BACKGROUND: Galcanezumab has shown efficacy and effectiveness in the treatment of episodic and chronic migraine (CM), however, the population represented in randomized clinical trials (RCTs) differs from the population observed in real-world setting. To describe the long-term effectiveness and tolerability of galcanezumab in clinical practice in patients excluded from RCTs. METHODS: Multicenter prospective cohort study of consecutive patients with chronic and high-frequency episodic migraine (HFEM) with prior failure to three or more migraine preventive drugs, treated with galcanezumab and followed up for 12 months. RESULTS: We enrolled 1055 patients, aged 50 (IQR: 42-58), 82.9% female, 76.4% chronic migraine, 69% with at least one exclusion criteria for RCTs, including age > 65 (n = 121), concomitant use of onabotulinumtoxinA (n = 185), daily headache at baseline (n = 347), chronic painful syndromes (n = 206), fibromyalgia (n = 101) or treatment resistance (n = 957). The median number of prior preventive treatments was 4 (IQR: 3-5). The retention rate was 90.8%, 76.8% and 71.4% at 3, 6 and 12 months. The main reasons for treatment discontinuation were lack of effectiveness (21.1%) and inadequate tolerability (6.6%). The 30%, 50% and 75% responder rates were 62.6%, 49.8% and 24.2% between weeks 8-12; 60.9%, 48.8% and 24.6% between weeks 20-24; and 59.7%, 48.3% and 24.6% between weeks 44-48. Daily headache at baseline (OR: 0.619; 95%CI: 0.469-0.817) and patient's age (OR: 1.016; 95%CI: 1.005-1.026) were associated with 50% response at weeks 20-24. The variables that were associated with a higher reduction of headache days between weeks 20-24 were patient's age (0.068; 95% CI: 0.018-0.119) and headache days per month at baseline (0.451; 95% CI: 0.319-0.583), while psychiatric comorbidity (-1.587; 95% CI: -2.626-0.538) and daily headache at baseline (-2.718; 95% CI: -4.58-0.869) were associated with fewer reduction in the number of headache days between weeks 20-24. CONCLUSION: This study provides class III evidence of effectiveness and tolerability of galcanezumab in patients with HFEM and CM with comorbidities that would result in exclusion of the pivotal RCTs. Nonetheless, the clinical results over a 12-month period were similar to the efficacy observed in randomized controlled trials. Few patients discontinued the drug due to inadequate tolerability.

BACKGROUND: To determine the percentage of patients who have silicone droplets in the vitreous after treatment with different anti-Vascular Endothelial Growth Factor (anti-VEGF) intravitreal injections (IVI) and how symptomatic they are. METHODS: One hundred fifty-two eyes of 140 patients who had at least received an IVI were recruited for this study. Data collection included the number and type of IVI (aflibercept, ranibizumab and bevacizumab) and the follow-up time. A complete ophthalmologic examination was carried out and patients were classified in four groups according to the amount of silicone droplets found in dilated fundoscopy (nonexistent, scarce, moderate and abundant). Measurement of intraocular pressure (IOP) was also carried out. An interview was conducted to report the presence and intensity of the symptomatology. RESULTS: Silicone oil droplets were reported in 109 eyes (71.7%). A positive correlation was found between the number of IVIs received and the quantity of droplets found, especially when aflibercept was used. Posterior vitreous detachment (PVD) was present in 65.8% of the patients, showing a positive correlation with the number of bubbles. Regarding the symptomatology, 60 eyes (39.5%) had floaters and the disturbance was reported to be 4 out of 10. The group with a moderate amount of silicone droplets had the highest percentage of floaters (60%). No statistical differences in the IOP were found between groups, although the group with abundant droplets had a higher mean IOP. CONCLUSION: A high prevalence of silicone droplets in vitreous of patients who undergo IVI treatment was found. It appears to have little impact on symptomatology and rise of IOP.

Background: Real-world studies have shown the sustained therapeutic effect and favourable safety profile of OnabotulinumtoxinA (BoNTA) in the long term and up to 4 years of treatment in chronic migraine (CM). This study aims to assess the safety profile and efficacy of BoNTA in CM after 5 years of treatment in a real-life setting. Methods: We performed a retrospective chart review of patients with CM in relation to BoNTA treatment for more than 5 years in 19 Spanish headache clinics. We excluded patients who discontinued treatment due to lack of efficacy or poor tolerability. Results: 489 patients were included [mean age 49, 82.8% women]. The mean age of onset of migraine was 21.8 years; patients had CM with a mean of 6.4 years (20.8% fulfilled the aura criteria). At baseline, patients reported a mean of 24.7 monthly headache days (MHDs) and 15.7 monthly migraine days (MMDs). In relation to effectiveness, the responder rate was 59.1% and the mean reduction in MMDs was 9.4 days (15.7 to 6.3 days; p < 0.001). The MHDs were also reduced by 14.9 days (24.7 to 9.8 days; p < 0.001). Regarding the side effects, 17.5% experienced neck pain, 17.3% headache, 8.5% eyelid ptosis, 7.5% temporal muscle atrophy and 3.2% trapezius muscle atrophy. Furthermore, after longer-term exposure exceeding 5 years, there were no serious adverse events (AE) or treatment discontinuation because of safety or tolerability issues. Conclusion: Treatment with BoNTA led to sustained reductions in migraine frequency, even after long-term exposure exceeding 5 years, with no evidence of new safety concerns.

Pleurotus ostreatus (Jacq. ex Fr.) P. Kumm mushroom contains bioactive compounds with both antimicrobial and prebiotic properties, which are distributed in the mushroom mycelium, fruiting body, and spent substrate. The mushroom is rich in nondigestible carbohydrates like chitin and glucan, which act as prebiotics and support the growth and activity of beneficial gut bacteria, thereby maintaining a healthy balance of gut microbiota and reducing the risk of antibiotic resistance. The bioactive compounds in P. ostreatus mushrooms, including polysaccharides (glucans, chitin) and secondary metabolites (phenolic compounds, terpenoids, and lectins), exhibit antibacterial, antiviral, and antifungal activities. When mushrooms are consumed, these compounds can help preventing the growth and spread of harmful bacteria in the gut, reducing the risk of infections and the development of antibiotic resistance. Nonetheless, further research is necessary to determine the efficacy of P. ostreatus against different pathogens and to fully comprehend its prebiotic and antimicrobial properties. Overall, consuming a diet rich in mushroom-based foods can have a positive impact on human digestion health. A mushroom-based diet can support a healthy gut microbiome and reduce the need for antibiotics.

Tako-tsubo-like cardiomyopathy (TTC) is much more common than originally thought. The exact pathophysiology of TTC is unclear. The most accepted theory proposes myocardial stunning of neurogenic origin, supported by the frequent antecedent of emotional or physical stress, suggesting a catecholamine-mediated mechanism. We present a patient with this syndrome and bilateral damage of the dorsal medulla oblongata likely affecting both solitary tract nuclei. Our case points to a link between baroreflex failure and TTC, highlighting the important role of sympathetic discharge in the pathophysiology of TTC.

Palytoxin (PTX) is produced by corals such as zoanthid corals. Here we present a case of bilateral PTX-induced keratoconjunctivitis. A 63-year-old man presented to the emergency department with symptoms of red eye, purulent discharge, and foreign body sensation in both eyes. On slit lamp examination, epithelial defects in both eyes with a ring-shaped corneal stromal infiltrate in the right eye and a marginal stromal infiltrate in the left eye were noted. High-resolution anterior segment optical coherence tomography (HR-AS-OCT) showed stromal hyperreflectivity and Descemet folds. Bacterial, fungal, and amoebic cultures were taken. Empirical treatment with topical dexamethasone as well as antibiotics and systemic doxycycline was started. The next day the patient stated that he had been handling zoanthid coral without gloves and had rubbed his eyes afterward. Bilateral PTX-induced keratoconjunctivitis was diagnosed. His eyes were irrigated abundantly with saline solution, and umbilical cord serum eye drops were added to the treatment. Treatment was tapered according to improvement of the corneal infiltrates and epithelial defects. After four months, the stromal infiltrates were resolved but corneal scars persisted in both eyes. HR-AS-OCT showed anterior stromal hyperreflectivity corresponding to corneal leucomas. PTX can cause ocular adverse effects such as keratolysis and corneal inflammation, and in some cases can lead to corneal perforation. It can also produce systemic adverse effects, hence the importance of the preventive measures when handling corals that can produce this toxin.

A 58-year-old African-American woman presented with a variably painful perianal eruption for 5 years (Figure 1). Prior treatment with topical zinc oxide, oral fluconazole, and Amoxicillin/clavulanic acid resulted in no improvement. She denied chronic diarrhea or cutaneous blistering. Past medical history included limited scleroderma and autoimmune hemolytic anemia treated with daily azathioprine; however, the eruption preceded iatrogenic immunosuppression. Physical examination revealed a well-defined glistening red, ovoid focally eroded plaque involving the intergluteal cleft. The vulva was uninvolved. Oral mucosa was also clear.

Herein we describe a comparative clinical and genetic study of Lrrk2-associated parkinsonism in Northern Spain. In our sample from the Basque region, Lrrk2 R1441G and G2019S account for 15 out of 50 kindreds (30%) with familial Parkinson's disease. We observe common founder haplotypes for both R1441G and G2019S carriers. Our findings highlight the importance of Lrrk2 parkinsonism in this population and may have important consequences for its extended Diaspora in North, Central and South Americas.

Twelve patients with Parkinson disease and psychosis were included in an open-label 12-week trial of ziprasidone. Two patients withdrew from the treatment because of adverse effects. The remaining 10 patients reported a significant improvement in psychiatric symptoms. Altogether, there was no deterioration of motor symptoms (UPDRS III score: basal 40.4 +/- 11.1, first month 41.1 +/- 10.8; final visit, 37.7 +/- 13.3). Two patients (20%) suffered a slight deterioration in motor symptoms and another patient suffered deterioration of gait. No analytic alterations or serious adverse effects that could limit the use of ziprasidone were observed. Although controlled trials are needed, the findings suggest that ziprasidone may be effective in parkinsonian patients with psychosis.

INTRODUCTION: The aim of this study is to analyze the clinical differences between Parkinson's disease patients with major (MD) and minor depression (md) and to see how both affect the quality of life. MATERIAL AND METHODS: 118 patients diagnosed with Parkinson's disease. The mean age of onset was 60.4+/-11.2 years with a mean duration of 8.5+/-6.2 years. Depression was diagnosed according to DSM-IV-TR criteria. Scores on the Hamilton depression inventory, MMSE, PDQ-39, NPI-10, UPDRS III, and UPDRS IV were recorded. RESULTS: Twenty-one patients (17.8%) met the criteria of major depression (MD) and 33 (28.0%) those of minor depression (md). The scores on the PDQ-39 and NPI-10 of patients with MD were higher than in patients with md, and control group. The MMSE scores were lower in patients with MD. In 52.2% of the patients with MD, the diagnosis of depression was made prior to that of PD, this occurred only in 24.2% of the patients with md (p<0.001). The presence of anhedonia was related to cognitive impairment and the presence of neuropsychiatric symptoms. DISCUSSION: MD is probably a part of the disease process of PD; it is associated with cognitive impairment and may precede motor symptoms.

Introducción. Las complicaciones infecciosas, hematológicas y metabólicas en los niños con leucemia linfoblástica aguda pueden tratarse de forma exitosa. Nuestro objetivo fue conocer cuáles son las características clínicas de los pacientes con complicaciones que contribuyen a la mortalidad de los niños con leucemia linfoblástica aguda, y los procesos de atención con apego a la Guías Clínicas del Children's Oncology Group. Métodos. Se realizó un estudio de casos y controles en nueve hospitales con diferentes tasas de sobrevida afiliados al Seguro Popular. Se analizaron las características clínicas y el proceso de atención en complicaciones como choque séptico, neutropenia y fiebre, anemia grave, hemorragias, síndrome de lisis tumoral e hiperleucocitosis. Resultados. En los pacientes con choque séptico, la neutropenia profunda y la confirmación de la fiebre influyeron en la mortalidad. También el retraso en la administración de la primera dosis de antibióticos, así como la falta de adherencia a las Guías Clínicas del Children's Oncology Group. No hubo diferencias clínicas ni de los procesos de atención que afecten significativamente en la anemia grave y hemorragias. Conclusiones. La falta de apego a las Guía Clínicas del Children's Oncology Group influye significativamente en la mortalidad por complicaciones infecciosas. Deben sistematizarse las acciones a realizar en algunos procesos de atención. En hospitales equipados, es evitable la muerte de pacientes con ciertas complicaciones.


Background. Infectious, hematologic and metabolic complications in children with acute lymphoblastic leukemia (ALL) can be successfully treated. The objective of this study was to determine the clinical characteristics of patients with complications that contribute to mortality in children with ALL and healthcare processes through adherence to the Children's Oncology Group Clinical Guidelines (GCCOG). Methods. We performed a case-control study in nine hospitals affiliated with the Seguro Popular insurance program, analyzing clinical characteristics and treatment of febrile neutropenia, septic shock, severe anemia, hemorrhage, lysis tumoral syndrome and hyperleucocytosis. Results. In patients with septic shock, profound neutropenia and confirmation of fever influences mortality. Antibiotic administration delays treatment and lack of adherence to the clinical guidelines of COG. There were no clinical differences in severe anemia and hemorrhage or in the health care processes. Conclusions. Lack of adherence to clinical guidelines has had an impact on mortality in infectious complications. There is a lack of sys-tematization in some care processes. In certified hospitals, there are reports that patients died from avoidable complications.

The present study explores the frequency of RLS in PD and focuses on the clinical differences between patients with and without restless legs syndrome (RLS). A cross-sectional study was designed, comprising 114 patients diagnosed with PD. Those patients positive for RLS were assessed for intensity of the syndrome (IRLS). We compared the clinical characteristics of the patients with and without RLS, using specific scales: Unified Parkinson's Disease Rating Scale (UPDRS I-IV), quality of life (Parkinson's Disease Questionnaire, PDQ 39), sleep symptoms (Parkinson's Disease Sleep Scale, PDSS), and diurnal hypersomnia (Epworth Sleepiness Scale). Twenty-five patients (21.9%) out of a total of 114 subjects diagnosed with PD met the RLS diagnostic criteria. RLS was more frequent in women (68%). The patients with RLS showed poorer scores on the PDSS (PD-RLS+: 102.4 +/- 15.1 vs PD-RLS-: 113.2 +/- 16.4) (P = 0.005) and in the bodily discomfort dimension of the PDQ-39 (PD-RLS+ 6.1 +/- 3.4 vs PD-RLS- 3.8 +/- 2.6) (P = 0.002). Analysis of the subscales of the PDSS showed significant differences (P < 0.001) between both groups of patients in items 4 and 10, and to a lesser degree in items 5 (P = 0.01) and 11 (P = 0.02) There was no increased incidence of diurnal hypersomnia in the group of patients with RLS. There were no differences in the rest of the variables. RLS is frequent in patients with PD, though this condition doesn't apparently affect quality of life or lead to an increased presence of diurnal hypersomnia. It would be advisable to validate the diagnostic criteria of RLS in this specific group of patients.

Introducción. El análisis causa-raíz es una técnica cualitativa caracterizada por un proceso secuencial de preguntas orientadas a la prevención de un evento centinela, al buscar conocer e incidir en los diversos componentes que rodean al acto mismo. La leucemia linfoblástica aguda es el cáncer más común en los niños. Debido a su manejo multidisciplinario, es susceptible de generar diversos errores latentes que ponen en riesgo la vida de los pacientes. Métodos. Se realizó un estudio de análisis causa-raíz a nivel nacional, con la participación de siete hospitales afiliados al Seguro Popular. Un panel de expertos analizó el expediente de un paciente pediátrico con leucemia linfoblástica aguda que hubiera fallecido recientemente en cada uno de los centros participantes. Los resultados se describieron y jerarquizaron para, finalmente, identificar las posibles recomendaciones para mejorar la atención al paciente. Resultados. Se consideró que en cuatro de los siete casos las causas de muerte eran prevenibles. Los problemas en la atención fueron relacionados con el manejo heterogéneo de complicaciones infecciosas y hematológicas. Los distintos centros hospitalarios compartieron la mayor parte de los problemas identificados, especialmente los referentes a la falta de personal capacitado y a la insuficiencia de productos hemáticos. Conclusiones. El estudio sienta la base para conocer las deficiencias en los distintos hospitales y propone el análisis de la información para la creación de comités de mejora. El análisis causa-raíz es una herramienta útil dentro de las unidades médicas, para identificar potenciales deficiencias en los procesos de atención.


Background: Root cause analysis is a qualitative technique characterized by a sequential order of questions to prevent errors by the acknowledgment and management of its diverse components. Acute lymphoblastic leukemia is the most common cancer in children. Due to the necessity of a multidisciplinary approach, it can be the target of diverse latent errors, which jeopardize the life of the patient. Methods: We developed a study including seven national institutions affiliated with the Seguro Popular insurance program. We conducted a summary of the clinical records of the most recent death of a pediatric patient with ALL at each institution. Clinical files were analyzed by an expert panel. Results were described and prioritized in order to recommend improvements for care. Results: We considered that 4/7 deaths were preventable. Problems in patient care are related to mismanagement of infectious and hematological complications. Many of the results obtained in the analysis are shared by the institutions with particular emphasis on lack of personnel and insufficient training as well as deficiency of blood products. Conclusions: This study will become the starting point to acknowledge the deficiencies among the different institutions and invites an analysis of the information to create committees for better care practices and to consider RCA as a useful tool within each medical institution to identify deficiencies in the care process.
