RESUMO

El Senado Federal de Brasil creó una Comisión Parlamentaria de Investigación (CPI) para investigar las irregularidades del gobierno de Bolsonaro en la gestión de la pandemia de COVID-19. Uno de los casos que llamó la atención fue la investigación llevada a cabo por Prevent Senior, una empresa privada de seguros de salud, sobre el tratamiento temprano de COVID-19. Este artículo analiza la validez científica de la investigación y los problemas éticos relacionados con su implementación. Se basa en un análisis del informe del estudio clínico de Prevent Senior, de los registros de ensayos clínicos de Brasil y Estados Unidos, del informe de la CPI del Senado y de información difundida por los medios de comunicación. Este caso de fraude científico y sesgo político-ideológico ejemplifica cómo Prevent Senior, utilizando un protocolo cuestionable para mejorar su reputación y obtener el apoyo del gobierno, contribuyó a la construcción de la narrativa de "tratamiento temprano" para COVID-19, y muestra cómo sirvió de base para una política pública del gobierno que promovió el uso de medicamentos ineficaces.

O Senado Federal brasileiro criou uma Comissão Parlamentar de Inquérito (CPI) para investigar as irregularidades do governo Bolsonaro na gestão da pandemia da COVID-19. Um dos casos que chamou a atenção foi a pesquisa realizada pela Prevent Senior, uma seguradora privada de saúde, sobre o tratamento precoce da COVID-19. O artigo analisa a validade científica da pesquisa e os problemas éticos relacionados à sua implementação. Baseia-se na análise do relatório do ensaio clínico da Prevent Senior, dos registros do ensaio clínico em plataformas do Brasil e dos EUA, do relatório da CPI do Senado e nas informações divulgadas pela mídia. Esse caso de fraude científica e viés político-ideológico exemplifica como a Prevent Senior, usando um protocolo questionável para melhorar sua reputação e ganhar o apoio do governo, foi fundamental na construção da narrativa do "tratamento precoce" para a COVID-19, e mostra como serviu de base para uma política pública governamental que promoveu o uso de drogas ineficazes.

Substantial declines in farmland biodiversity have been reported in Europe for several decades. Agricultural changes have been identified as a main driver of these declines. Although different agrienvironmental schemes have been implemented, their positive effect on biodiversity is relatively unknown. This raises the question as to how to reconcile farming production and biodiversity conservation to operationalize a sustainable and multifunctional agriculture. We devised a bioeconomic model and conducted an analysis based on coviability of farmland biodiversity and agriculture. The coviability approach extended population viability analyses by including bioeconomic risk. Our model coupled stochastic dynamics of both biodiversity and farming land-uses selected at the microlevel with public policies at the macrolevel on the basis of financial incentives (taxes or subsidies) for land uses. The coviability approach made it possible for us to evaluate bioeconomic risks of these public incentives through the probability of satisfying a mix of biodiversity and economic constraints over time. We calibrated the model and applied it to a community of 34 common birds in metropolitan France at the small agricultural regions scale. We identified different public policies and scenarios with tolerable (0-0%) agroecological risk and modeled their outcomes up to 2050. Budgetary, economic, and ecological (based on Farmland Bird Index) constraints were essential to understanding the set of viable public policies. Our results suggest that some combinations of taxes on cereals and subsidies on grasslands could be relevant to develop a multifunctional agriculture. Moreover, the flexibility and multicriteria viewpoint underlying the coviability approach may help in the implementation of adaptive management.

BACKGROUND AND OBJECTIVE: In this article we sought to characterize the Long Stay Establishments for the Elderly in Chile, besides presenting the services offered in the public and private. MATERIAL AND METHODS: It is a quantitative, cross-sectional descriptive study, with secondary information source. We analyze all establishments in the country registered with the National Service of the Elderly. As of November 2015, a total of 724 establishments were registered and distributed in 169 neighborhoods throughout the country in which 16,985 adults aged 60 and over were institutionalized. RESULTS: Most of the establishments (65.9% [n=246/724]) are private, 47.5% of them (n=344/724) are located in the metropolitan region of Santiago. Of the residents' health, only 26.5% are considered functional brave, 28.3% are physically handicapped and 8.8% are mentally handicapped. Most establishments offer manual activities, physical exercises, memory classes, cultural classes and recreational or touristic tours. Proportionally of the activities offered were mostly private ones. CONCLUSIONS: In Chile, most of the establishments are private, located in the metropolitan region where there is the largest supply deficit of this type of service, with an occupancy rate of 90.7%, with 72.4% women and almost half 47.7% with some physical or psychic dependence.

OBJECTIVE: To identify if the regional government health plans in Spain include specific objectives and measures related to physical activity. METHOD: Institutional websites were scanned to identify the most recent health plan of each regional government. The information included in the health plans on objectives and measures and the population groups targeted by these measures was extracted. The search was completed on 24 January 2022. RESULTS: Sixteen health plans were identified of which six are in force in 2022; 15 of these have incorporated specific objectives and measures on physical activity. Most of the actions are focus on general population, children-adolescents and elderly population. The plans of Cantabria, the Comunitat Valenciana and País Vasco cover physical activity more widely, proposing measures for seven of the eight identified population groups. CONCLUSIONS: Most of the health plans include measures related to physical activity and target two or more population groups. The objectives, specific measures and population groups were highly heterogeneous. This study could serve to prompt regional governments to review their health plans, update them and improve them in terms of physical activity. It seems advisable that the different health plans should have a homogeneous framework of proposals related to the promotion of physical activity.

OBJECTIVE: In September 2022, the Sociedad Española de Salud Pública y Administración Sanitaria (SESPAS) brought together a panel of experts with the aim of defining and prioritizing health policy proposals, from the perspective of the Spanish State as a whole, to adapt the National Health System (NHS) to current risks and to contemporary/present-day society. METHOD: Expert meeting structured using a mix of procedures adapted from brainstorming, nominal group and Rand consensus method techniques. Relevance and feasibility of proposals identified were assessed individually by each panelist. Proposals were then ordered thematically and ranked according to the median and quartile deviation of relevance scores. RESULTS: Panelists identified and prioritized several proposals in different areas: governance and funding of the NHS, reform of the portfolio of services and benefits and of the NHS human resources, public health and health policy, actions against inequality and poverty, and healthcare delivery reform, including socio-sanitary, primary and end-of-life care. CONCLUSIONS: The results of the meeting show the urgent need to address in-depth changes in many state-wide health policies, including a major reconfiguration of governance, public health, and health care structures. They also point out potential areas of improvement, constituting a tentative guide of prioritized issues to be addressed.

OBJECTIVES: To analyse the profile of tuberculosis (TB) among Bolivian immigrants, investigate the impact that this population has on the trends of TB and assess equity in access to TB treatment, in the city of São Paulo, Brazil. METHODS: Descriptive study of the epidemiological profile of TB in four city districts with large Bolivian populations, comparing cases among Brazilians and Bolivians, during the 1998-2008 period was carried out. We used logistic regression to adjust the treatment outcome for potential confounders. RESULTS: We identified 2056 new TB cases: 65.7% in Brazilians, 32.1% in Bolivians and 2.2% among other nationalities. Although TB incidence remained stable (high) over the study period, the annual proportion of cases among Bolivians increased from 15.0% to 53.0%. In comparison with the Brazilians, the Bolivians were younger (median age, 24 vs. 40 years; P < 0.0001) and presented a lower unemployment rate (3.1%vs. 11.6%; P < 0.0001), a lower rate of HIV co-infection (1.5%vs. 28.5%; P< 0.001), a higher proportion of individuals receiving supervised treatment (81.5%vs. 62.0%; P < 0.0001) and a higher proportion of cures (71.6%vs. 63.2%; P< 0.0001). After having been adjusted for potential confounder, cure after treatment was not associated with nationality. CONCLUSIONS: Bolivian immigrants influenced the incidence but not the trends of TB among Brazilians in the study area. We found no significant differences between Bolivians and Brazilians regarding healthcare access or treatment outcome. Guaranteed universal health care access for all, including undocumented individuals, contributes to health equity. Specific intervention strategies are warranted for immigrants with tuberculosis.

OBJECTIVE: To explore trends in childhood overweight/obesity according to socio-economic status and in relation to the preventive effort developed by the Spanish autonomous regions. METHOD: A series of multilevel cross-sectional studies were conducted using data from different waves of the Spanish National Health Survey, namely 2003, 2006, 2011 and 2017. Hierarchical logistic regression models were estimated, with individuals (level 1) nested within the region-period of study (level 2) and these, in turn, within the region (level 3). The main independent variables were the average effort made by the Spanish autonomous regions in child overweight-obesity prevention policies and the change in the effort made in these policies over the periods studied. RESULTS: The likelihood of obesity and/or overweight increases substantially if the adult respondent in the household is also obese or overweight. The joint effect of the policies implemented by the autonomous regions is associated with a significant decrease in prevalence only for children belonging to high and middle social classes (odds ratio [OR]: 0.89, 95% confidence interval [95%CI]: 0.82-0.96, and OR: 0.93, 95%CI: 0.88-0.97, respectively). CONCLUSIONS: The policies implemented by the Spanish autonomous regions seem to have a limited capacity to achieve significant reductions in the prevalence of childhood overweight and obesity. The results suggest that it is the middle and upper-middle class groups that seem to benefit most from these policies, which could indirectly contribute to increasing inequalities in childhood obesity.

OBJECTIVE: To analyse the gender composition of the advisory boards created for the management and policy decision-making during the COVID-19 pandemic in Spain. METHOD: A peer review was carried out to identify the advisory boards involved in the management of COVID-19 in the autonomous regions and in Spain. Name, number of members, sex and sources of information were collected, and the percentage of women was calculated. RESULTS: At the regional level, the composition of eleven advisory boards was identified, with a mean representation of women of 39.2%; 42.9% at the national level. 75% of all boards represented women below the parity threshold. CONCLUSIONS: There is a significant under-representation of women in the advisory boards for the COVID-19 management, which may limit a necessary feminist perspective for the crisis recovery.

OBJECTIVE: To summarize public health policy models reported in studies published between 2002-2017. METHOD: Using MesH vocabulary, we systematically searched articles in English, Spanish and Portuguese in the PubMed, Scielo, Jstor, ProQuest and Google Scholar data-bases. The quality of the articles was assessed using the tool for understanding a qualitative study by CASPe. RESULTS: A higher proportion of scientific studies were conducted in the African continent. The policies relating to health care were most frequently investigated. Walt and Gilson's model and Kingdon's multiple streams framework were more frequently reported. CONCLUSIONS: The articles set out the public policy analysis model, but not the approach to guide it. A more robust theoretical body needs to be built to enable discussion about the approaches and models for the analysis of public policies. Its distinction is relevant, since it can order methodological proposals for the study of policies.

One of the aims of the citizen's initiative #CienciaenelParlamento is helping to establishing a parliamentary office of scientific and technological advice in the Spanish parliament. Said office would be in charge of fostering networking spaces between scientific knowledge and public policies and of triggering public debate between policy-makers, experts and the general public. In this article, we first review the main parliamentary mechanisms of scientific advice, with special attention to one in particular: parliamentary offices of scientific and technological advice. These offices exist in 22 parliaments worldwide, but there are none in Spain. Second, we describe the activity undertaken by #CienciaenelParlamento in its collaboration with the Congress of Deputies during the 12th Spanish Legislature. This collaboration reached its peak with a two-day networking event in November 2018 with over 200 scientists and almost 100 deputies, who all debated twelve topics of social interest and the most up-to-date scientific knowledge. Thanks to this collaboration, the Congress has taken the first steps towards officially establishing a parliamentary science advice office. Lastly, we enumerate some examples about how these parliamentary offices in other countries have contributed with other stakeholders to better public debate and processing of public policies in public health and other areas. To conclude, we at #CienciaenelParlamento believe that a parliamentary science advice office would help to enhance the science-policy ecosystem in Spain.

Neste cenário, políticas públicas voltadas para auxiliar a população como o Projeto Respirar foram fundamentais para recuperar a funcionalidade cardiorrespiratória e física de pacientes. O objetivo deste estudo é apresentar as experiências de um grupo de ex-pacientes, e agora usuários do Projeto Respirar, desde a alta hospitalar até a alta ao final do projeto. A coleta de dados incluiu a realização de dois Grupos Focais. Os usuários relataram sentir muitos medos e incertezas no início da pandemia, contudo, após o ingresso no Projeto Respirar, mudanças aconteceram nas suas percepções de saúde, em seus comportamentos e hábitos (AU).

Tendo como pano de fundo o contexto brasileiro, o presente texto tem por objetivo tecer algumas reflexões sobre o cruzamento entre envelhecimento e cuidado no período pandêmico. Amparada por dados de uma pesquisa realizada no período que antecedeu a Covid-19 - 2019 - e por outras que aconteceram durante a pandemia, a proposta é pontuar os links de conexão entre esses dois momentos. Apesar da hiperexposição do idoso durante a pandemia de Covid-19, com destaque para a estigmatização e os diferentes níveis de violência, é possível perceber que se tratou de uma exacerbação da visibilidade, mas não de algo novo. No caso do cruzamento entre cuidado e velhice, podemos dizer que um grande segredo público veio à tona.


Using the Brazilian context as a backdrop, this text reflects on the intersection of aging and care during the pandemic. Based on data collected by a study conducted in the period leading up to Covid-19 - 2019 - and other studies during the pandemic, the aim is to highlight the connective links between these two moments. Despite the hyper exposure of older people during the pandemic, including stigmatization and different levels of violence, it is possible to see that it was more of an exacerbation of visibility, but not something new. In the case of the intersection of care and old age, we could say that a major public secret came to light.


Teniendo como telón de fondo el contexto brasileño, el objetivo de este texto es realizar algunas reflexiones sobre el cruce entre envejecimiento y cuidado en el período pandémico. Amparada por datos de una investigación realizada en el período que precedió a la Covid-19 - 2019 - y por otras que tuvieron lugar durante la pandemia, la propuesta es puntuar los enlaces de conexión entre esos dos momentos. A pesar de la súper exposición del anciano durante la Covid-19, con destaque para la estigmatización y los diferentes niveles de violencia, es posible percibir que se trató más de una exacerbación de la visibilidad, pero no de algo nuevo. En el caso del cruce entre cuidado y vejez, podemos decir que un gran secreto público salió a la superficie.

RESUMO

Nos últimos 40 anos, a atuação do psicólogo vem sendo orientada pela aproximação a diversas políticas públicas. A despeito dos avanços empreendidos quanto à capacitação das profissionais para o trabalho com beneficiários de tais políticas, os cursos de graduação ainda parecem carecer de um foco maior no estudo de tais políticas e de uma consolidação de referenciais teórico-metodológicos e epistemológicos de atuação psicológica. O presente ensaio busca discutir criticamente o papel dos fundamentos históricos e epistemológicos da psicologia na formação para atuação em políticas sociais, compreendendo um estudo teórico, de base bibliográfica e documental, acerca das discussões histórico-epistemológicas no campo psicológico. Destaca-se que tais discussões tanto oferecem a base para o desenvolvimento do senso crítico acerca da psicologia enquanto área dispersa e fragmentada quanto também possibilitam o desvelamento da condição escamoteada da psicologia enquanto saber auxiliar ao controle subjetivo e ideológico dos indivíduos pelo Estado.


For the last 40 years, the work of psychologists has been oriented by approaching several social policies. Despite advances in terms of professional training and education to work with beneficiaries of such policies, Psychology undergraduate courses still seem to lack the necessary emphasis on the study of such policies and on the consolidation of epistemological, theoretical, and methodological references for psychological practice. This essay critically discusses the role of the epistemological and historical foundations of Psychology on professional training to act in social policies, through a theoretical, bibliographical, and documental study about the historical-epistemological discussions in the Psychology field. It is noteworthy that such discussions set the basis for the development of critical thinking on Psychology as fragmented and scattered area, as well as enable to unveil the concealed condition of Psychology as an auxiliary knowledge for the subjective and ideologically control of individuals by the State.

The City of Girls and Boys of Huesca (Spain) is an international project that seeks to foster children's autonomy and full participation in the construction of the city. Following its formal approval at the City Council of Huesca, the Girls and Boys Board and the City Laboratory were created, both of which include children from all the schools of Huesca. To highlightthe work on health, activities such as "Healthy School Paths", in which the asset mapping technique is used, were incorporated. Besides the interdisciplinary work among different areas of the city council, professionals from education (schools and university), health and social services, effects on the urban physical environment, with one street, park and square remodelled; and on the children's lifestyles, with more children walking to school alone, have been observed.

Objetivo: Relatar a experiência do planejamento, elaboração do plano e realização de oficinas de trabalho da Estratégia Amamenta e Alimenta Brasil no âmbito da Atenção Primária à Saúde do município do Crato, Ceará. Métodos: Foram realizadas reuniões de educação permanente em saúde, essas chamadas de oficinas de trabalho, conduzidas por residentes do Programa de Residência Multiprofissional em Saúde Coletiva da Universidade Regional do Cariri, com profissionais de diversas áreas da saúde, pertencentes às equipes de saúde da família do município supracitado. Durante a realização das oficinas, a fim de levar maior conhecimento e aprofundamento sobre a temática em questão, houve a aplicação de atividades coletivas com a utilização de metodologias ativas, através de diálogos com trocas de conhecimentos e experiências empírico-científicas, difundindo as orientações preconizadas pelo Ministério da Saúde acerca do Aleitamento Materno e Alimentação Complementar Saudável. Conclusões: O plano de oficina de trabalho (roteiro final) foi construído a partir das observações e discussões de equipe realizadas, após a leitura dos materiais de apoio/consulta, objetivando consolidar de forma lúdica, clara e simples, as informações pertinentes à temática e que fossem condizentes às demandas e necessidades específicas dos territórios. As realizações das oficinas de trabalho em diferentes equipes de Saúde da Família proporcionaram um olhar ampliado e humanizado, além de possibilitar o aprimoramento e fortalecimento das ações de promoção, proteção e apoio ao aleitamento materno e a alimentação complementar saudável por parte dos profissionais de saúde, e a resolutividade de demandas no âmbito da alimentação e nutrição de crianças menores de 02 anos de idade do município.

A Lei Geral do Esporte (LGE) foi sancionada em junho de 2023 e apesar do veto presidencial a 40% do conteúdo, teve como inovação a inclusão a menção à participação das mulheres no esporte, rompendo com a omissão histórica sobre a temática. Considerando o cenário de desigualdade de gênero nas oportunidades de prática esportiva, nosso objetivo foi analisar a forma pela qual a LGE aborda a questão de gênero, considerando o esporte como um direito fundamental para o desenvolvimento humano de todas as pessoas. Analisamos a legislação com base nas premissas da análise cultural, que destaca a linguagem como artefato cultural e tecnologia de poder. Observamos como os discursos presentes na legislação enunciam, produzem ou limitam a inclusão das mulheres no esporte.


The General Sports Law (LGE) was enacted in June 2023 and despite the presidential veto affecting 40% of its content, LGE mentioned the right to sport for women, breaking historical silence on the matter. Given the historical inequality in access and participation for girls and women in sports, this essay examines how the LGE addresses gender issues, recognizing sports as a fundamental right for the comprehensive human development of all individuals. We analyze the legislation based on the premises of cultural analysis, highlighting language as a cultural artifact and a power technology. We observe how the discourses present in the legislation articulate, produce, or limit the inclusion of women in sports.

Resumo O estudo analisou como as inflexões político-econômicas de financiamento e de estruturação do Sistema Único de Saúde podem ter afetado as condições de implementação de ações de prevenção e controle da obesidade (PCO) na atenção primária à saúde (APS) no estado do Rio de Janeiro (ERJ) entre 2014 e 2021. Fundamentou-se em referenciais de análise de políticas, considerando contexto de implementação, antecedentes históricos, mecanismos de indução e incentivos governamentais para o desenvolvimento das ações de PCO. Baseou-se em dois projetos realizados nos 92 municípios do ERJ entre 2014 e 2018 (PPSUS-ERJ) e 2019-2021 (PEO-ERJ), pautados em análise documental, entrevistas e grupos focais com profissionais e gestores da APS. Até 2016, percebe-se os impactos positivos da estruturação da APS e dos mecanismos de indução federais. No entanto, as inflexões na expansão e no financiamento da APS contribuíram para o enfraquecimento de unidades, equipes e estratégias, além de uma retração de recursos para as ações estaduais e municipais. Entre 2016-2018, a crise política e financeira do ERJ foi potencializada pelas crises nacionais, e os contrapontos positivos desde então foram os mecanismos de indução e recursos federais que permaneceram, além das áreas técnicas da SES-ERJ e do cofinanciamento estadual.


Abstract The present study aimed to analyze how these changes, both at the national and state levels, could affect the conditions of the implementation of obesity prevention and control (OCP) actions in primary health care (PHC) in the Rio de Janeiro State from 2014 to 2021. This study was based on policy analysis methods that emphasize the understanding of the implementation contexts, as well as the induction mechanisms and government incentives for the development of actions and integration of two projects that analyzed the OPC actions in PHC in the 92 municipalities of RJS between 2014 and 2018 (PPSUS-RJS) and between 2019 and 2021 (PEO-RJS). The results indicate that, by 2016, it was possible to observe the positive impacts of the structuring of PHC and the federal induction mechanisms in RJS. However, inflections in the expansion and funding of PHC contributed to the weakening of units, teams, and strategies, and led to retraction of resources for both state and municipal actions. Between 2016-2018, RJS's political and financial scenario deteriorated due to national crises, and the positive counterpoints since then were the induction mechanisms and federal resources that remained, in addition to the technical areas of the RJS-HD and state co-financing resources.
