RESUMO

We report the case of a 76-year-old male previously known with ischaemic heart disease, diabetes mellitus, hypertension and hypercholesterolaemia, who presented with a 14-day history of radicular neck pain, binocular diplopia and a left partial oculomotor palsy. Cerebrospinal fluid analysis showed lymphocytic pleocytosis and an intrathecal synthesis of IgG and IgM antibodies against Borrelia burgdorferi was confirmed. This case report highlights the importance of remembering Lyme neuroborreliosis in the differential diagnosis of diplopia and oculomotor palsy.

RESUMO

Todd's paralysis is a clinical entity consisting of acute focal neurological deficits following an epileptic seizure. It occurs after 6-13% of seizures, and the symptoms may last from minutes to 36 hours. Stroke with seizure at symptom onset is difficult to differentiate clinically from Todd's paralysis. The use of advanced imaging such as cerebral CT and MRI with angiography is recommended. This is a review of the current knowledge on pathogenesis, clinical presentation and differential diagnoses, and we propose an investigation plan for patients presenting with symptoms of Todd's paralysis.

RESUMO

We report the case of a 37-year-old male patient with chronic amphetamine abuse who presented with vertebral artery dissection. Prior to presentation, he had increased the consumption of amphetamine from 5 times a year to once every week and had used amphetamine on the day of presentation. He attended with neck pain, vertigo and coordinating difficulties of his left arm. Computed tomography angiogram of the neck vessels showed a left vertebral stenosis and cerebral magnetic resonance imaging showed a left vertebral pseudolumen and a medullary stroke. Cervical artery dissection is a major cause of stroke in the young. To the authors' knowledge, this is the second reported case of vertebral artery dissection in a patient with amphetamine abuse. Amphetamine might contribute to an increased risk of vertebral artery dissection through its vasculopathic properties although more data are needed to establish a causal relationship.

RESUMO

In this case report we describe the presence of a dural sinus cavernosus fistula presenting with unilateral oculomotor palsy. Dural arterio-venous fistula is a rare abnormal connection between dural arteries and the venous system. Venous stasis, infarctions and intracranial haemorrhage might arise. Hence, timely diagnosis and intervention is crucial. Symptoms involve focal neurological deficits, cognitive impairment, movement disorders, seizures, symptoms of elevated intracranial or intraocular pressure and bruits or pulsatile tinnitus, which may be heard by auscultation of the skull. Invasive angiography is diagnostic.

RESUMO

This is a case report of a 55-year-old male, who was admitted due to right-sided sensorimotor deficits and one-and-a-half syndrome He had snorted 5 g of amphetamine over 24 h two months prior to admission. He was normotensive on admission. A CT brain scan showed left pontine haemorrhage, but a CT angiogram showed no vascular malformations nor vessel beading, and a two-month follow-up MRI showed no vascular malformations nor tumours. To the best of our knowledge, this is the fifth published case of pontine haemorrhage, and the first published case of one-and-a-half syndrome related to amphetamine abuse.

RESUMO

Cerebral aneurysm following a cardiac myxoma is a rare neurological complication. We report a 50-year-old man who developed cerebral aneurysms one year after resection of a cardiac myxoma. Magnetic resonance imaging of the brain showed features of intracranial haemorrhage. Digital subtraction angiography showed several fusiform intracranial aneurysms bilaterally in the middle cerebral artery. Finally, we discuss potential molecular mechanisms of the development of myxomatous aneurysms.
