BACKGROUND AND OBJECTIVES: Wastage of red blood cell units (RBCs) due to their inappropriate storage at the clinical ward has become both a financial and ethical challenge in the daily hospital practice. This study was aimed at identifying the extent of RBC wastage and evaluating the effects of various interventions in reducing this wastage. MATERIALS AND METHODS: From January 2011 to March 2011, baseline wastage level was evaluated using temperature-sensitive labels. Following this initial analysis, various interventions were implemented, including modifying the transfusion practice, intensifying training of and communication with the medical staff and improving the transport conditions. The impact of these interventions on wastage was measured during two periods, and results were compared with baseline wastage level. RESULTS: Based on the extent of label colouring, 7·5% of the units dispensed by the transfusion laboratory were determined as non-reusable at baseline. After implementation of the various interventions, wastage decreased to 1% of the units dispensed, potentially leading to an annual saving for our hospital of approximately €208·000/$230·600 on the total number of RBCs dispensed. CONCLUSION: Relative straightforward interventions, such as raising awareness among medical staff and particularly improving transport conditions, had a clear impact on the level of RBC wastage, accommodating the financial issue not to waste public money as well as the ethical issue that RBC wastage should be as low as possible.

BACKGROUND: Lung transplantation (LTx) is a last treatment option for patients with an end-stage pulmonary disease. Chronic lung allograft dysfunction, which generally manifests as bronchiolitis obliterans syndrome (BOS), is a major long-term survival limitation. During injury, inflammation and BOS monocytes are recruited. We determined whether changes in count, subset distribution, and functionality by surface marker expression coincided with BOS development. METHODS: Fresh whole-blood samples were analyzed from 44 LTx patients, including 17 patients diagnosed with BOS, and compared with 10 age-matched healthy controls and 9 sarcoidosis patients as positive controls. Monocytes were quantified and analyzed using flow cytometry. Based on surface marker expression, classical, intermediate, and nonclassical subsets were determined, and functional phenotypes were investigated. RESULTS: The absolute count of monocytes was decreased in LTx and slightly increased in BOS patients. The relative count shifted toward classical monocytes at the expense of nonclassical monocytes in LTx and BOS. Surface marker expression was highest on intermediate monocytes. The expression of both CD36 and CD163 was significantly increased in the LTx and BOS cohort. The difference between the BOS cohort and the LTx cohort was only subtle, with a significant decrease in HLA-DR expression on nonclassical monocytes in BOS. CONCLUSIONS: Monocyte subsets and surface marker expression changed significantly in transplantation patients, while BOS-specific changes were understated. More research is needed to determine whether and how monocytes influence the disease process and how current immunosuppressants affect their normal function in vivo.

RESUMO

A measurement station dedicated for quantitative radiological characterisation of naturally occurring radionuclides in a metallurgical company and based on gamma-ray spectrometry was developed. The station is intended for performing quality control of final non-ferrous metal products and for radiological checks of incoming materials. A low-background point-contact HPGe-detector was used and the signal was split in two branches to enable collecting simultaneously spectra with high amplification (for gamma-ray energies below 250keV) and low amplification.
