RESUMO

Transcultural nursing should be the main focus of the nurse in any setting. Increased mobility of society demonstrates the need to understand anthropologic and cultural differences. The Valley provides a laboratory in which transcultural nursing can be examined more closely. Cadena recommends that MA nurses remain open to personal feelings generated by relationships with MA patients. The astute nurse assesses each patient's level of assimilation and provides care based on the findings. Comprehensive communication and patient and family participation are the keys to successful transcultural nursing. Sensitivity to modesty and pride translates into professional understanding of holistic needs rather than humiliation and alienation of the patient. At VBMC, these concepts are translated into working systems through the UACs and the bedside-managed care delivery system.

RESUMO

Tuberculosis (TB) remains a major global disease, and diabetes, which is documented to increase susceptibility to TB threefold, is also becoming pandemic. This susceptibility has been attracting extensive research interest. The increased risk of TB in diabetes may serve as a unique model to understand host susceptibility to specific pathogens in humans. To examine this rationale, we investigated the expression of reported TB candidate genes in a longitudinal diabetes study. Two genes, HK2 and CD28, emerged as potential culprits in diabetes-increased TB susceptibility.

RESUMO

The authors present an institution-wide approach to restructuring nursing practice. Unit action committees, restorative care paths, permanent care teams, and a physician/nurse liaison committee were four major strategies implemented. Nursing's major goals from this project were to increase autonomy, enhance collaborative practice, and reward with salary commensurate with management responsibility.
