RESUMO

A virtual source (VS) is a hypothetical source instead of an actual physical entity, but provides a distinctive perspective to understand physical fields in a source-free area. In this work, we generalize the VS theory to structured partially coherent light fields (PCLFs) by establishing the partially coherent inhomogeneous Helmholtz equation, then demonstrate that PCLFs can be generated from the incoherent extended VS in imaginary space. Especially, we put forward an understanding of the Gaussian Schell-model beam, which consists of a group of partially coherent paraxial complex rays. The mutual coherence between these rays depends on the included angle between them. In previous studies, the analytical solution of the partially coherent Airy beam was obtained with difficulty by the Huygens-Fresnel integral; however, by applying the VS, we put forward, to our knowledge, an unprecedented analytical solution for a partially coherent Airy beam. We believe this example will qualify the VS as an important perspective to understand structured PCLFs.

RESUMO

We introduce a new type of multiple and off-axis optical bottles (OBs) based on the chirped circular Pearcey Gaussian vortex beam. This kind of beam allows the generation of the OBs with a perfect bottle shape through coherent superposition. Also, we show that the number and the position of the OBs can be precisely and flexibly controlled. The experimental results agree well with our numerical simulations, and we observe stable trapping of the mesocarbon microbeads particles by the proposed bottle beam.

A novel, to the best of our knowledge, class of coherent structures of inseparability, incorporating phases asymmetrically cross-coupled by two position vectors, is introduced in theory and experiment. These phases disappear in the environment of complete coherence, but the vanishment is avoidable in the coexistent state of extreme incoherence and full coherence. The radiated beams intrinsically possess a controllable rotation but undergo an intermediate process quite different from the twisted Gaussian Schell-model beams. Analysis shows a novel association between the magnitude and the phase of the coherent structure which displays both synergy and opposition. Our work further reveals the inner mechanism of the inseparable coherent structures and extends a new horizon for the optical twist.

We introduce the propagation of Pearcey Gaussian (PG) beams in a strongly nonlocal nonlinear medium (SNNM) analytically. Our results show that PG beams propagating in the SNNM have two different focusing positions. The intensity peak appears at different focusing positions depending on the selection of the nonlinear parameters. In addition, the effects of the nonlinear parameters and the scaling factor on the trajectory, the position of the intensity focusing, the intensity evolution between focus locations, and the radiation force are studied.

BACKGROUND: To date, most existing models for predicting neuromyelitis optica spectrum disorder (NMOSD) are based primarily on clinical characteristics. Blood-based NMOSD severity and prognostic predictive immune- and inflammation-related biomarkers are needed. We aimed to investigate the associations between plasma inflammatory biomarkers and relapse and attack severity in NMOSD. METHODS: This two-step, single-center prospective cohort study included discovery and validation cohorts. We quantified 92 plasma inflammatory proteins by using Olink's proximity extension assay and identified differentially expressed proteins in the relapse group (relapse within 1 year of follow-up) and severe attack group. To define a new molecular prognostic model, we calculated the risk score of each patient based on the key protein signatures and validated the results in the validation cohort. RESULTS: The relapse prediction model, including FGF-23, DNER, GDNF, and SLAMF1, predicted the 1-year relapse risk. The severe attack prediction model, including PD-L1 and MCP-2, predicted the severe clinical attack risk. Both the relapse and severe attack prediction models demonstrated good discriminative ability and high accuracy in the validation cohort. CONCLUSIONS: Our discovered biomarker signature and prediction models may complement current clinical risk stratification approaches. These inflammatory biomarkers could contribute to the discovery of therapeutic interventions and prevent NMOSD progression.

BACKGROUND: Previous meta-analyses examined only the short-term differences between lidocaine and steroids vs lidocaine alone in treating lumbar degenerative diseases. Long-term outcomes (1-2 years) in patients with lumbar disc herniation (LDH) and lumbar central spinal stenosis (LCSS) have not yet been systematically evaluated. OBJECTIVE: The objective of our study was to assess quantitatively the difference in efficacy at 1 to 2 years between lidocaine alone vs lidocaine and steroids for the management of LDH or LCSS. STUDY DESIGN: We conducted a meta-analysis. METHODS: PubMed, EMBASE, and the Cochrane library were electronically searched up to July 22, 2016, for randomized controlled trials comparing lidocaine alone vs in combination with steroids for the treatment of LDH and LCSS. Effective pain relief (EPR), Numeric Rating Scale (NRS-11), Oswestry Disability Index (ODI), opioid intake (OI), and total employed increased rate (TEIR) were the endpoints. Risk ratios (RRs) or weighted mean difference (WMD) with 95% confidence intervals (CIs) were calculated, and the pooled analysis was conducted using RevMan 5.2. RESULTS: Seven trials were included. EPR was not significantly different at 1 and 2 years, with RR = 1.08 (95% CI, 0.90-1.30; P = .39) and RR = 1.04 (95% CI, 0.92-1.18; P = .51), respectively, in patients treated with lidocaine alone vs in combination with steroids. The NRS-11 was also similar at 1 and 2 years. ODI and OI were not significantly different at 1 and 2 years. A similar TEIR effect was also observed for the 2 treatments. LIMITATIONS: This meta-analysis relied on a small sample size of trials. Significant heterogeneity among studies was observed. Several significant differences in terms of age of the patients were reported in one included trial. CONCLUSION: This meta-analysis confirmed the similar effects associated with lidocaine alone vs in combination with steroids for the management of LDH and LCSS. Studies with longer follow-up periods are still recommended.
