Currently, the aim of the resuscitation of burn patients is to maintain end-organ perfusion with fluid intake as minimal as possible. To avoid excess intake, we can improve the estimation using computer methods. Parkland and Brooke are the commonly used formulas, and recently, a new, an easy formula is been used, i.e. the 'Rule of TEN'. Fluid resuscitation should be titrated to maintain the urine output of approximately 30-35 mL/h for an average-sized adult. The most commonly used fluids are crystalloid, but the phenomenon of creep flow has renewed interest in albumin. In severely burn patients, monitoring with transpulmonary thermodilution together with lactate, ScvO2 and intraabdominal pressures is a good option. Nurse-driven protocols or computer-based resuscitation algorithms reduce the dependence on clinical decision making and decrease fluid resuscitation intake. High-dose vitamin C, propranolol, the avoidance of excessive use of morphine and mechanical ventilation are other useful resources.

BACKGROUND: The use of propranolol has been proposed to reduce the hypermetabolic response of patients with burn injuries. OBJECTIVES: To review the studies published up to December 2013 on the effects of propranolol in burn patients. METHODS: A PubMed search was conducted using the terms "burns", "thermal injury", "beta-blocker" and "propranolol", with the filters "human" and "English" and "Spanish". A total of 42 citations were retrieved, 15 of which were randomized clinical trials. The main results are summarized. MAIN RESULTS: Propranolol at doses adjusted to decrease the heart rate by 20% of the baseline value (4­6 mg/kg/day p.o.) reduces supraphysiological thermogenesis, cardiac work, resting energy expenditure and peripheral lipolysis. It likewise increases the efficiency of muscular protein synthesis and reduces central mass accretion. Most studies have been conducted in pediatric burn patients. CONCLUSIONS: Propranolol reduces the hypermetabolic response in pediatric burn patients. More studies on its effects in adult burn patients are needed.

OBJECTIVE: To evaluate the effect of selective decontamination of the digestive tract (SDD) on hospital-acquired infections (HAIs) in patients with acute burn injury requiring admission to a Burns Unit (BU). DESIGN: Retrospective before-and-after cohort study, between January 2017 and June 2023. SDD was implemented in March 2019, dividing patients into two groups. SETTING: Four-bed BU, in a referral University Hospital in Spain. PATIENTS: All the patients admitted during the study period were eligible for analysis. Patients who died or were discharged within 48hours of admission, and patients with an estimated survival less than 10% not considered for full escalation of therapy were excluded. INTERVENTION: SDD comprised the administration of a 4-day course of an intravenous antibiotic, and an oral suspension and oral topical paste of non-absorbable antibiotics during the stay in the BU. MAIN VARIABLE OF INTEREST: Incidence of HAIs during the stay in the BU. SECONDARY OUTCOMES: incidence of specific types of infections by site (bacteremia, pneumonia, skin and soft tissue infection) and microorganism (Gram-positive, Gram-negative, fungi), and safety endpoints. RESULTS: We analyzed 72 patients: 27 did not receive SDD, and 45 received SDD. The number of patients who developed HAIs were 21 (77.8%) and 21 (46.7%) in the non-SDD and the SDD groups, respectively (p=0.009). The number of hospital-acquired infectious episodes were 2.52 (1.21-3.82) and 1.13 (0.54-1.73), respectively (p=0.029). CONCLUSIONS: SDD was associated with a reduced incidence of bacterial HAIs and a decrease in the number of infectious episodes per patient.

Burned patients may need prolonged admissions in the Intensive Care Service, both for initial care and for the pre and postoperative treatment of the multiple surgeries they require. The initial resuscitation of critically burned patients requires adequate monitoring to calculate the fluid therapy necessary to replenish the losses and ensure tissue perfusion, but without excesses that increase interstitial edema. In addition, monitoring can evaluate the systemic inflammatory response that can lead to shock and organic dysfunctions. After this initial phase we will find a critical patient who requires multiple reinterventions in non-optimal situations, so he will need special care over a long period of time. In addition, the Intensive Care Service offers specific postoperative care for reconstructive surgery and the transplantation of composite tissues (upper limb and face) in which its success depends on a rigorous control through adequate monitoring and treatment.

Se revisan los nuevos conocimientos sobre la matriz extracelular (MEC), que han permitido descubrir su importante rol en la cicatrización de las heridas cutáneas. Se describen sus características morfofisiológicas y cómo interviene en la curación de las heridas cutáneas. Se presentan cuatro casos clínicos en los que se aplicó este enfoque terapéutico: los sustitutos de piel y la "cura húmeda"


We review the new knowledge about the extracellular ma-trix (ECM) that has allowed us to discover its important role in the healing of cutaneous wounds. The morpho-physiological characteristics of ECM and its role in the healing of cutaneous wounds are described. Four clinical cases are presented where this therapeutic approach was applied: the skin substitutes and the "moist wound healing".

Objetivo : Comparar las características clínicoepidemiológicas de pacientes pediátricos con quemaduras antes y durante la pandemia por COVID-19, en el Instituto Nacional de Salud del Niño San Borja. El estudio : Estudio analítico, retrospectivo. Se evaluó pacientes atendidos en el INSNSB, durante los períodos de marzo a diciembre 2019 y 2020. Se obtuvieron datos de las historias clínicas. Para la estadística se empleó el programa SPSS v25 (IC 95%). Hallazgos : Se analizaron 139 pacientes en cada periodo. El 60.4% y el 76.3% procedía de Lima, antes y durante la pandemia respectivamente. El grado de instrucción más frecuente del familiar responsable fue el de secundaria (42.3% y 58.2%). Además, las quemaduras menores mostraron un aumento antes y durante la pandemia (49.9% y 61%). Conclusiones: Se observó cambios significativos en la procedencia, grado de instrucción del familiar responsable y severidad de quemadura en ambos períodos


Objective: To compare the clinical-epidemiological characteristics of patients with burns before and during the COVID-19 pandemic treated at the "Instituto Nacional de Salud del Niño San Borja" . The study: Analytical, retrospective study. Patients treated at the INSNSB, during the periods from March to December 2019 and 2020. Data were collected from medical records. For statistical, the SPSS v25 program (95% CI) was used. Findings: 139 patients were analyzed in each period. 60.4% and 76.3% of patients came from Lima, before and during the pandemic, respectively; The most frequent level of education of the responsible family member was high school (42.3% and 58.2%). Also, minor burns showed an increase before and during the pandemic (49.9% and 61%). Conclusions: Significant changes were observed in the origin, degree of the responsible family member and the severity of burn between both periods.

RESUMO

Major burn injury triggers severe oxidative stress, a systemic inflammatory response, and a persistent hypermetabolic and hypercatabolic state with secondary sarcopenia, multiorgan dysfunction, sepsis and an increased mortality risk. Calorie deficit, negative protein balance and antioxidant micronutrient deficiency after thermal injury have been associated to poor clinical outcomes. In this context, personalized nutrition therapy with early enteral feeding from the start of resuscitation are indicated. Over the last four decades, different nutritional and pharmacological interventions aimed at modulating the immune and metabolic responses have been evaluated. These strategies have been shown to be able to minimize acute malnutrition, as well as modulate the immunoinflammatory response, and improve relevant clinical outcomes in this patient population. The purpose of this updating review is to summarize the most current evidence on metabolic response and nutrition therapy in critically ill burn patients.

Abstract: Introduction: burns are a serious public health problem, with several studies estimating that more than 11 million people were affected by burn injuries with approximately 300,000 deaths worldwide. Studies showed that the main causes of death were inhalation injuries, infection, and metabolic and hemodynamic complications ending in multi-organ failure. It has been shown that the increase in the systemic inflammatory response, whose parameters can be easily obtained, can be useful and directly related to poor prognosis. Several clinical studies indicate that the ratio of neutrophils to lymphocytes, an indicator of systemic inflammatory response, can signify the presence of inflammation in some diseases such as diabetes, coronary artery disease, cholangitis, rheumatoid arthritis, and recently COVID-19. Objective: to know the association between the neutrophil lymphocyte index as a biomarker of mortality in patients with major burns. Material and methods: an observational, retrospective, descriptive, longitudinal study will be carried out: with a user population of the National Center for Research and Attention to Burned Patients (CENIAQ) of the Luis Guillermo Ibarra Ibarra National Rehabilitation Institute. A review of the clinical records of the patients treated in the period will be carried out during the period from February 1, 2020 to February 28, 2022, the data of admission and discharge will be taken into account, as well as initial laboratory studies. The information will be recorded in an Excel spreadsheet to be later analyzed in the SPSS software, the results will be obtained and finally their analysis will be carried out. Results: in the analyzed population we found that the average age is 40 years, it was also found that the most affected gender in this population corresponds to the male gender with 81.2%. It was also found that, within the diagnoses of the population studied, fire burn was the one that most affected the population, this being 67.1% of all diagnoses. However, in this study no significant difference was found in the neutrophil lymphocyte index with respect to the type of burn. In our analysis, the neutrophil lymphocyte index was included as a biochemical predictor of mortality, since high levels of this index at admission are associated with increased mortality. In our population, a significant difference was found between the groups with a fatal clinical outcome and those who recovered, which is why it can be considered as a predictor of mortality in these patients since they presented a p value < 0.023, data that is consistent with what is reported in the international literature where the Neutrophil lymphocyte index value can be used as a predictor of mortality. Conclusions: an association was found between the elevation of the neutrophil/lynphocyte ratio and mortality in patients with severe burns.


Resumo: Introdução: as queimaduras são um grave problema de saúde pública, onde estima-se em diversos estudos que mais de 11 milhões de pessoas foram acometidas por queimaduras com aproximadamente 300.000 mortes em todo o mundo. Estudos mostraram que as principais causas de morte foram lesões inalatórias, infecções e complicações metabólicas e hemodinâmicas que culminaram em falência de múltiplos órgãos. Tem sido demonstrado que o aumento da resposta inflamatória sistêmica, cujos parâmetros podem ser facilmente obtidos, pode ser útil e estar diretamente relacionado ao mau prognóstico. Vários estudos clínicos indicam que a proporção de neutrófilos para linfócitos, um indicador de resposta inflamatória sistêmica, pode significar a presença de inflamação em algumas doenças como diabetes, doença arterial coronariana, colangite, artrite reumatóide e recentemente COVID-19. Objetivo: conhecer a associação entre o índice neutrófilo-linfócito como biomarcador de mortalidade em pacientes com grandes queimaduras. Material e métodos: será realizado um estudo observacional, descritivo, retrospectivo, longitudinal: com a população usuária do Centro Nacional de Pesquisa e Atenção ao Paciente Queimado (CENIAQ), do Instituto Nacional de Reabilitação Luis Guillermo Ibarra Ibarra. Realizaremos uma revisão dos prontuários clínicos dos pacientes atendidos no período de 1º de fevereiro de 2020 a 28 de fevereiro de 2022, serão levados em consideração os dados de admissão e alta, bem como os estudos laboratoriais iniciais. As informações serão registradas em planilha Excel para posteriormente serem analisadas no software SPSS, serão obtidos os resultados e por fim será realizada a análise. Resultados: na população analisada verificamos que a média de idade é de 40 anos, constatou-se também que o gênero mais acometido nesta população corresponde ao gênero masculino com 81.2%. Constatou-se também que, dentro dos diagnósticos da população estudada, a queimadura por fogo foi o que mais afetou a população, sendo este 67.1% do total de diagnósticos. No entanto, neste estudo não foi encontrada diferença significativa no índice neutrófilo-linfócito em relação ao tipo de queimadura. Em nossa análise, foram incluídos o índice neutrófilo-linfócito como preditor bioquímico de mortalidade, uma vez que altos níveis desse índice na admissão estão associados a aumento da mortalidade. Em nossa população, foi encontrada diferença significativa entre os grupos com desfecho clínico fatal e os recuperados, motivo pelo qual pode ser considerado como preditor de mortalidade nesses pacientes, pois apresentaram valor de p < 0.023, dado compatível com o relatado em a literatura internacional onde o valor do índice neutrófilo-linfócito pode ser utilizado como preditor de mortalidade. Conclusões: encontrou-se associação entre a elevação do índice neutrófilo/linfócito e mortalidade em pacientes com queimaduras graves.

Resumen Introducción: Existen distintos scores pronósticos para determinar probabilidad de muerte en pacientes quemados. El Índice de Garcés (IG) utilizado en Chile, no es aceptado mundialmente. Objetivo: Decidimos evaluar la correlación entre IG y Abbreviated Burn Severity Index (ABSI), de uso generalizado. Materiales y Método Estudio descriptivo analítico transversal de pacientes quemados subidos a la plataforma online "Registro Nacional de Quemados" y luego ingresados a nuestro centro entre julio de 2017 y julio de 2018. Se calculó coeficiente de correlación de Pearson entre IG y ABSI. Se normalizan ambas variables para hacerlas comparables y se calculó concordancia entre estas con coeficiente de correlación de Lin. Con análisis de regresión logística se calculó probabilidad de fallecer asociada para ambas variables y se compararon áreas de las curvas ROC de ambos scores. Resultados: De 141 pacientes, 15 fallecieron durante hospitalización, con medias de IG de 104 ± 49 puntos y ABSI 7 ± 2. El coeficiente de correlación de Pearson entre IG y ABSI demostró ser muy alto con un valor de 0,928. El coeficiente de correlación de Lin entregó concordancia moderada con un valor de 0,93. Al comprobar cuál de las dos puntuaciones predice mejor la probabilidad de muerte mediante análisis de sensibilidad y especificidad, la curva ROC cubre más área en IG (0,82 vs. 0,83), sin ser estadísticamente significativo. Conclusiones: Ambos índices, IG y ABSI, son muy similares para predecir la probabilidad de muerte, por lo cual son equiparables a la hora de compartir resultados de estudios científicos.


Introduction: There are different prognostic scores to determine the probability of death in burned patients. The Garces Index (GI) used in Chile is not accepted worldwide. Aim: We decided to evaluate the correlation between IG and Abbreviated Burn Severity Index (ABSI), which is widely used. Materials and Method: Cross-sectional analytical and descriptive study of burned patients uploaded to the online platform "National Burns Registry" and then admitted to our center between July 2017 and July 2018. Pearson's correlation coefficient between IG and ABSI was calculated. Both variables were normalized to be able to make them comparable and the agreement between them was calculated with Lin's correlation coefficient. With logistic regression analysis, the associated probability of death was calculated for both variables and areas of the ROC curves of both scores were compared. Results: Of 141 patients, 15 died during hospitalization, with a mean GI 104 ± 49 and ABSI 7 ± 2. Pearson's correlation coefficient between GI and ABSI showed a very high correlation with a value of 0.928. Lin's correlation coefficient gave moderate agreement with a value of 0.93. When checking which of the two scores best predicts the probability of death through sensitivity and specificity analysis, the ROC curve covers more area in IG (0.82 vs 0.83) without being statistically significant. Conclusions: Both scores, IG and ABSI, are very similar when it comes to predicting the probability of death, which is why they are comparable at the time of sharing the results in scientific studies.

Introducción: a partir de la pandemia por Covid19 se reportó variabilidad en la incidencia de las infecciones asociadas al cuidado de la salud (IACS). Con el objetivo de describir y comparar las tasas de IACS en la Unidad de Quemados de un hospital pediátrico de tercer nivel, antes y después del inicio de la pandemia se llevó a cabo este estudio. Material y métodos: estudio de cohorte, retrospectivo, descriptivo, de vigilancia epidemiológica. Se registraron todos los eventos de IACS en la Unidad de Quemados desde el 01/07/2018 hasta el 31/06/2021. Se compararon las tasas de las IACS entre el período I (PI) previo a la pandemia (07/2018-12/2019) y el período II (PII) posterior al inicio de la misma (01/2020- 06/2021). Resultados: se registraron 74 episodios de IACS, en un total de 8232 pacientes-día. Se registró una tasa global de IACS similar en ambos períodos, 10,08 ‰ pacientes-día (PI) vs 7,34 ‰ pacientes-día (PII), sin encontrarse diferencias estadísticamente significativas en las tasas de bacteriemia asociada a catéter venoso central (BSI-CVC) 3,32 ‰ días uso de CVC (PI) vs 3,20 ‰ (PII), neumonía asociada a ARM 1.43 ‰ días de uso de ARM (PI) vs un 2.02 ‰ (PII), ni infección urinaria asociada a sonda vesical (SV) 7,36 ‰ días de uso de SV (PI) vs 3,64 ‰ (PII). Conclusiones: no se observaron diferencias estadísticamente significativas en las tasas de IACS entre ambos períodos, lo cual podría justificarse con el estricto protocolo en control de infecciones implementado previo al inicio de la pandemia (AU)


Introduction: since the start of the Covid19 pandemic, variability in the incidence of healthcare-associated infections (HAIs) has been reported. This study was conducted to describe and compare the rates of HAIs in the burn unit of a tertiary pediatric hospital before and after the onset of the pandemic. Material and methods: a retrospective, descriptive, epidemiological surveillance cohort study was conducted. All HAI events in the burn unit from 01/07/2018 to 31/06/2021 were recorded. HAI rates between the pre-pandemic period I (PI) (07/2018- 12/2019) and post-pandemic period II (PII) (01/2020-06/2021) were compared. Results: 74 episodes of HAI were recorded in a total of 8232 patient-days. There was a similar overall rate of HAIs in both periods, 10.08 ‰ patient-days (PI) vs 7.34 ‰ patient-days (PII), with no statistically significant differences found in the rates of central venous catheter-related bloodstream infections (CVC-BSI) 3.32 ‰ days CVC use (PI) vs 3.20 ‰ (PII), ventilator-associated pneumonia 1. 43 ‰ days MV use (PI) vs a 2.02 ‰ (PII), or catheter-associated urinary tract infection 7.36 ‰ days catheter use (PI) vs 3.64 ‰ (PII). Conclusions: no statistically significant differences were observed in the rates of HAIs between both periods, which may be explained by the strict infection control protocol implemented prior to the onset of the pandemic (AU)

Objetivo: evidenciar os cuidados de enfermagem descritos na literatura nacional e internacional que são aplicados em pacientes queimados em terapia intensiva. Método: revisão integrativa, realizada no período de fevereiro a dezembro de 2020, nas bases de dados eletrônicas US National Library of Medicine, Medical Literature Analysis and Retrieval System Online e Biblioteca Virtual em Saúde. Resultados: foram selecionados oito artigos, os quais foram categorizados em Cuidados de Enfermagem com a pele em pacientes queimados, Cuidados de Enfermagem com a mobilidade em pacientes queimados e Cuidados de Enfermagem em pacientes queimados em relação à dor, dispositivos e prevenção de complicações. Conclusão: os cuidados de enfermagem para pacientes queimados em terapia intensiva estão intensamente atrelados aos cuidados com as lesões de pele e seus desdobramentos, assim como a prevenção de infecções.


Objective: to highlight the nursing care described in the National and International Literature that is applied to burned patients in intensive care. Methods: integrative review, carried out from February to December, 2020, using the following electronic bases: US National Library of Medicine, Medical Literature Analysis and Retrieval System Online and Biblioteca Virtual em Saúde. Results: it was selected eight articles, which were categorized into Nursing Care for skin in burned patients, Nursing Care for mobility in burned patients and Nursing Care for burned patients in relation to pain, devices and prevention of complications. Conclusion: nursing care for burned patients in intensive care is associated to the care of skin lesions and their consequences, as well as the prevention of infections.


Objetivo: destacar los cuidados de enfermería descritos en la literatura nacional e internacional que se aplican en pacientes quemados en cuidados intensivos. Método: revisión integradora, realizada de febrero a diciembre de 2020, en las bases de datos electrónicas US National Library of Medicine, Medical Literature Analysis and Retrieval System Online y Virtual Health Library. Resultados: se seleccionaron ocho artículos, los cuales se clasificaron en Cuidados de Enfermería con la Pelea en Pacientes Enfermos, Cuidados de Enfermería con la Movilidad en Pacientes Enfermos y Cuidados de Enfermería en Pacientes Enfermos en relación con el dolor, los dispositivos y la prevención de complicaciones. Conclusión: los cuidados de enfermería a los pacientes quemados en cuidados intensivos están intensamente ligados al cuidado de las lesiones cutáneas y sus desdoblamientos, así como a la prevención de infecciones.

Resumen: El conocimiento de la fisiopatología de las quemaduras son base del manejo perioperatorio. Todos los órganos y sistemas resultan alterados y los cambios hemodinámicos en la fase aguda se caracterizan por una disminución del gasto cardíaco. Después de seis días de la lesión, ocurre un nuevo cambio hemodinámico, esta fase es conocida como estado hipermetabólico y su duración está en relación con la extensión y profundidad de la lesión. La lesión por inhalación es un factor importante a considerar como mal pronóstico y, de igual manera, el número de comorbilidades y/o trauma asociado. Durante este segundo período, los anestesiólogos tienen una participación continua y estrecha. Los problemas farmacológicos, manejo de vía aérea, accesos vasculares, hipotermia, monitoreo y control del dolor, entre otros, se hacen muy marcados. Por último, la fase de secuelas acompaña al paciente para toda su vida y la magnitud de los daños, costos y discapacidad serán directamente relacionados con la atención administrada durante las primeras dos fases.


Abstract: Knowledge of the pathophysiology of burns is the basis of perioperative management. All organs and systems are altered and hemodynamic changes in the acute phase are characterized by a decrease in cardiac output. After six days of injury a new hemodynamic change occurs, this phase is known as hypermetabolic state and its duration is in relation to the extent and depth of the injury. Inhalation injury is an important factor to consider as a poor prognosis and also the number of associated comorbidities and/or trauma. During this second period the Anesthesiologists have a continuous and close participation. Pharmacological problems, airway management, vascular access, hypothermia, monitoring and pain control among others become very marked. Finally, the phase of sequelae is for life and the magnitude of damages, costs and disability will be directly related to the care administered during the first two phases.

ABSTRACT Objective: To implement the preoperative Thirst Management Model, measuring its adoption, coverage, acceptability, feasibility and fidelity by nursing in a burn unit. Method: Quasi-experimental, pre- and post-test intervention study. Sample of 59 patients at pre-implementation and 40 post-implementation and 36 nursing professionals participating in the implementation in a burn unit from August (2019) to March (2020). Statistical analysis used Mann-Whitney and Chi-square. Results: Adoption of management ranged from 0.0% to 72.5% post-implementation. The capacity coverage was 87.5% of nurses and 87.9% of nursing technicians. There was acceptability and feasibility of thirst management by professionals. In the plan-do-study-act cycles, three pillars of the Model reached the goals, showing fidelity. Conclusion: The implantation of the Preoperative Thirst Management Model had acceptability and feasibility by the nursing team, showing fidelity in achieving the proposed goals, in addition to the adoption of evidence in clinical practice after high coverage professional training.


RESUMEN Objetivo: Implementar el Modelo de Gestión de la Sed Preoperatoria, midiendo su adopción, cobertura, aceptabilidad, factibilidad y fidelidad por parte de enfermería en una unidad de quemados. Método: Estudio de intervención cuasi-experimental, pre- y post-test. Muestra de 59 pacientes en la pre-implementación y 40 en la post-implementación y 36 profesionales de enfermería participantes en la implementación en un centro de quemados desde agosto (2019) hasta marzo (2020). El análisis estadístico utilizó Mann-Whitney y Chi-cuadrado. Resultados: La adopción de la gestión osciló entre el 0,0% y el 72,5% después de la aplicación. La cobertura de la formación fue del 87,5% de los enfermeros y del 87,9% de los técnicos. Hubo aceptación y viabilidad de lagestión por parte de los profesionales. En los ciclos planear-hacer-estudiar-actuar tres de cuatro pilares del Modelo alcanzaron las metas, evidenciando fidelidad. Conclusión: La implantación del Modelo de Gestión de la Sed Preoperatoria fue aceptable y viable por el equipo de enfermería, demostrando fidelidad a la consecución de los objetivos propuestos, además de la adopción de evidencias en la práctica clínica tras alta cobertura de formación profesional.


RESUMO Objetivo: Implantar o Modelo de Manejo da Sede no pré-operatório mensurando a sua adoção, cobertura, aceitabilidade, viabilidade e fidelidade pela enfermagem em uma unidade de queimados. Método: Estudo de intervenção quase-experimental, pré e pós-teste. Amostra de 59 pacientes na pré-implantação e 40 pós-implantação e 36 profissionais de enfermagem participantes da implantação em um centro de queimados de agosto (2019) à março (2020). Análise estatística utilizou Mann-Whitney e Chi-quadrado. Resultados: Adoção do manejo variou de 0,0% para 72,5% pós-implantação. Cobertura da capacitação de 87,5% das enfermeiras e 87,9% dos técnicos de enfermagem. Houve aceitabilidade e viabilidade do manejo da sede pelos profissionais. Nos ciclos planejar-fazer-estudar-agir três dos quatro pilares do Modelo alcançaram as metas, evidenciando fidelidade. Conclusão: A implantação do Modelo de Manejo da Sede no pré-operatório teve aceitabilidade e viabilidade pela equipe de enfermagem, apresentando fidelidade pelo alcance das metas propostas, além da adoção da evidência na prática clínica após alta cobertura das capacitações dos profissionais.

Resumen Introducción: En el año 2017 se incorporó un registro de notificación en línea (Registro Nacional de Quemados) al flujo de derivación de pacientes quemados en Chile. Objetivo: A partir de la información obtenida de esta plataforma, se describe la epidemiología de las quemaduras y las variables que podrían explicar los traslados fallidos a nuestra unidad de quemados. Materiales y Método: Se analizaron los casos subidos a esta plataforma entre julio de 2017 y julio de 2018. Se caracterizó la población global y comparó variables relevantes entre el grupo de pacientes no trasladados a nuestra unidad y los que fueron trasladados con éxito. Resultados: Se analizaron 319 pacientes, 66% hombres, edad promedio 51 años, IMC de 27% y 47% con enfermedades previas. El fuego fue la principal causa de quemaduras. Se observó un 31% de injuria inhaladora. 107 pacientes no se trasladaron a nuestro centro de quemados. Los pacientes trasladados puntuaron más alto en comorbilidad, índice de gravedad, superficie corporal total quemada y aseo quirúrgico en el hospital base. El grupo de pacientes no trasladados puntuó más alto en injuria inhalatoria. La mortalidad global fue 20,4%. La mortalidad fue mayor en pacientes no trasladados (33,6% versus 13,7%; p < 0,001). Conclusiones: Además de facilitar el flujo de pacientes y ahorrar recursos, un uso noble de esta plataforma es ser fuente de información epidemiológica y de implementación de políticas públicas, lo cual puede ser tomado como ejemplo por otros países en vías de desarrollo. Además, se demuestra que ser trasladado constituye un factor protector de muerte por quemaduras.


Introduction: In 2017, an online notification register, the National Burn Registry, was incorporated into the referral flow of burned patients in Chile. Aim: Through the information obtained from this platform, we describe the epidemiology of burns in Chile, and identify variables that could explain failed transfers to our burn unit. Materials and Method: Cases uploaded to this platform between July 2017 - July 2018 were analyzed. We characterize the global population and relevant variables were compared between the group of patients that failed to be transferred to the burn unit and the ones who were successfully transferred. Results: 319 patients were analyzed, 66% men, average age 51 years, BMI of 27 and 47% with previous illnesses. Fire was the main cause of burn injury. Smoke inhalation injury was observed for 31%. 107 patients failed to reach to our burn center. Transferred patients rated higher in comorbidity, severity index, total burned body surface and surgical debridement at base hospital. The group of not transferred patients rated higher in inhalation injury. Overall mortality was 20.4%. Mortality was higher in non-transferred patients (33.6% versus 13.7%; p < 0.001). Conclusions: Aside from facilitating the flow of burned patients and resources saving, a noble use of this platform has been to serve as a source of epidemiological information and implementation of public policies, which can be taken as an example by other developing countries. Also, being transferred is a protective factor for death from burn injuries.

Resumen Sphingobacterium multivorum es un bacilo gramnegativo no fermentador, que infrecuentemente causa enfermedad en humanos. En la literatura médica existen escasos reportes de infecciones causadas por este microorganismo, en general, en hospederos con alguna comorbilidad. Presentamos el primer caso reportado, según nuestro conocimiento, de una infección de piel y tejidos blandos por Sphingobacterium multivorum en un paciente pediátrico gran quemado.


Abstract Sphingobacterium multivorum is a non-fermentative gram-negative bacillus that rarely causes human infections. In the medical literature, the few reported cases of infections resulting from S. multivorum usually occurred in patients with an associated comorbidity. We present the first case report, according our knowledge, of a skin and soft tissue infection by S. multivorum infection in a pediatric patient after a burn injury.

Resumen Se realizó un estudio retrospectivo de las infecciones fúngicas atendidas en un centro de quemados de alta complejidad situado en Buenos Aires, entre 2011 y 2014, mediante el análisis de las histo rias clínicas (n = 36). Las edades de los pacientes estuvieron entre 18 y 87 años, y 23 (63.9%) fueron mujeres. La extensión de la superficie corporal afectada más prevalente varió entre 30 y 50% (p = 0.03), y entre 71% y 100% en pacientes de menor edad, relacionándose con intentos de femicidio. En el 69.4% la profundidad de la quemadura fue grado 4, y en el 50% de los casos se observaron lesiones por inhalación, con una alta mortali dad (p = 0.04). El Candida score resultó de 3 en el 77.7% de los casos. Se usaron vías centrales y ventilación mecánica. Todos los pacientes recibieron tratamiento antibiótico y en 33 (91.7%) antifúngicos. La escisión qui rúrgica de la quemadura se practicó en 33 (91.7%) y los autoinjertos de piel en 29 (80.6%). La infección fúngica se desarrolló en una media de 21.4 días del ingreso en UCI. Se estudiaron 52 muestras de levaduras que se aislaron de urocultivo (42.3%), hemocultivo (26.9%), biopsia cutánea (9.6%), punta de catéter (15.4%) y aspirado traqueal (5.8%). Por cultivo microbiológico y métodos moleculares se identificaron a Candida albicans (53.8%), C. tropicalis (23.1%), C. parapsilosis sensu stricto (13.5%), C. krusei (5.8%), C. glabrata (1.9%) y C. dubliniensis (1.9%). Las infecciones fúngicas representan severas complicaciones en quemados con factores de riesgo.


Abstract A retrospective analysis of fungal infections was carried out in a health-care burn center between 2011 and 2014 using the patients' medical records (n = 36). Patients ranged from 18 to 87 years of age, with 23 (63.9%) being women. The most prevalent widespread total body surface area affected (TBSA) was 30-50% (p = 0.03), and 71-100% in younger patients, mainly associated with femicide. Fourth degree burns were revealed in 69.4% of the patients while in 50%, inhalation injuries were observed to represent a higher mortality rate (p = 0.04). The Candida score was 3 in 77.7% of cases respectively. Central venous catheter and mechanical ventilation were used. All patients received antibiotic treatment and 91.7% antifungal treatment. Surgical excision of the burn was performed in 33 (91.7%) patients, and skin autografting in 29 (80.6%). The median of the fungal infection devel oped was 21.4 days after admission to the ICU. The specimens analyzed involved 52 yeast samples isolated from different cultures: urine (42.3%), blood (26.9%), skin biopsy (9.6%), catheter tip (15.4%) and tracheal aspirate (5.8%). The use of microbiological culture and molecular methods allowed for the identification of Candida albicans (53.8%), C. tropicalis (23.1%), C. parapsilosis sensu stricto (13.5%), C. krusei (5.8%), C. glabrata (1.9%) and C. dubliniensis (1.9%). Fungal infections observed in skin burns lead to severe complications in at-risk patients.

Currently, there are few studies on candidaemia in the severely burned patient. These patients share the same risk factors for invasive fungal infections as other critically ill patients, but have certain characteristics that make them particularly susceptible. These include the loss of skin barrier due to extensive burns, fungal colonisation of the latter, and the use of hydrotherapy or other topical therapies (occasionally with antimicrobials). In addition, the increased survival rate achieved in recent decades in critically burned patients due to the advances in treatment has led to the increase of invasive Candida infections. This explains the growing interest in making an earlier and more accurate diagnosis, as well as more effective treatments to reduce morbidity and mortality of candidaemia in severe burned patients. A review is presented on all aspects of the burned patient, including the predisposition and risk factors for invasive candidiasis, pathogenesis of candidaemia, underlying immunodeficiency, local epidemiology and antifungal susceptibility, evolution and prognostic factors, as well as other non-Candida yeast infections. Finally, we include specific data on our local experience in the management of candidaemia in severe burned patients, which may serve to quantify the problem, place it in context, and offer a realistic perspective.

Introducción: El cuidado de enfermería para satisfacer necesidades humanas es un proceso individualizado de atención a cada persona, a partir de una valoración que se apoya en el razonamiento clínico y contribuye a la toma de decisiones. Objetivo: Parametrizar los cuidados de enfermería para satisfacer necesidades humanas en personas con lesiones por quemaduras. Métodos: Estudio descriptivo-transversal en el servicio de Caumatología del Hospital Miguel Enríquez, La Habana, durante 2019. Universo 80 enfermeras asistenciales. Se parametrizó la variable cuidados para satisfacer necesidades humanas en personas con lesiones por quemaduras, fraccionada en tres dimensiones, ocho subdimensiones y 29 indicadores. Se aplicaron: observación, encuesta y entrevista y se utilizó el criterio de expertos para constatar la validez de los instrumentos, así como la triangulación metodológica para comparar los datos obtenidos. Se calculó media ponderada y coeficiente alfa de Cronbach para medir confiabilidad de los instrumentos. La comparación de proporciones se realizó mediante chi-cuadrado, considerándose un nivel de significancia para p < 0,05. Resultados: La subdimensión comunicación obtuvo media ponderada 1.49, modo de actuación 2.65, formación de valores 4.98 y ética 4.99, las subdimensiones: necesidades humanas, razonamiento clínico, toma de decisiones y juicio clínico obtienen nivel de desarrollo bajo. Las categorías NANDA-NOC-NIC mostraron deficiencias en el conocimiento inadecuado (47,50 porciento, 58,75 porciento y 66,25 porciento). Conclusiones: La parametrización realizada permitió avalar que los cuidados para satisfacer necesidades humanas en personas con lesiones por quemaduras tienen un nivel medio de desarrollo. Se identificaron problemas y potencialidades que justifican el desarrollo de una intervención de enfermería(AU)


Introduction: Nursing care is an individualized process of care for each person aimed at satisfying her or his human needs, based on assessment supported by clinical reasoning and contributing to decision-making. Objective: To parameterize nursing care aimed at satisfying human needs in people with burn injuries. Methods: Descriptive-cross-sectional study carried out, during 2009, in the burn service at Miguel Enríquez Hospital, La Habana. The universe was eighty healthcare nurses. The variable care for satisfying human needs in people with burn injuries was parameterized, divided into three dimensions, eight subdimensions and twenty-nine indicators. Observation, survey and interview were applied. The method of expert judgment was used to verify the instruments' validity, as well as methodological triangulation to compare the data obtained. Weighted mean and Cronbach's alpha coefficient were calculated to measure the instruments' reliability. Comparison of proportions was carried out using chi-square, considering a level of significance for P < 0.05. Results: The subdimension communication obtained a weighted mean of 1.49, with a mode of action at 2.65, formation of values at 4.98, and ethics at 4.99. The subdimensions human needs, clinical reasoning, decision-making and clinical judgment obtained a low level of development. The NANDA-NOC-NIC categories showed deficiencies associated to inadequate knowledge (47.50 percent, 58.75 percent, and 66.25 percent,, respectively). Conclusions: The parameterization carried out permitted to guarantee that the cares for satisfying human needs in people with burn injuries have a medium level of development. Problems and potentialities that justify the development of a nursing intervention were identified(AU)

Objetivo: Analisar o estado da arte sobre suporte nutricional enteral ao paciente queimado criticamente enfermo. Método: Revisão integrativa de literatura realizada com buscas nas bases de dados PubMed, CINAHL e LILACS. Resultados: A busca possibilitou a seleção de 28 artigos científicos com disponibilidade on-line e no idioma inglês, os quais foram organizados e discutidos em seis categorias: hipermetabolismo e nutrição enteral; nutrição enteral precoce nas vítimas de queimaduras; estimativa de necessidade enérgica; suplementação na nutrição enteral; jejum e nutrição intraoperatória; intolerância à alimentação e sepse. Conclusão: O estudo proporcionou a exposição de diversas informações a respeito dos benefícios associados à introdução de nutrição enteral ao paciente queimado, investigações sobre o aporte calórico mais adequado, pesquisas sobre a nutrição suplementada e incentivo de práticas que ainda não participam totalmente do cotidiano hospitalar, como a nutrição intraoperatória. Entretanto, foi perceptível a carência de pesquisas nacionais voltadas ao paciente com queimaduras(AU)


Objective: To analyze the state of the art on enteral nutritional support to critically ill burned patients. Method: Integrative literature review conducted with searches in Pubmed, CINAHL and LILACS. Results: The search made possible the selection of 28 scientific articles available online and in the English language, which were organized and discussed in six categories: hypermetabolism and enteral nutrition; early enteral nutrition in burned patient; energetic need estimate; supplementation in enteral nutrition; fasting and intraoperative nutrition; food intolerance and sepsis. Conclusion: The study provided information on the benefits associated with the introduction of enteral nutrition to the burned patient, investigations on the most appropriate caloric intake, researches on supplemented nutrition and encouragement of practices that do not yet fully participate in the daily hospital, such as intraoperative nutrition. BACKGROUND: Burns are a public health problem, ranking among the 20 principal causes of morbidity in Mexico. Children are a more vulnerable group to suffer from these injuries. For that reason, we analyzed cases in order to gain a better comprehension and to propose preventive measures for this problem. The aim of the study was to determine the epidemiological profile of those injuries to allow us to plan and implement actions for burn prevention. METHODS: We conducted a cross-sectional study that included demographic characteristics of the child, the parents and the burn injuries from children treated at this Burn Unit. Statistical analysis was done to calculate amounts, proportions and 95% confidence intervals. RESULTS: The epidemiological profile of the injuries included males (63.4%) <5 years of age (65.2%) with educational level according to their age (56.2%). Parents of the included children were <35 years old with basic educational level. Burns were caused by hot water (56.2%) and occurred between noon and midnight (73.0%). The majority of the burns were shallow and deep second-degree or minor (69.2%) and affected <20% of body surface area (74.5%). CONCLUSIONS: This epidemiological profile paved the way for preventive actions beginning with scientific research to guide the actions in a progressive direction along with the cooperative actions of public and private institutions in the development of programs and strategies. The end result would be the establishment of a public policy for accidental injuries.
