RESUMO

5-fluorouracil (5-FU) is a potent antineoplastic agent used for the treatment of various malignancies. The L-arginine nitric oxide (NO) pathway involved in the pathogenesis of chemotherapy induced kidney damage. This work investigated the beneficial mechanism of L-arginine supplementation in 5-FU induced nephropathy. Eighty male Wistar rats were divided into four equal groups: control group; L-arginine group (378 mg/rat/day for 4 weeks); 5-FU group (189 mg/rat/week for 4 weeks) and L-arginine for 1 week before and 4 weeks concomitant with 5-FU group. At the end of experiment, the kidney functions were assessed and kidneys specimens were processed for paraffin sections and stained with haematoxylin and eosin (H&E), Masson's trichome (MT) and periodic acid-Schiff (PAS) stains. Other sections were processed for immunohistochemical demonstration of caspase-3 and inducible NO synthase (iNOS). Image analyser was used to analyse the results morphometrically and statistically. L-arginine administration to 5-FU treated animals elicited significant reduction in serum urea and creatinine levels, urine volume, urinary protein excretion and kidney/body weight ratio in comparison to fluorouracil treated group. L-arginine improved glomeruloscelerosis, degeneration of convoluted tubules and interstitial fibrosis in 5-FU treated animals. L-arginine attenuated effectively some biochemical and histological changes in 5-FU nephrotoxicity.

BACKGROUND: The aim of the present work was to provide evidence about the anatomical variations as regard the origin, distribution, and branching pattern of the musculocutaneous nerve (MCN). MATERIALS AND METHODS: Brachial plexus was dissected in 40 upper limbs of 20 male adult cadavers. The pattern of the MCN was photographed by a digital camera. RESULTS: The location and length of the nerve branches between left and right arms were recorded and statistically analysed. In 90% of specimens the MCN originates from the lateral cord of the brachial plexus, in 5% it arose from the median nerve (MN), while in the remaining 5% specimen, it was absent. The MCN pierced the coracobrachialis muscle in 90% of specimens, and in the remaining 10% did not pierce it. The motor branches to biceps brachii muscle were categorised into: type 1 (90%): one branch that divides to supply the two heads of biceps; type 2 (5%): double branches, innervating each head of biceps separately. The motor branches to brachialis muscle were categorised into: type 1 (82.9%): one branch; type 2 (14.2%): double branches and type 3 (2.9%): three branches that innervating brachialis muscle. Communications between the MCN and the MN were observed in 35% of specimens. CONCLUSIONS: The knowledge of the common and uncommon MCN variations is important especially to the surgeons for carrying out surgical procedures in axilla and arm.

BACKGROUND: Human anatomy texts in current use have very little precise information as to the frequency of variations in the bifurcation of the common carotid artery, and a clear description of the relation between external and internal carotid arteries as well as the variation of the origin of the ascending pharyngeal and superior thyroid arteries is limited. MATERIAL AND METHODS: Sixty common carotid arteries in the sagittal section of the head and neck of 30 human adult cadavers were obtained from the Anatomy Department of King Abdulaziz University. The data collected were analysed using the Chi square-test. RESULTS: The carotid bifurcation was at the level of the superior border of the thyroid cartilage in 48.3% of cases, 25% were opposite the hyoid bone, and 18.3% were at the level between the thyroid cartilage and the hyoid bone. The bifurcation appeared at a lower level than the superior border of the thyroid cartilage in 5% of cases, while in 3.3% of cases the bifurcation level was seen higher than the hyoid bone. The usual anteromedial position of the external carotid artery to the internal carotid artery was found in 51.7% of cases, whereas it was medial to the internal carotid artery in 36.7% of cases. In 10% it was seen in an anterior position and only in 1.7% the external carotid artery was lateral to the internal carotid artery. In 93.3% of the cases the ascending pharyngeal artery originated from one root, while in the remaining 6.7% of cases it originated from two roots. In 80% of cases the superior thyroid artery arose from the external carotid artery. In 18.3% of cases it originated from the common carotid artery, and in 1.7% it arose from a thyrolingofacial trunk. CONCLUSIONS: The carotid bifurcation can occur as high as the hyoid bone, or as low as the cricoid cartilage. The anteromedial position of the external carotid artery (ECA) in relation to the internal carotid artery (ICA) was the most common anatomical position. The origins and configurations of the ascending pharyngeal artery and the superior thyroid artery are variable.
