Abordagem acerca da sífilis congênita, doença transmissível resultante da transmissão do agente etiológico da gestante infectada para o seu concepto. Trata-se de doença evitável, desde que haja o diagnóstico e tratamento das gestantes infectadas, durante o período pré-natal. A abordagem pressupõe a existência de processos sociais envolvidos na ocorrência de casos de SC, além dos processos biológicos de transmissão vertical do agente etiológico. Entende-se que os determinantes sociais da saúde, enquanto atributos do processo de determinação social da saúde. Propõe-se a caracterização dos casos notificados de sífilis congênita e em gestantes, no período de 2011 a 2014, e associar a situação de saúde, com enfoque específico nos diferenciais de morbidade da sífilis congênita e em gestantes, às condições de vida das populações, incluindo a utilização de serviços de saúde, tendo o território enquanto categoria explicativa do processo saúde-doença. Utilizando-se dados do SINAN, SINASC e do censo do IBGE, buscou-se uma abordagem ancorada em modelo teórico de determinação da transmissão vertical da sífilis, que inclui os distintos níveis de realidade. No nível individual, a descrição dos casos registrados, segundo variáveis biológicas, socioeconômicas, de localização e de utilização de serviços de saúde, mostrou elevada proporção de casos residentes em bairros situados na zona norte suburbana e oeste da cidade, com baixa escolaridade, de cor da pele negra, bem como as falhas na resposta do setor saúde à transmissão vertical, no que diz respeito ao diagnóstico e tratamento. No nível agregado, foram utilizados os bairros do Rio de Janeiro enquanto unidades de análise, para descrever a distribuição espacial das taxas de incidência da sífilis congênita e em gestantes.
Approach about congenital syphilis (CS), a transmissible disease resulting from the transmission of the etiological agent of the infected pregnant woman to her concept. It is an avoidable disease, if there is the diagnosis and treatment of infected pregnant women, during the prenatal period. The approach presupposes the existence of social processes involved in the occurrence of CS, in addition to the biological processes of vertical transmission of the etiological agent. It is understood that the social determinants of health as attributes of the process of social determination of health. It is proposed to characterize the reported cases of congenital and pregnant syphilis in the period from 2011 to 2014, and to associate the health situation, with a specific focus on the differential of morbidity of congenital syphilis and in pregnant women, to the general conditions of existence of the populations, including the use of health services, with territory as an explanatory category of the health-disease process. Using data from SINAN, SINASC and the IBGE census, an approach was anchored in a theoretical model of the determination of the vertical transmission of syphilis, which includes the different levels of reality. At the individual level, the description of the registered cases, according to biological, socioeconomic, location and health services, showed a high proportion of cases in neighborhoods located in the suburban north and western areas of the city, of the black skin, as well as the failures in the response of the health sector to vertical transmission, with respect to diagnosis and treatment. At the aggregate level, the neighborhoods of Rio de Janeiro were used as units of analysis to describe the spatial distribution of the incidence rates of congenital syphilis and in pregnant women.
