RESUMO

Shellfish farming is considered a highly sustainable form of aquaculture that has developed rapidly worldwide. Unfortunately, today biological and chemical pollution of the oceans and marine waters is widespread and has multiple negative impacts on marine ecosystems, which are exacerbated by global climate changes. In addition, such impacts on fisheries and aquaculture are significant in inducing socio-economic losses. Therefore, it is necessary to develop innovative solutions to improve productivity and environmental performance in line with the blue sustainable economy (European Green Deal). However, one upcoming problem associated with shellfish consumption is shell waste and its disposal. In addition, the percentage of wasted shells destined for reuse is much lower than the one accumulated in landfills or in more or less well-managed sites. This represents a weakness of the shellfish farming sector that can only be mitigated through a project of shellfish waste recycling moving towards the circular economy, with undoubted environmental and economic advantages. In the present study, we present a possible solution for recycling clam shells coming from the waste of the fishing industry (circular economy). Indeed, three eco-friendly bio-reefs for the stabilization and implementation of marine biodiversity (blue economy) were realized using additive manufacturing technology (3D printing) for large dimensions (technological innovation). Furthermore, before deploying the reefs on the sea bottom, they were colonized with oysters to promote repopulation.

Experience gained with the repair of the defects of the long process of the incus with ionomer cement in 35 patients is reported. We describe a case in which reconstruction of the tendon of the tensor tympani muscle was necessary for the successful restoration of sound conduction. The right ear of a nine-year-old boy was treated for cholesteatoma with staged surgery. During the first operation, the tendon was cut to ensure good visibility in the tympanic cavity. Post-operatively, maintenance of aeration of the middle ear required ventilation tubes at first and Valsalva manoeuvres later on. The position of the reconstructed tympanic membrane varied a great deal, moving between the medial wall of the tympanic cavity and extreme bulging. This made exact measurement of a columella for ossicular reconstruction impossible. The preserved handle of the malleus was bound to the cochleariform process with ionomer cement, using a piece of surgical suture material as a substitute for the tendon. This arrangement prevented the tympanic membrane from undergoing excessive lateral movement after inflation and the ossicular chain was replaced with a successful ossiculoplasty with an autogenous bone 'drum to footplate' columella. The pre-operative 55.0 dB air-bone gap decreased immediately to 3.3 dB, widening after three years to 15.0 dB.

The results of treatment in 201 patients with rhinoconjuctivitis due to house dust mite-induced allergy are reported. Bronchial asthma was associated with this condition in 45 of the patients (22.3%). Of the 201, 143 patients in whom symptomatic therapy was not sufficiently efficacious, received immunotherapy (hyposensitization). In most of the 143 patients, hyposensitization with Alutard allergen extract resulted in marked improvement, rendering 107 patients (75%) free of symptoms and signs. The remaining 36 responded as follows: the patient's condition improved considerably in 14 (10%), in seven (5%) it improved, in 10 (7%) it remained unchanged, and in five (3%) it worsened. During the hyposensitization, generalized urticaria was seen in three patients, oedema of the upper part of the body in one, and generalized pruritus and dysponea in one.
