INTRODUCTION: Embedded smoking cessation support within lung cancer screening is recommended in the UK; however, little is known about why individuals decline smoking cessation support in this setting. This study identified psychosocial factors that influence smoking cessation and quit motivation among those who declined support for quitting smoking alongside lung cancer screening. METHODS: Qualitative interviews conducted between August 2019 - April 2021 with thirty adults with a smoking history, recruited from the Yorkshire Lung Screening Trial. Participants had declined smoking cessation support. Verbatim interview transcripts were thematically analysed. RESULTS: Fifty percent of participants were male and the majority were from the most deprived groups. Participants reported low motivation and a variety of barriers to stopping smoking. Participants described modifiable behavioural factors that influenced their quit motivation including self-efficacy, perceived effectiveness of stop-smoking services including smoking cessation aids, risk-minimising beliefs, lack of social support, absence of positive influences on smoking and beliefs about smoking/smoking cessation. Broader contextual factors included social isolation and stigma, COVID-19 and comorbid mental and physical health conditions that deterred smoking cessation. CONCLUSIONS: To encourage engagement in smoking cessation support during lung cancer screening, interventions should seek to encourage positive beliefs about the effectiveness of smoking cessation aids and increase confidence in quitting as part of supportive, person-centred care. Interventions should also acknowledge the wider social determinants of health among the lung screening-eligible population. IMPLICATIONS: This study provides an in-depth understanding of the beliefs surrounding smoking and smoking cessation and further potential psychosocial factors that influence those attending lung cancer screening. Many of the barriers to smoking cessation found in the present study are similar to those outside of a lung screening setting however this work offers an understanding of potential facilitators that should be considered in future lung screening programmes.

Background: Micro RNAs (miRs) expression is involved in the pathogenesis of type 2 diabetes mellitus (T2DM). This study investigates the expression levels of plasma miR-29a, miR-146a, and miR-147b and their correlations with clinical parameters in patients with T2DM. Methods: 105 patients with T2DM who categorized either as newly diagnosed T2DM (n=52) or treated T2DM (n=53) and 93 healthy individuals were included in this study. The expression levels of miR-29a, miR-146a, and miR-147b were quantified by real-time PCR and analyzed for possible association with T2DM. Results: The expressions of miR-29a and miR-147b were significantly increased in T2DM patients compared with healthy controls (P<0.0001). The expression levels of miR-29a in newly diagnosed T2DM patients were higher than that in the group of treated T2DM (P=0.002). The expression of studied miRs was correlated with several clinical parameters such as blood glucose levels, HbA1C, microalbuminuria, C-peptide, triglyceride levels as well as the HOMA-ß index. The expression levels of miR-29a and miR-147b show a potential diagnostic performance to discriminate newly diagnostic T2DM (AUCs=0.77 and 0.84, respectively) and beta-cell dysfunction (AUCs= 0.62 and 0.75, respectively). Conclusions: The plasma miR-29a and miR-147b expression levels in T2DM patients are significantly associated with T2DM while miR-146a shows poor evidence in relation to T2DM. miR-147b shows potential as a biomarker for the diagnosis of T2DM and pancreatic beta cell dysfunction.

RESUMO

We successfully fabricate a transversely disordered optical fiber made of AsSe2 and As2S5 glasses for high-resolution mid-infrared image transport. By using the fabricated fiber, we experimentally observe transverse Anderson localization of mid-infrared light at the wavelength of 3 µm. Moreover, we numerically evaluate the localization in the fiber by using a cross-sectional image of the fiber.

In this paper, we report a successful fabrication of a highly nonlinear chalcogenide all-solid hybrid microstructured optical fiber with polarization maintaining properties and a mid-infrared SC generation. Up to 4.5 × 10-4 at 10 µm of the fiber birefringence can be realized by employing a single As2Se3 core and two As2S5 rods horizontally aligned in the AsSe2 cladding. The fiber possesses a near-zero and flattened all-normal chromatic dispersion profile over the wavelength range from 5 to 10 µm. The polarization maintaining properties of the fiber is experimentally confirmed and a broadband supercontinuum spectrum from 2 to 10 µm in the mid-infrared window was experimentally demonstrated.

Objective: The study aims to evaluate long-term ovarian reserve change by serum anti-Mullerian hormone (AMH) level and determine the factors that affect the changes after laparoscopic endometrioma cystectomy. Methods: In a prospective longitudinal study, 104 patients with unilateral (n=77) and bilateral (n=27) endometrioma underwent laparoscopic endometrioma cystectomy. AMH levels were measured preoperatively and at 1, 3, 6, and 12 months postoperatively. Multivariate linear regression analysis was performed to determine factors related to AMH level changes. Results: Median preoperative AMH levels significantly declined from 3.77 ng/mL to 1.60 ng/mL (P<0.001), 1.66 ng/mL (P<0.001), 1.67 ng/mL (P<0.001), and 1.72 ng/mL (P<0.001) at 1, 3, 6, and 12 months postoperatively, respectively. The rate of decrease in AMH was unchanged six months after surgery, 52.2%, 53.7%, 54.8% at 1, 3, 6 months, respectively, and declined to 43.2% at 12 months. Although most factors were associated with AMH level changes in monovariant linear regression, multivariant linear regression analysis showed only three factors that reached the statistical significance, including bilateral endometriomas, mean size of the endometrioma, and preoperative AMH levels. Conclusions: Serum AMH levels decline significantly after laparoscopic cystectomy of endometriomas but recovered at 12 months compared with the first 6 months with unilateral endometrioma. Bilateral endometriomas, size of the cyst, and preoperative AMH levels might independently affect AMH levels at 12 months after surgery.

Aiming at maintaining the chromatic dispersion properties and fiber optical parametric amplification (FOPA) performance when fiber core fluctuation occurs, we propose a buffer step-index optical fiber. The AsSe2 chalcogenide glass is employed as the core material due to its high nonlinearity and broad transmission spectrum. The calculated results in this study show that the chromatic dispersion variation due to the change of core diameter can be greatly suppressed and a continuous and very broad FOPA signal gain spectrum can be obtained and maintained by carefully controlling the core, buffer and cladding properties such as refractive index and diameters. The calculated results in this study showed that by using the proposed 3-cm-long fiber pumped at 5.02 µm, a broad signal gain bandwidth from 3 to 14 µm at about 15 dB is attainable although the fiber core diameter Dc drastically fluctuated from 2 to 5 µm and the buffer diameter Db varies from 8.9 to 9.3 µm. Moreover, when Dc varies in smaller range from 3 to 4 µm, the FOPA signal gain spectra calculated at different fixed values of Db in the range from 8.9 to 9.3 µm are highly maintained. When Db is kept at 9.0 µm and Dc varies from 3 to 4 µm, the calculated FOPA signal gain spectra at different pump wavelengths from 4.98 to 5.02 µm are also nearly identical in the wavelength range from 3 up to 13 µm.

Twin-twin transfusion syndrome (TTTS) is an unusual and serious condition that occurs in twin pregnancies when identical twins share a placenta but develop discordant amniotic fluid volumes. TTTS is associated with an increased risk of fetal death and birth defects if untreated. This study investigated the soluble levels of biomarkers including growth factors and interleukins in pregnant women with and without TTTS during pregnancy. We quantified plasma levels of VEGF-R1, VEGF-R2, IL-1ß, IL-6 and IL-8 in twin pregnant women with (n=53) and without TTTS (n=72) and in women with single pregnancy (n=30) by ELISA and analyzed the association of maternal circulating biomarker levels with TTTS. Our results showed that maternal VEGF-R1 levels were significantly higher in twins compared to single pregnancy (P<0.05) and were decreased in the second trimester compared to the first trimester (P = 0.065, 0.019 and 0.072 for twins with and without TTTS and single pregnancy, respectively). VEGF-R2 levels had a trend to be lower in twins compared to single pregnancy. In addition, soluble VEGF-R1 and VEGF-R2 levels were significantly decreased while IL-6 levels were increased after surgical treatment with laser in twin pregnant women with TTTS (P = 0.016, 0.041 and 0.04, respectively). These results suggest that IL-6, VEGF-R1 and VEGF-R2 are involved in vascular regulation and stabilization in twin pregnancies and may contribute to the pathogenesis of TTTS and thus play a prognostic role in the surgical treatment of TTTS.

The complement receptor of the immunoglobulin superfamily (CRIg, encoded by the VSIG4 gene) is a macrophage receptor involved in the clearance of immune complexes and autologous cells. Our results suggest that the VSIG4 rs1044165T allele is a risk factor for severe functional status of rheumatoid arthritis in women, possibly by affecting VSIG4 gene expression.

We report the fabrication of a chalcogenide all-solid hybrid microstructured optical fiber and its application in supercontinuum generation for the first time, to the best of our knowledge. The fiber possesses all-normal and flattened chromatic dispersion, making it highly potential for broad and coherent supercontinuum generation. By pumping the fiber with a femtosecond laser at 3, 4, and 5 µm, broad supercontinua with good spectral flatness are generated. The broadest SC spectrum extending from 2.2 to 10 µm at -20 dB level was obtained when the fiber was pumped at 5 µm with an input power of 3.9 mW.

An all-fiber supercontinuum laser source with a piece of highly nonlinear fiber inserted into a mode-locked fiber laser is experimentally demonstrated. This laser achieves mode-locking based on the Mamyshev mechanism and realizes supercontinuum generation spanning from 1330 nm to 2030 nm directly. Mode-locking based on the Mamyshev mechanism can be obtained easily, and the influence of the parameters of the laser cavity on the supercontinuum laser source is investigated. This supercontinuum laser source has a simple structure, and no amplifier stages are required. It demonstrates intracavity supercontinuum generation in mode-locked fiber laser based on the Mamyshev mechanism and exploits its operation further.

BACKGROUND: While the World Health Organization (WHO) Southeast Asia region has the second highest incidence of malaria worldwide, malaria in Vietnam is focal to few provinces, where delayed parasite clearance to anti-malarial drugs is documented. This study aims to understand Plasmodium species distribution and the genetic diversity of msp1 and msp2 of parasite populations using molecular tools. METHODS: A total of 222 clinical isolates from individuals with uncomplicated malaria were subjected to Plasmodium species identification by nested real-time PCR. 166 isolates positive for Plasmodium falciparum mono infections were further genotyped for msp1 (MAD20, K1, and RO33), and msp2 allelic families (3D7 and FC27). Amplicons were resolved through capillary electrophoresis in the QIAxcel Advanced system. RESULTS: Mono-infections were high and with 75% P. falciparum, 14% Plasmodium vivax and 9% P. falciparum/P. vivax co-infections, with less than 1% Plasmodium malariae identified. For msp1, MAD20 was the most prevalent (99%), followed by K1 (46%) allelic family, with no sample testing positive for RO33 (0%). For msp2, 3D7 allelic family was predominant (97%), followed by FC27 (10%). The multiplicity of infection of msp1 and msp2 was 2.6 and 1.1, respectively, and the mean overall multiplicity of infection was 3.7, with the total number of alleles ranging from 1 to 7. CONCLUSIONS: Given the increasing importance of antimalarial drugs in the region, the genetic diversity of P. falciparum msp1 and msp2 should be regularly monitored with respect to treatment outcomes and/or efficacy studies in regions, where there are ongoing changes in the malaria epidemiology.

Background: ISGylation is the conjugation of ISG15 with target proteins. ISGylation occurs through an enzymatic cascade, which is similar to that of ubiquitination. Through ISGylation, ISG15 can bind to proteins involved in cell proliferation and differentiation, thus promoting genesis and progression of malignancies. The present study aims to investigate expression of genes involved in ISGylation and ubiquitination in patients with hepatocellular carcinoma and to correlate gene expression with clinical laboratory parameters of these patients. Methods: mRNA expression of genes encoding enzymes involved in the ISGylation process (EFP, HERC5, UBA1, UBC and USP18) was evaluated by quantitative real-time PCR in 38 pairs of tumour and adjacent non-tumour tissues from patients with hepatocellular carcinoma and correlated with distinct clinical laboratory parameters. Results: Relative mRNA expression of EFP, HERC5, UBA1 and USP18 was significantly higher in tumour tissues compared to adjacent non-tumour tissues (P=0.006; 0.012; 0.02 and 0.039, respectively). The correlation pattern of mRNA expression between genes in the tumours differed from the pattern in adjacent non-tumour tissues. Relative expression of EFP, HERC5 and UBA1 in adjacent non-tumour tissues was positively associated with direct bilirubin levels (Spearman's rho=0.31, 0.33 and 0.45; P=0.06, 0.05 and 0.01, respectively) and relative expression of USP18 in adjacent non-tumour tissues correlated negatively with ALT levels (Spearman's rho= -0.33, P=0.03). Conclusions: EFP, HERC5, UBA1, and USP18 genes are upregulated in tumour tissues of patients with HCC and, thus, may be associated with the pathogenesis of hepatocellular carcinoma.

We experimentally demonstrate for the first time a successful fabrication of a tellurite hollow core optical fiber which has a mid-infrared transmission range. The wall thickness of each cladding air-hole is about 2.8 µm and the outer diameter of the full air-hole structure D is approximately 110 µm. The results show that the measured transmission spectrum can expand up to 3.9 µm. In addition, it is expected that the transmission can extend to around 6 µm. When the input light is linearly polarized, it can be maintained after propagating through a 17-cm-long fiber.

Mode-locked fiber lasers that generate two frequency combs with different frequency intervals could find very important applications in low-complexity dual-comb metrology. We report a partially polarization-maintaining all-fiber dual-wavelength dual-comb mode locked laser. The polarization dependent loss of the wavelength division multiplexer combined with the polarization-maintaining fibers leads to the periodic Lyot filtering effect. Traditional single wavelength mode locking can be realized with the tunable central wavelength and spectral shape. By properly setting the state of the polarization controller, stable dual-wavelength asynchronized mode locked pulse trains with the repetition rate difference of hundreds of Hertz can be achieved. In this dual-wavelength mode locked fiber laser, the dispersion of the laser cavity leads to group velocity difference of the two mode locked pulse trains with different central wavelengths, which results in the generation of dual-comb output with the frequency difference of hundreds of Hertz. The central wavelengths of the dual-wavelength mode locked fiber laser are demonstrated to be tunable in a certain range as well. Meanwhile, a novel dual-wavelength soliton molecule mode locking is also demonstrated. Such compact all-fiber dual-wavelength dual-comb mode locked laser could find various practical applications that in need of dual-comb fiber laser system.

We demonstrate broadband highly coherent near-to-mid-IR supercontinuum generation using a short length of tapered tellurite step-index fiber pumped with an ultrafast laser in normal dispersion regime. The tapered tellurite fiber possesses all-normal dispersion characteristics within the whole range of the generated supercontinuum spectrum. A highly coherent near-to-mid-IR supercontinuum spectrum spanning 1.28 to 3.31 µm at a -40 dB intensity level is obtained using a 3.2 cm long tapered tellurite fiber when it is pumped with a 200 fs laser pulse with a peak power of 19.8 kW at 2 µm. To obtain the supercontinuum spectrum, we also carried out numerical modeling for the tapered tellurite step-index fiber with the same geometrical parameters and pump conditions used in the experiment. The numerical observation supports the experimentally obtained result. The findings of this work show that the fabricated tapered tellurite step-index fiber is a promising nonlinear medium to obtain a coherent near-to-mid-IR supercontinuum spectrum in a short length of the fiber.

For the first time, an all-solid tellurite optical glass rod with a transversely disordered refractive index profile was fabricated successfully as a medium to study the transverse localization of light and near-infrared (NIR) optical image transport. Two tellurite glass compositions of 70TeO2-8Li2O-17WO3-3MoO3-2Nb2O5 (TLWMN) and 75TeO2-15ZnO-5Na2O-5La2O3 (TZNL) which have a small difference in softening temperature (about 0.5 °C), compatible thermal expansions from room to 400 °C and broad transmission range from about 0.4 up to 6.0 µm were developed for a successful fabrication process. The tellurite transversely disordered optical rod (TDOR) consists of high and low-index units (TLWMN and TZNL, respectively). The diameter of each unit is 1.0 µm and their refractive index difference was about 0.095 at 1.55 µm. Experimental results showed that after a CW probe beam at 1.55 µm propagated in a 10-cm-long tellurite TDOR, the beam became localized. In addition, NIR optical images at 1.55 µm of numbers on a test target were transported. The captured images at the output facet of the tellurite TDOR are visually clear with high contrast and high brightness. The quality of our transported optical images can be comparable or higher than the results which were obtained by a polymer Anderson localized fiber and by a commercially available multicore imaging optical fiber.

Mid-infrared cascaded stimulated Raman scattering (SRS) is experimentally investigated in an As-S optical fiber which is fabricated based on As38S62 and As36S64 glasses and whose fiber loss is ∼0.08 dB/m at1545 nm. Using a nanosecond laser operated at ∼1545 nm as the pump source, mid-infrared cascaded SRS up to eight orders is obtained in a 16 m As-S fiber. To the best of our knowledge, this is the first demonstration of SRS of such high order in non-silica optical fibers, and it may contribute to developing tunable mid-infrared Raman fiber lasers using C-band pump sources.

Oncolytic measles and mumps viruses (MeV, MuV) have a potential for anti-cancer treatment. We examined the anti-tumor activity of MeV, MuV, and MeV-MuV combination (MM) against human solid malignancies (HSM). MeV, MuV, and MM targeted and significantly killed various cancer cell lines of HSM but not normal cells. MM demonstrated a greater anti-tumor effect and prolonged survival in a human prostate cancer xenograft tumor model compared to MeV and MuV. MeV, MuV, and MM significantly induced the expression of immunogenic cell death markers and enhanced spleen-infiltrating immune cells. In conclusion, MM combination significantly improves the treatment of human solid malignancies.

OBJECTIVE: Interleukin-10 (IL-10) is an anti-inflammatory cytokine produced by Th1 cells and macrophages. The rationale of this study was to examine and validate possible contributions of IL-10 promoter polymorphisms in sub-Saharan Africa in children infected with either Plasmodium falciparum or Schistosoma haematobium and in children co-infected with both parasites. MATERIALS AND METHODS: A total of 309 Nigerian children aged 4-15 years were recruited. The study group consisted of individuals infected either with P. falciparum (n = 76) or S. haematobium (n = 94) in mono-infections, a group of children co-infected with both P. falciparum and S. haematobium (n = 62) and matched healthy controls (n = 77). The IL-10 promoter polymorphisms -1082G/A, -819C/T and -592C/A were genotyped by direct sequencing. RESULTS: The frequencies of the IL-10 -1082GG genotype, the -1082G allele and haplotype GCC (positions -1082, -819 and -592) were higher in children infected with P. falciparum than in healthy controls, indicating that the -1082GG genotype and the -1082G allele and the GCC haplotype are associated with increased susceptibility to malaria infection (OR = 3.4, 95% CI = 1.2-10.8, P = 0.02; OR = 2.5, 95% CI = 1.1-3.4, P = 0.02; OR = 3.8, 95% CI = 2.0-7.2, P = 0.0001, respectively). Children with the -1082GG genotype had a higher parasitaemia than children with the -1082AA or -1082AG genotypes (P = 0.0017). Haplotype GCC occurred more frequently in children infected with S. haematobium, while haplotype GTA was less frequent than in controls (OR = 2.2, 95% CI = 1.2-4.4, P = 0.017 and OR = 0.1, 95% CI = 0.02-0.5, P = 0.0004, respectively). No differences in the frequencies of IL-10 promoter polymorphisms were observed between children with P. falciparum-S. haematobium co-infections and healthy controls. CONCLUSION: Although IL-10 promoter polymorphisms are not associated with P. falciparum and S. haematobium co-infection, variant -1082G/A and haplotype GCC are associated with malaria, whereas the IL-10 haplotypes GCC and GTA are associated with schistosomiasis.

BACKGROUND: Clinical progression of HBV-related liver diseases is largely associated with the activity of HBV-specific T cells. Soluble fibrinogen-like protein 2 (sFGL2), mainly secreted by T cells, is an important effector molecule of the immune system. METHODS: sFGL2 levels were determined by ELISA assays in sera of 296 HBV patients clinically classified into the subgroups of acute hepatitis B (AHB), chronic hepatitis B (CHB), liver cirrhosis (LC), hepatocellular carcinoma (HCC) and patients with LC plus HCC. As control group, 158 healthy individuals were included. FGL2 mRNA was quantified by qRT-PCR in 32 pairs of tumor and adjacent non-tumor liver tissues. RESULTS: sFGL2 levels were elevated in HBV patients compared to healthy controls (P <  0.0001). In the patient group, sFGL2 levels were increased in AHB compared to CHB patients (P = 0.017). sFGL2 levels were higher in LC patients compared to those without LC (P = 0.006) and were increased according to the development of cirrhosis as staged by Child-Pugh scores (P = 0.024). Similarly, HCC patients had increased sFGL2 levels compared to CHB patients (P = 0.033) and FGL2 mRNA was up-regulated in tumor tissues compared to adjacent non-tumor tissues (P = 0.043). In addition, sFGL2 levels were positively correlated with HBV-DNA loads and AST (Spearman's rho = 0.21, 0.25 and P = 0.006, 0.023, respectively), but reversely correlated with platelet counts and albumin levels (Spearman's rho = - 0.27, - 0.24 and P = 0.014, 0.033, respectively). CONCLUSIONS: sFGL2 levels are induced by HBV infection and correlated with the progression and clinical outcome of HBV-related liver diseases. Thus, sFGL2 may serve as a potential indicator for HBV-related liver diseases.
