RESUMO

Prostatic magnetic resonance images of 22 male volunteers less than 30 years old and with no known genito-urinary tract disease were obtained at 1.5 T. Normal anatomical features of the prostate were studied with spin-echo techniques. Different zones of the normal gland are shown by T2-weighted images: the anterior fibromuscular fascia, the central prostate, the peripheral prostate and the periurethral zone can be differentiated. The normal prostate gland is shown on T1-weighted images as a homogeneous appearance. It is important to recognize the normal zonal anatomy of the prostate since prostatic disorders arise in different anatomical zones.

RESUMO

A personal serie on the management of 13 testicular tumors is reported. Four seminomas were treated by castration and irradiations on both abdominal and mediastinal fields. All 4 patients are surviving from 4 to 8 years of follow-up. Among 7 patients with embryonal carcinoma, 5 are still living after a 3 to 10 year period, one patient is in widespread disease and another one is dead. An embryonal carcinoma was found associated with a teratocarcinoma in 2 patients who are alive respectively after 1 and 4 years.

RESUMO

Presentation of the results of a national, multicentric and retrospective study regarding the surgery of the octogenarian. The series counts 7,407 patients accepted for surgery, and which can be divided into 4,581 women (61.8%) and 2,826 men (38.2%) of an average age of 84.4 years old (extremities 80-101 years old). The global mortality of this population of elderly (whether operated on or not) amounts to 16.7%. The postoperative mortality of the 4,177 operated patients is 18.2% with different rates according to the surgical disciplines: 34.7% for surgery of the colon, 23.2% for bone surgery, 22.4% for vascular surgery, 15.3% for biliary surgery, 8.7% for urologic surgery and 5.6% for cardiac surgery.

RESUMO

The majority of studies with MR imaging in prostate disease are based on a semiology obtained using images weighted in T1 and T2. A study was carried out to evaluate effects of images calculated in T1 and T2 obtained at 0.5 T. This preliminary study concerns 28 prostate examinations with spin-echo acquisition and inversion-recuperation parameters, and provided images calculated in T1, weighted and calculated in T2. Images allowed detection and characterization of prostate lesions. However, although calculated images accentuate discrimination of the method, the weighted images conserve their place because of their improved spatial resolution.

Forty-eight patients with prostatic disease (benign prostatic hyperplasia (B.P.H.), carcinoma, cysts, myoma and prostatitis) and 10 normal volunteers underwent magnetic resonance imaging (M.R.I.) of the prostate. The prostatic parenchyma was best evaluated by a T2-weighted spin-echo pulse sequence. The prostate in patients with B.P.H. often had a homogeneous or more rarely a nodular appearance on T2-weighted images. In most cases, a peripheral dark rim is observed. All prostate in patients with carcinoma had an heterogeneous appearance on T2-weighted images. While most of the prostatic carcinomas appeared hypo-intense relative to adjacent prostatic parenchyma, some of the neoplasms had a high or mixed-high and low signal. The myoma showed a low-signal nodule like carcinoma. The cyst appears as a liquid tumor. The prostatitis had an homogeneous bright signal. With the used methodology, MRI can differentiate prostatic diseases in many cases. Nevertheless the technique has to be optimized to improve its accuracy.

A case of seminal vesicle cyst is reported. Magnetic resonance (M.R.) appearance of the seminal vesicle cyst is described first. A brief review of the literature for seminal vesicle cysts is made. New imaging modalities (ultrasound, C.T. scan an M.R.) are illustrated, their role is discussed.

RESUMO

Two cases of leiomyoma of the prostate are reported. They are the first studies by M.R.I. Based on the results of M.R.I. of these lesions, differential diagnosis with malignant processes of the prostate is discussed. Associated with a benign prostatic hypertrophy, the leiomyoma must be distinguished from additional neoplasm.

RESUMO

Fourteen patients with prostatic carcinoma, benign prostatic hyperplasia, or chronic granulomatous prostatitis underwent gadolinium-DOTA enhanced magnetic resonance imaging of the prostate. Gadolinium-DOTA was administered intravenously in a dosage of 0.1 mmol/kg body weight. All studies were performed on a superconductive 1.5-T system. As compared to noncontrast T1- and T2-weighted images, Gd-DOTA enhanced T1-weighted images were useful in distinguishing malignant tumor from granulomatous prostatitis in one case, but were uncontributive to the differentiation between carcinoma and benign prostatic hyperplasia.

The authors extracted 11 case files of patients undergoing radical prostatectomy following MRI studies from a series of 192 MRI examinations of the prostate with histological confirmation of the diagnosis. This series is part of a prospective study in which MRI was always interpreted prior to histological results. The diagnosis of prostatic cancer was confirmed in 9 cases on the basis of a hyposignal on T2-weighted scans. In 4 cases, extracapsular spread of the tumour was demonstrated by MRI and was confirmed at surgery. In the cases in which the cancer was visible on MRI, the diagnosis of local spread proved to be accurate. The debate concerning the role of MRI in prostatic disease is not definitively resolved, but this preliminary series indicates the potential and the limitations of this technique.

RESUMO

1433 transrectal ultrasonographies were performed for prostatic disease. In 453 cases, histopathologic correlation was obtained and compared to digital rectal examination and transrectal ultrasonography done after the clinical examination. The sensitivity of digital rectal examination was 91% and the specificity was 85%. The sensitivity and the specificity of ultrasonography were respectively 95 and 81%. 4 prostatic cancers, not suspected by digital rectal examination, were diagnosed by ultrasound (0,9%). The total number of subclinical cancer was 9 (2%). Interest of ultrasound compared to digital rectal examination concerns less than 1% of the total number of controlled patients. The staging of local extension of prostatic cancer is better by ultrasound than by the digital rectal examination. On 9 cases of clinical unsuspected extracapsular invasion, 6 were diagnosed by ultrasound.

Cysts of the prostate and seminal vesicles are confusing abnormalities because they are uncommon and their origin is uncertain. Several approaches to diagnosis have been used, most recently CT and transrectal sonography. In this study, we investigated the value of MR imaging in the diagnosis of six cases of prostatic cyst and five cases of cysts of the seminal vesicles. All patients were symptomatic. T1- and T2-weighted spin-echo images were obtained in all cases; pathologic confirmation was available in four patients. MR images were analyzed prospectively without knowledge of clinical data or the results of other imaging procedures. In all cases, low- and high-signal masses were observed on T1- and T2-weighted images, respectively. The cysts were unilocular with a sharply defined margin, ranging from 0.5 to 3.0 cm in diameter. The thin wall was of low signal intensity on T2-weighted images. Our experience suggests that MR imaging is useful in demonstrating the liquid content of prostatic and seminal-vesicle cysts and in establishing their size and location.

400 urinary stone patients were treated with the electromagnetic lithotriptor Siemens 'Lithostar': 66.3% had renal stones and 33.7% had ureteral stones. Ninety percent of the treatments were performed under intravenous sedation only: 14.5% of the patients had more than one session. After 3 months more than 80% of the patients with a single stone smaller than 1.5 cm and 46% of the patients with stones larger than 2.5 cm were 'stone-free'.
