PURPOSE: To compare 6 methods for intraoperative pupil dilatation in eyes with insufficient pupil size during phacoemulsification. METHODS: This was a prospective case-control study. 99 microcoria cataract patients (120 eyes) were collected and were divided into 6 groups(20 eyes each group), and their pupils were dilated by bimanual stretching pupil (group I), pupil radial cut open(group II), mechanical pupil dilatation with iris-retractor hooks (group III), OASIS iris expander (group IV), and Malyguin-ring (Microsurgical company, America) (group V), B-HEX Pupil Expander (Med Invent Devics, India)(group VI),respectively. 3.0 mm clear corneal incision were used in phacoemulsification. All cases were followed up at 1 week and 1, 3, 6 months after the surgery. The best corrected visual acuity (BCVA), intraocular pressure(IOP), corneal endothelium cell density(ECD), pupil diameter(PD) of before and after surgery were compared. RESULTS: One same doctor finished all cataract surgeries successfully. The eyes' condition before surgery and at 6 months after surgery were compared. There were no significant statistical differences for the conditions of the eyes before surgery among six groups. The ECDs were better at 6 months postoperatively in group III and V, median values: 2114/mm2, 1961/mm2. PD was largest in group II (median value: 5.5 mm), which was significantly larger than other groups (Padjusted < 0.05). CONCLUSIONS: All 6 methods used in this study were effective for the mechanical dilatation of small pupils and didn't affect the postoperative visual acuity and intraocular pressure in microcoria cataract phacoemulsification. Iris-retractor hooks and the Malyugin Ring can reduce intraoperative corneal endothelium cell loss. Postoperative PD is larger when the iris was cut open radially.

PURPOSE: The aim of this study was to evaluate the effectiveness of a disposable uniplanar pupil expansion device in small-pupil cataract surgery. METHODS: This is a feasibility study carried out at the Rothschild Foundation, Paris, France. Patients undergoing routine cataract surgery with a dilated pupil size < 6 mm, and who agreed to participate in the study were included. The trial enrolled 25 patients, of whom 21 proceeded to cataract surgery using the pupil expansion device to be evaluated. The pupil diameter was measured at defined stages during the cataract surgery, which was performed by a single surgeon, in a single center setting. The 1st generation Bhattacharjee pupil expansion ring was used if the preoperative pupil size was < 6 mm. Intraoperative and postoperative adverse events were recorded. RESULTS: Pupil size immediately after the Bhattacharjee ring implantation was ≥ 6 mm for 15 eyes (71.4%). The mean dilated pupil size before ring insertion was 4.5 ± 0.8 mm (range 2.5-5.8 mm), and the mean pupil size after ring insertion was 6.1 ± 0.3 mm (range 5.9-6.8 mm). Mean pupil size following removal of the ring was 4.2 ± 0.8 mm (range 2.5-5.4 mm). Two adverse events occurred during the surgeries: 1 Bhattacharjee ring broke prior to implantation, and 1 implanted Bhattacharjee ring was unstable and removed before the end of the surgery. No postoperative adverse event was recorded. CONCLUSIONS: The Bhattacharjee ring is an effective pupil expansion device, which facilitates stable pupil expansion during cataract surgery. This study was registered as a clinical trial at clinicaltrials.gov under the number NCT02434588.

OBJECTIVE: Usage of YouTube as an educational tool is gaining attention in academic research. To date, there has been no study on the content and quality of eye surgery videos on YouTube. The aim of this study was to analyze YouTube videos on phacoemulsification in eyes with small pupil. METHODS: We searched for the phrases "small pupil cataract surgery," "small pupil phacoemulsification," "small pupil cataract surgery complications," and "small pupil phacoemulsification complications" in January 2015. Each resulting video was evaluated by all authors, and Krippendorff's alpha was calculated to measure agreement. Videos were classified according to pupil size (small/very small) in the beginning of the surgery, and whether pupillary diameter was large enough to continue surgery safely after pupillary dilation by the surgeon in the video (safe/not safe). Methods of dilatation were also analyzed. Any stated ocular comorbidity or surgical complications were noted. RESULTS: A total of 96 videos were reviewed. No mechanical intervention for pupillary dilatation was performed in 46 videos. Fifty-eight operated eyes had no stated ocular comorbidity. Ninety-five operations ended successfully without major complication. There was fair agreement between the evaluators regarding pupil sizes (Kα = 0.670) but poor agreement regarding safety (Kα = 0.337). CONCLUSIONS: YouTube videos on small pupil phacoemulsification have low complication rates when compared to the literature, although no reliable mechanical dilatation methods are used in almost half of these videos. Until YouTube's place in e-learning becomes clearer, we suggest that viewers be cautious regarding small pupil phacoemulsification videos on YouTube.

AIM: to develop a differentiated approach to hybrid phacoemulsification (PE) in small pupils. MATERIAL AND METHODS: Modified hybrid PE was performed in 119 patients (130 eyes) with conditionally small pupils (preoperative dilated pupil diameter of less than 5.5 mm). RESULTS: A differentiated approach to hybrid PE has been developed and tested in patients with poor preoperative mydriasis. CONCLUSIONS: Small rigid pupils are considered a relative contraindication for hybrid PE. Developed modifications enable less surgical trauma and changes in efficient exposure time, while the latter is usually increased in standard PE in patients with poor preoperative pupil dilation.

RESUMO

Small pupil and Intraoperative Floppy Iris Syndrome (lFIS) has always been a challenge in cataract surgery. Iris dilation and mechanical stabilization can be achieved by using intraoperative iris retractors, preferably before capsulorhexis. Malyugin ring is a mechanical iris expansion device, which presents many advantages (gently relaxes the iris tissue, implants easier and less traumatic). The disadvantages of using Malyugin ring consists primarily in the high cost and in an increased duration of surgical procedure.

Aberrant regeneration occurs in forms of oculomotor motor nerve palsy and frequently involves the pupil, but the incidence and functional impact of ciliary muscle involvement in pediatric patients is sparsely reported in the literature. A 4-year-old girl presented with inflammatory oculomotor motor nerve paresis affecting the inferior division. Initial treatment focused on her inability to accommodate through her physiologic +2.5 D hyperopia and the prevention and treatment of amblyopia. She subsequently developed aberrant regeneration of the pupil, with miosis on adduction. Following eye muscle surgery for residual exotropia and hypertropia, her dry refraction was noted to be more myopic in the affected eye on adduction, mirroring aberrant pupillary constriction. Recognition of pediatric aberrant regeneration of accommodation may influence surgical planning for oculomotor nerve palsy and/or management of amblyopia.

PURPOSE: To describe methods for performing laser-assisted cataract surgery in small pupils. METHODS: Iris retractors or a Malyugin Ring (Microsurgical Technology, Redmond, WA) were used for mechanical pupil dilation before laser capsulotomy and lens fragmentation. The laser treatment can be performed with or without use of ophthalmic viscosurgical devices (OVD). RESULTS: In 73 eyes with small pupils, laser treatment was possible after mechanical dilation device installation. With and without an OVD, no flattening of the anterior chamber or other complications occurred during re-docking, laser treatment, or the manual part of the operation. Small adhesions of the anterior capsulotomy were more frequent when the anterior chamber was still filled with the OVD and did not occur when the OVD was removed. CONCLUSION: A small, poorly dilating pupil constitutes a major challenge in cataract surgery, particularly when performing capsulorhexis. Femtosecond laser treatment can be employed with considerable benefit for the patient, even in these difficult cases.

Insufficient pupillary dilatation is a significant challenge during cataract surgery, as it increases the risk of various intraoperative complications. Implantation of toric intraocular lenses (TIOL) is particularly difficult in eyes with small pupils, as the toric marks are provided in the periphery of the IOL optic, making the visualization of the same difficult for proper alignment. Attempts at visualizing these marks using a second instrument such as a dialler or iris retractor lead to additional manipulations in the anterior chamber resulting in increased chances of postoperative inflammation and intraocular pressure rise. A new intraocular lens (IOL) marker to guide the implantation of TIOLs in eyes with small pupils is described, which can potentially be beneficial in achieving accurate alignment of toric IOLs in small pupils, without the need for additional manipulations, thus improving safety, efficacy, and success rates of TIOL implantation in these eyes.

Objective: To analyze the utility and the nuances of implanting the B-HEX® Pupil Expander (Med Invent Devices, India) at various stages in small pupil phacoemulsification. Methods: This prospective interventional case series was undertaken to assess the utility of B-Hex in small pupil phacoemulsification under topical anesthesia. Our series comprised of 50 cataract cases with small pupils of various etiologies, operated by a single surgeon at our private practice, beginning February 2018. Cataract cases with pharmacological dilation of < 4 mm underwent phacoemulsification with B-Hex implantation. It was employed at the outset or interim, whenever the need arose. Results: The B-Hex ring is extremely handy and useful in small pupil phacoemulsification. The average age of our study cohort was 62 years. In 38 cases B-Hex was employed before making a capsulorhexis. Progressive pupillary constriction during nuclear disassembly warranted the utilization of the device in 9 cases, while the remaining 3 cases had a retained epi-nuclear plate or significant cortex and the ring was placed after nucleus removal. No significant complication was noted. Postoperatively, the pupil dimensions, notably the architecture, were maintained. No significant AC reaction was noted. Intraocular pressure was not high. Conclusion: Our experience showed that B-Hex is secure and easy to use under topical anesthesia. The learning curve is shallow and the technique is precise. B-Hex is truly an ace up the sleeve.

PURPOSE: To investigate outcomes for small versus large pupils in cataract surgery using different pupil expansion techniques. METHODS: Retrospective case-series reviewing 20,175 patients' cataract surgery electronic medical records at Moorfields Eye Clinic in Bedford Hospital NHS Trust from January 2010 to April 2020. Outcomes such as visual acuity (VA), intraocular pressure, intraoperative, post-operative complications were recorded and small pupil expansion device outcome. RESULTS: One thousand, four hundred twenty-six patients were identified as having small pupil (SP). Of these, 1110 patients (77.8%) had interventions to expand the pupil including 447 (31.3%) with intracameral phenylephrine (IC PE) alone, 194 (13.6%) with iris hooks and 469 (32.9%) with a Malyugin ring. The large pupil (LP) group had a statistically significant greater gain in VA than the SP group (p < 0.05). SPs had a significantly higher rate of intraocular complications including posterior capsular rupture (PCR) with vitreous loss (OR 2.75, p < 0.001). There was also a significantly higher rate of post-operative complications such as corneal oedema (OR 2.64, p < 0.001) and anterior uveitis (OR 2.11, p < 0.001) in the SP group. However, VA improvement and complications between the different pupil expansion groups showed no significant differences (p > 0.05) except for a greater rate of iris tears in the Malyugin group (p < 0.05). CONCLUSION: To date, this is the largest reported case-series comparing Malyugin rings and iris hooks with other pupil expansion techniques. The various techniques to expand pupil size appear to be safe and equally effective in improving VA with a similar rate of complications except for a greater rate of iris tears with Malyugin ring.

Purpose: To detect the presence of urotensin-2 (U-II) in the aqueous humor and evaluate the relationship between aqueous humor level and systemic diseases and pupil size. Methods: The study included 88 patients who underwent cataract surgery. Those with a pupil diameter (PD) of up to 4 mm were considered to have small dilation, those with 4-7 mm of dilatation were considered to have moderate dilation, and those with a PD of more than 7 mm considered to have large dilation. Patients with HT (hypertension) were classified as group 1, those with DM (diabetes mellitus) as group 2, and those with HT+DM as group 3, and those without any systemic disease as group 4. The U-II levels in humor aqueous samples taken from anterior chamber were measured. Results: When compared with the control group, it was observed that the level of U-II in the aqueous humor of the HT, DM, and DM+HT groups was significantly higher (P < 0.05). At the same time, when we compared the DM+HT group with the other groups, the level of U-II in the aqueous humor was significantly higher compared to the group with DM (P < 0.05). The U-II levels of the aqueous humor were higher in the patients with small pupils compared to the remaining groups (P < 0.005). Conclusion: U-II may play a role in small pupil pathophysiology. In addition, it was determined that patients with HT and/or diabetes had higher U-II levels in the aqueous humor than healthy individuals.

PURPOSE: To evaluate a new disposable, small-pupil expansion device in tamsulosin patients with intraoperative floppy-iris syndrome (IFIS) having cataract surgery. SETTING: Private practice, Los Altos, California, USA. METHODS: The 5-0 polypropylene Malyugin pupil expansion device (MicroSurgical Technology) was used in 30 eyes from 21 tamsulosin patients having routine cataract surgery. The pupil diameter was measured at the beginning and end of surgery, and the severity of IFIS was graded. Intraoperative and postoperative complications were recorded. RESULTS: The device maintained a constant 6.0 mm pupil diameter throughout surgery. Although iris prolapse was still possible, there were no significant intraoperative or postoperative complications despite the fact that 93% of the eyes had moderate to severe IFIS. All eyes achieved a best corrected visual acuity of at least 20/25. There was a tendency for ring deformation if flash sterilization was used without sufficient cooling time. CONCLUSIONS: The disposable Malyugin pupil expansion device was highly effective at maintaining an adequate pupil opening in eyes with IFIS. It is easier and faster to use than iris retractors and other pupil expansion rings and represents an excellent small-pupil strategy.

RESUMO

As cataract surgery continues to evolve, the intraoperative small pupil continues to pose challenges to even the most experienced cataract surgeon. Several steps can be taken preoperatively to decrease the chance of intraoperative miosis. Even so, the problem of miosis during cataract surgery remains a relatively common occurrence. This paper discusses many steps, both preoperative and intraoperative, that can make surgery technically easier and safer, thus maximizing the postoperative outcomes and patient satisfaction. Complications associated with small-pupil cataract surgery, risk factors for intraoperative miosis, the preoperative and intraoperative management of the small pupil during cataract surgery, and postoperative care are reviewed.

Postoperative outcome of diffractive multifocal intraocular lens (MIOL) implantation in eyes with a small-diameter pupil was evaluated. This non randomized case series involved 23 eyes of 23 patients who underwent diffractive MIOL implantation and whose preoperative photopic pupil diameter was ≤3.0 mm [small-diameter pupil (SDP) group], and 79 eyes of 79 patients implanted with the same MIOL whose pupil diameter was >3.0 mm as controls (LDP group). Contrast sensitivity of high spatial frequency (12, and 18 cycle per degree) and both 12.5% and 6% low-contrast visual acuity (VA) were significantly worse in the SDP group than in the LDP group (P = 0.04, 0.05, 0.05, 0.03). However, no significant difference was found between eyes in the SDP group with a postoperative pupil diameter greater than 3.0 mm and LDP group eyes. No significant differences were found in uncorrected and corrected distance VA (UDVA, CDVA), uncorrected and corrected near VA (UNVA, CNVA), contrast sensitivity with low spatial frequency, and defocus curve between the two groups. In conclusion, in eyes implanted with a diffractive MIOL, a pupil diameter of ≤3.0 mm deteriorates contrast sensitivity. This effect was eliminated postoperatively when pupil size was enlarged to >3.0 mm during surgery.

PURPOSE: To evaluate the outcomes of the first 30 cataract surgeries performed with a new disposable, injector-free, small-pupil expansion device. METHODS: This consecutive case series included 30 eyes from 29 patients who underwent cataract surgery using a new disposable small-pupil expansion device called the Canabrava Ring (AJL Ophthalmic S.A, Spain). It is the first iris expansion ring produced with indents that do not align with each other in the superior and inferior regions, resulting in a small vertical length (0.4 mm) that minimizes the risk of endothelial contact. All eyes had poorly dilated pupils of less than 5 mm preoperatively. Fifteen eyes had significant infective or traumatic pathologies preoperatively. Vertical and horizontal pupil diameters were evaluated preoperatively, intraoperatively, and 1 month postoperatively. RESULTS: The mean patient age was 64 ± 11.8 (standard deviation) years. The Canabrava Ring remained engaged throughout all surgeries, except one. All pupils were intraoperatively expanded to a diameter of 6.3 mm. Although preexisting pathology on the innervation of the pupils, the mean pupil diameter returns to a close preoperative size after 1 month surgery. The mean pupil diameters postoperatively and preoperatively were 4.41 and 3.77 mm, respectively (p<0.05). Postoperative complications occurred in eight eyes (one toxoplasmosis reactivation, one retinal detachment, one posterior capsule rupture, one posterior capsule opacification, and four posterior synechiae). These complications occurred in eyes with preexisting traumatic or infective pathologies or synechiae. CONCLUSION: The Canabrava Ring is effective for expanding and maintaining expansion of small pupils in cataract surgery. The increase in postoperative pupil diameter is clinically diminutive and can most likely be attributed to preexisting pathologies affecting pupil innervation. Further large-scale studies are required to support the present findings.

PURPOSE: Sphincterotomy, an alternative to iris hooks or pupil stretching, is a technique that can aid in small pupil phacoemulsification. The incidence of post-operative complications of this procedure, however, has not been studied. Our study evaluates the post-operative outcomes of phacoemulsification surgery with adjunctive pupillary sphincterotomy. METHODS: We conducted a retrospective review of case notes and Medisoft ® electronic record of patients that had undergone simultaneous sphincterotomy, phacoemulsification and intraocular lens (IOL) implantation by a single surgeon between March 2012 and February 2017. Our main outcome measures were post-operative ocular hypertension (IOP > 21 mmHg), uveitis and cystoid macular oedema (CMO). RESULTS: A total of 114 eyes of 114 patients were included in this study. The mean age was 81.2 years (range: 26-100). All patients had uncomplicated surgery. Transient (<1 month) ocular hypertension developed in five (4%) eyes. Sustained ocular hypertension (>1 month) occurred in one (1%) eye, which had pre-existing glaucoma. All four (4%) eyes that developed a persistent uveitis (>1 month) resolved with topical therapy except for one eye with a history of uveitis. The 6 (5%) eyes that developed CMO had a history significant for uveitis (n = 4), diabetic macular oedema (n = 1) and epiretinal membrane (n = 1). All CMO maculae resolved to their baseline. CONCLUSION: The incidence of post-operative complications following uncomplicated phacoemulsification and IOL implantation with pupillary sphincterotomy is low. The most important predisposing factors for development of a complication are ocular co-morbidities such as glaucoma, uveitis and the presence of a macular pathology.

RESUMO

This paper presents the review of historical aspects and the current state-of-the-art in various pupil dilatation methods to be used in cataract surgery. The surgical algorithm in managing small pupil cases should include topical and intraocular mydriatics, appropriately selected viscosurgical device and mechanical dilatation with instruments, iris hooks, and/or pupil expanders.

PURPOSE: To determine the effects of topical 0.45% ketorolac tromethamine on intraoperative miosis and prostaglandin E2 (PGE2) release during femtosecond laser-assisted cataract surgery. SETTING: Seoul St. Mary's Hospital, Seoul, Korea. DESIGN: Prospective case series. METHODS: The study comprised the following 3 groups: conventional cataract surgery without topical NSAIDs (conventional group); femtosecond laser-assisted cataract surgery with preoperative topical ketorolac tromethamine 0.45% (femtosecond NSAID group), and femtosecond laser-assisted cataract surgery without topical NSAIDs (femtosecond no-NSAID group). To measure the aqueous humor PGE2 concentration, a 100 µL aqueous humor sample was collected from the anterior chamber after femtosecond laser pretreatment. The PGE2 concentration was measured using an enzyme immunoassay. RESULTS: Topical ketorolac tromethamine 0.45% led to a significant reduction in intraoperative miosis in the femtosecond NSAID group compared with the femtosecond no-NSAID group (P < .001). Absolute and relative reductions in pupil area in the femtosecond NSAID group were significantly lower than in the femtosecond no-NSAID group (P = .019 and P = .007, respectively). The mean aqueous humor PGE2 concentrations were 893.60 pg/mL ± 843.10 (SD) in the conventional group, 1911.43 ± 1178.63 pg/mL in the femtosecond NSAID group, and 743.63 ± 927.46 pg/mL in the femtosecond no-NSAID group (P < .001, conventional versus femtosecond NSAID and femtosecond NSAID versus femtosecond no NSAID; P > .05, conventional versus femtosecond NSAID). CONCLUSION: Preoperative topical ketorolac tromethamine 0.45% reduced miosis induced by femtosecond laser pretreatment and inhibited aqueous humor PGE2 elevation.

The B-HEX® Pupil Expander (Med Invent Devices) is a disposable 6.5 mm flexible hexagonal device with notches at corners and flanges at sides. Alternate flanges with positioning holes are tucked under the iris to engage the notches to the margin of the pupil to provide a 5.5 mm expanded pupil. Unlike devices with scrolls or pockets which require an injector to avoid snagging the incision, the preloaded B-HEX is inserted and removed through a 1 mm or larger incision using a manipulator or 23-gauge micro-forceps. The thin profile and uniplanar design allow unhindered instrument movement during phacoemulsification, cortical cleaning, and intraocular lens implantation. The B-HEX is safely used even after capsulorhexis since the thin uniplanar notches are directly visualized to avoid the capsule margin. It is useful in coaxial phacoemulsification, coaxial sub 2.00 mm micro incision cataract surgery (MICS), biaxial 1.5 mm MICS, femtosecond laser assisted cataract surgery (FLACS), small pupil pars plana vitrectomy, and shallow anterior chamber eyes.
