6- to 14-year-old children. Our findings suggested that AM depth detection using a naturally produced vowel at the rate of the fundamental frequency was particularly difficult for children and with acoustic distortions. While the VOT cue salience was monotonically attenuated with increasing signal-to-noise ratio of SSN, its utility for consonant discrimination was completely removed even under mild reverberation. The reverberant energy decay in distorting critical temporal cues provided further evidence that may explain the error patterns observed in consonant identification. By 11-14 years of age, children approached adult-like performance in consonant discrimination and identification under adverse acoustics, emphasizing the need for good acoustics for younger children as they develop auditory skills to process distorted speech in everyday listening environments.

This study examined the role of visual speech in providing release from perceptual masking in children by comparing visual speech benefit across conditions with and without a spatial separation cue. Auditory-only and audiovisual speech recognition thresholds in a two-talker speech masker were obtained from 21 children with typical hearing (7-9 years of age) using a color-number identification task. The target was presented from a loudspeaker at 0° azimuth. Masker source location varied across conditions. In the spatially collocated condition, the masker was also presented from the loudspeaker at 0° azimuth. In the spatially separated condition, the masker was presented from the loudspeaker at 0° azimuth and a loudspeaker at -90° azimuth, with the signal from the -90° loudspeaker leading the signal from the 0° loudspeaker by 4 ms. The visual stimulus (static image or video of the target talker) was presented at 0° azimuth. Children achieved better thresholds when the spatial cue was provided and when the visual cue was provided. Visual and spatial cue benefit did not differ significantly depending on the presence of the other cue. Additional studies are needed to characterize how children's preferential use of visual and spatial cues varies depending on the strength of each cue.

Children with cochlear implants (CI) communicate in noisy environments, such as in classrooms, where multiple talkers and reverberation are present. Speakers compensate for noise via the 'Lombard effect'. The present study examined the Lombard effect on the intensity and duration of stressed vowels in the speech of children with Cochlear Implants (CIs) as compared to children with Normal Hearing (NH), focusing on the effects of speech-shaped noise (SSN) and speech-shaped noise with reverberation (SSN+Reverberation). The sample consisted of 7 children with CIs and 7 children with NH, aged 7-12 years. Regarding intensity, a) children with CIs produced stressed vowels with an overall greater intensity across acoustic conditions as compared to NH peers, b) both groups increased their stressed vowel intensity for all vowels from Quiet to both noise conditions, and c) children with NH further increased their intensity when reverberation was added to SSN, esp. for the vowel /u/. Regarding duration, longer stressed vowels were produced by children with CIs as compared to NH in Quiet and SSN conditions but the effect was retained only for the vowels /i/, /o/ and /u/ when reverberation was added to noise. The SSN+Reverberation condition induced systematic lengthening in stressed vowels for children with NH. Furthermore, although greater intensity and duration ratios of stressed/unstressed syllables were observed for children with NH as compared to CIs in Quiet condition, they diminished with noise. The differences observed across groups have implications for speaking in classroom noise.

Antimicrobial resistance poses a serious threat to public health and is one of the major challenges worldwide. As global social, economic, and environmental changes lead to increased exposure of populations to antimicrobials, the antimicrobial resistance of pathogens has accelerated and resulted in weakened clinical infection treatment effects. This article reviews the main mechanisms and driving factors of the production and spread of antimicrobial resistance from the perspective of "One Health"and discusses methods and strategies for controlling antimicrobial resistance from multiple dimensions. It also looks forward to the prospects of research and prevention of drug resistance to explore antimicrobial resistance prevention and control strategies based on "One Health".

Objective: To investigate the epidemiological characteristics of human respiratory syncytial virus (HRSV) in patients with Severe Acute Respiratory Infection (SARI) in Tianjin from 2015 to 2020. Methods: The study data were obtained from the Third Center Hospital of Tianjin, a designated sentinel hospital, from 2015 to 2020, with 1 597 SARI patients enrolled in this study. The clinical specimens of the research participants were subjected to respiratory multi-pathogen testing. HRSV-positive specimens were subtyped to analyze the differences in HRSV detection rates among cases of different age groups and periods and their mixed infection situations. Results: A total of 1 597 nasopharyngeal swabs were collected, with an HRSV detection rate of 4.20%. Among 67 HRSV-positive specimens, there were 19 pure HRSV-A nucleic acid-positive specimens, 19 pure HRSV-B nucleic acid-positive specimens and 29 mixed HRSV-A and HRSV-B nucleic acid-positive specimens. The difference in HRSV detection rate among different age groups was statistically significant (P<0.05), and the HRSV detection rate in children under five years old was higher than that in other age groups. From 2016 to 2020, the detection rate of HRSV showed an increasing trend year by year. The HRSV detection rate of SARI cases was highest in the winter season, at 7.15%. There were 10 (14.93%) mixed positive cases for HRSV and other viruses, of which four were mixed positive for HRSV and influenza A. Conclusion: The incidence of HRSV in Tianjin exhibits an increasing trend from 2016 to 2020, peaking during the winter season, with children under five years old constituting a high-risk demographic for HRSV infection.

Environmental noise and reverberation challenge speech understanding more significantly in children than in adults. However, the neural/sensory basis for the difference is poorly understood. We evaluated the impact of noise and reverberation on the neural processing of the fundamental frequency of voice (f0 )-an important cue to tag or recognize a speaker. In a group of 39 6- to 15-year-old children and 26 adults with normal hearing, envelope following responses (EFRs) were elicited by a male-spoken /i/ in quiet, noise, reverberation, and both noise and reverberation. Due to increased resolvability of harmonics at lower than higher vowel formants that may affect susceptibility to noise and/or reverberation, the /i/ was modified to elicit two EFRs: one initiated by the low frequency first formant (F1) and the other initiated by mid to high frequency second and higher formants (F2+) with predominantly resolved and unresolved harmonics, respectively. F1 EFRs were more susceptible to noise whereas F2+ EFRs were more susceptible to reverberation. Reverberation resulted in greater attenuation of F1 EFRs in adults than children, and greater attenuation of F2+ EFRs in older than younger children. Reduced modulation depth caused by reverberation and noise explained changes in F2+ EFRs but was not the primary determinant for F1 EFRs. Experimental data paralleled modelled EFRs, especially for F1. Together, data suggest that noise or reverberation influences the robustness of f0 encoding depending on the resolvability of vowel harmonics and that maturation of processing temporal/envelope information of voice is delayed in reverberation, particularly for low frequency stimuli.

Web-based testing is an appealing option for expanding psychoacoustics research outside laboratory environments due to its simple logistics. For example, research participants partake in listening tasks using their own computer and audio hardware and can participate in a comfortable environment of their choice at their own pace. However, it is unknown how deviations from conventional in-lab testing affect data quality, particularly in binaural hearing tasks that traditionally require highly precise audio presentation. Here, we used an online platform to replicate two published in-lab experiments: lateralization to interaural time and level differences (ITD and ILD, experiment I) and dichotic and contralateral unmasking of speech (experiment II) in normal-hearing (NH) young adults. Lateralization data collected online were strikingly similar to in-lab results. Likewise, the amount of unmasking measured online and in-lab differed by less than 1 dB, although online participants demonstrated higher speech reception thresholds overall than those tested in-lab by up to ∼7 dB. Results from online participants who completed a hearing screening versus those who self-reported NH did not differ significantly. We conclude that web-based psychoacoustics testing is a viable option for assessing binaural hearing abilities among young NH adults and discuss important considerations for online study design.

Objective: To analyze the related factors of futile recanalization (FR) after emergency endovascular treatment of large artery occlusion in anterior circulation. Methods: Three studies on endovascular treatment of acute anterior circulation large vessels occlusion stroke were selected, and their data were merged for retrospective analysis. Patients were divided into the FR group and favorable prognosis group according to the functional prognosis. Risk factor analysis was conducted using multivariate logistic regression. Results: A total of 1 581 patients were finally included, with 858 (48.9%) patients in favorable prognosis group and 926 (51.91%) patients in FR group. Among them, there were 939 males and 642 females, with a mean age of (65±12) years. Multivariate logistic regression analysis showed that National Institute of Health Stroke Scale (NIHSS) score (OR=1.089,95%CI:1.066-1.113), puncture to recanalization time (OR=0.756, 95%CI:0.586-0.971), age (OR=1.04,95%CI:1.029-1.051), serum glucose (OR=1.101,95%CI:1.062-1.143), systolic blood pressure (OR=1.005,95%CI:1.001-1.010), passes≥3(OR=1.941,95%CI:1.294-2.941)Alberta stroke program early CT (ASPECT) score (OR=0.919,95%CI:0.847-0.996), occlusion site (M1 segment of middle cerebral artery, OR=0.744,95%CI:0.565-0.980) and collateral circulation [(2 points, OR=0.757, 95%CI:0.581-0.985); (3-4 points, adjusted OR=0.640, 95%CI: 0.472-0.866)] were independent factors of FR. Conclusion: The incidence of FR in patients with large artery occlusion in anterior circulation who achieve satisfied reperfusion after endovascular treatment is high. Higher NIHSS score, longer puncture to recanalization time, older age, higher serum glucose and systolic blood pressure are risk factors, while lower ASPECTS, occlusion in cerebral middle M1 segment, better collateral circulation are protective factors.

Objective: To understand the antimicrobial resistance and genome characteristics of Campylobacter isolates recovered from retailed poultry meat samples in 20 provinces in China in 2020. Methods: In 2020, 265 Campylobacter strains including 244 Campylobacter jejuni and 21 Campylobacter coli collected from retailed poultry meat samples in China were tested for antimicrobial resistance to 9 antimicrobial compounds by using the agar dilution method. Forty-two selected isolates were sent for whole genome sequencing and 38 high-quality genomes were analyzed for their antimicrobial resistance genes, virulence genes, sequence types and genetic diversity. Results: The resistance rates of Campylobacter isolates from poultry meats to tetracycline, nalidixic acid and ciprofloxacin were the highest (84%-100%), with 53.2% of the isolates showing multidrug resistance in this study. The resistance rates of C. coli to erythromycin, azithromycin, telithromycin, gentamicin and clindamycin were significantly higher than those of C. jejuni (P<0.05). The resistance genes conferring resistance to ß-lactams (100%, 38/38), quinolones (94.7%, 36/38), tetracycline (81.6%, 31/38) and aminoglycosides (50%, 19/38) were the most frequently detected among 38 Campylobacter genomes. C. jejuni carried more virulence genes than C. coli. In total, 19 and 17 sequence types (ST) were obtained from 20 sequenced C. jejuni and 18 C. coli isolates, respectively, including 5 novel STs. The isolates showed a high genetic diversity based on their sequence types. Conclusion: The phenomenon of antimicrobial resistance in Campylobacter from poultry meat sources in China is relatively serious, and resistance and virulence genes are widely distributed in Campylobacter. There is genetic diversity in Campylobacter.

With the outbreak of infectious diseases, more and more attention has been paid to surveillance and early warning work. Timely and accurate monitoring data is the basis of infectious diseases prevention and control. Effective early warning methods for infectious diseases can improve the timeliness and sensitivity of early warning work. This paper briefly introduces the intelligent early warning model of infectious diseases, summarizes the emerging surveillance and early warning methods of infectious diseases, and seeks the possibility of diversified surveillance and early warning in different epidemic stages and different outbreak scenarios of infectious diseases. This paper puts forward the idea of constructing a diversified method system of infectious diseases surveillance and early warning based on multi-stages and multi-scenarios and discusses the future development trend of infectious diseases surveillance and early warning, in order to provide reference for improving the construction level of infectious diseases surveillance and early warning system in China.

Objective: To understand the epidemiological characteristics of suspected occupational diseases cases, and to track the subsequent diagnosis of suspected cases in Hubei Province from 2020 to 2021, and to provide theoretical basis for the supervision of suspected occupational diseases. Methods: In April 2022, the data of suspected occupational diseases cases and occupational diseases in Hubei Province from 2020 to 2021 were collected by the Occupational Diseases and Health Risk Factors Information Surveillance System. The distribution and diagnosis of suspected occupational diseases cases were analyzed. We investigated undiagnosed suspected occupational diseases by telephone. Results: From 2020 to 2021, a total of 1872 cases of suspected occupational diseases in 6 categories and 18 species were reported in Hubei Province. The top three suspected occupational diseases were suspected occupational noise deafness (36.75%, 688/1872), suspected coal worker's pneumoconiosis (33.07%, 619/1872) and suspected silicosis (20.99%, 393/1872). The diagnosis rate of suspected occupational diseases was 33.60% (629/1872). The rate of confirmed diagnosis was 63.59% (400/629). The diagnosis rate (26.86%, 456/1698) and rate of confirmed diagnosis (55.48%, 253/456) of suspected occupational diseases detected by occupational health examination were the lowest. The diagnosis rate of suspected occupational diseases detected by comprehensive medical institutions and private medical institutions were lower than disease prevention and control institutions and occupational disease prevention center (P<0.05). The main reasons for not entering the diagnostic procedure included that workers were not informed that they were diagnosed as suspected occupational diseases (31.55%, 124/393), workers were unwilling to apply for occupational disease diagnosis (18.56%, 73/393), and some workers planned to apply for diagnosis but had not yet applied (10.69%, 42/393) . Conclusion: Occupational noise deafness, coal worker's pneumoconiosis and silicosis are the main diseases of suspected occupational diseases in Hubei Province. In order to increase the diagnosis rate and confirmed diagnosis rate of suspected occupational diseases, it is suggested to strengthen management and supervision from the aspects of case management, information warning and worker notification.

Objective: To investigate the occurrence of work-related musculoskeletal disorders (WMSDs) in logistics workers of automobile manufacturing enterprises in Guangzhou, and explore their potential categories and influencing factors, so as to provide basis for the prevention and control of WMSDs in logistics workers of automobile manufacturing enterprises. Methods: From April to May 2020, a convenient sampling method was adopted to select 1442 logistics workers in 2 automobile manufacturing enterprises in Guangzhou as the research objects. The incidence of WMSDs among them was investigated by using the Nordic Musculoskeletal Disorders Questionnaire, and the incidence pattern of WMSDs among the subjects was analyzed by the method of potential category analysis. Chi-square test and multiple logistic regression model were used to analyze the influencing factors of the disease. Results: The total prevalence of WMSDs was 42.9% (619/1442). The three parts with high prevalence of WMSDs were neck (23.5%, 339/1442), shoulder (21.3%, 307/1442) and lower back (19.1%, 275/1442). 69.0% (427/619) of the patients showed simultaneous disease in two or more sites. The subjects were divided into four latent groups by the analysis of latent categories: the group with multiple body parts (4.51%), the group with very low or no disease (70.39%), the group with shoulder and neck and upper back (11.37%), and the group with limbs and lower back (13.73%). Multivariate logistic regression showed that compared with the "very low or no disease group", the risk factors of multiple body parts of automotive logistics workers suffering from WMSDs mainly included: Working posture uncomfortable (OR=3.10, 95%CI: 1.44~6.70, P=0.004), often bend and turn (OR=2.36, 95%CI: 1.05~5.29, P=0.037), bend your knees homework for a long time (OR=2.29, 95%CI: 1.17~4.47, P=0.015) ; Working posture uncomfortable (OR=2.26, 95%CI: 1.51~3.38, P<0.001), bending the neck (OR=1.68, 95%CI: 1.03~2.74, P=0.036), neck minister time keep the same position (OR=1.81, 95%CI: 1.22~2.67, P= 0.003) It is a risk factor for shoulder, neck and upper back WMSDs; Risk factors for limb-lower back WMSDs include: Length > 10 years (OR=2.16, 95%CI: 1.24~3.78, P<0.001), the working position uncomfortable (OR=2.38, 95%CI: 1.63~3.48, P<0.001), bending the neck (OR=1.66, 95%CI: 1.03~2.68, P=0.039) . Conclusion: The prevalence rate of WMSDs among logistics workers in automobile manufacturing enterprises is high, which has obvious characteristics of classification and comorbis, and is closely related to individual characteristics such as sufficient rest time and working posture of workers.

Some infectious diseases, including COVID-19, can undergo airborne transmission. This may happen at close proximity, but as time indoors increases, infections can occur in shared room air despite distancing. We propose two indicators of infection risk for this situation, that is, relative risk parameter (Hr) and risk parameter (H). They combine the key factors that control airborne disease transmission indoors: virus-containing aerosol generation rate, breathing flow rate, masking and its quality, ventilation and aerosol-removal rates, number of occupants, and duration of exposure. COVID-19 outbreaks show a clear trend that is consistent with airborne infection and enable recommendations to minimize transmission risk. Transmission in typical prepandemic indoor spaces is highly sensitive to mitigation efforts. Previous outbreaks of measles, influenza, and tuberculosis were also assessed. Measles outbreaks occur at much lower risk parameter values than COVID-19, while tuberculosis outbreaks are observed at higher risk parameter values. Because both diseases are accepted as airborne, the fact that COVID-19 is less contagious than measles does not rule out airborne transmission. It is important that future outbreak reports include information on masking, ventilation and aerosol-removal rates, number of occupants, and duration of exposure, to investigate airborne transmission.

OBJECTIVE: The antenatal screening strategy remains inadequate for eliminating congenital syphilis. To further eliminate maternal fetal transmission, preconception syphilis screening is considered an option. In this study, we investigated syphilis seropositivity and intra-couple discordance among married couples planning a pregnancy in China to provide essential baseline evidence for preconception syphilis screening. DESIGN: Population-based survey. SETTING: National preconception registered data. POPULATION: Married Chinese couples planning conception within 6 months between 2013 and 2018. METHODS: Syphilis was screened using rapid plasma reagin (RPR); infection self-reporting and sociodemographic characteristics were collected through questionnaires and medical records, respectively. r 3.2.2 and arcgis 10.2 were used for statistical analyses and geographic mapping. MAIN OUTCOME MEASURES: RPR seropositivity. RESULTS: Among 31 955 041 couples, 29 737 172 (93.06%) had complete RPR results for both spouses; of those, 0.62% (186 100) were seropositive, with dramatic intra-couple discordance, with 0.33% positivity in wives, 0.24% positivity in husbands and 0.05% positivity in both spouses. Across time, both seropositivity and intra-couple discordance remained stable. Seropositivity in different regions varied significantly, with provincial rates ranging geographically from Tibet (0.8%) to Hebei (0.2%) (P < 0.05). Economic level was an independent factor for this regional variation, with seropositivity increasing as gross domestic product income decreased (P < 0.05). CONCLUSIONS: Intra-couple discordance in seropositivity for syphilis is notable among couples, with a considerable rate of pre-existing syphilis before pregnancy. Thus, screening both spouses during integrated preconception health care is recommended for further eliminating maternal-fetal transmission. OBJECTIVE: To evaluate the association between the vaginal microenvironment and fecundability among women. DESIGN: Register-based nationwide cohort study. SETTING: Chinese National Free Pre-conception Check-up Project from 2015 to 2018. POPULATION: Our study included a total of 3 388 554 eligible women who were attempting to become pregnant. METHOD: We assessed the vaginal microenvironment at baseline by considering four indices: vaginal pH, clue cell examination, whiff test and vaginal cleanliness grading. If any of these indicators was abnormal, the vaginal microenvironment was defined as poor. Propensity score matching was used to control for potential confounders and reduce bias. Logistic models were used to estimate the fecundability odds ratios (FORs) after adjustment for covariates. MAIN OUTCOME MEASURES: Achievement of a pregnancy within 1 year. RESULTS: Of the total study population, 379 718 women (11.2%) had a poor vaginal microenvironment and their pregnancy rate after 1 year was significantly lower than the group with a normal microenvironment (71.8% versus 76.1%, P < 0.001). After adjusting for potential confounders, the women with a poor vaginal microenvironment were associated with a 9% reduction in fecundability compared with the normal microenvironment group (FOR 0.91, 95% CI 0.90-0.92). The adverse effects of a poor vaginal microenvironment were stronger among multipara (FOR 0.89, 95% CI 0.87-0.90) or women with irregular menstruation (FOR 0.86, 95% CI 0.84-0.89). CONCLUSION: There was a negative association between a poor vaginal microenvironment and the fecundability of women. These findings highlight the significance of assessing the vaginal microenvironment during pre-pregnancy health examinations. OBJECTIVES: To investigate the role of auditory cues for spatial release from masking (SRM) in children with bilateral cochlear implants (BiCIs) and compare their performance with children with normal hearing (NH). To quantify the contribution to speech intelligibility benefits from individual auditory cues: head shadow, binaural redundancy, and interaural differences; as well as from multiple cues: SRM and binaural squelch. To assess SRM using a novel approach of adaptive target-masker angular separation, which provides a more functionally relevant assessment in realistic complex auditory environments. DESIGN: Children fitted with BiCIs (N = 11) and with NH (N = 18) were tested in virtual acoustic space that was simulated using head-related transfer functions measured from individual children with BiCIs behind the ear and from a standard head and torso simulator for all NH children. In experiment I, by comparing speech reception thresholds across 4 test conditions that varied in target-masker spatial separation (colocated versus separated at 180°) and listening conditions (monaural versus binaural/bilateral listening), intelligibility benefits were derived for individual auditory cues for SRM. In experiment II, SRM was quantified using a novel measure to find the minimum angular separation (MAS) between the target and masker to achieve a fixed 20% intelligibility improvement. Target speech was fixed at either +90 or -90° azimuth on the side closer to the better ear (+90° for all NH children) and masker locations were adaptively varied. RESULTS: In experiment I, children with BiCIs as a group had smaller intelligibility benefits from head shadow than NH children. No group difference was observed in benefits from binaural redundancy or interaural difference cues. In both groups of children, individuals who gained a larger benefit from interaural differences relied less on monaural head shadow, and vice versa. In experiment II, all children with BiCIs demonstrated measurable MAS thresholds <180° and on average larger than that from NH children. Eight of 11 children with BiCIs and all NH children had a MAS threshold <90°, requiring interaural differences only to gain the target intelligibility benefit; whereas the other 3 children with BiCIs had a MAS between 120° and 137°, requiring monaural head shadow for SRM. CONCLUSIONS: When target and maskers were separated at 180° on opposing hemifields, children with BiCIs demonstrated greater intelligibility benefits from head shadow and interaural differences than previous literature showed with a smaller separation. Children with BiCIs demonstrated individual differences in using auditory cues for SRM. From the MAS thresholds, more than half of the children with BiCIs demonstrated robust access to interaural differences without needing additional monaural head shadow for SRM. Both experiments led to the conclusion that individualized fitting strategies in the bilateral devices may be warranted to maximize spatial hearing for children with BiCIs in complex auditory environments.
