RESUMO

Culturally significant interventions to prevent lead exposure of battery recyclers are required. Sulfuric acid used in batteries causes skin lesions and could facilitate the recognition of hazards. This study explored whether joint exposure to lead and sulfuric acid can be used in interventions to manage hazardous work conditions. Data were collected from 120 informal workers, and the blood lead level was measured. Predictors of blood lead levels were explored using a tobit model. The median blood lead level was 9.45 µg/dL (Q75-Q25: 48.9 µg/dL); when lead and sulfuric acid exposure occurred, the level increased to 11.44 µg/dL, and when exposure to lead and other substances occurred, the level was 11.50 µg/dL. Workers are unaware of the risks of obvious and acute silent chronic exposure. Future preventive interventions could confront the economic benefits of battery recycling with the recognition of susceptibility and severity related to lead and sulfuric acid exposure.

The contexts where there are mining and agriculture activities are potential sources of risk to human health due to contamination by chemical mixtures. These contexts are frequent in several Colombian regions. This study explored the potential association between the frequency of micronuclei and pesticides and elements in regions with ferronickel (Montelibano, Córdoba) and gold (Nechí, Antioquia) mining, and a closed native mercury mine (Aranzazu, Caldas), with an emphasis in the potential effect of selenium as a potential chelator. A cross-sectional study was carried out with 247 individuals. Sociodemographic, occupational, and toxicological variables were ascertained. Blood and urine samples were taken for pesticide analysis (5 organophosphates, 4 organochlorines, and 3 carbamates), 68 elements were quantified in hair, and micronuclei were quantified in lymphocytes. The mixtures of elements were grouped through principal component analysis. Prevalence ratios were estimated with robust variance Poisson regressions to explore associations. Interactions of selenium with toxic elements were explored. The highest concentrations of elements were in the active mines. The potentially most toxic chemical mixture was observed in the ferronickel mine. Pesticides were detected in a low proportion of participants (<2.5%), except paraoxon-methyl in blood (27.55%) in Montelibano and paraoxon-ethyl in blood (18.81%) in Aranzazu. The frequency of micronuclei was similar in the three mining contexts, with means between 4 to 7 (p = 0.1298). There was great heterogeneity in the exposure to pesticides and elements. The "hormetic effect" of selenium was described, in which, at low doses, it acts as a chelator in Montelibano and Aranzazu, and at high doses, it can enhance the toxic effects of other elements, maybe as in Nechí. Selenium can serve as a protective agent, but it requires adaptation to the available concentrations in each region to avoid its toxic effects.

Objetivo: Identificar los factores asociados a la intoxicación intencionada por plaguicidas en Colombia durante 2007-2017. Metodología: Se llevó a cabo un estudio analítico transversal en la población de intoxicados con intención suicida por plaguicidas de Colombia entre 2007 a 2017. Se calcularon prevalencias ajustadas por edad, expresadas por 100 000 personas contadas por sexo. Se realizó un modelo de regresión bivariado, en el cual la condición final se constituyó en la variable de interés (variable y; 0=vivo 1=muerto) y las variables explicativas fueron (variables x): sociodemográficas (sexo, edad, escolaridad y procedencia) y específicas del evento (vía de exposición). Resultados: el comportamiento de los Odds Ratio evidencia que, los hombres tienen un riesgo de fallecer de 1.39 (95 % IC: 1.238-1.567) más veces que las mujeres. Con respecto al régimen de afiliación en salud, las personas no aseguradas tienen un riesgo de fallecer por suicidio de 1.33 más veces con respecto a los que sobreviven (95 % IC: 1.131-1.555). Para la mortalidad por suicidio se identifican los siguientes factores asociados a la supervivencia (intentos de suicidio no consumados): en primer lugar, el área de residencia urbana y, en segundo lugar, los grupos de edad, adolescente y la adultez temprana. Conclusiones: Al analizar la mortalidad por suicidio asociada a la intoxicación por plaguicidas los hombres son identificados y caracterizados como el grupo de mayor riesgo, siguiendo tendencias generales (todos los métodos) similares a estudios realizados a nivel mundial.


Objective: To identify the social factors associated with intentional poisoning by pesticides in Colombia, during the 2007-2017 period. Methodology: A cross-sectional analytical study was carried out in the population of those intoxicated by pesticides with suicidal intent in Colombia between 2007 and 2017. Age-adjusted prevalences were calculated, expressed per 100,000 people, counted by sex. A bi-variate regression model was carried out, in which the final condition was constituted in the variable of interest (variable y; 0 = alive 1 = dead), and the explanatory variables were (variables x): sociodemographic (sex, age, education and origin) and specific to the event (route of exposure). Results: the behavior of the Odds Ratio shows that men have a risk of dying 1,393 (95% CI: 1,238-1,567) more times than women. Regarding the health affiliation scheme, uninsured people have a risk of dying by suicide 1.33 times more than those who survive (95% CI: 1.131-1.555). For mortality due to suicide, the following factors associated with survival (unsuccessful suicide attempts) are identified: firstly, the urban residence area and secondly, age groups: adolescents and early adulthood. Conclusions: When analyzing suicide mortality associated with pesticide poisoning, men are identified and characterized as the highest risk group, following general trends (all methods) similar to studies carried out worldwide.

Resumen El propósito de este revisión sistemática fue describir la situación actual del síndrome de burnout en Colombia en estudiantes y trabajadores de las ciencias de la salud. Se realizó una búsqueda en las bases de datos Medline, Scopus, Science Direct, Lilacs y repositorios de revistas como Scielo, Pubmed, Redylacs, BIRE-ME. Se llevó a cabo un filtro inicial mediante títulos y resúmenes de acuerdo con los criterios de inclusión y exclusión. Posteriormente, se evaluó la calidad metodológica y con los estudios incluidos se estimó la prevalencia global. Finalmente se seleccionaron tres estudios. La población total fue de 5874 estudiantes y profesionales de las ciencias de la salud, 96,1 % (n=5647) y 3,86 % (n=227), respectivamente. Esta revisión deja en evidencia que en Colombia, aunque se han realizado varios estudios relacionados con el síndrome de Burnout, aún se requieren más investigaciones con mayor rigurosidad metodológica que profundicen en las intervenciones una vez sea diagnosticado.


Abstract The aim of the present systematic review was to describe the current situation of the Burnout Syndrome in Colombia in students and workers of the health sciences. A search was made in the databases: Medline, Scopus, Science Direct, Lilacs and magazine repositories: Scielo, Pubmed, Redylacs, BIREME. An initial filter was carried out through titles and abstracts according to the inclusion and exclusion criteria. Subsequently, the methodological quality was evaluated and with the included studies, the overall prevalence was estimated. Finally, three studies were selected, the total population was of 5874 students and professionals of the health sciences, 96.1% (n = 5647) and 3.86% (n = 227) respectively. This review shows that in Colombia, although several studies related to Burnout Syndrome have been carried out, there is still more research with greater methodological rigor that delves into complications once the syndrome is diagnosed.
