RESUMO

BACKGROUND: The relationship between venous congestion in cardiopulmonary bypass (CPB) and acute kidney injury (AKI) in cardiac surgery has not utterly substantiated. This study aimed at investigate the relationship between CVP in CPB and the occurrence of AKI. METHODS: We retrospectively reviewed 2048 consecutive patients with cardiovascular disease undergoing cardiac procedure with CPB from January 2018 to December 2022. We used the median CVP value obtained during CPB for our analysis and patients were grouped according to this parameter. The primary outcomes were AKI and renal replacement therapy(RRT). Multivariable logistic regression was used to explore the association between CVP and AKI. RESULTS: A total of 2048 patients were enrolled in our study and divided into high CVP group (CVP ≥ 6.5 mmHg) and low CVP group (CVP < 6.5 mmHg) according to the median CVP value. Patients in high CVP group had the high AKI and RRT rate when compared to the low CVPgroup[(367/912,40.24%)vs.(408/1136,35.92%),P = 0.045;(16/912,1.75%vs.9/1136;0.79%), P = 0.049]. Multivariate logistic regression analysis displayed CVP played an indispensable part in development of renal failure in surgical. CONCLUSIONS: Elevated CVP(≥ 6.5mmH2OmmHg) in CPB during cardiac operation is associated with an increased risk of AKI in cardiovascular surgery patients. Clinical attention should be paid to the potential role of CVP in predicting the occurrence of AKI.

RESUMO

Background: At present new epidemic has entered a stage of normalized management, but there is still sporadic distribution, public already had certain protective knowledge of coronavirus disease 2019 (COVID-19). G County of Liangshan Yi Autonomous Prefecture is located in the mountainous area of southwest Sichuan Province, which also is ethnic minorities and as national-level poverty-stricken areas, residents in the region to the migrant workers as the main economic source of personnel with high mobility. In order to ensure the resumption of work and production, the effective implementation of epidemic prevention measures has certain guiding significance for epidemic prevention and control and economic recovery. This study investigated and analyzed the status quo of villagers' attitudes and behaviors toward COVID-19 prevention and control in Liangshan Yi Autonomous Prefecture, providing evidence for COVID-19 prevention and control measures in the resumption of rural work and agricultural production. Methods: Snowball sampling was used to survey 117 villagers from an impoverished village in Liangshan Yi Autonomous Prefecture on February 10-19, 2020. A total of 120 questionnaires were collected, the recovery rate is 97.5%. Based on literature review, a self-designed questionnaire on attitudes and behaviors related to COVID-19 prevention and control was designed, the expert validity score was 0.912, and Cronbach α coefficient was 0.903. Results: The overall score for respondents' attitude toward COVID-19 prevention and control was 29.65±3.23, which was considered a good level. The total score for prevention and control behavior was 114.74±17.09, which was medium level. A statistically significant difference was found for the attitudes and behaviors of different ethnic groups toward epidemic prevention and control. Conclusions: The people in this village had a positive attitude toward epidemic prevention and control, but there was still room for improvement in prevention and control behavior. Training on hand hygiene and wearing masks outside should be strengthened, and relevant training for ethnic minorities should be further strengthened.

RESUMO

Background Day surgery has been gradually accepted by health professions globally, which can shorten the hospital stay and reduce medical costs. The ambulatory laparoscopic cholecystectomy (LC) has been performed in China since over 10 years. However, few studies focus on its 30-day unplanned readmission rate of ambulatory LC and no standard of unplanned readmission rate for that now. Purpose This study aimed to evaluate the unplanned readmission rate and the reasons readmission after ambulatory LC in a tertiary hospital, which is the earliest ambulatory surgery implementation place in western China. Methods A retrospective analysis was conducted. The patients who underwent ambulatory LC from September 2015 to September 2019 in West China Hospital were screened. The 30-day unplanned readmission rate was calculated as the first outcome. The reasons for unplanned readmission were analyzed and classified as the second outcome. Results The study included 3,014 patients, and the unplanned readmission rate was 1.53%. The rate of patients diagnosed with cholecystolithiasis with cholecystitis was significantly higher in the unplanned readmission group (73.9% vs. 48.9%, p=0.003), and medical cost of unplanned readmission patients was significantly more than that of non-readmission patients (8,102.4±1,375.7 Yuan vs. 7,574.61±10,14.0 Yuan; p=0.008). It was observed that 71.7% readmission happened in the first seven days. Wound problems (60.9%) and abdominal pain (26.1%) went the two main reasons for unplanned readmission. Conclusions The analysis revealed that the unplanned readmission rate of 1.53% was low for ambulatory LC. Some causes of unplanned readmission, such as abdominal pain and wound site pain, wound exudate could be reduced by some simple interventions of the clinical professions.

RESUMO

The aim of this study was to investigate the effects of feedstock material and pyrolysis temperature on the content and spectral properties of dissolved organic matter(DOM) in biochars. Biochars were produced from the pyrolysis of rice straw and Cunninghamia lanceolata litter at three temperatures(350, 500, and 650℃). The results showed that the pH values of the two biochars with pyrolysis temperature increases were improved from 8.10 and 6.56 to 10.53 and 8.23, respectively. The pyrolysis temperature had no significant effect on the total C content of biochar, but the feedstock material and their interaction had significant effects on the total C content of the biochar(P<0.05). The dissolved organic carbon(DOC) content of the two types of biochar first decreased and then increased with increasing pyrolysis temperature, and the content of DOC of the biochar derived from rice straw was significantly higher than that from Cunninghamia lanceolata litter under the same temperature(P<0.05). The feedstock material had no significant effect on the SUVA254 value of DOM, but temperature and its interactive effect with the feedstock material had a significant effect on the SUVA254 values(P<0.05). Maximum DOC SUVA254 values occurred at 500℃ in the two types of biochar, indicating the highest degree of aromatization. Three-dimensional fluorescence spectra showed that the DOM components of the two types of biochar were dominated by fulvic acid-like and humic acid-like material, which had different responses to pyrolysis temperature. FTIR spectra suggested that the DOM of the biochars had absorption peaks at similar positions, in five regions, and the stretching vibration of aliphatic C-H gradually weakened with an increase in pyrolysis temperature. Therefore, the biochars produced at higher pyrolysis temperatures(500℃ and 650℃) had lower DOC contents but a higher aromatization degree and humification degree, and were more stable, compared to the biochars produced at a lower pyrolysis temperature(350℃).

RESUMO

The plant homeodomain (PHD) finger-containing proteins have been implicated in many human diseases including cancer. In this study, we found that PHF14, a newly identified PHD finger protein, is highly expressed in lung cancer. The high expression level of PHF14 was associated with adenocarcinoma and poor survival in lung cancer patients. Knocking down PHF14 suppressed cancer cell growth and carcinogenesis, while over-expressing PHF14 promoted cell proliferation. During cell division, PHF14 directly bound to and co-localized with KIF4A (a nuclear motor protein involved in lung carcinogenesis) to form a functional complex. Similarly to the effect of KIF4A depletion, silencing PHF14 in several cell lines caused cell mitotic defects, prolonged M phase, and inhibited cell proliferation. What's more, these two proteins had a synergistic effect on cell proliferation and were significantly co-overexpressed in lung cancer tissues. Our data provide new insights into the biological significance of PHD finger proteins and imply that PHF14 may be a potential biomarker for lung cancer.

RESUMO

Heat shock proteins (HSPs) are induced by various physical, chemical, and biological stresses. HSPs are known to function as molecular chaperones, and they not only regulate various processes of protein biogenesis but also function as lifeguards against proteotoxic stresses. Because it is very useful to discover nontoxic chaperone-inducing compounds, we searched for them in herbal medicines. Some herbal medicines had positive effects on the induction of HSPs (Hsp70, Hsp40, and Hsp27) in cultured mammalian cells. We next examined 2 major constituents of these herbal medicines, glycyrrhizin and paeoniflorin, with previously defined chemical structures. Glycyrrhizin had an enhancing effect on the HSP induction by heat shock but could not induce HSPs by itself. In contrast, paeoniflorin had not only an enhancing effect but also an inducing effect by itself on HSP expression. Thus, paeoniflorin might be termed a chaperone inducer and glycyrrhizin a chaperone coinducer. Treatment of cells with paeoniflorin but not glycyrrhizin resulted in enhanced phosphorylation and acquisition of the deoxyribonucleic acid-binding ability of heat shock transcription factor 1 (HSF1), as well as the formation of characteristic HSF1 granules in the nucleus, suggesting that the induction of HSPs by paeoniflorin is mediated by the activation of HSF1. Also, thermotolerance was induced by treatment with paeoniflorin but not glycyrrhizin. Paeoniflorin had no toxic effect at concentrations as high as 80 microg/ mL (166.4 microM). To our knowledge, this is the first report on the induction of HSPs by herbal medicines.
