RESUMO

Boceprevir drug is a ketoamide serine protease inhibitor with a linear peptidomimetic structure that exhibits inhibition activity against 2019-nCoV main protease. This paper reports electronic properties of boceprevir and its molecular docking as well as molecular dynamics simulation analysis with protein receptor. For this, the equilibrium structure of boceprevir has been obtained by DFT at B3LYP and ωB97XD levels with 6-311+G(d,p) basis set in gas and water mediums. HOMO-LUMO and absorption spectrum analysis have been used to evaluate the boceprevir's toxicity and photosensitivity, respectively. Molecular docking simulation has been performed to test the binding affinity of boceprevir with 2019-nCoV MPRO; which rendered a variety of desirable binding locations between the ligand and target protein's residue positions. The optimum binding location has been considered for molecular dynamics simulation. The findings have been addressed to clarify the boceprevir drug efficacy against the 2019-nCoV MPRO.

INTRODUCTION: The ongoing visceral leishmaniasis (VL) elimination programme in India is targeting the elimination of the disease VL but not the pathogen. The persistence of hidden parasite pool may initiate a resurgence in suitable conditions. This study dealt with a novel approach to unearth such pathogen pool and their proper management to prevent the resurgence of VL. MATERIALS AND METHODS: We deployed a new approach for detection of pathogen pool by following up the VL and post kala-azar dermal leishmaniasis patients treated during the last 10 years along with mass sero-surveillance within a radius of 500 m of recently treated individuals. RESULTS: We followed up 72.6% (3026/4168) previously treated VL and post kala-azar dermal leishmaniasis patients and diagnosed 42 (1.4%) new and 38 (1.3%) recurrent post kala-azar dermal leishmaniasis. We detected 93 asymptomatic leishmanial infection, 8 VL and 1 post kala-azar dermal leishmaniasis by mass sero-surveillance. CONCLUSION: Our three-step process including mapping and follow-up of previously treated cases, mass surveillance within 500 m of radius of known cases, and 6 monthly follow-on clinical and serological screening of asymptomatic cases, enabled detection of previously undetected cases of post kala-azar dermal leishmaniasis and VL. Recurrent post kala-azar dermal leishmaniasis deserves special attention regarding their treatment guideline. Early diagnosis and effective treatment of all leishmaniasis cases will hasten pathogen elimination and prevent resurgence of VL. This may help the policymakers to develop appropriate strategy for elimination of pathogen to prevent resurgence of VL.

Inguinal bladder hernia is an uncommon pathology found in less than 4% of all inguinal hernias. Pre-operative diagnosis is made in only 10% of cases and most are identified intra-operatively. The failure to diagnose bladder herniation may result in bladder injury and in rare instances are diagnosed later in post-operative period when the patient presents with peritonitis. Here we present a case of a 52-year-old male who underwent left mesh hernioplasty and presented to us after 10 days of surgery with features of peritonitis. On exploration, he was found to have a urinary bladder injury.

Intraguild predation (IGP) is common in the freshwater insect communities, involving a top predator, intraguild prey (IG prey) and a shared prey. Influence of the habitat complexity on the prey-predator interactions is well established through several studies. In the present instance, the IGP involving the heteropteran predators and the dipteran prey were assessed in the background of the habitat complexity. The three predators Diplonychus rusticus, Ranatra filiformis, and Laccotrephes griseus, one intraguild prey Anisops bouvieri and two dipteran prey Culex quinquefasciatus and Chironomus sp. were used in different relative density against the complex habitat conditions to deduce the impact on the mortality on the prey. In comparison to the open conditions, the presence of the macrophytes and pebbles reduced the mortality of the shared prey under intraguild system as well as single predator system. The mortality of the shared prey was however dependent on the density of the predator and prey. Considering the shared prey mortality, predation on mosquito larvae was always higher in single predator system than chironomid larvae irrespective of identity and density of predators. However, for both the shared prey, complexity of habitat reduced the prey vulnerability in comparison to the simple habitat condition. Higher observed prey consumption depicts the higher risk to predation of shared prey, though the values varied with habitat conditions. Mortality of IG prey (A. bouvieri) in IGP system followed the opposite trend of the shared prey. The lower mortality in simple habitat and higher mortality in complex habitat conditions was observed for the IG prey, irrespective of shared prey and predator density. In IGP system, the shared prey mortality was influenced by the habitat conditions, with more complex habitat reducing the vulnerability of the shared prey and increased mortality of the IG prey. This implies that the regulation of the mosquitoes, in the IGP system will be impeded by the habitat conditions, with the heteropteran predators as the top predator.

Flagellate dermatitis caused by bleomycin is a rare side effect with a distinctive pattern of whip-like, linear streaks. The clinical presentation has become uncommon nowadays as bleomycin use in conventional chemotherapy regimens has decreased. We present a case of a 30-year-old female diagnosed with ovarian germ cell tumour, managed with bleomycin, etoposide, and cisplatin (BEP) and later developed a widespread rash indicative of classic flagellate dermatitis. This brief report emphasizes the significance of detection and management of this transient dermatological complication in patients receiving bleomycin.

In this modern era, giant inguinoscrotal hernias are very rare to experience in a medical career. We discuss a case of a 65-year-old man with a history of an inguinoscrotal hernia with progressive growth for the past 35 years. On examination, he had a 20 cm × 15 cm non-reducible swelling with multiple ulcers over the skin surface extending to the mid-thigh with otherwise no other bladder and bowel complications. These large hernias pose a different set of surgical problems. Open surgery was performed, hernial sac opened, contents reverted and left orchidectomy were done with scrotal reconstruction. The defect was closed with Vicryl 1-0 over the muscle layer and the skin was stapled. Daily wound care was provided. Besides, this case also compels us to explore possible reasons for the occurrence of such potentially dangerous surgical problems in low-to-middle income countries (LMIC).
