RESUMO

The slowdown in economic development caused by traffic accidents in Spain, together with the disparity in the risk of death or injury due to traffic accidents in its provinces, makes it necessary to evaluate their road safety performance. Therefore, the objectives of the present research are, on the one hand, to find out the level of road safety efficiency of Spanish provinces in the period 2014-2018. On the other hand, it is also aimed to determine to what extent the annual efficiency change is originated by pure changes in efficiency or by technological changes. To achieve both objectives, the nonparametric technique of Data Envelopment Analysis and the Malmquist Index have been used based on the consideration of the Safety Performance Indicators (SPI) as the inputs of the process. The research findings show changes in the level of road safety efficiency in each of the years of the study period. Thus, it is possible to identify a geographical delimitation of the causes that generate changes in efficiency in recent years. A contraction in efficiency and technological progress is identified in part of the Spanish provinces in the north, northeast, and south of Spain.

Herein is described the case of a 39-year-old female agronomist who was admitted to hospital after a syncopal episode. She had had fever, abdominal pain, nausea, and vomiting for the previous month. The patient showed signs of hypoperfusion, so a trans-thoracic echocardiography was done, demonstrating the presence of a cardiac tamponade. An emergency pericardiocentesis was performed, draining 500 ml of hematic content. Thoracic-abdominal computed tomography showed bilateral pleural effusion and also peritoneal effusion. Laboratory tests were compatible with an inflammatory situation with neutrophilic leukocytosis, alteration of hepatic function, and a plateau elevation of high-sensitivity troponin T. Colchicine was initiated but the evolution of the patient was torpid, making necessary the performance of a pericardial window due to an abrupt increase of pericardial effusion and echocardiographic signs of impending cardiac tamponade. Two chest tubes were inserted due to an increasing bilateral pleural effusion. Serology was positive for Leptospira spp. so doxycycline was initiated. She reported that she had inspected a rice-field the previous month. The patient presented a good response to the treatment, being discharged from hospital completely asymptomatic, with no pericardial effusion and practically resolved pleural effusions. She was evaluated again one month later, with no trace of effusions or symptoms. .

Littoral-cell angioma (LCA) is a primary splenic vascular tumor that arises from the normal littoral cells lining the sinus channels of the splenic red pulp. We report a case of LCA of the spleen, which has been infrequently communicated in the literature. A 76-year-old man with a 2-wk history of weight loss, abdominal pain and changes in bowel habits was admitted to our hospital. Imaging studies (CT and MRI) showed multiple lesions in the spleen. Splenectomy was performed. Lining cells were positive for CD31/CD68 markers. Our case was associated with a serrated colonic adenoma. LCA is a benign vascular tumor of the spleen that needs to be included in the differential diagnosis of multiple splenic nodules.

OBJECTIVE: Billroth II procedures (gastrojejunostomy with vagotomy) are seldom performed now but were popular before the advent of histamine 2 receptor blockers and proton pump inhibitors. Such procedures can be a cause of late postprandial hypoglycemia. METHODS: We performed a formal evaluation and discussion of postprandial hypoglycemia. RESULTS: We present a case of an 85-year-old man who presented to the endocrinology clinic with symptoms of "fainting spells after large meals." The patient previously had extensive evaluations by a neurologist and a cardiologist. Because of the history of Billroth II surgery and the description of his fainting spells, a 5-hour glucose tolerance test was performed in the clinic using the foods that uniformly resulted in symptoms. This confirmed that the late dumping syndrome with associated hypoglycemia was the cause of his spells. CONCLUSIONS: Late dumping syndrome manifesting with hypoglycemia should be considered in the workup of patients with a history of gastric surgery and unusual postprandial symptoms. This case highlights the importance of an appropriate workup that can lead to avoidance of unnecessary testing in such patients.

Resumen Se presenta el caso de una paciente ingresada por cuadro clínico de infarto agudo de miocardio con elevación del segmento ST. Tras la realización de una coronariografía emergente en la que se evidencia un trombo en la arteria coronaria derecha, sin objetivar lesiones ateroscleróticas, es diagnosticada de infarto de probable origen embólico. Tras realizar un ecocardiograma transtorácico, se detecta la presencia de trombos en las cavidades derechas e izquierdas del corazón. Por ello se realiza angiografía por tomografía computarizada con protocolo de embolia pulmonar, que confirma la presencia de embolia pulmonar bilateral masiva. Tras esto, se repitió el ecocardiograma transtorácico, hallándose un aneurisma del septo interauricular, con aparente protrusión de una masa trombótica a través de él. Se inició terapia anticoagulante con buena evolución por parte de la paciente, habiendo desaparecido los trombos en el ecocardiograma con test de burbujas que se realizó días después, en el que se demostró la presencia de un foramen oval permeable. Con la presentación de este caso se pretende, en primer lugar, ilustrar una afección que en contadas ocasiones se puede objetivar mediante prueba de imagen, y en segundo lugar, hacer una breve revisión del diagnóstico y el manejo del foramen oval permeable como parte del estudio de fuente embólica.


Abstract The authors describe a case of a patient admitted with ST elevation myocardial infarction. An emergent coronariography was done, showing a thrombus in the right coronary artery, without evidence of atherosclerotic lesions in the coronary bed. Due to this, she is diagnosed with probable embolic myocardial infarction. When she was performed a transthoracic echocardiography, the presence of free thrombi was detected in right and left chambers of the heart. Due to this, a CT angiography with pulmonary embolism protocol was done, confirming the presence of a massive bilateral pulmonary embolism. After that, a new trans-thoracic echocardiography was done, finding an aneurism at interatrial septum, with an apparent protrusion of thrombotic mass through it. The decision taken was initiating therapeutic anticoagulation, having the patient a positive evolution with disappearance of thrombi in the bubble test performed days later, and showing the presence of a patent foramen ovale. The purpose of the presentation of this case report is, firstly, illustrating an entity which in very few occasions can be demonstrated by imaging tests, and secondly, making a brief review of the diagnosis and management of the patent foramen ovale as a part of the study of embolic sources.

BACKGROUND: Similarities between human inflammatory bowel disease (IBD) and ruminant paratuberculosis have fueled a heated discussion on the role of Mycobacterium avium subsp. paratuberculosis (MAP) in the etiology of IBD. METHODS: In order to determine microbiological and immunological evidence of an association between MAP and IBD, blood from 222 inflammatory bowel disease patients and 80 healthy donors from the Basque Country (Spain) were subjected to nested PCR for MAP-specific insertion sequence IS900, interferon-gamma (IFN-gamma) release test with PPA-3 MAP antigen (IFNMAP) or phosphate-buffered saline (IFNPBS), and antibody ELISA with PPA-3 MAP antigen (ABMAP). RESULTS: Highly significant differences in the proportion of PCR-positive IBD patients (17%) and healthy controls (43%) as well as lower IFNMAP and higher ABMAP and IFNPBS responses were observed. Treatment was associated with decreases in IFNMAP and PCR-positive frequency. CONCLUSIONS: These results indicate the existence of immune responses and treatment interactions with MAP that strongly support an etiological role of this agent in IBD.

BACKGROUND: Mycobacteria, such as M. leprae and M. tuberculosis infect billions of humans. However, because of appropriate immune responses and antibiotic therapy, overt mycobacterial diseases occur far less frequently. M. avium subspecies paratuberculosis (MAP) causes Johne's disease in ruminants, an affliction evocative of inflammatory bowel disease (IBD). Several agents used to treat IBD (5-ASA, methotrexate, azathioprine and its metabolite 6-MP) have recently been shown to be antiMAP antibiotics. We herein evaluate the prevalence of MAP DNA in healthy individuals and compare them with IBD patients on antiMAP antibiotics. METHODS: We studied 100 healthy individuals (90 blood donors) and 246 patients with IBD. IS900 MAP DNA was identified using a nested primer PCR in the buffy coat of blood. Positive signal was confirmed as MAP by DNA sequence analysis. PCR positive results frequencies were compared according to medications used. Significance was accepted at p<0.05. RESULTS: 47% (47/100) healthy controls and 16% (40/246) IBD patients were IS900 positive (p<0.0001). MAP DNA was identified in 17% of 143 patients receiving mesalamine and 6% of 16 receiving sulfasalazine. None of the IBD patients receiving methotrexate (n = 9), 6-MP (n = 3), ciprofloxacin (n = 5) or Tacrolimus (n = 3) had MAP DNA detectable in their blood. DISCUSSION: We found a disquietingly large percentage of healthy individuals have MAP DNA in their blood, the significance of which remains to be determined. Counter-intuitively, the incidence of MAP DNA was significantly lower in patients with IBD. Agents with the most potent in vitro antiMAP activity were associated with clearance of blood MAP DNA. We posit that the use antiMAP antibiotics was responsible for the decreased prevalence of MAP DNA in patients with IBD.

La dopamina (DA) estimula el factor liberador de hormona luteinizante (LHR), incrementa la hormona luteinizante (LH) e induce ovulación. El precursor inmediato, la L-dopa, ha sido usado para inducir la ovulación y en el síndrome amenorrea-galactorrea, pero esta droga produce varios efectos indeseables. Por esta razón pareció conveniente investigar drogas capaces de elevar la DA cerebral con menos efectos colaterales. La acción del veneno de la serpiente Lachesis trigonocephalus sobre la DA cerebral fue estudiada en ratas Wistar. Treinta y nueve animales recibieron el veneno a una dinamización de 1 x 10-24 = 12CH en etanol al 22.5%. La droga fue administrada per os con un catéter a la dosis de 0.25 ml, con intervalos de 8 horas, durante 10 días. Treinta y tres ratas sirvieron como controles y recibieron la misma dosis de etanol sin la droga. El grupo control mostró un nivel medio de 998 ± 43 ng/g (X ± SE) y los animales que recibieron la droga 1136 ± 57 ng/g (P < 0.05). La concentración de noradrenalina (NA) fue de 528 ± 29 ng/g en el grupo control y de 452 ± 16 ng/g en el grupo que recibió Lachesis (P < 0.025). Dopamine (DA) stimulates hormone luteinizing liberator (LHR), increases hormone luteinizer (LH) and induces ovulation. L-Dopa, its immediate precursor, has been used to induce ovulation and in the amenorrhea-galactorrhea syndrome, but this drug produces several undesirable effects. It was therefore considered convenient to investigate drugs which can raise Brain DA with less side effects. The effect of Lachesis trigonocephalus snake poison was studied in rats. Thirty nine animals received the venom at a 1 x 10-24 dynamization (Lachesis trig. 12CH), in ethanol at 22.5%. The drug was administered per os through a catheter on a 0.25 ml dose, at 8 hour intervals during 10 days. Thirty three rats were used as controls and received the same dosage of ethanol without the drug. The control batch gave a mean level of 998 ± 29 ng/g (X ± SE) and animals which received the drug showed 1136 ± 57 ng/g (P < 0.025). The concentration of noradrenaline (NA) was 528 ± 29 ng/g in the group which received Lachesis trig. 12CH (P < 0.025). No significant variations of adrenaline levels were apparent with the drug...

Se estudio la frecuencia con que aparecen algunos de los principales factores de riesgo coronario en ejecutivos bancarios de la Ciudad de Mexico. Se investigo la incidencia de tabaquismo, anormalidades ECG indicativas de cardicangioesclerosis, hipercolesterolemia, intolerancia a los hidratos de carbono (HC) e hipertension arterial (HA). En funcion de estos datos se valoro la probabilidad de presentar cardiopatia isquemica. Se estudiaron 420 sujetos cuyas edades variaron entre 22 y 77 anos, divididos en tres grupos de diferente edad: 21-40; 41-60 y 61-80 anos. Para la valoracion cuantitativa del riesgo coronario se usaron las tablas de la "American Heart Association". Del total de casos estudiados, 263 (62.6%) presentaron tabaquismo; 122 (29%), hipercolesterolemia; 59 (14.1%), intolerancia a los HC y 45 (10.7%), HA. El tabaquismo ocupo el primer lugar en los grupos de 21-40 y 41-60 anos y paso a segundo lugar, por un corto margen, en el grupo de 61-80 anos. La incidencia de anomalias ECG, HA e intolerancia a los HC aumento al avanzar la edad. Por el contrario, la hipercolesterolemia se encontro en segundo lugar en el grupo de 21-40 anos y paso a ocupar el ultimo lugar en los grupos del 41-60 y 60-80 anos. La probabilidad de presentar cardiopatia isquemica dentro de un periodo de seis anos a partir de la fecha de evaluacion fue, en promedio para toda la muestra estudiada, de 4.87% con limites de 0.3 a 28.2%. En el grupo de 21-40 anos fue de 1.59% (lim. 0.3-11.3%); en el de 41-60 anos, de 6.82% (lim. 0.7-27.0%) y en el de 61-80 anos llego al 14.59% (lim. 5.3-28.2%)

Se han observado elevaciones de la dopamina (DA) cerebral durante dos fases del ciclo estral de la rata: en el proestro y en el diestro 1; así como en el climaterio de este animal. También se ha encontrado aumentada la excreción urinaria de DA durante el periodo premenstrual de mujeres sanas. En todas estas condiciones se presentan cifras bajas de progesterona sérica. Si estos aumentos en la DA estuvieron condicionados a los descensos en la progesterona circulante, cabría esperar que en el climaterio, donde se encuentran niveles especialmente bajos de progesterona, se observará una elevada excreción urinaria de DA, mayor aún que la correspondiente al periodo premenstrual

La esteroclerosis está íntimamente ligada a los niveles de lípidos, básicamente al colesterol circulante. En la actualidad se piensa que existe una tendencia para el desarrollo de la aterosclerosis y por lo tanto es posible detectar esta tendencia tempranamente, inclusive al nacimiento. Esta identificación temprana permitiría una prevención adecuada. Este proyecto fue diseñado para determinar en una etapa los niveles sanguíneos de lípidos, colesterol, triglicéridos, HDL-C y LDL-C en un grupo de recién nacidos (558) y sus respectivas madres. El promedio de colesterol para los recién nacidos fue de 75, 74 mgrs% triglicéridos 64 mgrs%, HDL-C 21, 91 y LDL-C 40, 74 mgrs%. Se encontró una correlación significación entre la HDL-C materna y la de los recién nacidos (p 0.001). La muestra fue separada en percentiles para permitir la correcta ubicación de los lípidos de acuerdo al peso de los niños y también para determinar los que tuvieran niveles elevados de lípidos. Un segundo paso en el estudio mostrará la correlación entre cambios nutricionales y lípidos. La correlación entre la HDL-C materna y la de los recién nacidos sugiere la posibilidad de una influencia directa de la madre sobre los lípidos y lipoproteínas a través de la barrera placentaria. También se presentan aquí los resultados obtenidos en el análisis de niños de 6 meses, 1 año y 2 años

Se investigó el efecto de un extracto hidroalcohólico de Guatteria gaumeri sobre el colesterol de las diferentes lipoproteínas (LP) séricas del conejo. Se estudiaron 33 animales divididos en tres grupos: el testigo y otros dos que recibieron el fármaco a las dosis de 25 y 50 ul/kg/dl. Con estas dos dosis se observaron disminuciones de distinta cuantía en el colesterol de las diferentes fracciones estudiadas. Se encontró una disminución de 23 por ciento y 37 por ciento en el colesterol total (p<0.05 y <0.01) de 27 por ciento y 39 por ciento en el de las LP de baja densidad (p< 0.05 y < 0.01), y de 11 por ciento y 29 por ciento en las LP de alta densidad (p > 0.01 < 0.01). La relación entre colesterol total y colesterol de LP de alta densidad disminuyó de 3.8 a 3.3 y 3.7, respectivamente

En trabajos previos se encontró que durante el ciclo estral de la rata se presentan dos aumentos den la dopamina (DA) cerebral: uno en el proestero y otro en el diestro I. También se observaron cifras elevadas de DA cerebral en ratas climatéricas. Tanto en el climaterio como en los momentos en que subió la DA durante el ciclo estral se encuentran niveles bajos de progesterona circulante. Estos hechos sugieren que los descensos de progesterona sérica son capaces de inducir, quizás por un mecanismo de retroalimentación, aumentos en la concentración de DA cerebral. Por este motivo pareció interesante estudiar si se observan cambios en la excreción urinaria de DA en el periodo premenstrual, durante el cual descienden los niveles de progesterona sérica

Se estudió la concentración de Da cerebral en 56 ratas Wistar, durante las diferentes fases del ciclo estral. Los valores promedio, + ó - error estándar, fueron los siguientes: proesto 14.05 + ó - 3.25 micronmol/g (2,152 + ó - 949 ng/g); estro 7.73 + ó - 0.97 miconmol/g (1,185 + ó - 148 ng/g); diestro I 18.06 + ó - 2.21 micronmol/g (2,767 + ó - 339 ng/g) y diestro II 6.02 + ó - 0.45 micronmol/g (922 + ó - 70 ng/g). Los niveles más elevados de DA se observaron poco tiempo después de que los valores de progesterona descienden en la rata. Inversamente las cifras más bajas de DA se presentaron al poco tiempo de que la progesterona alcanza sus niveles máximos. Estos hallazgos son compatibles con la hipótesis de que la DA pudiera estar involucrada en ciertos mecanismos de retroalimentación que regulan el eje hipotálamo-hipófisis-ovario
