RESUMO

BACKGROUND: Breast cancer multifocality and multicentricity diagnosis influences the surgeon's choice between applying breast conservative therapy or performing mastectomy. PURPOSE: To assess the role of contrast enhanced mammography (CEM) and breast magnetic resonance imaging (MRI) in the assessment of preoperative breast cancer multifocality and multicentricity and to assess their accuracy, agreement and impact on the surgical management. MATERIAL AND METHODS: The study retrospectively included cases over a 5-year period. After analysis and interpretation of suspicious breast lesions, a comparative evaluation of CEM and MRI was conducted with the assessment of diagnostic indices, including sensitivity, specificity and diagnostic accuracy. The kappa (κ) measure of agreement between both modalities was measured. The postoperative specimen pathology was the reference standard. RESULTS: One hundred and twenty-two female cases with 126 breast lesions were evaluated. Specimen pathology, MRI and CEM showed a single neoplastic lesion in 67.5%, 35% and 48.5% of cases, respectively, and multiple neoplastic lesions in 32.5%, 65% and 51.6% of cases, respectively. The sensitivity, specificity and accuracy of MRI were 95.12%, 49.41%,and 64.29%, and the CEM values were 85.37%, 64.71% and 71.43%, respectively. The κ value was 0.592 with an intermediate agreement between both modalities. When comparing between both modalities, enhancing foci showed a statistically significant difference, although there were no statistically significant difference in terms of high breast density or molecular subtype. CONCLUSION: In terms of breast cancer multifocality and multicentricity evaluation, MRI showed a higher sensitivity, while CEM showed a higher specificity, and there was moderate agreement between the two modalities.

RESUMO

The corrosion of carbon steel infrastructure in acidic environments poses significant economic and safety challenges. Traditional inhibitors such as chromates are being phased out due to toxicity concerns. Thus, there is a need to develop effective and sustainable green alternatives. In this work, we evaluated an epoxy-based inhibitor, bisphenol A tetrabromo dipropoxy dianiline tetraglycidyl ether (TGEDADPTBBA), for protecting carbon steel against corrosion in 1 M hydrochloric acid. An integrated experiment-computation approach was employed. Polarization curves and electrochemical impedance spectroscopy were used to assess the inhibition efficiency and mechanism of TGEDADPTBBA. Quantum chemical calculations and molecular dynamics simulations provided atomic-level insights into adsorption behavior. Scanning electron microscopy with energy-dispersive X-ray spectroscopy characterized the surface morphology. The results showed that TGEDADPTBBA acted as a highly effective mixed-type inhibitor, achieving over 95% inhibition efficiency at a 10-3 M concentration. It suppressed corrosion currents while increasing the charge transfer resistance. Theoretical studies revealed that TGEDADPTBBA adsorbed onto steel surfaces via both electrostatic and van der Waals interactions. This stable adsorption facilitated the formation of a protective barrier layer, as observed experimentally. Notably, our work demonstrated the synergistic potential of combining experimental corrosion testing with computational modeling to develop structure-property relationships for innovative inhibitor design. This integrated approach offers insight into inhibition mechanisms and presents TGEDADPTBBA as an attractive green corrosion inhibitor alternative for industrial applications.

RESUMO

Anticorrosive protection efficiency of novel tetrafunctional epoxy prepolymer, namely 2,3,4,5-tetraglycidyloxy pentanal (TGP), for mild steel in 1 M HCl medium was assessed through potentiodynamic polarization (PDP), electrochemical impedance spectroscopy (EIS), scanning electron microscope (SEM), energy dispersive X-ray spectroscopy (EDS), contact angle (CA), adsorption isotherm model, temperature effect and thermodynamic parameters. The synthesized TGP was characterized and confirmed by Fourier transform infrared (FTIR) spectroscopy and nuclear magnetic resonance (NMR). The inhibitory efficiencies found at lower concentration of the prepolymer TGP were85% (PDP) and 87.17% (EIS). PDP measurement illustrated that the TGP behaved as a mixed-type inhibitor in the realized solution. SEM and EDS analysis showeda significant decrease in the corrosion of the MS surface in the presence of the inhibitory prepolymer compared with the blank (1 M HCl). Langmuir adsorption isotherm is the most acceptable modelto describe the TGP epoxy prepolymer on the MS area.

RESUMO

This paper summarizes a review of the viscosimetric, viscoelastic and rheological properties of polymers and hybrid nanocomposite polymers. Hybrid nanocomposites can be combined from natural fibers or synthetic fibers and/or both. The hybrid nanocomposite polymer offers the designer the opportunity to achieve the required characteristics to a considerable extent controlled by the choice of appropriate fibers or fillers and the polymer architecture. The rheological behavior of hybrid nanocomposite depends on fiber content, fiber length, fiber orientation, fiber-to-matrix bonding, fiber configuration and filler, respectively. Further, rheological properties of hybrid nanocomposite polymers by introducing various charges were examined discussed.

RESUMO

We present the case of a 54-year-old male patient diagnosed with a right upper lobe lung cancer and was referred for resection. Positron emission tomography-computed tomography scan showed a prominent vascular structure in the right lung, suspicious for vascular malformation. A computed tomography angiography was done, demonstrating an intercostobronchial trunk-pulmonary artery fistula. There was also non-tapering dilated wandering pulmonary artery coursing through the right lower lung without any abnormal connection with pulmonary veins and were supplying normal lung parenchyma. Amplatzer vascular plugs were used for the treatment of the intercostobronchial trunk-pulmonary artery fistula. Our case highlights these very rare vascular anomalies, and their management.

RESUMO

PURPOSE: We aimed to evaluate the frequency of persistence and complication rates of pulmonary arteriovenous malformations (PAVMs) treated with Amplatzer vascular plug (AVP) or Amplatzer vascular plug type 2 (AVP2). METHODS: We retrospectively reviewed a total of 22 patients with 54 PAVMs between June 2004 and June 2014. We included 12 patients with 35 PAVMs who received percutaneous embolization using AVP or AVP2 only without the use of any other embolic devices. The mean follow-up was 54±24.3 months (range, 31-97 months). The primary end-points of the study were the efficacy of embolotherapy, the increase in oxygen saturation, and the persistence of PAVM on follow-up. Secondary end point was the incidence of complications. RESULTS: The study included 10 female and two male patients with a mean age of 50.2±13.7 years (range, 21-66 years). All PAVMs had a simple angioarchitecture. The technical success of the procedure for PAVM occlusion was 100%. There was a significant increase in the oxygen saturation following embolotherapy (P < 0.0001). Follow-up computed tomography angiography revealed successful treatment in 34 PAVMs (97%) and failed treatment in one PAVM (3%). Twenty-three aneurysmal sacs (67%) showed complete disappearance. The failed treatment was due to persistence of PAVM caused by subsequent development of systemic reperfusion, which did not require further intervention. There were two minor complications but no major complications were encountered. CONCLUSION: Embolotherapy of PAVMs using AVP or AVP2 devices is safe and effective, with high technical success rate, low persistence and complication rates, and with excellent long-term results.

RESUMO

We report a case of extensive acute portal vein thrombosis (PVT) presenting with severe diffuse abdominal pain and impending small bowel infarction. The patient was successfully treated with ultrasound-accelerated catheter-directed thrombolysis (EKOS endowave system; Covidien, Mansfield, Massachusetts), which resulted in prompt recanalization of his portal vein (PV) and its tributaries. The patient eventually had ischemic stricture that necessitated bowel resection. However, we believe that our technique was successful in rapidly restoring the patency of the PV and its tributaries, and therefore, avoiding a life-threatening complication of more extensive bowel infarction. To our knowledge, the use of ultrasound-accelerated thrombolysis in treatment of PVT has not been previously described in the literature.
