OBJECTIVE: To compare the diagnostic accuracy of 3-T magnetic resonance imaging (MRI) with thin-slice 3D T1 VIBE sequence to 128-slice computer tomography (CT) in pars stress fractures of the lumbar spine. MATERIALS AND METHODS: 3-T MRI and CT of 24 patients involving 70 pars interarticularis were retrospectively reviewed by four blinded radiologists. The fracture morphology (complete, incomplete, or normal) was assessed on MRI and CT at different time points. Pars interarticularis bone marrow edema (present or absent) was also evaluated on MRI. RESULTS: In total, 14 complete fractures, 31 incomplete fractures and 25 normal pars were detected by CT. Bone marrow edema was seen in seven of the complete and 25 of the incomplete fractures. The overall sensitivity, specificity and accuracy of MRI in detecting fractures (complete and incomplete) were 97.7, 92.3, and 95.7 %, respectively. MRI was 100 % accurate in detecting complete fractures. For incomplete fractures, the sensitivity, specificity, and accuracy of MRI were 96.7, 92.0, and 94.6 %, respectively. CONCLUSIONS: 3-T MRI with thin-slice 3D T1 VIBE is 100 % accurate in diagnosing complete pars fractures and has excellent diagnostic ability in the detection and characterization of incomplete pars stress fractures compared to CT. MRI has the added advantages of detecting bone marrow edema and does not employ ionizing radiation.

INTRODUCTION: MR and CT are excellent complimentary diagnostic modalities for evaluation of hip pain which are often used together along with radiographs. However, CT involves radiation, which is a concern particularly in younger patients. T1VIBE is a 3D gradient echo MR sequence with high intrinsic contrast between the bone and soft tissues with multiplanar capabilities. AIM: We performed a study to assess if TIVIBE can be used to complement MR for evaluation of hip pain in young adults and to see if measurements and angles can be calculated using T1VIBE inversion images with similar accuracy to CT scan. MATERIAL AND METHODS: A retrospective review of 50 patients aged less than 40 years, who had MR (including TIVIBE) and CT of pelvis was performed. Post surgical patients were excluded. Some important measurements such as Centre edge angle, Tonnis' angle, anterior acetabular sector angle, posterior acetabular sector angle and acetabular version were independently measured by two readers on T1 VIBE inversion and CT images separately and measurements were compared. One reader performed the measurements again to assess for intra-observer error. RESULTS: There was a female predominance (37 F, 13M) with an average age of 27.6 years (range of 17-39). There was no significant difference in the measurements between CT and TI VIBE inversion and there was good intra and interobservor reliability. CONCLUSION: TI VIBE inversion sequence can be used as an alternative to CT with added advantage of alleviating the radiation exposure.

OBJECTIVE: To differentiate prostate cancer lesions in transition zone by diffusion-weighted-MRI (DW-MRI). METHODS: Data from a total of 63 patients who underwent preoperative DWI (b of 0-1000 s/mm2) were prospectively collected and processed by a monoexponential (DWI) model and compared with a biexponential (IVIM) model for quantitation of apparent diffusion coefficients (ADCs), perfusion fraction f, diffusivity D and pseudo-diffusivity D*. Histogram analyses were performed by outlining entire-tumor regions of interest (ROIs). These parameters (separately and combined in a logistic regression model) were used to differentiate lesions depending on histopathological analysis of Magnetic Resonance/transrectal Ultrasound (MR/TRUS) fusion-guided biopsy. The diagnostic ability of differentiate the PCa from BHP in TZ was analyzed by ROC regression. Histogram analysis of quantitative parameters and Gleason score were assessed with Spearman correlation. RESULTS: Thirty (30 foci) cases of PCa in PZ and 33 (36 foci) cases of BPH were confirmed by pathology. Mean ADC, median ADC, 10th percentile ADC, 90th percentile ADC, kurtosis and skewness of ADC and mean D values, median D and 90th percentile D differed significantly between PCa and BHP in TZ. The highest classification accuracy was achieved by the mean ADC (0.841) and mean D (0.809). A logistic regression model based on mean ADC and mean D led to an AUC of 0.873, however, the difference is not significant. There were 7 Gleason 6 areas, 9 Gleason 7 areas, 8 Gleason 8 areas, 5 Gleason 9 areas and 2 Gleason 10 areas detected from the 31 prostate cancer areas, the mean Gleason value was(7.5 ± 1.2). The mean ADC and mean D had correlation with Gleason score(r = -0.522 and r = -0.407 respectively, P < 0.05). CONCLUSION: The diagnosis efficiency of IVIM parameters was not superior to ADC in the diagnosis of PCa in TZ. Moreover, the combination of mean ADC and mean D did not perform better than the parameters alone significantly; It is feasible to stratify the pathological grade of prostate cancer by mean ADC.
