RESUMO

BACKGROUND: The incidence of post-operative stenosis after esophageal Endoscopic Submucosal Dissection (ESD) ranks high. This study aimed to investigate the association between the degree of muscular injury and the incidence of post-operative stenosis. RESEARCH DESIGN: This was a retrospective study of 133 patients with superficial esophageal lesions treated by non-circumferential ESD enrolled between January 2016 and May 2021 at two endoscopy centers. Demographic and clinical parameters were analyzed. A novel muscular injury classification system was proposed. Stenosis risk factors were identified using multivariate logistic regression. The association between the different degrees of muscular injury and the incidence of post-operative stenosis was investigated further by propensity score matching (PSM). RESULTS: There were 133 cases evaluated in this study, 33 of which developed stenosis. Multivariate analysis suggested lesions located in the upper 1/3 of the esophagus, resections >5/6 of the circumference, and muscular injury (Grade 3 or 4 according to our proposed classification) were risk factors for stenosis. Correlation analysis suggested a positive association between the degree of muscular injury and the incidence of post-operative stenosis (r = 0.408, p < .05). For PSM, 29 stenosis cases were matched and univariate analysis further corroborated that muscular injuries of grade 3 (OR = 6.429, 95%CI = 1.318-31.367, p = .021) or 4 (OR = 7, 95%CI = 1.068-45.901, p = .043) were risk factors for stenosis. CONCLUSION: Grade 3-4 muscular injury was identified as a risk factor of post-operative stenosis.

RESUMO

Introduction: The aim of this study was to determine the effects of muscular injuries in the lower limbs on the technical and physical performance of professional soccer players when they return to the league competition. Methods: Seventy-six muscular injuries incurred by Spanish male professional soccer players (Age: 27.5 ± 3.5 years) were analyzed during two consecutive competitive seasons: 2014-2015 and 2015-2016. The players' performance was studied during Spanish First Division competitive matches using a multi-camera computerized tracking system (Mediacoach Desktop). Results: After muscular injury relative total distance covered in sprints decreased by 8.6 ± 30.2% (P = 0.013) in the first half and 7.7 ± 36.6% (P = 0.038) in the second half. Similarly, maximal running speed decreased by 2.78 ± 6.91 km.h-1 (pre: 27.3 ± 6.4 km.h-1vs. post: 24.5 ± 6.6 km.h-1, P = 0.013) in the first half, and 1.50 ± 5.68 km.h-1 (29.1 ± 3.9 km.h-1vs. 27.6 ± 5.3 km.h-1, P = 0.043) in the second half. Muscle injury also affected technical performance significantly decreasing successful passes (P = 0.045). There were no differences in the number of possession gains (P = 0.277), and possession losses (P = 0.178). Conclusions: After a moderate or severe muscular injury (causing >8 days lay off), player performance is significantly lower in high-intensity efforts and technical skills such as sprints, maximal running speed, or successful passes.

RESUMO

UNLABELLED: Midkine (MK) is a growth factor with neurotrophic and neurite outgrowth activities. It was expressed in the peri-ischaemic area in the acute phase of cerebral infarction in rat brains. Astrocytes were the origin of MK in this occasion. MK has been assessed in terms of its effects on neural injury. The administration of MK into the lateral ventricle immediately prior to ischaemia prevented cell death in the hippocampal CA1 neurons degenerated by transient forebrain ischaemia in gerbils. MK administration was also beneficial in rats with neural injury, especially after kainic acid-induced seizures. Gene therapy with mouse MK cDNA using an adenovirus was effective in reducing the cerebral infarction volume and in increasing the number of neuronal precursor cells in the subventricular zone of the rat brain. MK mRNA and MK protein were found in spinal cord motor neurons of the anterior horn in both the acute phase of sciatic nerve injury and 3 weeks later. MK immunoreactivity was also found in the proximal side of a sciatic nerve-injured site in sciatic nerve axons. MK receptors were expressed in Schwann cells after injury, suggesting crosstalk between axons and Schwann cells. MK was also present in nerve terminals and influenced ACh receptor clustering during neuromuscular development in Xenopus. Thus, MK may also be involved in reinforcing and maintaining the synapse. All these findings indicate the therapeutic potential of MK for promoting repair of the nervous system after injury. LINKED ARTICLES: This article is part of a themed section on Midkine.

INTRODUCTION: Futsal or indoor soccer is currently the sport with the largest number of players in Brazil, other South American countries and also in Europe. It is derived from the soccer played in an outdoor grass field, adapted because of the lack of space to play. It can be practiced in three different types of floors, carefully leveled, without slopes or depressions, preventing slips and accidents. The movement on the court requires sudden acceleration and deceleration with sudden changes of direction, exposing the musculoskeletal structures to major impacts, increasing the risk of injury, which may be due to intrinsic or extrinsic factors, among which we consider the conditions and type of flooring used. OBJECTIVE: The purpose of this study was to analyze the possible influence of the type of flooring used in the indoor soccer courts and the adaptive muscular responses in the male futsal player. METHODS: The sample was composed by 10 indoor soccer players aged 20-30 years old. By surface electromyography, electrical activity was recorded by means of the root mean square (RMS) in microvolts and also the strength of major muscle groups of the dominant leg at rest after two futsal matches in two types of different court floors. RESULTS: The findings of this study of biomechanical parameters showed no significant difference between the practice of futsal in wooden and synthetic courts, with a tendency of increased electrical activity of anterior muscles on the court with a wooden floor and later in the synthetic flooring. CONCLUSION: It is necessary to make further investigations on the possible mechanisms that can reveal musculoskeletal reactions to different types of court flooring in sports like indoor soccer. .
