RESUMO

Studies were carried out in 528 women hospitalized in the Department of Obstetrics and Gynecology Medical Academy in Lublin. Besides the control group, patients were classified according to the observed histopathological changes in the cervix (CIN) and found infections with human papillomavirus (HPV). In all cases beta-carotene content in blood serum was examined. HPV infection was probably a cause of decrease of beta-carotene content. It was found that with increased advancement of cervical dysplasia the level of beta-carotene in serum decreased.

Increased systemic vascular resistance and blood pressure during pregnancy may be due to impaired prostacycline PGI(2)-thromboxan (TX) A(2) balance. The aim of the study was to compare the influence of magnesium sulphate and isradipine on the placental vascular resistance induced by stable thromboxan A(2) analogue U 46619 in experimental bilateral perfusion of the human placental cotyledon. The research used the experimental model described by Schneider et al. [Am. J. Obstet. Gynecol. 114 (1972) 822]. The control group consisted of six placental lobule perfusions lasting 120 min each. Constant increase in perfusion pressure of mean value 185% of the initial pressure was obtained from about the 60th min of the experiment and maintained till the end. Having obtained constant increase in perfusion pressure from the 60th min of the experiment, magnesium sulphate and isradipine were administered along with thromboxan A(2) analogue U 46619 into the fetal circulation. Both drugs lowered perfusion pressure, experimentally increased by thromboxan A(2) analogue U 46619. The onset of magnesium sulphate activity was quicker than isradipine. According to the results of our in vitro study, magnesium sulphate and isradipine may have a beneficial effect on vascular resistance of cotyledon vessels in PIH in vivo, too.

Human papillomavirus (HPV) can be a cause of cervical intraepithelial neoplasia (CIN) and cervical cancer. The level of antioxidant compounds in plasma (retinol, alfa-tocoferol, vitamins C and E, glutathione) can modulate the progression of latent HPV infection to subclinical lesions and CIN. Our studies show that in the cases of HPV infection glutathione (GSH) content decreased, but glutathione disulfide (GSSG) levels increased. The quantitative GSH/GSSG ratio reflects the above changes of both peptides which, the more distinct, the greater the pathological changes in the uterine cervix.

The presence of HPV, using the Digene Hybrid Capture System, was identified in a group of 324 women with CIN and in 228 women with normal cytological smears. Risk of occurrence of CIN was 40 times higher for high risk HPV types. The serum folic acid level and the level of antioxidant compounds in plasma (retinol, alpha-tocopherol, vitamins C and E) in women of the studied and control group was determined by HPLC (high-performance liquid chromatography-reversed phase). Statistically lower levels of folic acid were found in the women with CIN-HPV (+) (OR: 7.5: 95% CI: 1.2-9.7). Studies have shown that lower levels of antioxidants coexisting with low levels of folic acid increases the risk of CIN development.

RESUMO

A rare case of a term-pregnancy and delivery in a patient with microprolactinoma is presented. The patient had been treated with bromocriptine since she was 17 due to primary amenorrhea for 60 months with short intervals. Pharmacological tests with dopamine antagonists and X-ray diagnostics revealed the presence of microprolactinoma. The patient was constantly under specialist endocrinological care and the level of prolactin in her blood serum was continuously being monitored. After getting pregnant she was placed under strict gynecological scrutiny with special attention devoted to the developing fetus and possible congenital defects. The patient continued receiving bromocriptine therapy up to the 16th week of pregnancy. In the course of pregnancy in the 26th week, a circular suture was placed on the uterine cervix due to symptoms of isthmus-cervical insufficiency. In the 38th week of pregnancy a cesarean (C) section was performed and a healthy child in good general condition (Apgar score 10) was born. The child weighed 2,900 grams.

RESUMO

The effect of HPV infection on cervix dysplasia development and ascorbic acid level in 528 women was studied. HPV-DNA was estimated using Digene Hybride Capture System and ascorbic acid by color metric method with 2,4-dinitrophenylhydrazine at 520 nm. HPV infection can be connected with the risk of cervical pathology and cervical cancer. Besides, we observed lower level of ascorbic acid in groups of women with CIN, Ca in situ and HPV infected.

We have investigated the effect of HPV infection on the development of cervix dysplasias and GSH level in the plasma of 312 women, ranging from 18 to 34 years (x = 23.2). HPV-DNA was estimated using the Digene Hybride Capture System and GSH by the high performance liqvid chromatography method (HPLC). HPV infectior can be connected with a risk of cervical pathology and cervical cancer. Besides HPV infection the other risk factor is a low level of one of the most important antioxidant-glutathione (GSH). The level of GSH decreased while the concentration of GSSG increased in the plasma of women with HPV infection, cervix dysplasias and cancer. Simultaneous investigation of biochemical and virologic parameters in conjugation with the known cytopathologic and epidemiologic risk variables can show the factors that may inhibit or promote cervical carcinogenesis.

RESUMO

In the program of semen preparation to intra uterine insemination (IUI) the comparison of the routine semen analysis with the results of the Hypoosmotic Test (HOS-T) and the Hamster Oocyte Penetration Test (HOP-T) was performed. The positive correlation between the sperm quality and the results of the used tests (HOP and HOS) was revealed. It was found that the essential indicator of the decreased fertilizing capacity of the semen is it's motility. The significance of the studied tests in the diagnosis and treatment of infertility was discussed.

RESUMO

One of the method of treatment of the infertile couple due to the male factor is the Intra Uterine Insemination (IUI). The references on the subject are divergent. This is caused by the lack of the similar, comparable classification in patients selection for the treatment, procedure of semen preparation and the results estimation. The different efficiency of this treatment method and it's direct and indirect costs are emphasized. In our studies IUI was applied in 42 women from the couples infertile due to the male factor. There were 3 pregnancies obtained (7.0%) which gives the result of 4.3% of pregnancies respectively per treated cycle. Each stage of the procedure was analysed, beginning from the criteria of patients selection, the way of the semen preparation as well as the time of treatment in respect to the obtained results--the percentage of the patients who became pregnant.

RESUMO

Aging of human organism is usually related with a decline of gonadal activity. Age associated reproductive and hormonal changes in men are not as dramatic and clear as in women. During the process of physiological aging three hormonal systems show the decrease in circulating hormone concentrations: estrogen (menopause), testosterone (andropause) dehydroepiandrosterone sulfate (adrenopause) growth hormone and it's tissue mediator IGH-I (somatopause). Emphasizing the probable causes of hypoandrogenism consequences of the above hormonal shortage have been presented. Additionally some factors participating in the process of human organism aging have been discussed.

RESUMO

Retrospective study was performed upon incidence of tubal pregnancy and histopathologically proved post-infection changes in oviducts. The incidence of tubal pregnancy was significantly decreasing in studied period in our institution despite the fact that salpingitis was found more often. Authors conclude that recently reported increase of tubal pregnancy is valid only in the populations studied and not in general. Relation between salpingitis and tubal pregnancy is not always evident.

The opioid system modulates the growth and development of CNS. Hypoxia increases endogenous opioids significantly. The density and distribution of mu-opioid receptors in MPOA is cyclical and sexually dimorphic. To test the hypothesis that perinatal hypoxia affects the development of the opioid receptor system in fetus brain in both sexes differentially, we performed the following study. We divided 24 pregnant Sprague-Dawley rats to two treatment groups and control group. We exposed one group (CH) to chronic prenatal hypoxia (10.5% O2) continuously 24 h/day and second group (AH) to acute, intermittent hypoxia (9.0% O2) 6 h/day from day E15 to E20. We analyzed the brain regions: CPu, ZG, NA, OT, MMPoA, LMPoA by quantitative autoradiography following incubation with (3H) DAMGO. We didn't notice any significant changes in mu-opioid receptor density between sexes in selected brain regions. We found mu-opioid receptor a significant changes in mu-opioid receptor of all brain regions in female group.

This retrospective study was based on hospital records analysis. The incidence of tubal pregnancy and salpingitis were evaluated and possible correlation between these two diseases was estimated. Despite of many other authors reports about increase of tubal pregnancy incidence we observed decline of tubal pregnancy incidence in studied period. Increase of salpingitis incidence did not alter this trend. Authors conclude that recently reported increase of tubal pregnancy incidence in many countries is valid only in populations studied and can not be generalized. The increase of salpingitis incidence does not necessarily influence tubal pregnancy incidence.

UNLABELLED: The opioid system modulates the growth and development of CNS. Hypoxia increases endogenous opioids significantly. The early development of the opioid receptor system has been reported to be susceptible to perinatal opioid exposure so the developing brain may be influenced by circulating opioids. To test the hypothesis that perinatal hypoxia affects the development of the opioid receptor system in developing brain, we performed the following study. We divided 36 pregnant Sprague-Dawley rats to three treatment groups and control group. We exposed one group (C) to chronic prenatal hypoxia (10.5% O2) continuously 24 h/day and second group (AH) to acute, intermittent hypoxia (9.0% O2) 6 h/day from day E15 to E20. Group of newborns (PH) we exposed to postnatal hypoxia (10.5% O2) from day P2-10. We analyzed the several brain regions: CPu, NA, MMPoA, LMPoA, CA3, VM, AH, Habenula by quantitative autoradiography following incubation with (3H) DAMGO. RESULTS: 1) Chronic prenatal hypoxia decreased mu-receptor binding in all brain regions examined. 2) In contrast, mu-receptor binding following acute, intermittent hypoxia tended to be increased in the several brain regions. 3) Postnatal hypoxia did not change mu-receptor binding in characteristic way. 4) Such receptor changes may be associated with endocrinologic or behavioral abnormalities.

RESUMO

The results of transvaginal ultrasound diagnostic assessment of endometrium were compared with histopathological examination of endometrium by following diagnostic abrasion on examined group of 54 postmenopausal women. The method seems to be useful for diagnosis of postmenopausal metrorrhagia, when the "safe" thickness of endometrium layer is taken as 5 mm. It occurs in effect of our study the transvaginal ultrasound examination may decrease the number of subsequent diagnostic abrasions in those cases.

UNLABELLED: In our study the management and outcomes in 211 women with premature rupture of the membranes at term was analyzed. According to the time elapsed since PROM to delivery the study group was divided into four groups: within 6 h, within 6-12 h, within 12-24 h and over 24 h after PROM. The analysis was focused on the maternal age, obstetric history, induction of labor, mode of delivery, duration of interval between PROM and active labor, use of maternal antibiotics, newborns evaluation in Apgar score and the neonatal infections. CONCLUSIONS: 1. In our study PROM at term occurred more frequently in primigravidae than in multigravidae. 2. The relationship between duration of time from the membranes rupture to delivery and frequency of labor induction, use of maternal antibiotics and neonatal infections was observed. 3. The higher percentage of cesarean sections in patients with interval from PROM to delivery less than 6 and over 24 hours was found.

OBJECTIVES: Preeclampsia is the pregnancy specific disorder in which reduced organ perfusion and ischemia of the kidney, liver and brain can be detected. In the kidney, the most typical anatomopathological lesion is glomerular endotheliosis but renal tubular damage was also demonstrated. The NAG is an enzyme which is released into urine from the lysosomes of the proximal renal tubules epithelium and therefore increased urinary NAG activity is a marker of tubular damage or dysfunction. DESIGN: The aim of the study was to assess the renal tubular damage in women with preeclampsia by measuring of urinary NAG activity. MATERIALS AND METHODS: The study group consisted of 46 pregnant women in third trimester with preeclampsia. 36 normal pregnant women in third trimester and 26 healthy nonpregnant women served as two control groups. Urinary activity of NAG was measured in the second morning urine sample by colorimetric method and the results were expressed as NAG/creatinine ratio (U/mmol). RESULTS: Urinary NAG/creatinine ratio was increased in normal pregnant women in third trimester (0.77 +/- 0.44 U/mmol) exceeding that of nonpregnant women (0.23 +/- 0.16). In women with preeclampsia, the urinary NAG/creatinine ratio was significantly higher when compared to the normal pregnant women in third trimester (1.46 +/- 1.63 U/mmol). There were no correlation between urinary NAG excretion and blood pressure results in women with preeclampsia. CONCLUSIONS: The increased urinary NAG activity in women with preeclampsia seems to be a sign of proximal tubular damage. The renal function follow up study in such women is indicated.

beta 1 integrins, receptors responsible for the binding of extracellular matrix proteins, mediate activation, adhesion, migration and differentiation in different tissue compartments. It has been recently demonstrated that integrins are distributed on sperm surface and on T cells, as well. The aim of this study was to establish influence of antisperm antibodies on T cell adhesion to extracellular matrix proteins in women with a history of unexplained infertility.

OBJECTIVES: Captopril, an agiotensin converting enzyme inhibitor (ACEI) is widely used drug in antihypertensive treatment. After maternal therapy with ACEI several adverse effects on fetus and neonate have been described but the patomechanism is not well known. DESIGN: The purpose of the study was to determine the effects of varying Captopril concentrations on the perfusion pressure in fetal vessels in the in vitro placental double-sided perfusion model. MATERIALS AND METHODS: Human placental cotyledons from term normal pregnancies were perfused with 100 ng/ml and 1000 ng/ml of Captopril. The perfusion pressure in fetal circuit in every 30 minutes of 3-hours experiment was measured. RESULTS: The highest mean perfusion pressure in fetal circuit was observed in placentas perfused with Captopril in the dose of 100 ng/ml, whereas Captopril in the dose of 1000 ng/ml significantly decreased perfusion pressure after 180 minutes of the experiment. CONCLUSION: The decrease in perfusion pressure in fetal vessels after relative high dose of Captopril may indicate its direct or indirect influence on capillaries.
