RESUMO

The authors considered systemic approaches to disclosing factors that promote metabolic syndrome formation and associated risk of ciculatory system diseases in drivers. Examination covered drivers with metabolic syndrome and without it. Findings are that metabolic disorders developing in drivers are occupationally conditioned. Length of service can determine cases numbers of cardiovascular disorders, diabetes mellitus manifestations, more frequent combination of hyperglycemia and/or hyper-, dyslipidemia with abdominal obesity. The drivers with length of service over 15 years, aged over 35, demonstrated arterial hypertension in 35% of cases, postinfarction cardiosclerosis in 43% of cases, disorders of cardiac rhythm and conductivity in 55% of cases, and the number increases with length of service and reaches 80% of total examinees number by 55 years of age. Diabetes mellitus was diagnosed in 37% of cases (in 75% of the workers first diagnosed) with over 20 years of service, average age of 50 years.

The objective of the present work was to study the contribution of biofilms to the development of chronic bacterial rhinosinusitis. A total of 50 patients with this pathology were available for the examination. Mucosal swabs were taken from the middle nasal passages of all the patients to be used for the detection of biofilms by luminescence microscopy. The lactoferrine content in mucosal secretion from the nasal cavity was determined by an immunoenzymatic assay. Two groups of the patients presenting with bacterial rhinosinusitis were distinguished in the study of impression smears from nasal cavity mucosa by luminescence microscopy; one of them was comprised of biofilm-positive patients the other of biofilm-negative ones (56% and 44% respectively). The patients showing biofilms over nasal cavity mucosa had the lactoferrine content in mucosal secretion on the order of 0.0033±0.0008 mg/l compared with 0.0068±0.00075 mg/l in the biofilm-negative patients and 0.55±0.0005 mg/l in the healthy volunteers (controls). In other words, the biofilm-positive patients presenting with chronic bacterial rhinosinusitis had two times lower content of lactoferrine in mucosal secretion from the middle nasal passages than those in the biofilm negative group and 126 times lower content of lactoferrine than in the control group.

RESUMO

Anaerobic gas-producing clostridial infection is one of most severe complications of mainly gunshot wound of war period, in peace time occurs very seldom. The rare clinical case of anaerobic gas-producing clostridial endogenic infection with fulminant course in patient with salmonellosis produced by group B salmonella typhimurium. The disease was finished with fatal outcome in 8.5 hours after appearance of pain syndrome in right thigh with consequent fulminant development of gas gangrene with arterial hypotension on 4th day after onset of gastroenteric form of medium severity salmonellosis. Clinical, microbiological and postmortem data are represented.

Identification of aquaporin-4 antibodies in neuromyelitis optica (NMO) is highly important to provide early diagnosis for starting pathogenetic treatment, and for differential diagnosis in neuromyelitis optica spectrum disorders (NMO-SD). In this paper, we review current pathogenetic and clinical aspects of NMO and NMO NMO-SD. We present our data on the identification of aquaporin-4 antibodies in CNS disorders. Aquaporin-4 antibodies were detected in 86.36% of patients with neuromyelitis optica, in 12.5% of patients with partial syndromes and in 100% of patients with systemic lupus erythematosus with longitudinally extensive transverse myelitis or optic neuritis. There was the correlation between the extent of lesion in the spinal cord and positive aquaporin-4 antibodies.

RESUMO

An observation over a chronic course of primary tuberculosis of the lung in a 78-year-old female patient with involvement of the perihilar lymph nodes, development of periadenitis, ingrowth of tuberculous granulations into the lumen of bronchi and with formation of fistulas is described. Clinically the disease proceeded as asthmatic bronchitis.

In hypertension, all the components are affected in the microcirculatory bed of serous membranes: arterioles, precapillaries, capillaries, postcapillaries, venules, lymph capillaries and postcapillaries. Convolution of microvessels, formation of loops, skeins, torsions along the longitudinal axis, deformation of walls (folds and angularity of the contours, multiple unilateral and bilateral protrusions and invaginations, sacciform and cylindrical microaneurisms, multiplication of venules) are observed. In the control material collected from subjects who had died of various traumas vascular changes in serous membranes are poorly manifest or absent indicating a relative specificity of the microvessel involvement in hypertension. The results of the study are in accord with known from literature descriptions of changes in the microcirculatory bed of the eye conjunctiva observed in biomicroscopy in patients with hypertension which confirms the generalized nature of MCB involvement in this disease.

This paper presents an analysis of publications, mostly by Soviet authores, on clinical studies and morphological examinations of the microcirculatory bed in different pathology. It is concluded that the microcirculatory bed should be regarded as an integral system responding to the pathological effects by a local and general reaction of its structural components and by changing the rheological properties of blood. Two types of changes develop in the microcirculatory system -- sterotyped ones, typical for extreme states (various kinds of shock, hypertensive crisis, stress situations), and those specific for certain diseases (diabetes melitus, essential hypertension, athersclerosis, collagenoses, etc.). In all the above diseases the pathological process affects the functional structures of microcirculation that undergo a rearrangement in accordance with the requirements of the body. In the initial period of the disease this re-arrangement is of a compensatory nature and passes ahead of the clinical manifestations. A comparison of the pictutrs obtained by biomicroscopy of the bulbconjunctiva of the eye and of other mucosae with film preparations of the serosae demonstrates their complete similarity. Therefore, the method of biomicroscopy of the eyeball and of the mucosae as a method reflecting the state of microcirculation in the body as a whole should become an integral part of the clinical examination of patients.

RESUMO

In multiple experiments on golden hamsters, highly sensitive to glanders, the method of the transfer of antibacterial resistance was worked out. The most positive effect was obtained after the subcutaneous injection of saline-suspended marrow cells, obtained from white rats, infected with Burkholderia mallei C5 in a single inoculation and passaged through noninfected white rats 5 times. Out of 60 golden hamsters, treated with such material and subsequently (on day 21) injected with 1.0 x 10(2) B. mallei cells, 42 animals survived (70%).

RESUMO

Identification of biomarkers in multiple sclerosis is a very complex problem, and intensive work is being made in this field for the last decades. The importance of establishing biomarkers of multiple sclerosis is related to the high disease heterogeneity and lack of characteristic symptoms that leads to diagnostic, prognostic challenges and difficulties in making decisions about therapy. In this paper, we review the most important biomarkers and discuss their diagnostic and prognostic value. Experimental results on neurofilament heavy chains and antibodies to sulfatide in multiple sclerosis are presented.

The microvasculature (MV) of serous membranes was compared in rats with spontaneous genetic hypertension (SHR) and in normotensive Wistar rats. The study showed that in hypertension MV lesions had a systemic distribution, as structural changes were present in every MV component (arterioles, precapillaries, capillaries, postcapillaries, venules, lymphatic capillaries, and postcapillaries, nerve fibers); these lesions were generalized, as similar alterations could be found in all serous membranes studied. A close resemblance observed between MV of serous membranes in SHR and in patients who died of hypertensive disease confirms the concept suggesting the existence of MV changes which were relatively specific for hypertension, along with those of the nonspecific nature. Specificity of the hypertensive process is manifested in severe vascular lesions of a peculiar type, while the nonspecific phenomena are represented by a combination of intravascular, perivascular, and minimal vascular alterations represented by a universal MV response to various stresses.
