RESUMO

Nightmares are common among the general population and psychiatric patients and have been associated with signs of nocturnal arousal such as increased heart rate or increased high-frequency electroencephalographic (EEG) activity. However, it is still unclear, whether these characteristics are more of a trait occurring in people with frequent nightmares or rather indicators of the nightmare state. We compared participants with frequent nightmares (NM group; n = 30) and healthy controls (controls; n = 27) who spent 4 nights in the sleep laboratory over the course of 8 weeks. The NM group received six sessions of imagery rehearsal therapy (IRT), the 'gold standard' of cognitive-behavioural therapy for nightmares, between the second and the third night. Sleep architecture and spectral power were compared between groups, and between nights of nightmare occurrence and nights without nightmare occurrence in the NM group. Additionally, changes before and after therapy were recorded. The NM group showed increased beta (16.25-31 Hz) and low gamma (31.25-35 Hz) power during the entire night compared to the controls, but not when comparing nights of nightmare occurrence to those without. Moreover, low gamma activity in rapid eye movement sleep was reduced after therapy in the NM group. Our findings indicate, cortical hyperarousal is more of a trait in people with frequent nightmares within a network of other symptoms, but also malleable by therapy. This is not only a new finding for IRT but could also lead to improved treatment options in the future that directly target high-frequency EEG activity.

Nightmares are a common symptom in narcolepsy that has not been targeted in prior clinical trials. This study investigated the efficacy of Cognitive Behavioural Therapy for Nightmares (CBT-N), adapted for narcolepsy, in a small group of adults. Given the high prevalence of lucid dreaming in narcolepsy, we added a promising adjuvant component, targeted lucidity reactivation (TLR), a procedure designed to enhance lucid dreaming and dream control. Using a multiple baseline single-case experimental design, adults with narcolepsy and frequent nightmares (≥3/week, N = 6) were randomised to a 2 or 4 week baseline and received seven treatment sessions (CBT-N or CBT-N + TLR). Across the groups, there was a large effect size (between-case standardised mean difference [BC-SMD] = -0.97, 95% CI -1.79 to -0.14, p < 0.05) for reduced nightmare frequency from baseline (M = 8.38/week, SD = 7.08) to posttreatment (M = 2.25/week, SD = 1.78). Nightmare severity improved significantly with large effect sizes on sleep diaries (BC-SMD = -1.14, 95% CI -2.03 to -0.25, p < 0.05) and the Disturbing Dream and Nightmare Severity Index (z = -2.20, p = 0.03, r = -0.64). Treatment was associated with a reduction for some participants in sleep paralysis, sleep-related hallucinations, and dream enactment. NREM parasomnia symptoms (z = -2.20, p = 0.03, r = -0.64) and self-efficacy for managing symptoms (z = -2.02, p = 0.04, r = -0.58) improved significantly with large effect sizes. Participants who underwent TLR (n = 3) all recalled dreams pertaining to their rescripted nightmare. In interviews, participants noted reduced shame and anxiety about sleep/nightmares. This study provides a proof of concept for the application of TLR as a therapeutic strategy with clinical populations, as well as preliminary evidence for the efficacy of CBT-N in treating narcolepsy-related nightmares.

INTRODUCTION: With the advancement of surgical technology, the opportunity to integrate novel surgical preparation is imperative to improve patient outcomes and enhance safety. METHODS: Patient specific perfused kidney phantoms including the tumor, parenchyma, artery, vein, and calyx were fabricated using 3D-printing and hydrogel injection molding from scans of 25 patients scheduled for robotic partial-nephrectomy (RAPN). Models are validated for anatomical accuracy, mechanical, functional properties and surrounded by the other models of relevant anatomy in a body cast for a simulated surgical rehearsal. We investigated the impact of these preoperative rehearsals preceding complex RAPN by analyzing changes in surgeons' decisions following review of both axial-imaging and following rehearsal simulation. Predictive ability of these rehearsal platforms was compared to live surgery outcomes and trifecta of cases as an outcome was calculated. RESULTS: 25 patients with complex renal tumors, average 9.8 nephrometry score and 4.9 cm mean tumor diameter were consented. Mean blood loss and WIT were 193.2 ml and 19.8 min. Two Clavien 2 complications were reported at 30-day postoperative. Trifecta was achieved in 17 (68%) of surgical cases. Surgeons' confidence in their ability to complete a partial nephrectomy and ability to predict WIT significantly increased from initial viewing the axial imaging to after the rehearsal. The correlation coefficient between the procedural clinical data collected during the rehearsals and live surgery were calculated. CONCLUSION: The application of this versatile method for creating authentic perfused kidney phantoms can result in increased confidence in surgical plan and thus improved surgical performance and outcomes.

BACKGROUND: To solve the problem of workplace bullying among nurses, it is necessary to review the effects of interventions and generalize the findings. We conducted a systematic literature review and meta-analysis to evaluate the effects of cognitive rehearsal programs on workplace bullying among hospital nurses. METHODS: Data were collected from March 30 to April 11, 2021, and 11,048 journal articles published in South Korea and internationally were examined across eight databases. Nine articles were selected for inclusion in the systematic literature review; five of the nine studies were included in the meta-analysis. For randomized controlled trials, the risk of bias was evaluated, and for non-randomized controlled trials, the study quality was evaluated using the Risk of Bias for Non-randomized Studies version 2.0. Egger's regression test was performed to determine publication bias. RESULTS: Of the nine articles selected for this study, two were randomized controlled trials and seven were non-randomized controlled trials. The I2 value was 18.9%, indicating non-significant heterogeneity. The overall effect size of the cognitive rehearsal programs was -0.40 (95% confidence interval: -0.604 to -0.196; Z = -3.85; p = .0001) in a random-effects model, indicating a large effect size with statistical significance. CONCLUSIONS: Therefore, cognitive rehearsal programs that address workplace bullying among hospital nurses are effective. Health policymakers must implement cognitive rehearsal programs in a policy manner to address the problems of bullying in the workplace.

The emotions attributed to an event can change from occurrence to recall. Autobiographical memories (AMs) exhibit fixed affect (i.e., no change in emotionality), fading affect (i.e., a decrease in emotional intensity), flourishing affect (i.e., an increase in emotional intensity), and flexible affect (i.e., change of valence). Mixed-effects multinomial models were used to predict the likelihood of the different affect change categories. Mixed-effects regression models were used to predict the amount of emotional change within each category. Predictors at the event-level were initial intensity, social rehearsal, and recall frequency. Predictors at the participant-level were components of subjective wellbeing. Analyses were based on 1,748 AMs reported by 117 young participants in response to 16 event cues. Frequency biases, and biases in the amount of change across positive and negative emotionality, were found for all types of emotional change. Specifically, there was more fading of negative (29.98%) than positive affect (11.90%), more flourishing of positive (34.27%) than negative affect (9.61%), and more AMs changing from negative to positive valence (13.33%) than vice versa (3.95%). These biases were also evident in the amount of change within the categories. Moreover, slightly more AMs remained fixed in positive (49.89%) than negative affect (47.08%). Both event and participant level predictors were significantly associated with the likelihood of different affect change categories and the amount of emotional change within the categories. The present findings highlight the importance of considering the different ways in which AMs change emotionally from occurrence to later recall.

A series of four experiments tested the assumptions of the most prominent and longstanding account of item-method directed forgetting: the selective rehearsal account. In the item-method directed forgetting paradigm, each presented item is followed by its own instructional cue during the study phase - either to-be-forgotten (F) or to-be-remembered (R). On a subsequent test, memory is poorer for F items than for R items. To clarify the mechanism underlying memory performance, we manipulated the time available for rehearsal, examining instructional cue durations of 1 s, 5 s, and 10 s. Experiments 1a and 1b, where the order of cue durations was randomized, showed no effect of cue duration on item recognition of unrelated single words, for either R or F items. Experiment 2, using unrelated word pairs, again showed no effect of randomized cue duration, this time on associative recognition. Experiments 3 and 4 blocked cue duration and showed equivalent increases in recognition of both R and F single words and word pairs with increasing cue duration. We suggest that any post-cue rehearsal is carried out only when cue duration is predictable, and that such limited rehearsal is equally likely for F items and R items. The consistently better memory for R items than for F items across cue duration depends on selective retrieval involving (1) a rapid retrieval check engaged for R items only and (2) a rapid removal process implemented for F items only.

Encoding and recalling spoken instructions is subject to working memory capacity limits. Previous research suggests action-based encoding facilitates instruction recall, but has not directly compared benefits across different types of action-based techniques. The current study addressed this in two experiments with young adults. In Experiment 1, participants listened to instructional sequences containing four action-object pairs, and encoded these instructions using either a motor imagery or verbal rehearsal technique, followed by recall via oral repetition or enactment. Memory for instructions was better when participants used a motor imagery technique during encoding, and when recalling the instructions by enactment. The advantage of using a motor imagery technique was present in both verbal and enacted recall. In Experiment 2, participants encoded spoken instructions whilst implementing one of four techniques (verbal rehearsal, motor imagery, observation of others' actions or self-enactment), and then recalled the instructions by oral repetition or enactment. For both verbal and enacted recall, memory for instructions was least accurate in the rehearsal condition, while the other encoding conditions did not differ from each other. These novel findings indicate similar benefits of imagining, observation and execution of actions in encoding spoken instructions, and enrich current understanding of action-based benefits in working memory.

Repetitive thinking is a common phenomenon, also implicated in a variety of mental disorders. The content of repetitive thoughts can take the form of prediction of aversive events (in worry) or evoking negatively valenced information (in rumination), for instance. Investigating the influence of repetitive thinking through the lens of associative learning could help deepen our understanding of the mechanisms involved in its effects.In two experiments, non-clinical participants were exposed to CS-US pairings with the aim of creating non-threatening expectancy learning (Experiment 1) or evaluative conditioning (Experiment 2). After each conditioning trial, participants were instructed to initiate repetitive thoughts about the pairing (i.e. rehearse) or follow control instructions.Experiment 1 (N = 64) showed that such intervention strengthen the association between the mental representations of the CS and the US, leading to a stronger US expectancy in response to the CS. In Experiment 2 (N = 107), an evaluative conditioning effect was observed; however, it was not influenced by instructions.The study demonstrated that simulated repetitive thinking strengthens the CS-US association and leads to greater US expectancy in expectancy learning, despite appearing not to influence the evaluative conditioning effect. The potential implications of these findings on repetitive thinking are discussed.

Posttraumatic nightmares (PTN) are a frequent symptom after a traumatic event and often play part in the psychopathology of patients with borderline personality disorder (BPD). Imagery rehearsal therapy (IRT) currently offers the best evidence for an effective treatment to reduce PTNs, although high drop-out rates are common. Art therapy in IRT may counteract this, by its indirect, nonverbal, and often playful approach that helps to break through avoidance. This case study focusses on the perception of a patient with BPD in an art therapy based IRT treatment for patients with PTNs. It tells the story of Aurelia, a 40-year-old woman who, within this treatment, processes traumatic contents of her childhood like physical and sexual violence, but also current interactional problems that manifest themselves in her nightmares. Following the IR-AT treatment for PTNs Aurelia noticed a reduction in her nightmares, was less afraid of them and felt calmer towards her trauma. She expressed herself in the art medium and by this developed more self-efficacy. Her process resulted in an integration of the trauma and a perceived decrease in borderline symptoms. Future research can build on this basis to further explore the mechanisms and effects of IR-AT for PTNs.

Most theories of free recall emphasize the importance of retrieval in explaining temporal and semantic regularities in recall; rehearsal mechanisms are often absent or limit rehearsal to a subset of what was last rehearsed. However, in three experiments using the overt rehearsal method, we show clear evidence that just-presented items act as retrieval cues during encoding (study-phase retrieval) with prior related items rehearsed despite well over a dozen intervening items. Experiment 1 examined free recall of categorized and uncategorized lists of 32 words. In Experiments 2 and 3, we presented categorized lists of 24, 48, and 64 words for free recall or cued recall, with the category exemplars blocked in successive list positions (Experiment 2) or randomized throughout the list (Experiment 3). The probability of rehearsing a prior word was affected by its semantic similarity to the just-presented item, and the frequency and recency of its prior rehearsals. These rehearsal data suggest alternative interpretations to well-known recall phenomena. With randomized designs, the serial position curves were reinterpreted by when words were last rehearsed (which contributed to the list length effects), and semantic clustering and temporal contiguity effects at output were reinterpreted by whether words were co-rehearsed during study. The contrast with the blocked designs suggests that recall is sensitive to the relative (not absolute) recency of targeted list items. We discuss the benefits of incorporating rehearsal machinery into computational models of episodic memory, and suggest that the same retrieval processes that generate the recalls are used to generate the rehearsals.

Strategy use is an important source of individual differences during immediate serial reconstruction. However, not all strategies are equally suited for all tasks. Therefore, assessing participants' dynamic strategy selection across contexts is an important next step for reliable interpretation of individual differences in short-term memory span - in both experimental and clinical settings. Strategy use during reconstruction of phonologically similar and phonologically distinct word sets was directly assessed using a self-report questionnaire. In two experiments, participants reported consistent use of phonological strategies across word sets; however, participants reported additionally using non-phonological strategies (i.e., mental imagery and sentence generation) when tasked with remembering phonologically similar words. In particular, strategy selection was most impacted when the phonologically similar word set was either the only word set or the first word set participants received. When the phonologically similar lists were presented after a classic list of phonologically distinct words, participants continued using the phonological strategies that had been effective for the distinct lists. Moreover, in both experiments, accuracy of phonologically similar lists was better predicted by use of non-phonological strategies than use of phonological strategies. Specifically, reported use of verbalization or rehearsal did not predict accuracy, but participants who reported regularly using mental imagery and/or sentence generation (typically in conjunction with rehearsal) displayed greater serial memory for similar words. These results do not undermine the general assumptions of the phonological similarity effect, but they do indicate that its interpretation is less straightforward than previously thought.

In this study, we determined whether information in eye-hand/hand-eye cross-modal memory is maintained by the input modality used for encoding, the output modality used for testing, or both. In experiments, two categories of effect were examined: facilitatory, produced by rehearsal work with eyes or hands corresponding to the movement of the stimulus to be memorized after its presentation, and interference, formed through the performance of a noncorresponding movement. The results indicated that both the eye and hand facilitated the eye-hand cross-modal memory tasks (Experiment 1A), confirming that both serve a rehearsal function. Subsequently, we conducted an interference effect experiment (Experiment 1B) using the same memory task as that used in Experiment 1A and found that neither modality produced interference effects. This result indicates that information was preserved via output-modality-specific representations when the eye-interference task interfered with the information retention of input-modality-specific representations and via input-modality-specific representations when the hand-interference task interfered with the information retention of output-modality-specific representations. We observed the same facilitation and disappearance of interference effects for the hand-eye cross-modal memory task (Experiments 2A and 2B). In the eye-hand/hand-eye cross-modal memory tasks, the effects of eye and hand rehearsals were found to be comparable, which indicated that the two types of representations functioned together during encoding and testing. From the disappearance of interference effects, the possibility that modality-specific representations have functional aspects arises.

In a recently published study, (Parker, A., Parkin, A., & Dagnall, N. (2021). Effects of survival processing on list method directed forgetting. Memory (Hove, England), 29(5), 645-661) examined directed forgetting in a survival processing context using the list-method directed forgetting procedure. (Parker, A., Parkin, A., & Dagnall, N. (2021). Effects of survival processing on list method directed forgetting. Memory (Hove, England), 29(5), 645-661) found that the costs of directed forgetting were greater when engaging in survival processing than when making moving relevance or pleasantness ratings. However, according to most current accounts of directed forgetting, engaging in survival processing should not have enhanced the directed forgetting effect but rather should not have impacted the directed forgetting effect. In the present study, we further investigated how survival processing impacts directed forgetting using both the list (Experiment 1) and item method of directed forgetting (Experiment 2). In Experiment 1, we did not replicate the findings of (Parker, A., Parkin, A., & Dagnall, N. (2021). Effects of survival processing on list method directed forgetting. Memory (Hove, England), 29(5), 645-661) - that the directed forgetting effect is enhanced when engaging in survival processing. Rather, we demonstrated that making survival ratings and moving ratings yielded a similar cost of directed forgetting for List 1 items. In Experiment 2, survival processing provided an overall memory benefit (but not when recalling to-be-remembered and to-be-forgotten items in separate recall tests) but did not differentially impact to-be-remembered and to-be-forgotten words. Thus, we did not find evidence that survival processing influences directed forgetting.

After we listen to a series of words, we can silently replay them in our mind. Does this mental replay involve a reactivation of our original perceptual dynamics? We recorded electrocorticographic (ECoG) activity across the lateral cerebral cortex as people heard and then mentally rehearsed spoken sentences. For each region, we tested whether silent rehearsal of sentences involved reactivation of sentence-specific representations established during perception or transformation to a distinct representation. In sensorimotor and premotor cortex, we observed reliable and temporally precise responses to speech; these patterns transformed to distinct sentence-specific representations during mental rehearsal. In contrast, we observed less reliable and less temporally precise responses in prefrontal and temporoparietal cortex; these higher-order representations, which were sensitive to sentence semantics, were shared across perception and rehearsal of the same sentence. The mental rehearsal of natural speech involves the transformation of stimulus-locked speech representations in sensorimotor and premotor cortex, combined with diffuse reactivation of higher-order semantic representations.

BACKGROUND: COVID-19's infection control policies have hindered the Deliberate Practice of clinical examinations. Guided Mental Rehearsal (GMR) may overcome this obstacle by facilitating independent, repetitive practice. Underpinned by the 'Motor Simulation Theory,' GMR reinforces similar neuro-circuit activations during physical practice and was proven effective in surgical training. METHODS: This prospective, randomized controlled study evaluated the efficacy of GMR versus 'peer-learning' of Confrontational Visual Field Examination (CVFE). Third-year medical-students without clinical Ophthalmology experience were recruited. Controls (n = 40) watched an e-learning instructional video (8-min CVFE tutorial) followed by 6-min of 'peer-learning.' GMR-students (n = 40) had 'peer-learning' replaced by a 6-min GMR audio-recording (CVFE running commentary). Pre-test and post-test MCQs were administered to determine baseline knowledge and knowledge acquisition, respectively. 28 controls and 26 GMR-students performed CVFE on simulated patients with right homonymous hemianopia. Four Ophthalmologists graded their performances using a checklist-based marking scheme. RESULTS: Both groups did not exhibit a significant difference in pre-test scores (8.550 vs. 7.947, p = 0.266); outcome of sub-group analysis of CVFE-performing candidates was similar (8.214 vs. 7.833, p = 0.561). Post-test scores were significantly higher than pre-test in both groups (all p < .001), without inter-group difference (14.000 vs. 15.000, p = 0.715). GMR-group had significantly higher scores on CVFE performance than controls (85.354 vs. 73.679%, p = .001). CONCLUSIONS: GMR improved psychomotor but not cognitive aspect of learning CVFE. This may be attributable to GMR's theoretical resemblance with physical practice, with additional expert guidance. By enabling independent learning, GMR may also reduce the demand for teaching manpower and thus education cost in the future.

OBJECTIVES: Posttraumatic stress disorder (PTSD) is associated with sleep disturbances including insomnia and nightmares. This study compared cognitive behavioral therapy for insomnia (CBT-I) with CBT-I combined with imagery rehearsal therapy (IRT) for nightmares to evaluate if the combined treatment led to greater reductions in trauma-related sleep disturbances in Australian veterans. METHODS: Veterans with diagnosed PTSD, high insomnia symptom severity, and nightmares (N = 31) were randomized to eight group CBT-I sessions or eight group CBT-I + IRT sessions. Self-reported sleep, nightmare, and psychological measures (primary outcome: Pittsburgh Sleep Quality Index), and objective actigraphy data were collected; the effect of obstructive sleep apnea (OSA) risk on treatment outcomes was also examined. RESULTS: No treatment condition effects were detected for the combined treatment compared to CBT-I alone, and no moderating effect of OSA risk was detected. On average, participants from both groups improved on various self-report measures over time (baseline to 3 months posttreatment). Despite the improvements, mean scores for sleep-specific measures remained indicative of poor sleep quality. There were also no significant differences between the groups on the actigraphy indices. CONCLUSIONS: The findings indicate that there is potential to optimize both treatments for veterans with trauma-related sleep disturbances.

BACKGROUND: Expressive writing about previous interactions has been found to improve mental health, but the effect of expressive writing about future imagined interactions has not been explored. METHODS: To examine the effects of rehearsal expressive writing on mental health, we recruited 132 people, and 100 (76%) completed writing tasks (31 control, 34 replay, and 35 rehearsal). Each group was measured at baseline, after the 4th consecutive day of 15-minute-per-day writing, and at 1-month follow-up. Measures included psychological adjustment (Mental Health Inventory-18, MHI), self-perceived stress, PROMIS depression and anxiety, work and social adjustment, loneliness, coping (Brief COPE), and daily writing expressiveness. Repeated measures ANOVA examined time and time-by-group effects. RESULTS: Significant time effects were found for MHI, self-perceived stress, depression and anxiety symptoms, and self-blame. Marginally significant group-by-time effects on behavioural disengagement and venting, subscales of the coping measure, were found, with rehearsal associated with decreased disengagement and increased venting compared to the replay group. The rehearsal group consistently reported significantly higher perceived writing expressiveness than the other groups over time. Rehearsal and replay expressive writing showed different effects on writing expressiveness and coping via behavioural disengagement and venting. CONCLUSION: This study established rehearsal as a promising extension to the affordable therapeutic expressive writing paradigm.

INTRODUCTION: Simulators provide a safe method for improving surgical skills without the associated patient risks. Advances in rapid prototyping technology have permitted the reconstruction of patient imaging into patient-specific surgical simulations that require advanced expertise, potentially continuing the learning curve. OBJECTIVES: To evaluate the impact of preoperative high-fidelity patient-specific percutaneous nephrolithotomy hydrogel simulations on surgical and patient outcomes. MATERIALS AND METHODS: Between 2016 and 2017, a fellowship-trained endourologist performed 20 consecutive percutaneous nephrolithotomy procedures at an academic referral center. For the first ten patients, only standard review of patient imaging was completed. For the next ten patients, patient imaging was utilized to fabricate patient-specific models including pelvicalyceal system, kidney, stone, and relevant adjacent structures from hydrogel. The models were tested to confirm anatomic accuracy and material properties similar to live tissue. Full procedural rehearsals were completed 24-48 h before the real case. Surgical metrics and patient outcomes from both groups (rehearsal vs. standard) were compared. RESULTS: Significant improvements in mean fluoroscopy time, percutaneous needle access attempts, complications, and additional procedures were significantly lower in the rehearsal group (184.8 vs. 365.7 s, p < 0.001; 1.9 vs. 3.6 attempts, p < 0.001; 1 vs. 5, p < 0.001; and 1 vs. 5, p < 0.001, respectively). There were no differences in stone free rates, mean patient age, body mass index, or stone size between the two groups. CONCLUSION: This study demonstrates that patient-specific procedural rehearsal is effective reducing the experience curve for a complex endourological procedure, resulting in improved surgical performance and patient outcomes.

While items learned immediately before testing are generally remembered better than prior items in a study list, in delayed testing this relationship is reversed, yielding a negative recency effect. To adjudicate between the strategic rehearsal and spacing accounts of this phenomenon, we examined performance of 169 participants on a delayed recognition test following multiple sessions requiring the study and immediate free recall testing of 16 lists of 16 words. This revealed a strong effect of the amount of spacing between initial study position and initial free recall position on the degree of negative recency, supporting the spacing account. Furthermore, these spacing effects were nonmonotonic, suggesting that they are mediated by consolidation processes. Additional analyses indicate that strategies and rehearsal opportunities may also contribute to the effects of within-list encoding position on subsequent long-term memory, but for recall more than for recognition.

Do the properties of to-be-remembered events influence the ability to remember, and also intentionally forget, these events in similar ways? Prior work has examined how the font size, animacy, emotionality, concreteness (the degree to which a word denotes something perceptible), frequency (how often a word appears in language), and length of to-be-remembered words influence memory. However, it was previously unclear whether the forgetting of information is also influenced by these characteristics. In six experiments, we used an item-method directed forgetting task where we presented participants with to-be-remembered and to-be-forgotten words varying in font size (large or small), animacy (animate or inanimate), emotionality (negative or neutral), concreteness (high or low), frequency (high or low), and word length (long or short). Results revealed that animacy, emotionality, concreteness, frequency, and word length (but not font size) influenced both remembering and forgetting. Together, the present findings indicate that the characteristics of presented words can influence remembering as well as directed forgetting, providing further evidence that the remembering and forgetting processes are governed by similar properties.
