RESUMO

The global problems of children health include overweight and obesity and associated high risks of disability, premature death and complications of mental health in adult age. Purpose of the study is to identify alimentary and social risk factors of development of extra body mass in children and adolescents 7-17 years old residing in industrial center of the Eastern Siberia. The average age of studied group of children was 10.95 years. The sampling survey and anthropometric examination of children of school-age was carried out with participation of their parents (legal representatives). The results were evaluated by parametric statistical methods. The analysis testifies predominantly fat-based dietary model in children with normal and extra body weight. It is established that diet of children is inadequate in fish, curds, cultured milk food, meat, vegetables, fruits and drinking water. And 51,67-61,90% of children never consumed seafood. The most significant factors associated with extra body mass and obesity in children and adolescents were status of working mother and her specialized secondary education, high body mass index of father, satisfactory family income, high body mass index of mother. The results of study can be useful for correcting and elaborating further strategies of development of healthy diet in institutions of general education.

RESUMO

The diseases of digestive apparatus significantly contribute into premature mortality of population. In the Republic of Buryatia, the diseases of digestive apparatus occupy third place in the structure of primary morbidity of population (6.7%) and fourth place in the structure of total mortality (6.6%). The purpose of the study is to establish characteristics of structure and dynamics of morbidity and mortality of population because of diseases of digestive apparatus in the Republic of Buryatia in 2009-2018. The analysis was based on data from state statistical reporting forms and official reports of the Territorial Authority of the Federal State Statistics Service in Republic of Buryatia for 2009-2018, reporting and accounting forms of medical organizations. The study was carried out using statistical, analytical methods, as well as elements of comparative analysis. The analysis testifies increasing of indicator of primary morbidity of diseases of digestive apparatus from 4051.0 to 4226.1 per 100 thousand of population and of mortality of diseases of digestive apparatus in the Republic of Buryatia from 60.6 to 62.5 per 100 thousand of population. The main causes of increasing of morbidity and mortality from diseases of digestive apparatus in the Republic of Buryatia are liver diseases, in particular liver fibrosis and cirrhosis because of alcoholization of population, undue diagnostics and treatment of liver pathology in the group "older than able-bodied age".

The article presents results of comparative analysis of morbidity and mortality of diseases of circulatory system in the Republic of Buryatia in 2003-2018. The population mortality depends on identification of risk factors for cardiovascular diseases, diseases of circulatory system and subsequent coverage of population with medical care, including dispensary monitoring. The analysis was based on data from State statistical reporting forms and official data of the Territorial Board of the Federal State Statistics Service in the Republic of Buryatia, including the form № 12 "The information on number of diseases registered in patients residing in area of medical organization servicing activity"; the form № 025-12/s "The Out-Patient Registration Card"; the form № 25-2/y "The Statistical Coupon for Final (updated) Diagnosis Registration". The study was carried out using statistical, analytical and comparative analysis methods. Currently, in the Republic of Buryatia, diseases of circular system occupying second place in the structure of total morbidity (15.7%) and primary disability of adult population (28.6%) are among leading cause of total population mortality (41.6%).

The evaluation of social economic losses based on the analysis of mortality of alcohol-associated causes in the Republic of Buryatia was carried out. It is established that in the Republic in 1993-2017 the average mortality due to alcohol-associated causes made up to 3.52% of total population mortality. This indicator is significantly higher among died individuals of able-bodied age, reaching 6.98±0.11% in this cohort. The economic damage due to direct alcohol-associated causes of death of individuals of able-bodied age made up to 4,123.9 million rubles, including 2038.3 million rubles in male population and 319.3 million rubles in female population, considering employment of analyzed population cohort. The social and economic damage due to population premature mortality in the Republic of Buryatia and directly related to alcohol consumption was calculated on the basis of potential demography principle. In average, the annual social losses made up to 80,248.37 man-years not survived till the end of able-bodied age. In total, the concomitant economic losses for the mentioned above period made up to 4.1 billion rubles. The obtained data testify both high level of alcoholization of population of the Republic of Buryatia and necessity of the activation of activities targeted to primary prevention.

Evaluation of the actual nutrition of the population of individual regions allows the development of targeted practical measures to improve nutrition, the structure of food consumption and appropriate recommendations to ensure optimal development and health status of a person. In order to study the nutrition of the adult population of the Republic of Buryatia, a retrospective analysis of the feeding structure data was conducted according to the data of the territorial department of the Federal State Statistics Service for the Republic of Buryatia, and the results obtained in the course of the questionnaire survey of 426 people (chosen by selective method with observance of representativeness) were analyzed. The data are presented obtained on the results of comparison with physiological needs in energy and nutrients for men and women in the most employable age of 30-39 years, having the third group of labor intensity. The structure of the food indicates its irrationality, which was confirmed by an increase in the consumption of food products with high content of saturated fats and added sugars, a decrease in the consumption of biologically valuable food products with high food density. In the product mix of the adult population of the republic, both among men and women, there was a lack of consumption of products from aquatic bioresources, vegetables, dairy products and eggs. At the same time, the consumption of bakery, flour-grinding products and potatoes in aggregate amounted to 120% of the recommended level. According to the results of the observation, 91.6% of the interviewed did not take into account the distribution of caloric intake by meals, in 50.8% the highest calorie intake was for dinner. Actual nutrition included elements that are risk factors for the development of endocrine pathology, disorders of the hormonal system and other metabolic diseases. There was a high level of energy intake for a number of years, which averaged 2891±43.6 kcal in men, 2407±38.4 in women, due to excess of fat and carbohydrates with a protein content of 82.4-87.3% of the recommended level. As a result, the problem of healthy nutrition, bringing the diet in line with the real physiological needs of a person requires serious systemic decisions in terms of organizing, systematizing and standardizing nutrition.

One of the main parameters characterizing workers' health state is transitory disablement morbidity. Among the occupational groups of railway workers in Mongolia, conductors occupy first place in morbidity, with values over 2.4 times higher than an average for all railway workers. Findings are high level of occupational conditionality for cardiovascular diseases (RR = 3.22; EF = 69; c2 = 7.46) and for genito-urinary tract diseases (RR = 4.84; EF = 79; c2 = 20.21), and diseases concerning pregnancy, delivery and post-partum (other obstetric conditions) (RR = 11.85; EF = 92; c2 =.11.21).

Selenium is one of the essential elements which adequate consumption is strictly necessary for human health. Despite unfavorable geographical position of Mongolia and registered cases of selenium deficiency diseases among domestic animals in some provinces of the country there are still no concrete data of selenium content in objects of the environment in this region. The aim of the present work was the evaluation of selenium consumption levels by adults from different provinces of Mongolia and the development of methods of prophylactic for the selenium status optimization. Detection of selenium deficiency in Mongolia was caused by inadequate selenium content in foodstuffs. Determined mean consumption levels were equal to 41.8±4.9 µg/day for males and 34.1±3.1 µg/day for females. The main reason of low selenium content in the diet was its deficiency in native foodstuffs: selenium concentrations in meat of domestic animals µg/kg d.w.) reached 109-296 in beef, 94-200 in mutton, 120-225 in horseflesh and 124-197 in goat's flash, values did not differ between each other statistically (p>0.05). The exception were samples of horseflash with selenium concentration exceeding 400 µg/kg d.w. from Gov-Altai province. Selenium content in wheat was in the range 6-36µg/kg d.w., with the lowest levels in Dornod, Ucs and Selenge provinces. Selenium content in eggs was equal 6.7-7.8 µg/egg. The correction of selenium deficiency is recommended to achieve using preparations of Astragalus Mongolicus from Bulgan province with selenium concentration 278±26µg/kg d.w. Besides this utilization of Mongolicum Tricholoma Mushrooms (from the vicinity of Ulaanbaatar and from Zavkhan province) seems to be highly promising due to high selenium concentrations (616±26 µg/kg d.w. and 352±17µg/kg d.w. respectively).

The transition to a new stage of the development - the information society is an objective reality and has an influence on all areas of the activity of the society, including the establishment of a child as an object of the hygienic research. In conditions of the general informatization of the society, the appearance of so-called "clip thinking," explains the maladjustment of educational technologies to mechanisms of children ' and teenagers 'perception and is confirmed by the growth of the school pathology and the gain in the morbidity rate. In the investigation on the example of the educational institutions of Irkutsk it was executed the evaluation of the impact of the intensification of informatization of education and personal development. For the investigation there were formed 2 groups ofpreschools with different levels of informatization in the same preschool institution of the central district of the city of Irkutsk but in different periods of time. In total there were observed 211 children aged of 5.5 to 6.5 years. For the study the influence of the intensification (and informatization of training there were formed 2 groups of small schoolchildren with different levels of intensification (and informatization) of education. The total number of cases accountedfor 465 children aged of 7-9 years. There were suggested methodical approaches to the estimation of the health status of the children, with taking into account the inevitable influence offactors of informatization and the intensification of education. The performed investigations have allowed to reveal the following tendencies in the shaping of the psychophysical state of health and development of children: an increase of level of informatization of education and personal and accomplishment; intensification of learning working; reduction of the attention level; imagination and visual divergence; capability to the linear differentiation and construction of inferences; fear to fail to meet the expectations of surrounding people and low resistance to stress; the increase speed of data processing along with fall in quality; the gain in hyperactivity.

Among the risk factors that have priority in shaping the health of the population, a leading place is held by a rational nutrition. Presented results indicate to the absence of a stable group eating regularly, with the rational regularity and having all the necessary meals and the recommended food set. In the population of the adult people of the city of Irkutsk there is revealed the role of the nutritional factor in the formation of morbidity rate level of the adult population. It was found that the risk of diseases of the circulatory system accounted of 2.5 (95% CI: 2.1-3.6), for infectious and parasitic diseases - 2.4 (95% CI: 1.7-3.5), for the endocrine system - 2.2 (95% CI: 1.4-3.4), andfor the urinary system - 2.3 (95% CI: 1,7-3,0).

Among risk factors possessing a main importance in the shaping of theA public health the leading place is featured to the rational nutrition. Presented results testify to the absence of stable group in the adult population of Irkutsk who eats regularly, with a rational multiplicity and having all essential meal reception and the recommended food package. There was revealed the role of nutrition in shaping of morbidity rate in the adult population. The risk of circulatory diseases was established to account of 2.5 (95% CI 2.1-3.6), the infectious and parasitic diseases - 2.4 (95% CI 1.7-3.5), the endocrine system - 2,2 (95% CI 1.4-3.4), and urinary system - 2.3 (95% CI 1.7 to 3.0).
