BACKGROUND/OBJECTIVES: This study investigated adherence to physical activity (PA) guidelines and associated sociodemographic factors among older Koreans from 2011 to 2020. METHODS: Utilizing four public data sets from the National Survey of Older Koreans, the study included the data on 40,993 older adults 65 years and older in South Korea, collected between 2011 and 2020. Adherence to PA guidelines and sociodemographic factors were assessed through self-reported questionnaires. The data were analyzed using a two-way analysis of variance and post hoc tests. RESULTS: Overall adherence increased from 39.1% in 2011 to 48.2% in 2017, then decreased to 37.6% in 2020 (p < .001). Men had higher adherence than women (p < .001). Age-related adherence peaked in the young-older group (65-74 years old) and was lowest in the oldest-old group (85+ years old) (p < .001). Marital status, education, and income were also significantly related to PA adherence (p < .001) across the years. CONCLUSION: Although continuous increase in adherence to PA among Koreans 65 years and older was observed, the decline in PA levels during the COVID era underscored the need for targeted interventions and well-informed health care policies to address demographic challenges. Still, considering that data were collected during the recommended social distancing period, a cautions interpretation of these findings is warranted. Significance/Implications: Health policies aiming to improve adherence to PA guidelines should prioritize Korean older adults who are female, belong to the oldest-old group, are single, and have low education and income levels, with the goal of enhancing health equity.

As global concern over the negative impacts of global warming, primarily caused by using passenger vehicles (PVs), the transition to hydrogen fuel cell vehicles (HFCVs) is an essential alternative for reducing greenhouse (GHG) emissions. This research employs a bottom-up approach to analyze road vehicle fleet's GHG emissions. We calculated GHG emissions from PVs in 15 Group of Twenty (G20) countries based on four scenarios adopting the global HFCVs from 2024 to 2050. This paper introduces business-as-usual (BaU), moderate, aggressive, and non-HFCVs scenario. The results show that the aggressive scenario has the highest sales, estimated between 62,000 and 29.48 million vehicles by 2050, with global hydrogen market penetration rates 48.48%. Building on countries' respective national strategies, the findings highlight China and India as the leading markets for hydrogen demand, with Germany and Japan also showing significant interest. The aggressive scenario further demonstrates that transitioning from internal combustion engine vehicles (ICEVs) to battery electric vehicles (BEVs), plug-in hybrid electric vehicles (PHEVs), and HFCVs can significantly reduce annual GHG emissions. Ultimately, this study finds that the transition to HFCVs could reduce emissions by up to 67.09% by 2050.

Designation of protected areas has enjoyed global application as a means of biodiversity conservation for over 100 years. National conservation policy is essential as a means of protecting biodiversity, but is contingent on, amongst others, changing values and international drivers, and remains dynamic in many countries. As conservation policies evolve, the role of pre-existing protected areas within broader strategies for conservation can become unclear, with consequences both for the ability of the conservation policies to achieve their stated goals, and also for biodiversity outcomes within a nation. In order to map evolving inconsistencies between conservation policy and the role of protected areas within it, we develop a conceptual conservation policy framework synthesised from different policy orientations reported in the literature. Using South Africa as a case study, the conceptualisation is used to characterise the policy goals for protected areas in the recently adopted conservation policy, and the pre-existing protected areas system which remains on the statute books. The results indicate that the conceptual conservation policy framework can be used identify misalignment between policy and practice, and has enabled a mismatch to be identified between South Africa's pre-existing protected areas system and its contemporary conservation policy, which suggests that the management of protected areas is likely to significantly change towards greater access and monetisation at the expense of their intrinsic value.

The increasing availability of large-scale, complex data has made research into how human genomes determine physiology in health and disease, as well as its application to drug development and medicine, an attractive field for artificial intelligence (AI) approaches. Looking at recent developments, we explore how such approaches interconnect and may conflict with needs for and notions of causal knowledge in molecular genetics and genomic medicine. We provide reasons to suggest that-while capable of generating predictive knowledge at unprecedented pace and scale-if and how these approaches will be integrated with prevailing causal concepts will not only determine the future of scientific understanding and self-conceptions in these fields. But these questions will also be key to develop differentiated policies, such as for education and regulation, in order to harness societal benefits of AI for genomic research and medicine.

Approximately 1-in-5 children have a diagnosed mental, behavioral, and/or developmental disorder or delay by age 8 in the United States. Children with such conditions often require complex, complicated diagnostic and specialty care, making them susceptible to repeated referrals and ongoing unmet healthcare needs. Patient navigation programs (PNPs) are designed to integrate care from primary care providers to community-based services, using trained navigators to help patients and their families manage referrals and connect with referred services. This study examines factors associated with repeated referrals to an active PNP to inform ongoing referral patterns and adaptations to standard navigation support within a large healthcare system in South Carolina (SC). Data is sourced from the inception of the PNP in 2017 through 2022, including 15,702 referrals. Overall, 71.07% had no repeated referrals. Children who are older, diagnosed with attention deficit disorder(s), behavioral concerns, depression, multiple referral needs, and insured by Medicaid were found to be most susceptible to repeated referrals. Conversely, children who are non-Hispanic Black, were referred at a well-child visit, and are primarily insured by private insurance or Tricare were least likely to have repeated referrals. Children who are insured by Medicaid are more likely to be younger, identify as non-Hispanic Black, Hispanic, or another race/ethnicity, and have multiple needs at time of initial referral, identifying a potentially compounded risk for those who hold multiple risk factors to experiencing repeated referrals. Findings may inform adaptations to this PNP model to adjust navigator protocol for at-risk populations and equitably optimize referral-to-service connection.

Photovoltaic (PV) panels have a crucial role in coping with the global warming mitigation and the energetic crisis currently affecting the European Community. However, from the circular perspective of end-of-life (EoL) management, there are still big issues to be solved in order to recover materials from this kind of e-wastes. Because of several reasons (e.g. type of embedded materials, illegal shipments, location of manufacturers) EoL businesses do not have the interest in approaching them. This poses a significant environmental concern in terms of their management. This work wants to assess the profitability of a specific PV module recycling plant, by evaluating several market contexts in which multiple scenarios of material price, investment and process costs will be considered. The results for a 3000 tonnes plant show that profitability is not verified in the absence of an avoided landfill cost. Instead, when a value of 200 /tonnes is applied, the net present value is positive in 35.2% of the scenarios and at 87.6% when a value of 350 /tonnes is considered. The policy choice of this value requires linking the PV module disposal fee to the circular benefits associated with its recovery.

Effective policies to address poor food choices and dietary patterns need to consider the complex set of motives affecting eating behavior. This study examined how different eating motives are associated with anthropometry, body composition, and dietary intake. Our analysis is based on a cross-sectional sample with 429 healthy adults in three different age groups collected in Germany from 2016 to 2018. Dietary intake, Body Mass Index (BMI), waist circumference (WC), and fat-free mass (FFM) were measured by standardized methods. Eating motives were measured using The Eating Motivation Scale (TEMS). Regressing dietary intakes and anthropometric indicators on TEMS motives, we identify the main sources of variation in diet and nutritional status separately for men and women. Results indicated the Health motive to be positively associated with FFM (B±SE=1.72±0.44) and negatively with WC (B±SE=-3.23±0.81) for men. For women, the Need & Hunger motive was positively associated with FFM (B±SE=1.63±0.44) and negatively with WC (B±SE=-2.46±0.81). While Liking and Habits were the most frequently stated eating motives, we did not find them to be significantly related to the nutritional status. Other motives were associated with dietary intake but not anthropometry or body composition. The Price motive was positively and the Convenience motive was negatively associated with energy (B±SE=63.77±19.98;B±SE=-46.96±17.12) and carbohydrate intake (B±SE=7.15±2.65;B±SE=-5.98±2.27) for men. The results highlight the need for more differentiated analyses of eating motives, beyond comparing the relative importance of motives based on mean values, towards the association of motives with dietary intake and nutritional status.

Over the last few years, the number of Rohingya refugees in Bangladesh has increased exponentially. On arrival, they experience poor mental health and psychological well-being. This commentary explores the risk factors and contemporary challenges that deteriorate Rohingyas' psychological well-being in Bangladesh. The onslaught of Covid-19 compounds the pre-existing psychological health conditions of the Rohingyas living in cramped and flimsy camps. The recent relocation to Bhashan Char may likely trigger tensions and eventually exacerbates their existing psychological well-being. The relocation to Bhashan Char has presumably happened against their volition. Meanwhile, the Myanmar military's coup has added additional fear about their bleak future of a safe and dignified return from Bangladesh to Myanmar. All these have contributed to the worsening of their existing psychological well-being. In order to subside their psychological health challenges, this commentary suggests: (a) the immediate execution of 'National Deployment and Vaccination Plan for Covid-19 Vaccines and initiation of vaccine rollout among the refugees; and (b) involving Rohingyas in economic activities and making them a self-reliant and economically empowered community. We suggest that safe and dignified repatriation is the only solution to the challenges they have been going through in Bangladesh and to preserve their psychological well-being.

In order to discern the causes of the health and developmental problems of children, and thence develop preventative strategies, longitudinal cohort studies offer major advantages. They can monitor the consequences of exposure to physical and psychological events and thence identify antecedents of various disorders. We describe the historical background to the development in the UK of this study design, and the uptake of longitudinal birth cohorts in low- and middle-income countries (LMICs), using the cohorts in Jamaica, South Africa and Brazil as exemplars. We describe the benefits of such studies and show how undertaking longitudinal cohort studies can have major health and financial benefits to the populations concerned. Additionally, the paper outlines the advantages of collaboration between studies and the pooling of data.

In response to the rapid spread of coronavirus disease-2019 (COVID-19) within and across countries and the need to protect public health, governments worldwide introduced unprecedented measures such as restricted road and air travel and reduced human mobility in 2020. The curtailment of personal travel and economic activity provided a unique opportunity for researchers to assess the interplay between anthropogenic emissions of primary air pollutants, their physical transport, chemical transformation, ultimate fate and potential health impacts. In general, reductions in the atmospheric levels of outdoor air pollutants such as particulate matter (PM), nitrogen dioxide (NO2), carbon monoxide (CO), sulfur dioxide (SO2), and volatile organic compounds (VOCs) were observed in many countries during the lockdowns. However, the levels of ozone (O3), a secondary air pollutant linked to asthma and respiratory ailments, and secondary PM were frequently reported to remain unchanged or even increase. An increase in O3 can enhance the formation of secondary PM2.5, especially secondary organic aerosols, through the atmospheric oxidation of VOCs. Given that the gaseous precursors of O3 (VOCs and NOx) are also involved in the formation of secondary PM2.5, an integrated control strategy should focus on reducing the emission of the common precursors for the co-mitigation of PM2.5 and O3 with an emphasis on their complex photochemical interactions. Compared to outdoor air quality, comprehensive investigations of indoor air quality (IAQ) are relatively sparse. People spend more than 80% of their time indoors with exposure to air pollutants of both outdoor and indoor origins. Consequently, an integrated assessment of exposure to air pollutants in both outdoor and indoor microenvironments is needed for effective urban air quality management and for mitigation of health risk. To provide further insights into air quality, we do a critical review of scientific articles, published from January 2020 to December 2020 across the globe. Finally, we discuss policy implications of our review in the context of global air quality improvement.

Being a Re-Emerging Infectious Disease, dengue causes 390 million cases globally and is prevalent in many urban areas in South America. Understanding the fine-scale relationships between dengue incidence and environmental and socioeconomic factors can guide improved disease prevention strategies. This ecological study examines the association between dengue incidence and satellite-based vegetation greenness in 3826 census tracts nested in 474 neighborhoods in Belo Horizonte, Brazil, during the 2010 dengue epidemic. To reduce potential bias in the estimated dengue-greenness association, we adjusted for socioeconomic vulnerability, population density, building height and density, land cover composition, elevation, weather patterns, and neighborhood random effects. We found that vegetation greenness was negatively associated with dengue incidence in a univariate model, and this association attenuated after controlling for additional covariates. The dengue-greenness association was modified by socioeconomic vulnerability: while a positive association was observed in the least vulnerable census tracts, the association was negative in the most vulnerable areas. Using greenness as a proxy for vegetation quality, our results show the potential of vegetation management in reducing dengue incidence, particularly in socioeconomically vulnerable areas. We also discuss the role of water infrastructure, sanitation services, and tree cover in lowering dengue risk.

Being declared a global emergency, the COVID-19 pandemic has taken many lives, threatened livelihoods and businesses around the world. The energy industry, in particular, has experienced tremendous pressure resulting from the pandemic. In response to such a challenge, the development of sustainable resources and renewable energy infrastructure has demonstrated its potential as a promising and effective strategy. To sufficiently address the effect of COVID-19 on renewable energy development strategies, short-term policy priorities should be identified, while mid-term and long-term action plans should be formulated in achieving the well-defined renewable energy targets and progress towards a more sustainable energy future. In this review, opportunities, challenges, and significant impacts of the COVID-19 pandemic on current and future sustainable energy strategies were analyzed in detail; while drawing from experiences in identifying reasonable behaviors, orientating appropriate actions, and policy implications on the sustainable energy trajectory were also mentioned. Indeed, the question is that whether the COVID-19 pandemic will kill us or provide us with a precious lesson on future sustainable energy development.

The outbreak of the COVID-19 epidemic brings huge consequences on the global economy and health. Diverse restrictive policies have been enforced to prevent the novel Coronavirus from spreading. During the COVID-19 pandemic, the express logistics companies transport essential goods among different regions in China, supporting people's normal lives. This study explores the dynamic impacts of COVID-19 epidemic on the intra-provincial and inter-provincial express parcel flows based on the statistical and econometric analysis considering the individual-specific effects of each province. The key results obtained from the unique long-run panel data analysis are: (1) the temporal fluctuation of inter-provincial express logistics flows affected by the epidemic is stronger than that of intra-provincial flows, both of which also show significant spatial heterogeneity; (2) the process of China's fighting against the COVID-19 pandemic is divided into four stages according to the severity of pandemic and implications of restrictive policies, which have moderating effects for the impacts on the express logistics; (3) the dynamic effects of the pandemic on the express logistics are obviously heterogeneous in different stages, confirming the effectiveness of restrictive and support policies; (4) the delayed effects of COVID-19 epidemic on the regional express logistics vary with different setting of time lags. In the short term, the typical restrictive policies and Chinese Spring Festival holidays have significant negative effects on the express logistics. In the long term, the resumption of work and production stimulate the demands of express logistics, presenting significant positive effects in post-epidemic era. This study can provide the policy implications for the logistics planning and management under the public health intervention.

The rising number of COVID-19 cases and economic implications of lockdown measures indicate the tricky balancing act policy makers face as they implement the subsequent phases of 'unlock'. We develop a model to examine how lockdown and social distancing measures have influenced the behavioral conduct of people. The current situation highlights that policy makers need to focus on bringing awareness and social restraint among people rather than going for stringent lockdown measures. We believe this work will help the policy makers gain insights into the troubled COVID-19 times ahead, and based on the estimates, they can frame policies to navigate these wild waves in the best possible way.

BACKGROUND: General Practitioners (GPs) are the main providers of primary palliative care (PPC). At the same time they are the main initiators of specialised palliative homecare (SPHC). In Germany, little is known about factors which influence GPs in their involvement of SPHC. Aim of our study is to identify factors that drive GPs to give value to and involve SPHC. METHODS: A cross-sectional survey was performed. In 2018, questionnaires were mailed to 6000 randomly selected GPs from eight German federal states, focusing on the extent of GPs' palliative care activities and their involvement of SPHC. RESULTS: With a response rate of 19.4% and exclusion of GPs working in SPHC-teams, n = 1026 questionnaires were appropriate for analysis. GPs valued SPHC support as the most "important/very important" for both "technical/invasive treatment measures" (95%) and availability outside practice opening hours (92%). The most relevant factor influencing perceived SPHC-importance was GPs' self-reported extent of engagement in palliative care (ß = - 0.283; CI 95% = - 0.384;-0.182), followed by the perceived quality of utilised SPHC (ß = 0.119; CI 95% = 0.048;0.190), involvement in treatment of palliative patients after SPHC initiation (ß = 0.088; CI 95% = 0.042;0.134), and conviction that palliative care should be a central part of GPs' work (ß = - 0.062; CI 95% = - 0.116;-0.008). Perceived SPHC-importance is also associated with SPHC-referrals (ß =0.138; p < 0.001). The lower the engagement of GPs in palliative care, the more they involve SPHC and vice versa. CONCLUSIONS: GPs with low reported activity in palliative care are more likely to initialise SPHC for palliative care activities they do not deliver themselves for various reasons, which might mean that the involvement of SPHC is substitutive instead of complementary to primary palliative care. This finding and its interpretation should be given more attention in the future policy framework for (specialised) palliative homecare. BACKGROUND: Ghana is challenged with shortage of critical human resources for health particularly nurses and midwives in rural hard-to-reach communities. This shortage potentially hinders efforts towards attaining universal access to basic healthcare. More importantly, poor quality of pre-service training for health trainees has the potential to worsen this predicament. There is therefore the need to leverage emerging digital innovations like e-learning to complement existing efforts. This study was conducted several months before the outbreak of COVID-19 to investigate the preparedness, acceptability and feasibility e-learning innovation for nursing and midwifery trainees. METHODS: The study is a cross-sectional descriptive survey involving nursing and midwifery students (n = 233) in one of Ghana's public universities, located in the Volta region of Ghana. Simple random sampling technique was used to collect responses from eligible respondents using a structured questionnaire. Descriptive statistical analysis was done using STATA software (version 12.0). RESULTS: It was found that nearly 100% of respondents owned smartphones that were used mostly for learning purposes including sharing of academic information. Over 70% of respondents particularly used social media, social networking applications and internet searches for learning purposes. Health trainees were however constrained by low bandwidth and lack of seamless internet connectivity within their learning environments to maximize the full benefits of these e-learning opportunities. CONCLUSION: Respondents were predominantly prepared for an e-learning pilot project. These feability findings suggest e-learning is a huge potential that can be used to augment existing approaches for pre-service training of health trainees in Ghana, when implementation threats are sufficiently addressed. Compelling findings of this study are therefore timely to inform evidence-based policy decisions on innovative digitial solutions for pre-service training of health workforce even as the world adapts to the "new normal" situation induced by COVID-19.

Background and Objective: The concept of a "disruptive innovation," recently extended to health care, refers to an emerging technology that represents a new market force combined with a new value system, that eventually displaces some, or all, of the current leading "stakeholders, products and strategic alliances." Naturopathy is a distinct system of traditional and complementary medicine recognized by the World Health Organization (WHO), emerging as a model of primary care. The objective here is to describe Naturopathy in the context of the criteria for a disruptive innovation. Methods: An evidence synthesis was conducted to evaluate Naturopathy as a potentially disruptive technology according to the defining criteria established by leading economists and health technology experts: (1) The innovation must cure disease; (2) must transform the way medicine is practiced; or (3) have an impact that could be disruptive or sustaining, depending on how it is integrated into the current healthcare marketplace. Results: The fact that Naturopathy de-emphasizes prescription drug and surgical interventions in favor of nonpharmacological health promotion and self-care could disrupt the present economic model that fuels health care costs. The patient-centered orientation of Naturopathy, combined with an emphasis on preventive behaviors and popular complementary and integrative health services like natural products, mind and body therapies, and other therapies not widely represented in current primary care models increase the likelihood for disruption. Conclusions: Because of its patient-centered approach and emphasis on prevention, naturopathy may disrupt or remain a durable presence in healthcare delivery depending on policymaker decisions.

The Huang-Huai-Hai Plain (HHH) is typical of China's farming area, and was predicted as one of the fastest growing areas of urbanization in the world. Since the turn of the new millennium, construction land and farmland transitions in this region driven by rapid urbanization have resulted in dramatic loss of farmland, which triggered a serious threat to regional even national food security. In this paper, the coupling relationships between per capita construction land transition (PCCT) and per capita farmland transition (PCFT) in the HHH and their implications for regional food security are analyzed. During 2000-2015, the farmland decreased by 8.59%, 72.25% of which were occupied by construction land. There are two major coupling types between PCCT and PCFT, one is the double increasing of per capita construction land area (PCCA) and per capita farmland area (PCFA); another is the increasing of PCCA and the decreasing of PCFA. The fluctuant increasing of PCCT and decreasing of PCFT coexisted and presented symmetrical coupling characteristics in space. Physical, location, transportation and socio-economic factors play significantly different roles in driving PCCT and PCFT. The implications for ensuring food security involve promoting the reclamation and redevelopment of inefficient and unused urban-rural construction land, reducing inefficient occupation of farmland resources, developing appropriate scale management of agriculture, and establishing a better social security system to smoothly settle down the floating rural population in the city.

This study aims to provide a better understanding of the impact of New Zealand's low-cost carrier (LCC) on domestic tourism demand and growth. The panel data regression model and the two-stage least-square (2SLS) model (aims to control for the endogeneity effects) are used to empirically investigate the impact of LCC and the key determinants affecting New Zealand's domestic tourism using five regions (Auckland, Canterbury/Christchurch, Dunedin, Queenstown, and Wellington) from June 2009 to July 2015. The findings suggested that the LCC's services, GDP per capita, the regional tourism indicators (accommodation, and food and beverage), and land transport costs affected New Zealand's domestic tourism. The policy implications of the key finding regarding the significance of the LCC's operations on New Zealand's domestic tourism (local/regional tourism authorities and tourism operators), airline competition between incumbent airline (Air New Zealand) and the LCC (Jetstar), and airport authorities are discussed.

AIM: The aim of this study is to obtain insights pertaining to disaster management among Indian general dental practitioners (GDPs). MATERIALS AND METHODS: All GDPs in Mangaluru city, Karnataka, India, were included in the present study. Their willingness to participate in disaster management and their objective knowledge, attitude, behavior, and perceived effectiveness related to disaster management were assessed by a structured, pretested, self-administered questionnaire. Demographic information was also collected. RESULTS: Overall, 101 GDPs volunteered for the study, and 96.04% of respondents were willing to participate in disaster management. Mean knowledge, attitude, behavior, and perceived effectiveness scores were 52.65, 79.60, 41.55, and 64.20% respectively. Religion (odds ratio [OR] = -0.194, p = 0.022), marital status (OR = -0.222, p = 0.040), attachment to college (OR = -0.256, p = 0.037), familiarity with standard operating procedures (SOP; OR = -0.502, p = 0.000), and knowledge (OR = 0.265, p = 0.003) were significant predictors of behavior. CONCLUSION: The GDPs reported knowledge and behavior scores which were low, while their attitude and willingness to participate were high. Demographic determinants might be critical indicators in disaster management scenario among GDPs. CLINICAL SIGNIFICANCE: The present study has crucial implications for policymakers and curriculum changes to integrate dentists effectively into disaster response teams. As responsible members of the society, the dental fraternity has critical contributions to make toward disaster mitigation. Integration of GDPs in a multidisciplinary team managing disasters might be crucial, especially in highly disaster-prone areas, such as India, with a definite paucity of resources.
