RESUMO

The purpose of this study is to clarify whether swallowing function can be inferred from grip strength. Based on the diagnostic criteria of sarcopenia, patients were divided into two groups according to grip strength, and it was analyzed whether there was a difference in the evaluation index for swallowing function between the two groups. Among the cases requesting evaluation of swallowing function from June 10, 2020 to October 28, 2020, 83 cases (mean age: 71.7 years, 59 males and 24 females) who received assessment tests and swallowing endoscopy were included. According to the diagnostic criteria for grip strength in the Asian working group in Sarcopenia, less than 28 kgf and 18 kgf were defined as the weak group for men and women, respectively. Hyodo scores, repeated salivary swallowing tests (RSST), maximum vocalization time (MPT), and dysphagia severity classification (DSS) were compared between the two groups. Of the 83 patients, 29 and 54 were in the normal group and weak group, respectively. In all indicators, the normal group showed significantly better results than the weak group: Hyodo score (2.4 vs. 4.0, p < 0.01), RSST (4.1 times vs. 2.4 times, p < 0.01), MPT (12.1 s vs. 5.9 s, p < 0.001), DSS (4.5 vs. 5.9, p < 0.001), respectively. In multiple regression analysis with DSS as the dependent variable, grip strength was a significant independent variable of DSS even after adjusting for age, gender, and body mass index. Grip strength assessment based on sarcopenia criteria can be a useful tool for estimating swallowing function.

PURPOSE: Post-stroke dysphagia (PSD) is the most common type of dysphagia. Stroke patients with sustained dysphagia have poorer outcomes. The severity of PSD is assessed using miscellaneous scales with unknown consistencies. We aim to investigate the consistencies among miscellaneous scales, which could aid in the assessment of PSD. METHODS: A total of 49 PSD patients were enrolled. Functional Oral Intake Scale (FOIS), Dysphagia Severity Scale (DSS), Ohkuma Questionnaire, Eating Assessment Tool-10, and Repetitive Saliva Swallowing Test were performed. FOIS was performed by physicians, and DSS was conducted by both the physicians and nurses; the physicians used either videofluoroscopy (VF) or videoendoscopy (VE) for evaluation; while, the nurses assessed PSD by observation and subjective judgment. RESULTS: When using VF (VF-DSS and VF-FOIS) as the gold standard for the evaluation, VE-FOIS (κ = 0.625, 95% CI 0.300-0.950, p < 0.001) has a substantial agreement with VF-FOIS, and VE-DSS (κ = 0.381, 95% CI 0.127-0.636, p = 0.007) has a fair agreement with VF-DSS. The weighted kappa of FOIS to DSS in VE (weighted κ = 0.577, 95% CI 0.414-0.740, p < 0.001) is not lower than that in VF (weighted kappa = 0.249, 95% CI 0.136-0.362, p < 0.001). CONCLUSION: For both DSS and FOIS, only VE has a statistically significant agreement with VF. Though VF has been viewed as the traditional gold standard of dysphagia screening, it has the limitations of being invasive and equipment dependent. For PSD, VE could be considered as a substitution when VF is not available or suitable.

This study focused on the Kuchikara Taberu Balance Chart (KTBC) as a tool for swallowing function evaluation. To clarify the relationship between videoendoscopic (VE) examination of swallowing function and the KTBC, we compared median KTBC scores with and without laryngeal penetration identified by VE. Sixty-five patients with a mean age of 84.3 ± 7.9 years were examined at the Towada City Hospital. The patients were classified into groups based on laryngeal penetration, including 28 patients with and 37 patients without penetration. We found no significant differences in patient backgrounds. The median KTBC score (interquartile range) was 36.5 (31-44.5) in the group with laryngeal penetration and 42 (35-48.5) in the group without penetration, but the scores were not significantly different (level of statistical significance at α = 0.0036 determined by the Bonferroni correction method) when compared with the Mann-Whitney U test (36.5 vs. 42, z = -2.33, p = 0.020). The median respiratory condition (3 vs. 4, z = - 3.23; p < 0.0036), oral preparatory and propulsive phases (3 vs. 4, z = - 2.96; p < 0.0036), and position and endurance (1 vs. 3, z = - 3.25; p < 0.0036) scores were significantly lower in the group with laryngeal penetration. This study revealed a correlation between laryngeal penetration confirmed by VE and KTBC scores. Consequently, respiratory condition, oral preparatory and propulsive phases, and position and endurance may be useful as tools for the assessment of swallowing. In particular, we recommend adding respiratory status to dysphagia screening.

OBJECTIVE: The loud and severely distorting form of audible nasal emission (commonly known as nasal turbulence or nasal rustle) typically occurs with a small velopharyngeal opening during production of pressure-sensitive consonants. The purpose of this study was to determine whether bubbling of the secretions, which commonly occurs on the superior aspect of the velopharyngeal port when there is a small opening, is a periodic process that can generate sound in the nasal cavity. PARTICIPANTS: Ten pediatric patients were included in the study. All participants had normal articulation and resonance but exhibited audible nasal emission characterized as nasal rustle. MEASURES: For each participant, high-speed video (HSV) nasopharyngoscopy and acoustic signals were recorded simultaneously. The acoustic recordings were captured in a manner similar to nasometry using nasal and oral microphones connected to a separation plate. Spectral analysis of the audio recordings and the HSV images was used to determine correlation between the acoustic and visual measurements. RESULTS: This study showed that secretion bubbling is a periodic process and its frequency, measured from the HSV data, was also captured by the acoustic measurements. The nasal acoustic signal correlated more strongly with the video of bubbling than the oral acoustic signal in the majority of the cases where bubbling occurred. CONCLUSION: These findings are strong evidence that secretion bubbling plays a significant role in the mechanism that generates undesired sound in the nasal cavity. Further work is needed to determine whether this sound is perceived as nasal rustle.

RESUMO

The patulous Eustachian tube syndrome was described more than 150 years ago, but up to the present time some of its aspects continue to be a challenge for both fundamental and clinical medicine. Certain issues of etiology and pathogenesis of this condition have until recently remained unclear which probably accounts for the failure of choosing the adequate methods for its medical correction. Despite the seemingly broad spectrum of the modern tools for the purpose including first and foremost the endoscopic and radiological techniques, verification of the patulous Eustachian tube may encounter difficulties for the specialists. In connection with this, we have undertaken to summarize the views and opinions as regards the syndrome of interest available in the relevant world literature. In the first part of this communication, the authors discuss the modern concepts of etiology and pathogenesis of the patulous Eustachian tube syndrome with special reference to its clinical manifestations and methods of diagnostics.

Aspiration pneumonia is a cause of mortality in the elderly. Evaluating swallowing function is important. Videofluoroscopy and flexible endoscopic evaluation of swallowing are comparable; however, observing all swallowing dynamics is impossible using the latter approach. We examined the significance of flexible endoscopic evaluation of swallowing using videofluoroscopy. Thirty-seven patients with dysphagia [70.0 ± 8.9 (range 49-84) years] were included. In random order, patients underwent videofluoroscopy with 10 cc contrast material, once without, and once with an endoscope inserted. Laryngeal elevation delay time, Penetration-Aspiration Scale score, and Pharyngeal Residue Severity Rating Scale score were evaluated. Laryngeal elevation delay time without or with endoscope insertion was similar (0.35 ± 0.16 s vs. 0.36 ± 0.16 s, P = 0.29). The Penetration-Aspiration Scale (3.59 ± 2.71 vs. 4.41 ± 2.85; P < 0.001) and Pharyngeal Residue Severity Rating Scale (0.97 ± 0.93 vs. 1.46 ± 1.10; P < 0.001) scores differed significantly. The cases that showed no aspiration without endoscope insertion showed greater aspiration with endoscope insertion, and the cases that did not show aspiration with an endoscope inserted also showed no aspiration without an endoscope. Flexible endoscopic insertion resulted in more severe aspiration and residue than non-insertion, as assessed using videofluoroscopy.

Mochi is highly cohesive and adhesive, and easy to choke on. Many of the fatal suffocation accidents with mochi occur in the elderly aged 65 years or older. These circumstances prompted us to investigate a special property of waxy wheat which is similar in texture to waxy rice, but is less cohesive and adhesive. We compared the differences in chewing and swallowing movements associated with eating waxy rice mochi and waxy wheat mochi between healthy adults and healthy elderly. Healthy elderly chewed mochi more and longer than healthy adults. Although there was no difference in the number of chewing cycles or total duration of chewing between the two types of mochi, waxy wheat mochi was easier to chew and left less pharyngeal residue. These findings lead us to suggest that waxy wheat mochi is promising as a food that is easy to swallow and difficult to choke on.

OBJECTIVE: Adductor laryngeal dystonia (AdLD) disrupts fine motor movements of vocal folds during speech, resulting in a strained, broken, and strangled voice. Laryngeal high-speed videoendoscopy (HSV) in connected speech enables the direct visualization of detailed laryngeal dynamics, hence, it can be effectively used to study AdLD. The current study utilizes HSV to investigate supraglottic laryngeal tissue maneuvers obstructing the view of the vocal folds, in AdLD and normophonic speakers during connected speech. Characterizing the laryngeal maneuvers in these groups can facilitate a deeper understanding of the normophonic voice physiology and AdLD voice pathophysiology. METHODS: HSV data were obtained from six normophonic speakers and six patients with AdLD during production of connected speech. Three experienced raters visually analyzed the data to determine laryngeal tissues leading to obstructions of vocal folds in HSV images. The raters recorded the duration of each obstruction and indicated the specific tissue(s) leading to the obstruction. After the completion of their individual visual analysis, the raters came to consensus about their observations and measurements. RESULTS: Statistical analysis indicated that AdLD patients exhibited higher occurrences of vocal fold obstructions and longer durations of obstructions compared with the normophonic group. Similar obstruction types were found in both groups, with the epiglottis being the primary site of obstruction for both. Participants with AdLD displayed significantly elevated occurrences of sphincteric compression resulting in vocal fold obstruction. CONCLUSION: HSV can be used to study the movements of laryngeal tissues in detail during connected speech. The analysis of supraglottic laryngeal tissue dynamics in speech can help us characterize the AdLD pathophysiology. The study's findings regarding the tissues implicated in obstructions may potentially inform the development of patient-specific therapeutic strategies targeting individual control over specific laryngeal muscles during phonation and speech production.

OBJECTIVES: This study aimed to estimate vocal loading in loud phonation of a vowel and two widely used semiocclusion voice exercises (SOVTEs). Impact stress (IS) was estimated from glottal closing speed, inertial forces from the second derivative of glottal opening and closing. STUDY DESIGN: Experimental study in vivo. METHODS: A vocally healthy male sustained the [o:] vowel with habitual loudness and loudly: (1) without a tube, (2) into a silicone "Lax Vox" type tube (35 cm in length, 10 mm in diameter) outer end submerged 10 cm in water, and (3) into a straw (length 12.6 cm, diameter 2.5 mm) the outer end in air. He tried to use equal effort in all loud samples. High-speed video-laryngo-endoscopy was performed with a rigid scope. Oral air pressure (Poral) was registered in a mouthpiece through which the endoscope was inserted into the larynx and to which the tubes were attached air-tightly. RESULTS: Compared with vowel phonation at habitual loudness, mean of maximal glottal width (max GW) increased by 44.1% for loud tube phonation and decreased by 1.8% for loud straw phonation, and mean absolute value of minimum GW time derivative dmin increased by 57.1% for tube and by 29.5% for straw suggesting faster glottal closing. Compared with loud vowel phonation, max GW increased by 22.6% for loud tube phonation, while it decreased by 16.6% for loud straw phonation. For the tube, dmindecreased by 7.6% and for the straw by 23.8%. Maximal acceleration (ACC) and deceleration (DC) values were larger for the tube and smaller for the straw than the values for both vowel phonations. CONCLUSIONS: IS, deduced from dmin, increased in loud SOVTEs compared to vowel phonation at a conversational loudness, but remained lower in loud SOVTEs than in loud vowel phonation, particularly with a narrow straw, which also reduced inertial forces, as suggested by the reduced ACC and DC.

OBJECTIVES: This study investigates the vocal fold vibratory dynamics of singers, which are postulated to differ from those of normal speakers due to the singers' regular vocal training. The measurement of vocal fold vibration involved the utilization of laryngeal high-speed videoendoscopy (LHSV) and subsequent LHSV-based analysis. The focus of the present study is to characterize and compare the LHSV-based measures derived from the glottal area waveform (GAW), namely fundamental frequency (F0GAW), glottal perturbation (jitterGAW and shimmerGAW), open quotient (OQGAW), and Nyquist plots, between singers and normal speakers across genders. METHODS: Participants comprised 13 singers from a local cultural and heritage academy and 56 normal speakers from a local university, all were evaluated to have normal voices. Each participant underwent LHSV procedures to capture images of vocal fold vibration, which were subsequently analyzed to generate the LHSV-based measures. RESULTS: Male singers exhibited lower F0GAW, jitterGAW, shimmerGAW, and OQGAW than female singers. When compared to normal speakers, male singers demonstrated higher F0GAW, and lower jitterGAW and shimmerGAW. No difference in OQGAW was found between male singers and normal speakers. Female singers exhibited lower jitterGAW compared to normal speakers, but no differences were observed in shimmerGAW and OQGAW. The results of Nyquist plots indicated no gender-related associations between types of rim width and among singers. However, for rim pattern, male singers were associated with a higher percentage of clustered rim, suggesting more regular vocal fold vibration, compared to female singers and normal male speakers. CONCLUSIONS: Singers, particularly male singers, demonstrate distinct and potentially superior vocal fold vibrations compared to normal speakers, likely attributed to their regular vocal training, resulting in refined vocal fold configurations even during speaking. Despite the limited sample of singers, the study offers valuable insights into the vocal fold vibratory behaviors in singers analyzed using LHSV.

PURPOSE: The study aimed to assess the relevance of objective vibratory parameters derived from high-speed videolaryngoscopy (HSV) as a supporting tool, to assist clinicians in establishing the initial diagnosis of benign and malignant glottal organic lesions. METHODS: The HSV examinations were conducted in 175 subjects: 50 normophonic, 85 subjects with benign vocal fold lesions, and 40 with early glottic cancer; organic lesions were confirmed by histopathologic examination. The parameters, derived from HSV kymography: amplitude, symmetry, and glottal dynamic characteristics, were compared statistically between the groups with the following ROC analysis. RESULTS: Among 14 calculated parameters, 10 differed significantly between the groups. Four of them, the average resultant amplitude of the involved vocal fold (AmpInvolvedAvg), average amplitude asymmetry for the whole glottis and its middle third part (AmplAsymAvg; AmplAsymAvg_2/3), and absolute average phase difference (AbsPhaseDiffAvg), showed significant differences between benign and malignant lesions. Amplitude values were decreasing, while asymmetry and phase difference values were increasing with the risk of malignancy. In ROC analysis, the highest AUC was observed for AmpAsymAvg (0.719; p < 0.0001), and next in order was AmpInvolvedAvg (0.70; p = 0.0002). CONCLUSION: The golden standard in the diagnosis of organic lesions of glottis remains clinical examination with videolaryngoscopy, confirmed by histopathological examination. Our results showed that measurements of amplitude, asymmetry, and phase of vibrations in malignant vocal fold masses deteriorate significantly in comparison to benign vocal lesions. High-speed videolaryngoscopy could aid their preliminary differentiation noninvasively before histopathological examination; however, further research on larger groups is needed.

OBJECTIVE: To examine changes in lateral and vertical vibratory motion along the anterior, middle, and posterior sections of the vocal folds, as a function of vocal frequency variations. METHODS: Absolute measurements of vocal fold surface dynamics from high-speed videoendoscopy with custom laser endoscope were made on 23 vocally healthy adults during sustained /i:/ production at 10%, 20%, and 80% of pitch range. The 3D parameters of amplitude (mm), maximum velocity opening/closing (mm/s), and mean velocity opening/closing (mm/s) were computed for the lateral and vertical vibratory motion along the anterior, middle, and posterior sections of the vocal folds. Linear mixed model analysis was conducted to evaluate the differences in (a) vocal frequency levels (high vs. normal vs. low pitch), (b) axis level (vertical vs. lateral), (c) position level (anterior vs. middle vs. posterior), and (d) gender differences (male vs. female). RESULTS: Overall, the superior surface vertical motion of the vocal fold is greater compared with the lateral motion, especially in males. Along the superior surface, the mean and maximum closing velocities are greater posteriorly for low pitch. The location (anterior, middle, and posterior) along the superior surface is relevant only for vocal fold closing rather than opening, as the dynamics are different along the various locations. CONCLUSIONS: The study highlights the significance of assessing the vertical motion of the superior surface of the vocal fold to understand the complex dynamics of voice production. Objective: Videoendoscopic evaluation of swallowing is an objective swallowing function evaluation method used in dysphagia rehabilitation. However, it is anatomically difficult to detect the entry of foreign substances through the posterior tracheal wall using a conventional endoscope (CE). In this study, we developed an endoscope that can observe the posterior tracheal wall and investigated its reliability and validity in healthy adults. Methods: Twenty healthy adults were included. The trachea was observed from inside the larynx using a CE and a portable, flexible two-step angulation endoscope (two-AE) with a two-step curved shaft tip. The visibility of the anterior and posterior walls was recorded. The time from the endoscope tip entering the larynx to the posterior tracheal wall was measured. Additionally, discomfort events were assessed after the examination. McNemar's test and a paired t-test were used for statistical analysis. Kappa coefficients and concordance rates were calculated. Results: The anterior tracheal wall was observed using both endoscopes. The posterior tracheal wall was significantly observed in 18 participants with the two-AE (p < .001), compared to only three of 20 participants with the CE. The time to observation of the posterior tracheal wall for examiners 1 and 2 was 13.3 ± 6.5 and 12.0 ± 6.7 s, respectively, with no difference between groups (p = .400). The kappa coefficients of examiners 1 and 2 and between the examiners were 0.444, 0.643, and 0.643, respectively, with concordance rates of 90%, 95%, and 95%, respectively. Conclusion: Regardless of the examiner's years of experience, we observed that the two-AE could observe the posterior tracheal wall. Level of Evidence: Step 5.

OBJECTIVE: This study aimeed to examine the characteristics of formed and unformed trillo, an essential ornament found in 17th-century Italian vocal music, using simultaneous multimodality voice measurements. PARTICIPANT AND METHODS: A 28-year-old female with 12 years of classical voice training and 7 years of advanced training in historical performance produced formed trillo, unformed trillo, oscillating trill, vibrato, and straight tone on the vowel /i/. Simultaneous high-speed videoendoscopy, inductive plethysmography, and acoustic recordings were conducted to examine the laryngeal motion, respiratory kinematics, and output sound characteristics. RESULTS: The study findings reveal that trillo in this single participant is not only produced by the periodic adduction/abduction of the vocal fold but also with underlying differences in oscillatory mechanisms and increased glottal flow (use of percent vital capacity) controlled by increased activation of abdominal muscles and/or decreased activation (inspiratory braking) of the diaphragm relative to tidal breathing when compared with straight tone, vibrato, and oscillating trill. The formed trillo differs from the unformed trillo in the oscillatory mechanisms and glottal airflow utilization. CONCLUSIONS: The physiological mechanism responsible for trillo is more complex than simply adduction and abduction. Future studies with a greater number of participants are needed to evaluate the mechanisms responsible for the formation of and the auditory-perceptual differences between the formed versus unformed trillo.

The aim of the study was to utilize a quantitative assessment of the vibratory characteristics of vocal folds in diagnosing benign and malignant lesions of the glottis using high-speed videolaryngoscopy (HSV). METHODS: Case-control study including 100 patients with unilateral vocal fold lesions in comparison to 38 normophonic subjects. Quantitative assessment with the determination of vocal fold oscillation parameters was performed based on HSV kymography. Machine-learning predictive models were developed and validated. RESULTS: All calculated parameters differed significantly between healthy subjects and patients with organic lesions. The first predictive model distinguishing any organic lesion patients from healthy subjects reached an area under the curve (AUC) equal to 0.983 and presented with 89.3% accuracy, 97.0% sensitivity, and 71.4% specificity on the testing set. The second model identifying malignancy among organic lesions reached an AUC equal to 0.85 and presented with 80.6% accuracy, 100% sensitivity, and 71.1% specificity on the training set. Important predictive factors for the models were frequency perturbation measures. CONCLUSIONS: The standard protocol for distinguishing between benign and malignant lesions continues to be clinical evaluation by an experienced ENT specialist and confirmed by histopathological examination. Our findings did suggest that advanced machine learning models, which consider the complex interactions present in HSV data, could potentially indicate a heightened risk of malignancy. Therefore, this technology could prove pivotal in aiding in early cancer detection, thereby emphasizing the need for further investigation and validation.

OBJECTIVE: The first goal of this study was to investigate the coverage of laryngeal structures using two potential administration techniques for synthetic mucus: inhalation and lozenge ingestion. As a second research question, the study investigated the potential effects of these techniques on standardized voice assessment parameters. METHODS: Fluorescein was added to throat lozenges and to an inhalation solution to visualize the coverage of laryngeal structures through blue light imaging. The study included 70 vocally healthy subjects. Fifty subjects underwent administration via lozenge ingestion and 20 subjects performed the inhalation process. For the first research question, the recordings from the blue light imaging system were categorized to compare the extent of coverage on individual laryngeal structures objectively. Secondly, a standardized voice evaluation protocol was performed before and after each administration to determine any measurable effects of typical voice parameters. RESULTS: The administration via inhalation demonstrated complete coverage of all laryngeal structures, including the vocal folds, ventricular folds, and arytenoid cartilages, as visualized by the fluorescent dye. In contrast, the application of the lozenge predominantly covered the pharynx and laryngeal surface toward the aryepiglottic fold, but not the inferior structures. All in all, the comparison before and after administration showed no clear effect, although a minor deterioration of the acoustic signal was noted in the shimmer and cepstral peak prominence after the inhalation. CONCLUSIONS: Our findings indicate that the inhalation process is a more effective technique for covering deeper laryngeal structures such as the vocal folds and ventricular folds with synthetic mucus. This knowledge enables further in vivo studies on the role of laryngeal mucus in phonation in general, and how it can be substituted or supplemented for patients with reduced glandular activity as well as for heavy voice users.

OBJECTIVE: Diagnosis of adductor laryngeal dystonia (AdLD) is challenging as it mimics voice features of other voice disorders. This could lead to misdiagnosis (or delayed diagnosis) and ineffective treatments of AdLD. This paper develops automated measurements of glottal attack time (GAT) and glottal offset time (GOT) from high-speed videoendoscopy (HSV) in connected speech as objective measures that can potentially facilitate the diagnosis of this disorder in the future. METHODS: HSV data were recorded from vocally normal adults and patients with AdLD during the reading of the "Rainbow Passage" and six CAPE-V (Consensus Auditory-Perceptual Evaluation of Voice) sentences. A deep learning framework was designed and trained to segment the glottal area and detect the vocal fold edges in the HSV dataset. This automated framework allowed us to automatically measure and quantify the GATs and GOTs for the participants. Accordingly, a comparison was held between the obtained measurements among vocally normal speakers and those with AdLD. RESULTS: The automated framework was successfully developed and able to accurately segment the glottal area/edges. The precise automated measurements of GAT and GOT revealed minor, nonsignificant differences compared to the results of manual analysis-showing a strong correlation between the measures by the automated and manual methods. The results showed significant differences in the GAT values between the vocally normal subjects and AdLD patients, with larger variability in both the GAT and GOT measures in the AdLD group. CONCLUSIONS: The developed automated approach for GAT and GOT measurement can be valuable in clinical practice. These quantitative measurements can be used as meaningful biomarkers of the impaired vocal function in AdLD and help its differential diagnosis in the future.

OBJECTIVE: This study proposes a new computational framework for automated spatial segmentation of the vocal fold edges in high-speed videoendoscopy (HSV) data during connected speech. This spatio-temporal analytic representation of the vocal folds enables the HSV-based measurement of the glottal area waveform and other vibratory characteristics in the context of running speech. METHODS: HSV data were obtained from a vocally normal adult during production of the "Rainbow Passage." An algorithm based on an active contour modeling approach was developed for the analysis of HSV data. The algorithm was applied on a series of HSV kymograms at different intersections of the vocal folds to detect the edges of the vibrating vocal folds across the frames. This edge detection method follows a set of deformation rules for the active contours to capture the edges of the vocal folds through an energy optimization procedure. The detected edges in the kymograms were then registered back to the HSV frames. Subsequently, the glottal area waveform was calculated based on the area of the glottis enclosed by the vocal fold edges in each frame. RESULTS: The developed algorithm successfully captured the edges of the vocal folds in the HSV kymograms. This method led to an automated measurement of the glottal area waveform from the HSV frames during vocalizations in connected speech. CONCLUSION: The proposed algorithm serves as an automated method for spatial segmentation of the vocal folds in HSV data in connected speech. This study is one of the initial steps toward developing HSV-based measures to study vocal fold vibratory characteristics and voice production mechanisms in norm and disorder in the context of connected speech.

Adductor spasmodic dysphonia (AdSD) disrupts laryngeal muscle control during speech and, therefore, affects the onset and offset of phonation. In this study, the goal is to use laryngeal high-speed videoendoscopy (HSV) to measure the glottal attack time (GAT) and glottal offset time (GOT) during connected speech for normophonic (vocally normal) and AdSD voices. A monochrome HSV system was used to record readings of six CAPE-V sentences and part of the "Rainbow Passage" from the participants. Three raters visually analyzed the HSV data using a playback software to measure the GAT and GOT. The results show that the GAT was greater in the AdSD group than in the normophonic group; however, the clinical significance of the amount of this difference needs to be studied further. More variability was observed in both GATs and GOTs of the disorder group. Additionally, the GAT and GOT time series were found to be nonstationary for the AdSD group while they were stationary for the normophonic voices. This study shows that the GAT and GOT measures can be potentially used as objective markers to characterize AdSD. The findings will potentially help in the development of standardized measures for voice evaluation and the accurate diagnosis of AdSD.

OBJECTIVES: The Nyquist plot provides a graphical representation of the glottal cycles as elliptical trajectories in a 2D plane. This study proposes a methodology to parameterize the Nyquist plot with application to support the quantitative analysis of voice disorders. METHODS: We considered high-speed videoendoscopy recordings of 33 functional dysphonia (FD) patients and 33 normophonic controls (NC). Quantitative analysis was performed by computing four shape-based parameters from the Nyquist plot: Variability, Size (Perimeter and Area), and Consistency. Additionally, we performed automatic classification using a linear support vector machine and feature importance analysis by combining the proposed features with state-of-the-art glottal area waveform (GAW) parameters. RESULTS: We found that the inter-cycle variability was significantly higher in FD patients compared to NC. We achieved a classification accuracy of 83% when the top 30 most important features were used. Furthermore, the proposed Nyquist plot features were ranked in the top 12 most important features. CONCLUSIONS: The Nyquist plot provides complementary information for subjective and objective assessment of voice disorders. On the one hand, with visual inspection it is possible to observe intra- and inter-glottal cycle irregularities during sustained phonation. On the other hand, shaped-based parameters allow quantifying such irregularities and provide complementary information to state-of-the-art GAW parameters.
