Coagulative properties of the blood and exudate accumulated in the pleural cavity following lung operations were studied. On the grounds of the studies of plasmin potential and active antiplasmin of the exudate a method of local fibrionolytic therapy of coagulated hemothorax with intrapleural injections of fibrinilysis activators was devised. Fibrinolytic therapy was applied in 106 patients, streptokinase preparations were injected to 28 of them.

[Characteristics of the aggregate state of autogenous blood transfusion media]. / Osobennosti agregatnogo sostoianiia autogennykh gemotransfuzionnykh sred.

About 300 samples of the autogenous blood prepared by different methods were examined in order to study peculiarities of the accumulation and structure of microclots of autogenous hemotransfusion media. The morphological and coagulation properties of the blood of patients before exfusion were found to be related to the velocity of accumulation of microaggregates after it. The role of methods of stabilization, conservation and terms of storage is shown in the dynamics of the aggregation state and the role of fibrin in the microaggregate structure. The filtration "in the depth" is shown to be efficient for the prevention of posthemotransfusional microembolism of the pulmonary vascular bed.

[Hemodynamics in double plasmapheresis in lung cancer]. / Gemodinamika pri dvoinom plazmoforeze u bol'nykh rakom legkogo.

The effect of double plasmaphoresis upon the central hemodynamics in 37 cases of pulmonary carcinoma was studied. Plasmaphoresis was aimed at the storage of an autologous transfusional medium to compensate the operative blood loss. Taking 400-500 cc of plasma does not cause any hemodynamic changes in patients without heart disease signs. The choice between surgical intervention and repeated plasmaphoresis should be made with regard to the time necessary for normalization of the circulatory parameters following plasmaphoresis.

[Effect of coagulated hemothorax and early fibrinothorax on the state of patients after pneumonectomy]. / Vliianie svernuvshegosia gemotoraksa i rannego fibrinotoraksa na sostoianie bol'nykh posle pnevmonéktomii

An estimation of the postoperative course after pneumonectomy in 328 patients, depending on the character and rate of distinguishing the pleural fluid, is given. The development of collapsed hemothorax and early (during the first week) fibrinothorax is predominantly related with separation of massive pleural adhesions, it aggravates the postoperative period and predisposes to the development of pleural empyema. Rethoracotomy with the removal of clots especially in later terms fail to prevent this complication. Intrapleural injections of fibrinolytic drugs is found to offer more perspectives.

[The rheological properties of the peripheral blood in acute diffuse suppurative peritonitis and their changes during infusion therapy using Modegel]. / Reologicheskie svoistva perifericheskoi krovi pri ostrom razlitom gnoinom peritonite i ikh izmeneniia pri infuzionnoi terapii s primeneniem Modezhelia.

The rheological properties of blood and deformability of erythrocytes were investigated in 85 patients with acute diffuse peritonitis. It was found that higher viscosity of blood mainly with slow shift rates were observed in the reactive and toxic phases of the disease. In the reactive phase these changes can be considered as adaptational, in the toxic phase--as pathological ones. Lessened viscosity in the terminal phase of the disease is an unfavorable prognostic sign.

[The effect of different methods of photohemotherapy on the rheological properties of the blood in patients with ischemic heart disease]. / Vliianie raznykh metodov fotogemoterapii na reologicheskie svoistva krovi u bol'nykh s ishemicheskoi bolezn'iu serdtsa.

The authors made an analysis of results of examination of 41 patients with ischemic heart disease treated by the standard medicamentous therapy and when using different methods of photohemotherapy against its background. It was established that medicamentous therapy during 2 weeks failed to result in a substantial improvement of rheological properties of blood, while its combination with photohemotherapy could give a considerable positive effect coinciding with clinical improvement of the patient's state. Shorter terms are required to correct hemorheological indices when autotransfusions of photomodified blood are used.

In experiments on dogs with acute massive hemorrhage the effectiveness has been studied of red blood cell suspension in glucose-saline solution N 8 prepared in the Central Research Institute of Hematology and Blood Transfusion, and in a preservative based on gelatin developed in the Leningrad Research Institute of Hematology and Blood Transfusion. The advantage of the colloid preservative has been proved by the parameters of macro- and microdynamics of the blood, oxygen-basic state, oxygen budget and rheologic properties of the blood.

[Transbronchial transthoracic sanation of chronic suppurative foci in the lungs]. / Chrezbronkhial'naia transtorakal'naia sanatsiia khronicheskikh gnoinykh ochagov v legkikh.

A new method of bronchial sanitation consisting in conducting a polyurethane tube through the pulmonary tissue and thoracic wall outward under the bronchoscopy control is suggested. The method has been used successfully in 22 patients.

Functional and morphological investigations have shown that the transfusion of preserved blood used to fill up the operation blood loss results in pulmonary microembolism by microaggregates. The authors believe functional manifestations of microembolism include disturbances of respiratory function, central hemodynamics, water-electrolyte and protein metabolism, the hemotransfusion pulmonary mciroembolism being an additional stress factor of genesis of the postoperative disease.

[Production of transfusion media on the base of erythrocytes and plasma substitute solutions]. / Sozdanie transfuzionnykh sred na osnove éritrotsitov i plazmo- zameshchaiushchikh rastvorov.

A review of investigations has been made conducted in the USSR and abroad on the production of combined polyfunctional hemotransfusion media--red blood cell suspensions in plasma-substituting solutions possessing preservative properties and hemodynamic action. The advantages of the hemotransfusion therapy with the use of these media have been shown associated with the diminution of the risk of posttransfusion complications and reactions (especially under extreme conditions) and depending on economic aspects. The properties of red blood cell suspension in the modified deionization gelatin solution have been considered in detail.

[An erythrocyte suspension in Modegel in the treatment of acute gastrointestinal hemorrhages]. / Vzves' éritrotsitov v "Modezhele" pri lechenii ostrykh gastroduodenal'nykh krovotechenii.

The effectiveness of a suspension of erythrocytes in the new colloid preservative "Modegel" (in 1:1 ratio) was studied in the complex treatment of 31 patients with severe gastroduodenal bleedings. It was established that the hemodynamic, antianemic action of the suspension was not inferior (and even excels) to the action of the whole preserved blood and the erythrocytic mass. The suspension transfusion has a favorable effect upon functions of the liver and kidneys, corrects the acidic-alkaline state of the recipients' blood, decreases the lipid peroxidation and has detoxicating properties. During transfusion of the suspension hypocoagulation of blood doesn't take place. The temperature reactions following the suspension transfusion are rare. The suspension of erythrocytes "Modegel" deserves wide introduction to clinical practice.

[Modezhel' erythrocyte preservative as an experimental and clinical blood substitute]. / Eritrokonservant "Modezhel'" kak krovezamenitel' v éksperimente i klinike.

The model of hemorrhagic shock in dogs was used to show that the infusion of "Modegel" normalizes the arterial pressure, substantially increases the minute volume of blood circulation. The circulating blood volume rises up to the initial level at the expense of the increased plasma volume. "Modegel" gives stable correction of the acid-base balance of blood. In clinic "Modegel" was used for filling the apparatus of artificial circulation and compared with the blood substitute "Gelatinol" for operations on the open heart. Examinations of the patients have shown that "Modegel" unlike "Gelatinol" does not induce hypercalcemia and hyperosmolarity of plasma while moderate hyperoncoticity of plasma is responsible for the circulating blood volume and favorable conditions for microcirculation.

[Leukocyte transfusions in the treatment of sepsis]. / Leikotransfuzii v lechenii sepsisa.

Leukotransfusion from ordinary donors and those immunized by staphylococcal anatoxin were used in complex treatment of 55 patients in a catabolic phase of different forms of sepsis. It was found that transfusion of leukosuspensions, immune ones in particular, had pronounced immunostimulating, detoxicating, antibacterial and anti-inflammatory effects. A short course of transfusions of allogenic leukocytes can be fulfilled without a selection by the HLA system.

[Erythrocyte preservative based on a solution of modified deionized gelatin with adenine and sodium phosphate]. / Konservant dlia éritrotsitov na osnove rastvora modifitsirovannogo deioinizirovannogo zhelatina s adeninom i natriia fosfatom.

The paper reports storage properties of a new erythro-preservative made of the solution of modified deionized gelatin with addition of adenine and disodium phosphate. 42-day storage of red cell suspension in the above preservative was assessed in comparison to Modejele preservative according to the following characteristics: free hemoglobin, the count of osmotically unstable red cells, red cell ATP concentration, red cell deformability index, zeta-potential, extracellular potassium and sodium concentrations, amount of microclots, acid-base condition of the suspensions, blood rheology. Control and test parameters showed the greatest difference by ATP concentrations: on storage day 42 ATP in the controls made up 58.9%, in the test series 77.8% against the baseline. Red cell deformability under storage in the new preservative also tended to a decrease. The rest test parameters did not differ much in the control and test series. Acute mouse, chronic rat, rabbit and dog experiments proved low toxicity of the preservative. The repeated introduction of the preparation brought about a slight shift to hypercoagulation.
